Optic nerve hypoplasia: a review.

Optic nerve hypoplasia is a developmental anomaly of the retina and optic nerves in which there is a reduction in the number of ganglion cells in the retina and of their centripetal fibers projecting through the optic nerve to the lateral geniculate body. The condition may be unilateral or bilateral and is frequently misdiagnosed as optic atrophy. In about 25% of cases, bilateral optic nerve hypoplasia is associated with a variety of cerebral malformations of which the commonest single disturbance is absence of the septum pellucidum (septo-optic dysplasia). Cerebral malformations and their end

Hypothalamic defects in two adult patients with septo-optic dysplasia.

Two adult patients with unilateral hypoplastic optic nerves, absent septa pellucida and hypopituitarism are described. Patient 1, aged 20, presented with diabetes insipidus due to partial vasopressin deficiency. Patients 2, aged 29, presented with focal epilepsy. Both had short stature. They showed absent growth hormone (GH) response to insulin-hypoglycaemia or glucagon, but responded to 100 micrograms growth hormone releasing factor (GRF-44) with a rise in circulating GH, suggesting a hypothalamic defect in GH release though a co-existing pituitary defect cannot be excluded. Other hypothalami

Conjugate lateral eye movements and cognitive mode: blindness as a control for visually-induced oculomotor effects.

The relationship between lateral eye movement directionality and type of cognitive task was investigated in a single blind subject with septo-optic dysplasia. Results from an 80-item test showed that rightward eye movements were significantly associated with verbal tasks and leftward eye movements with spatial tasks. As visual influences on oculomotor behaviour were eliminated, the findings suggested that lateralized eye movements might have arisen as a consequence of asymmetrical activation of cognitive origin triggering the frontal eye field orienting mechanism.

Extreme variant of septo-optic dysplasia.

A newborn female is demonstrated, the first child of healthy unrelated parents who was born after an uneventful pregnancy. Computerized tomography of the brain revealed gross malformations of the parietal and occipital lobes, of the cerebellum and of midline structures. The pictures are reminiscent of hydranencephaly. The association with bilateral coloboma of the papilla and diabetes insipidus is considered an extreme variant of septo-optico dysplasia.

Absence of the septum pellucidum. Overlapping clinical syndromes.

A patient with absence of the septum pellucidum, optic hypoplasia, congenital nystagmus, hemiatrophy, and seizures fulfilled clinical and radiological criteria for diagnosis of both septo-optic dysplasia and the syndrome of absent septum pellucidum with porencephalies. The anatomical and clinical similarities between these two syndromes suggest a common embryological basis. Their simultaneous presence in this case further supports this explanation. Clinically mild forms of both septo-optic dysplasia and the syndrome of absent septum pellucidum with porencephalies are now detected with the aid

Neuroendocrine study of a male infant with septo-optic dysplasia.

Septo-optic dysplasia includes abnormalities of the optic nerves and tracts with absence of the septum pellucidum. Most of the recently reported patients were deficient in growth hormone. We describe a male infant with septo-optic dysplasia in whom extensive endocrine evaluation revealed central diabetes insipidus, hypothalamic hypothyroidism and combined (hypothalamic-pituitary) hypoadrenalism, along with normal pituitary growth hormone reserve. This is the first reported case of a patient with septo-optic dysplasia who underwent corticotropin-releasing factor and growth hormone-releasing hor

Computed tomographic findings in septo-optic dysplasia: discordance between clinical and radiological findings.

The clinical and radiological findings in four children with septo-optic dysplasia are reported. All four had growth retardation associated with growth hormone deficiency, as well as varying degrees of ophthalmologic dysfunction. The CT scan findings spanned a spectrum from normal to the expected abnormalities involving the ventricular system and orbits. Only two of the four had an absent septum pellucidum. A third patient had normal CT scans except for optic nerve hypoplasia, while the fourth had entirely normal CT scans of both the brain and orbits. Because the anatomic defects of septo-opti

[Optic nerve hypoplasia and growth hormone deficiency: de Morsier's syndrome].

The association of optic nerve hypoplasia and deficiency of the septum pellucidum and hypopituitarism is known as the De Morsier syndrome or septo-optic dysplasia. Four children with blindness and growth retardation due to growth hormone deficiency are described. Frequent recurrent episodes of severe hypoglycaemia, starting in the newborn period, were observed in three of them. Although visual impairment was noted during the first year and growth failure was detected between the first and sixth year of life in all four patients, the diagnosis of septo-optic dysplasia was delayed for two to six

Specific spatial defect in a child with septo-optic dysplasia.

A blind girl with septo-optic dysplasia was compared with a blind, age-matched control on three spatial tests. Previous observations had suggested a specific spatial learning disorder. Results of the tests showed good verbal intelligence but severe impairment of topographical orientation, route-learning and kinaesthetic memory. It is argued that these dysfunctions were a direct consequence of the malformation syndrome, and could not be accounted for by factors such as sex or congenital blindness.

