STRADA deficiency impairs cortical interneuron development in humans and mice.

UNLABELLED: Polyhydramnios, Megalencephaly, and Symptomatic Epilepsy syndrome (PMSE/STRADA-related disorder) is a rare neurodevelopmental disorder characterized by megalencephaly (ME), early-onset drug-resistant epilepsy, neurocognitive impairment, and high early mortality, often due to status epilepticus. PMSE is caused by a multi-exon deletion in STRADA , encoding STRADA, which regulates the mechanistic target of rapamycin (mTOR) pathway. GABAergic inhibitory interneurons (INs) critically modulate the excitatory:inhibitory balance in cortical and hippocampal networks, and IN deficits contrib

Exploring ferroptosis: Unraveling its potential role in autistic spectrum disorder.

Autism spectrum disorder (ASD) is a diverse neurodevelopmental disorder characterized by ambiguous etiological mechanisms and the absence of recognized disease-modifying pharmacotherapies. Ferroptosis, an iron-dependent and lipid peroxidation-driven mechanism of regulated cell death, has been associated with neurodevelopment and neurodegeneration, prompting interest in its potential role in ASD. This narrative review consolidates from molecular and clinical studies, animal models, and in vitro systems to assess ferroptosis as a candidate mechanistic pathway, biomarker source, and therapeutic t

Craniometaphyseal dysplasia: a rare cause of persistent macrocephaly.

Craniometaphyseal dysplasia is a rare sclerosing bone disorder that may be difficult to recognise in early childhood. We report a patient initially evaluated in the toddler period for macrocephaly and craniofacial dysmorphism, in whom early neuroimaging was unremarkable, supporting the diagnosis of macrocephaly of infancy. During early childhood, following head trauma, incidental radiological findings revealed progressive cranial sclerosis. Persistent macrocephaly and craniofacial features prompted further investigation, leading to the diagnosis of craniometaphyseal dysplasia, confirmed by ide

Severe, rapidly progressive, atypical scoliosis with macrocephaly and a de novo phosphatase and tensin homolog missense variant: A case report.

RATIONALE: Early-onset, rapidly progressive, markedly asymmetric, or otherwise atypical scoliosis should prompt evaluation for an underlying genetic etiology. Variants in phosphatase and tensin homolog (PTEN) are associated with macrocephaly and a broad spectrum of developmental phenotypes; however, their clinical relevance to severe atypical scoliosis remains infrequently described. This case highlights the potential diagnostic value of genetic evaluation in patients with atypical scoliosis accompanied by abnormal head growth. PATIENT CONCERNS: A 13-year-3-month-old boy presented with scolio

Loss of Akt3 enhances antiviral immunity while disrupting corneal homeostasis during ocular HSV-1 infection.

BACKGROUND: Akt1 and Akt2 isoforms reveal distinct immune-regulatory roles in HSV-1 pathogenesis. Akt3 is a developmentally important protein, and mutations in Akt3 gene are associated with a wide spectrum of developmental disorders, including extreme megalencephaly; however, its role in corneal homeostasis and pathologies remains poorly understood. Here we investigated the role of Akt3 in HSV-1 infection. METHODS: In vivo HSV-1 infection was performed using WT, Akt3 homozygous, and heterozygous mutant mice via corneal scarification to assess disease outcomes. Viral replication was quantified

Antenatal diagnosis and maternal sirolimus treatment of polyhydramnios, megalencephaly, and symptomatic epilepsy (PMSE) syndrome.

BACKGROUND: Polyhydramnios, megalencephaly, and symptomatic epilepsy (PMSE) syndrome is a rare autosomal recessive mTORopathy caused by biallelic STE20-related kinase adaptor alpha (STRADA) loss-of-function variants. Animal models demonstrate that in utero mechanistic target of rapamycin (mTOR) inhibition can prevent cortical dyslamination, suggesting a prenatal therapeutic window. METHODS: A gravida 3 para 2 patient presented at 32+2 weeks with medication-refractory preterm labor and severe polyhydramnios. Lysosomal metabolism in pancreatic cancer and its crosstalk with host physiology.

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine

Do not Forget to Measure the Head: Hydrocephalus Can Phenotypically Mimic Developmental Coordination Disorder.

Developmental Coordination Disorder (DCD) is a neurodevelopmental condition presenting with poor motor skill development and impaired coordination at a young age. To diagnose DCD, neurologic conditions explanatory for the phenotype, including structural brain abnormalities like hydrocephalus, must be first ruled out. However, these neurologic conditions may phenotypically mimic DCD, which can hamper their distinction. In this article, we report a patient in whom the initial diagnosis of DCD was withdrawn after the identification of acquired hydrocephalus. An important cue in this case was seco

The Prominent Subarachnoid Space in Children: How has a Normal Variant Become Medicolegally Life-Threatening Pathology?

