Vaginal spermicides and congenital disorders.

Among 763 liveborn infants of white women who had obtained a vaginal spermicide in the ten months before conception, the prevalence of certain major congenital anomalies was 2.2%. The prevalence of such anomalies in a comparison group of 3,902 infants was 1.0%. The difference between these groups was due to an excess of limb-reduction deformities, neoplasms, syndromes associated with chromosomal abnormalities, and hypospadias in the infants whose mothers are presumed to have used vaginal spermicides. Pregnancies in women who had used vaginal spermicides ended in spontaneous abortion requiring

[Prognostication of neonates by compact polygraphy (author's transl)].

The polygraphic methods currently available for the prognostication of high risk neonates are timeconsuming, almost inaccessible to visual analysis and unsuited for clinical practice. A polygraphic method has been devised using a restricted number of standardized evaluation criteria arranged according to optimalisation principles with a reduction of observation time to three sleep cycles to an average of 90 minutes. Using this type of "compact polygraphy" 135 newborn infants were divided in a "simple" and a "complicated" group. Their follow-up over the first year of life showed that abnormal E

Abnormalities in the leukocyte sodium pump in advanced cirrhosis.

Previous studies have demonstrated abnormalities of intracellular electrolyte content and sodium transport in leukocytes of patients with fulminant hepatic failure. The current study was undertaken to establish whether similar abnormalities were present in patients with encephalopathy from advanced cirrhosis. Results from 19 patients with advanced cirrhosis showed values for the leukocyte total sodium efflux-rate constant were significantly reduced in patients, 3.02 +/- 1 SEM 0.12 h-1, compared to control values, 3.80 +/- 0.06 h-1. This reduction was due primarily to a lowering of the ouabain-

[Value of brain sonography in newborns and children (author's transl)].

Intracranial changes of newborns and young children can easily be discovered by modern sonography, especially as long as fontanelles are still open. Sonography of brain first should be done before computed tomography, encephalography and/or ventriculography are performed. Correspondence of most sonographic findings and computed tomography by other authors, is supporting reduction of CT-investigations in favour of sonography. We cannot agree with the idea that computed tomography should be the most important first investigation. Technical reasons as thick skull or closed fontanelles are making

Characterization of abnormalities in the visual system of the mutant mouse pearl.

Mice of the mutant strain pearl (pe/pe) differ from the wild strain by a single gene mutation, which leads to a lightening of the coat color. We tested this strain to see if this mutant gene also expressed itself in one or more visual abnormalities. Pearl mice were found to lack totally the optokinetic nystagmus reflex that was present in every normal mouse that we examined. This lack of optokinetic nystagmus was not due to oculomotor defects, since postrotatory nystagmus was normal. As described for other pigmentation mutants, we found that pearl mutants had a reduced ipsilateral projection t

Arthrogryposis multiplex congenita: an autopsy case of a fatal form.

A case of a severe and fatal form of arthrogryposis multiplex congenita with a full necropsy examination is presented in which the central nervous system and many muscles of the four extremities were examined histologically. The most striking feature was a great reduction in the muscular tissue of the limbs with a marked increase in the adipose tissue. The muscular changes were thought to be caused by neurogenic atrophy, and microscopy of the spinal cord revealed developmental abnormalities, including degenerative changes of the anterior horn cells. Neonatal and fatal cases of arthrogryposis m

Antibiotic prophylaxis in ventricular shunt surgery. I. Reduction of operative infection rates with methicillin.

Administration of prophylactic antibiotics is not a proven or universal practice in cerebrospinal fluid (CSF) shunt surgery although case and operative infection rates in hydrocephalic patients average 20 and 8%, respectively. In sequential series from 1969 through 1978 the authors achieved a reduction in case infection rates from 10.9 to 8.9% and in operative infections from 8 to 2.6% with the use of short-term prophylactic methicillin. Comparison of these results to those of other reported series supports the case for short-term prophylactic antibiotics in shunt surgery. Further reduction in

Neuronal depletion of cerebellum in late infantile metachromatic leukodystrophy.

