Neurosurgical management of epilepsy: a personal perspective in 1983.

The therapeutic goal in the neurosurgical treatment of medically intractable epilepsy is complete seizure control, for both biologic and psychosocial reasons. Cortical resections are more likely to accomplish this than other surgical alternatives for epilepsy. Although abnormalities on new imaging techniques (CT, positron emission scanning) aid in identifying the epileptic focus, interictal epileptiform EEG changes remain the main indicator of focal origin of the seizures. Where this is equivocal, direct brain recording of spontaneous seizures with subdural electrodes is of value in identifyin

Abnormalities in cortical and subcortical morphology after neonatal neocortical lesions in rats.

The brain weight, cortical thickness, cross-sectional areas of subcortical structures, and various retrograde changes were compared in rats with neonatal or adult ablation of all or part of the neocortex. Neonatal lesions produced a widespread reduction in brain size accompanied by a variety of major structural changes including modification of the thickness of the residual cortex, necrosis and calcification of subcortical structures, and gross distortion of the structure of the hippocampus. The modification of the cortical thickness, but not the other changes, depended on the site and extent

Not 'indifference to pain' but varieties of hereditary sensory and autonomic neuropathy.

Three children, from different kinships, with generalized insensitivity to pain, showed unusual manifestations of congenital, presumably inherited, sensory and autonomic neuropathy. The first child appeared to have a syndrome resembling those previously described as congenital indifference to pain, congenital universal loss of pain sensation from infancy without other apparent neurological deficit. Unlike most types of hereditary sensory and autonomic neuropathies (types I, II, III), but like type IV, she had normal sensory nerve action potentials. Abnormalities of sudomotor function and of so

Quantitative analysis of the lateral geniculate nucleus in the mutant microphthalmic rat.

A quantitative analysis of the lateral geniculate nucleus was carried out in the mutant microphthalmic rat. In the dorsal lateral geniculate nucleus (LGNd) of the microphthalmic rat we found the total volume and neuronal population were reduced by 45 and 68% of normal values, respectively. The size of normal LGNd neurons was 8 to 20 microns and that of mutant LGNd cells from 6 to 16 microns. Neurons of the normal LGNd were medium-size and round or oval, and their cell bodies were filled with Nissl substance. Microphthalmic LGNd neurons, on the other hand, had narrow cytoplasmic spaces with few

The clinical significance of changes in cerebral perfusion pressure during open-heart surgery.

In 34 high-risk patients, the intracranial epidural pressure (EDP) and the cerebral perfusion pressure (CPP) were monitored in connection with open-heart surgery. The findings were clinically evaluated, with special attention to affection of the central nervous system. Reduction of EDP to 10-30 mmHg for periods up to 10 min at the start of extracorporeal circulation and transient rise of EDP in the early postoperative period were relatively well tolerated. Late postoperative EDP rise, on the other hand, denoted an unfavourable trend, associated with permanent cerebral damage. Mannitol and/or b

Developmental deficiencies of the upper facial skeleton due to partial elimination of mesencephalic neural crest cells in the chick embryo.

With the aim to test the hypothesis that cells derived from the mesencephalic portion of the neural crest, are involved in the process of differentiation of various upper facial bones, in 41 chick embryos of the 6-somite stage (approx. 26 hours of incubation) the anterior and middle thirds of this part of the neural crest were partially eliminated by micro-laser irradiation, either unilaterally or bilaterally. Of the 14 embryos sacrificed at the age of 12 days, a number of 6 proved to have developed harelip and/or cleft palate conditions. In these embryos, in addition a reduction or absence of

Craniofacial malformation in Xenopus laevis tadpoles caused by the exposure of early embryos to ethanol.

The principal features of fetal alcohol syndrome (FAS) include microcephaly, reduced body size, and characteristic facial malformation, most of which are associated with hypoplasticity of structures around the mouth. A recent study using pregnant mice suggested that the malformations in FAS are caused by the effects of ethanol on early embryos during gastrulation and neurulation. Exposure of Xenopus laevis early embryos to 1-2% ethanol until they developed to the late neurula stage produced craniofacial malformations in tadpoles which have many similarities with the described features of FAS.

[Crossed aphasia in right-handed patients. I. Review of the literature].

More than 70 cases of crossed dextral aphasia have been reported in the literature since the end of the XIXth century. If a genetic, environmental or even pathological factor--or lack of information about it--could be suspected to be responsible of a majority of these cases, 10 of them in which all these factors were eliminated still remain. A summary of the neurological, neuropsychological and neurolinguistic features of these 10 cases shows, among other things: 1. that nearly all of them present a (left) motor deficit associated with a quite large and deep right-hemispheric lesion; 2. that m

Evidence for long-term functional plasticity in the visual cortex of adult cats.

1. Vision was investigated with behavioural and electrophysiological techniques in three groups of cats: (a) two normally raised kittens in which one eye was rotated at an age of 3 months, (b) three adult cats in which one eye had been rotated and the other closed 6 months prior to recording, (c) two adult cats in which first one eye had been rotated and the other closed and subsequently, after one year, the rotated eye had been closed and the normal eye re-opened. The latter two cats were investigated 6 and 12 months after reverse suture, respectively. All adult cats were at least 2 years old

Norepinephrine regulation of cerebral glycogen utilization during seizures and ischemia.

Norepinephrine (NE) depletion of the cerebral cortex after lesion of the ipsilateral locus ceruleus (LC) causes abnormalities of cerebral oxidative metabolism when the cortex is stimulated to increased energy demand (Harik, S. I., J. C. LaManna, A. I. Light, and M. Rosenthal (1979) Science 206: 69-71; LaManna, J. C., S. I. Harik, A. I. Light, and M. Rosenthal (1981) Brain Res. 204: 87-101). These abnormalities were exhibited as decreased mitochondrial reducing equivalent flow. One possible cause of this would be the decreased availability of oxidative metabolic substrates in the NE-depleted co

Development of polyamine and biogenic amine systems in brains and hearts of neonatal rats given dexamethasone: role of biochemical alterations in cellular maturation for producing deficits in ontogeny of neurotransmitter levels, uptake, storage and turnover.