Hormonal, metabolic, and neuroradiologic abnormalities associated with septo-optic dysplasia.

The clinical, neuroradiologic, and endocrine features in 16 patients with septo-optic dysplasia are reviewed. All of the patients had clinical optic nerve hypoplasia with varying degrees of nystagmus and visual impairment. Only one-half of the patients had absence of the septum pellucidum. Fourteen children were growth hormone deficient, 9 were adrenocorticotrophic hormone deficient, 3 were thyroid-stimulating hormone deficient, and 4 had diabetes insipidus. In most instances, the diagnosis of septo-optic dysplasia can be established by physical examination and neuroradiologic findings, at whi

The endocrine spectrum of septo-optic dysplasia.

Six children with septo-optic dysplasia were evaluated at the Children's Hospital of Philadelphia. There was a common history of young, nulliparous mothers. The clinical presentation of these children differed according to age of presentation and was a function of the degree and type of hypothalamic-pituitary and ophthalmologic impairment. Endocrine evaluation revealed four of six children to be growth hormone deficient, four of six to have hypothalamic hypothyroidism, and two of four to have elevated prolactin levels. One of the patients demonstrated adrenocorticotropic hormone deficiency, an

Holoprosencephaly and related entities.

Holoprosencephaly is a brain anomaly of varying severity with associated extracranial, symptomatic abnormalities in only a minority of cases. The class of brain defects known as holoprosencephaly represents a continuum usually divided into three types: alobar, semilobar, and lobar. Each has both distinctive radiological characteristics and some similarities. Typical facial anomalies are found in the severe forms. Absent septum pellucidum and septo-optic dysplasia, possibly of similar embryological origin, have some resemblances to lobar holoprosencephaly, but are clinically and radiologically

Craniotelencephalic dysplasia in sisters: further delineation of a possible syndrome.

We describe two sisters with a complex of anomalies involving the cranium and brain. The changes in the former are consistent with those previously described as craniotelencephalic dysplasia and those in the latter indicate primary developmental abnormalities of the central nervous system including septo-optic dysplasia, absent olfactory nerves, agenesis of the corpus callosum, and lissencephaly. Per se, these cerebral malformations are causally heterogeneous, but their occurrence in association with craniotelencephalic dysplasia suggests that this combination is a distinct, probably autosomal

Septo-optic dysplasia and median cleft face syndrome in a patient with isolated growth hormone deficiency and hyperprolactinemia.

A pituitary evaluation was carried out in a 12-year-old girl who had early puberty, short stature, optic nerve hypoplasia, and agenesis of the corpus callosum. Her growth hormone (GH) response to insulin-induced hypoglycemia and arginine infusion was blunted. Basal and stimulated levels of prolactin were elevated. The association of GH deficiency and hyperprolactinemia suggests a disruption of the dopaminergic modulation of these hormones. The facial features seen in our patient, such as hypertelorism, V-shaped frontal hairline, and cleft nose and upper lip, are enough to justify the diagnosis

Evolving hypopituitarism in children with central nervous system lesions.

At the time children with suspected hypopituitarism are seen with a subnormal growth rate, it is expected that testing will demonstrate growth hormone (hGH) deficiency. Seven patients with organic CNS lesions--three with histiocytosis X, one with septo-optic dysplasia, one with neonatal meningitis, one with an anterior encephalocele and meningitis, and one with neurofibromatosis who had normal growth hormone concentrations (greater than 7 ng/mL) despite a subnormal growth rate--were studied. Subsequent retesting 0.5 to 4.6 years later demonstrated the development of growth hormone deficiency.

[Septo-optic dysplasia. Apropos of a new case report].

Septo-optic dysplasia was detected in a 3 month-old boy. Abnormalities consisted of agenesis of septum lucidum as shown by cerebral CT scan and of hypoplasia of optic nerves resulting in pendular nystagmus and visual impairment. A slight visual improvement was observed at 10 months of age. Growth curve was normal and blood GH determination was not performed. Rubella infection had occurred in the 2nd month of pregnancy with rising antibody titers up to the 7 1/2 month.

Agenesis of the vermis with fusion of the cerebellar hemispheres, septo-optic dysplasia and associated anomalies. Report of a case.

Agenesis of the cerebellar vermis with fusion of the dentate nuclei and cerebellar hemispheres (rhombencephalosynapsis) is a rare cerebral malformation. We report the case of a 7-h-old girl whose mother had taken the drug phencyclidine during the first 6 weeks of pregnancy. Absence of septum pellucidum, hypoplasia of the commissural system, optic nerves, chiasm and tracts, moderate hydrocephalus, and agenesis of the posterior lobe of the pituitary were also found. Extracranial congenital anomalies involved the cardiovascular, respiratory, urinary, and musculoskeletal systems. Death was seconda