The cerebellums of two children with late infantile metachromatic leukodystrophy were investigated by light and electron microscopy. These observations showed that the morphological abnormalities were comprised not only of severe demyelination and presence of metachromatic granules but also marked loss of Purkinje cells and granule cells. The depletion of Purkinje cells is considered to have resulted from demyelination of their myelinated axons. On the other hand, the reduction of granule cells is considered to be caused by the loss of Purkinje cells and presynaptic elements such as mossy fibe

[A psychometric study of a group of Parkinsonian patients].

A group of 45 parkinsoniens patients were studied by means of Wechsler's adult intelligence scale (WAIS). The mean verbal intelligence quotient (VIQ) was not significantly different from that of the theoretical mean of the general population, whereas the performance intelligence quotient (PIQ) was significantly lower. Superior verbal over performance scale ratings were found significantly more frequently in the parkinsonian group than in the general population. Dispersion analysis tended to show that some parkinsonian patients have a fairly marked reduction in intelligence efficiency which is

Abnormal retino-geniculate and geniculo-cortical pathways in several genetically distinct color phases of the mink (Mustela vison).

Several genetically distinct color phases of mink, which all show an abnormal reduction of pigment in the retinal pigment epithelium and which also show abnormalities of the retinofugal pathways, have been studied. Autoradiographic methods have been used to demonstrate the retino-geniculate pathways, and retrograde degeneration or the retrograde transport of horseradish peroxidase has been used for the geniculo-cortical pathways. The retino-geniculate abnormality is mild in some of the color phases and extremely severe in others, but within any one color phase the variability is relatively low

Morphology of experimental spina bifida in the chick embryo.

Open malformations of the central nervous system may involve the brain or spinal cord, or both. Preliminary experiments in which a window was cut in the shell overlying early chick embryos (with removal of 2 ml of albumen) produced a range of neural and non-neural malformations. Exposure of Stage 5--10 embryos at 26 hours of incubation produced open brain and cord defects. Embryos were recovered at 11--12 days for gross examination. Open cord defects in 12 day experimental embryos could be divided morphologically into 2 types. One group showed an everted symmetrical plaque of neural tissue. In

N-ethyl-N-nitrosourea-induced teratogenesis of brain in the rat. A cellular and cytoarchitectural analysis of the neocortex.

N-Ethyl-N-nitrosourea (ENU) was administered intravenously to pregnant Wistar-albino rats on days 14--21 of gestation in order to study the teratological effects of the carcinogen on the developing brain. Offspring were killed 60 days postnatally, and the brains examined histologically by cresyl violet staining and Golgi-Cox preparations. Macroscopic examination of the brains revealed a graded pattern of microcephaly. Injection of ENU on day 14 of gestation gave the most severe effects and injection on day 21 the least effects. Microscopic examination of the neocortex revealed a graded reducti

Effects of low-level x-irradiation on cat cerebella at different postnatal intervals. I. Quantitative evaluation of morphological changes.

Whole head fractionated doses of 200 r and 150 r were initiated postnatally in five experimental age groups (birth, 1-week, 2-week, 3-week and 4-week) and continued over a period of 14 or 20 days to prevent reconstitution of the external granular layer. Animals irradiated at birth displayed minor deficits in behavior, which included ataxia, tremor, hypertonus and dysmetria, while animals irradiated at 1-week showed only mild symptoms of hypermetria. All other animals displayed no motor deficits. Animals irradiated at birth had smaller eyes and ears, a reduction in the size of the entire head a

EEG in presenile dementia related to cerebral blood flow and autopsy findings.

EEG, regional cerebral blood flow (rCBF) and neuropathological autopsy data were studied in 17 patients with presenile dementia. RCBF was measured in 14 cases using the intra-arterial-133-Xenon clearance technique. The neuropathological study included semi-serial whole brain microscopical sections in which the severity and regional distribution of the neuronal degeneration was studied, particularly in the cortex and the brain stem. There were two main diagnostic groups, seven cases of Alzheimer's disease and five cases with a cortical atrophy of the Pick (DFT) type. Both groups showed fronto-t

Neurotransmitter enzyme abnormalities in senile dementia. Choline acetyltransferase and glutamic acid decarboxylase activities in necropsy brain tissue.