A prominent subarachnoid space (SAS) in infants under 24 months is a very common finding and is a normal variant that can be associated with macrocephaly. This must be differentiated from various pathological conditions that also cause a prominent SAS, including a reduction in brain volume, obstruction to the cerebrospinal fluid (CSF) or malformations of the skull. The inappropriate labelling of normal SAS prominence as enlargement due to pathology and misrepresentation of published literature by some author groups has created confusion medicolegally, contributing to inappropriate conclusions

Discovery of a DNA methylation profile in individuals with Sifrim-Hitz-Weiss syndrome.

Pathogenic heterozygous variants in CHD4 cause Sifrim-Hitz-Weiss syndrome, a neurodevelopmental disorder associated with brain anomalies, heart defects, macrocephaly, hypogonadism, and additional features with variable expressivity. Most individuals have non-recurrent missense variants, complicating variant interpretation. A few were reported with truncating variants, and their role in disease is unclear. DNA methylation episignatures have emerged as highly accurate diagnostic biomarkers in a growing number of rare diseases. We aimed to study evidence for the existence of a CHD4-related DNA me

m6A-mRNA Reader YTHDF2 Identified as a Potential Risk Gene in Autism With Disproportionate Megalencephaly.

Among autistic individuals, a subphenotype of disproportionate megalencephaly (ASD-DM) seen at three years of age is associated with co-occurring intellectual disability and poorer prognoses later in life. However, many of the genes contributing to ASD-DM have yet to be delineated. In this study, we identified additional ASD-DM candidate genes with the aim to better define the genetic etiology of this subphenotype of autism. We expanded the previously studied sample size of ASD-DM individuals ten fold by including probands from the Autism Phenome Project and Simons Simplex Collection, totaling

Novel De Novo RALA Missense Variants Expand the Genotype Spectrum of Hiatt-Neu-Cooper Neurodevelopmental Syndrome.

BACKGROUND: RALA is a small GTPase from the RAS superfamily implicated in signal transduction and cytoskeletal dynamics. Recently, de novo variants in RALA have been associated with a neurodevelopmental syndrome characterized by intellectual disability (ID), developmental delay (DD), and seizures. So far, only < 12 patients have been reported. METHODS: In this study, we report two novel patients with neurodevelopmental impairment and epilepsy carrying previously unreported RALA variants. We performed a thorough clinical investigation of these patients and performed brain MRI to detect potenti

Cellular mechanisms of early brain overgrowth in autistic children: elevated levels of GPX4 and resistance to ferroptosis.

Autistic individuals with disproportionate megalencephaly (ASD-DM), characterized by enlarged brains relative to body height, have higher rates of intellectual disability and face more severe cognitive challenges than autistic children with average brain sizes. The cellular and molecular mechanisms underlying this neurophenotype remain poorly understood. To investigate these mechanisms, we generated human induced pluripotent stem cells from non-autistic typically developing children and autistic children with and without disproportionate megalencephaly. We assessed these children longitudinall

The phenotypic and genetic spectrum of AKT3-related neurodevelopmental condition.

This study was undertaken to expand the phenotypic and genetic spectrum of AKT3-related neurodevelopmental disorders and to investigate genotype-phenotype correlations. To date, more than 200 patients with AKT3-related disorders have been identified, including those with AKT3 single nucleotide variants and copy number variations affecting the AKT3 gene. Adding our three newly diagnosed patients, the total number of patients with AKT3 single nucleotide variant-related neurodevelopmental disorders is now 61. A total of 20 distinct AKT3 variants have been identified, with p.E17K and p.R465W being

Functional verification and allele-specific silencing of a novel AKT3 variant that causes megalencephaly, polymicrogyria and intractable epilepsy.

AKT3, a key component of the PI3K-AKT-MTOR pathway, is highly expressed in the brain, and its activating variants cause megalencephaly and cortical malformations. In this study, we functionally verified a novel missense AKT3 variant (p.Q78R) identified in a patient with extreme megalencephaly and intractable epilepsy. We transiently transfected HEK-293T cells with the AKTWT or AKT3Q78R and observed a significant increase of phospho-S6, a marker of mTOR complex 1 (mTORC1) activity, in AKT3Q78R transfected cells. Furthermore, considering its application in epilepsy treatment research, we identif

Assessing the health hazards of low-dose ethylmercury: Neurochemical and behavioral impacts in neonatal mice through matrix metalloproteinase activation and brain-derived neurotrophic factor release.

Ethylmercury (EtHg) primarily enters the body through contaminated fish and mercury-containing vaccines, raising concerns about its neurotoxic risks, particularly for infants and young children. Although its neurodevelopmental impact has been suggested, research remains inconclusive. Given that neurite outgrowth, matrix metalloproteinase (MMP) activity, and brain-derived neurotrophic factor (BDNF) expression play critical roles in brain development and synaptic plasticity, we hypothesized that EtHg exposure disrupts these processes, leading to behavioral abnormalities. To test this hypothesis,