Excessive levels of glucocorticoids are thought to interfere with synaptic development in the central nervous system. In the present study, dexamethasone given to newborn rats produced deficits in brain growth associated with shifts in the developmental pattern of the ornithine decarboxylase/polyamine system consistent with delays in cellular maturation. The effects on the brain were of smaller magnitude and shorter duration than those on the heart or on general growth and were indicative of "brain sparing." Although some biochemical indices of synaptic development of central noradrenergic sys

Cerebral blood flow, brain pH, and oxidative metabolism in the cat during severe insulin-induced hypoglycemia.

The effects of severe hypoglycemia on brain pH, cerebral blood flow (CBF), and other physiologic and metabolic parameters were studied in 26 cats subjected to insulin hypoglycemia. Two groups were utilized to compare the effects of anesthesia. The halothane group was composed of 14 animals and the barbiturate group contained 12 animals. Insulin was administered by both the intravenous and intramuscular routes until there was a severe electroencephalographic (EEG) change or until 6 h had elapsed. The cerebral responses to hypoglycemia demonstrated the following: CBF was unaffected by severe hyp

A change in susceptibility of rat cerebellar Purkinje cells to damage by alcohol during fetal, neonatal and adult life.

The sensitivity of rat cerebellar Purkinje cells to ethanol exposure during fetal, neonatal or adult life was assessed by histological techniques. Pregnant female rats were exposed to ethanol vapour during the last 2 weeks of gestation. Purkinje cells were counted 5 days after the pups were born. The number of Purkinje cells in lobe VIII was reduced by 45%, and the linear density of Purkinje cells in lobe I was 47% less than in controls not exposed to ethanol. Smaller reductions were found in other lobes. The weight of the cerebellum was reduced by 34%. Neonatal rats were exposed to ethanol va

Fever and evoked potentials in multiple sclerosis.

The somatosensory evoked potentials (SEPs) and visual evoked potentials (VEPs) were studied in 19 patients with multiple sclerosis; 17 controls were studied during fever (38.0 degrees - 39.7 degrees C) and 2-3 days following return to normal temperature. The latencies of components N20 and P114 were measured and specified as abnormal when their value exceeded the standard deviation of the controls by 2.5 times. The corresponding criterion for the evaluation of the amplitude of components N20 and P114 was a reduction in amplitude of more than 50%. In the controls fever did not cause significant

Neurochemical activities in human temporal lobe related to aging and Alzheimer-type changes.

Activities relating to 3 neurotransmitter and 4 neuropeptide systems have been examined in human temporal lobe (post mortem) for their relationships with age and Alzheimer-type changes (senile plaques and cognitive function). Significant alterations with increasing age (from 61 to 92 years) in a series of non-demented cases included a reduction of the cholinergic enzyme, choline acetyltransferase, and an increase in vasoactive intestinal peptide immunoreactivity. In cases of alzheimer's disease the only neurochemical activity investigated which correlated significantly with cognitive impairmen

Toxicity and teratogenicity studies with the hypolipidemic drug RMI 14,514 in rats.

The hypolipidemic drug RMI 14,514 (5-tetradecyloxy-2-furoic acid) has an oral LD50 of over 5000 mg/kg in rats. In a chronic toxicity study (6 months drug diet) doses of 30, 100, or 300 mg/kg/day produced no obvious signs of toxicity or abnormal clinical pathology parameters, other than prominent growth retardation at 300 mg/kg, which was somewhat alleviated when the dose was reduced to 200 mg/kg after 6 weeks. Hepatic change in the form of mild lipid accumulation was noted histopathologically after 6 months of treatment at 100 or 300 mg/kg/day, but was not present at 3 months or after 4 weeks

Late results of operation in temporal lobe epilepsy in adults and children.

In our clinic at the Medical Academy in Warsaw 282 patients with temporal lobe epilepsy were treated surgically. The causes of the illness were trauma in 43.2%, including birth trauma in 19.5%, infections diseases in early life in 19.8%, and microtumours in 6.8%. In 30% the aetiology was unknown. Fifty per cent had fits before 10 years and 75% before 20 years of age. The period of conservative treatment was on average nine years. Serial and stereo-EEG recordings including activation by ketamine, as well as intracarotid sodium amytal tests were performed routinely. The interictal EEG epileptifo

Neurotransmitter amino acids in dominantly inherited cerebellar disorders.

We measured amino acid contents in the brains of 11 patients with dominantly inherited cerebellar disorders. Despite clinical similarities, three biochemically different disorders were found. One disorder, with demonstrated HLA linkage in one pedigree, was characterized by moderate reduction of aspartate and glutamate contents in cerebellar cortex alone. In a second disorder, aspartate and glutamate contents were reduced markedly in other brain areas as well as in cerebellar cortex. Aspartate and glutamate contents were normal in cerebellar cortex in the third disorder. GABA content in cerebel

Morphogenesis of experimentally induced Arnold--Chiari malformation.

The administration of a single dose of vitamin A to pregnant hamsters, early during the morning of their 8th day of gestation, induces types I and II Arnold--Chiari malformation (ACM), as well as various types of axial skeletal-dysraphic disorders known to be associated with the human disease. This new model provides a means of carrying comparative studies between the axial skeletal defects and neurological anomalies of this complex developmental malformation with those which characterize the other induced disorders related to it. Study of this experimental model has demonstrated that the basi