Reductions in 2 neurotransmitter synthesizing enzymes in brain, glutamic acid decarboxylase (GAD) and choline acetyltransferase (CAT), have been found in dementias of different origins, including senile dementia (Alzheimer type). Significant reductions in cerebral GAD have also been found in depression (unipolar). The GAD reductions did not generally appear to be localised in any specific region of the brain examined. However, the reduction of CAT in the hippocampus, relative to reductions in other areas examined, was substantially greater in the brains with Alzheimer-type changes. GAD and CAT

Thyroid dysfunction in chronic renal failure. A study of the pituitary-thyroid axis and peripheral turnover kinetics of thyroxine and triiodothyronine.

Thyroid function was evaluated in 46 patients with end-stage kidney disease and 42 normal subjects. Patients were studied before and after the institution of maintenance hemodialysis (HD) and after renal transplantation (RT). Serum total triiodothyronine concentrations (TT(3), ng/100 ml, mean+/-SD) were 63+/-17 and 83+/-22 in the non-HD and HD groups, respectively. Values from normal subjects were 128+/-25 and from RT patients 134+/-20. The TT(3) was in the hypothyroid range (<78 ng/100 ml; 2 SD below normal mean) in 80% of non-HD and 43% of HD patients. Mean serum total thyroxine concentratio

Effect of prenatal heat stress on brain growth and serial discrimination reversal learning in the guinea pig.

Exposure of pregnant guinea pigs to an environmental temperature of 42 degrees C for 1 hr daily on Days 20-24 of gestation resulted in a significant reduction in the birthweight and brainweight of newborn offspring. These deficits persisted to early maturity and were still evident at 250 days of age following behavioral testing. Although some degree of postnatal neurogenesis and brain growth had occurred, this was not sufficient to compensate for the retarding influence of prenatal hyperthermia. Heat stress was also observed to seriously impair learning performance on the original discriminati

Effects of visual deprivation and alterations in binocular competition on responses of striate cortex neurons in the cat.

Electrophysiological recordings were made from single neurons in striate cortex of normally reared kittens (group N), kittens raised with binocular lid-suture (group BD), and kittens raised with one eye lid-sutured and other eye removed (group MD-E). The MD-E group represents a condition in which inputs from the deprived eye have been placed at a competitive advantage over those from the other eye. In agreement with previous studies, fewer cells were responsive to visual stimulation in BD kittens than in N kittens. Among the responsive cells, fewer were direction selective, fewer were orientat

Pyramidal cell abnormalities in the motor cortex of a child with Down's syndrome. A Golgi study.

The neuronal organization of the motor cortex of a 19-month old child with Down's syndrome (mongolism) has been studied with the rapid Golgi method. This congenital syndrome, also known as 21 Trisomy is caused by a chromosomal abnormality consisting of the presence of an extra chromosome in the group 21. Various structural abnormalities have been found in the dendritic spines (postsynaptic structures) of the pyramidal neurons of the motor cortex of this child. The axo-spinous synapses of these neurons are considered to be altered by these spine abnormalities. In addition, a peculiar form of in

Periodic sleep apnea: chronic sleep deprivation related to intermittent upper airway obstruction and central nervous system disturbance.

Periodic sleep apnea may be due to repeated episodes of upper airway obstruction in patients who have a short thick neck and/or large jowls. Apnea due to complete cessation of breathing may occur to a lesser extent. Anaylsis of the sleep electroencephalogram shows that these patients rarely achieve deep sleep and have less stage 1-REM sleep than normal subjects of comparable age. They are chronically sleep-deprived, a manifestation expressed by daytime somnolence, chronic fatigue and often by personality disturbances marked by paranoia, agitated depression and hostility. The definitive diagnos
