Lumbosacral posterior meningocele in adult patient: a case report.

Spinal dysraphism is the incomplete fusion of the neural arch, which can be seen as an occult or open neural tube defect. Meningoceles are a form of open neural tube defect characterized by cystic dilatation of the meninges containing cerebrospinal fluid without the involvement of neural tissue. Neurosurgical intervention is necessary in the newborn period since survival in advancing ages is often impossible. Therefore, meningoceles are rarely reported in adulthood. Here, we discussed a case of a 23-year-old female who presented with a meningocele in the lumbosacral area since birth, which had

Do routine antibiotics change the myelomeningocele infection rate? A case series.

BACKGROUND: Myelomeningocele (MMC) is the most common type of congenital spinal malformation, typically requiring surgical intervention. While prenatal repair is increasingly favored, postnatal repair remains the standard in many settings. This study aims to evaluate the antibiotics prescribed to neonates with MMC and their correlation with central nervous system (CNS) infection rates following postnatal surgical repair. METHODS: A retrospective cohort study was conducted, including newborns who underwent postnatal MMC repair between August 2017 and June 2021 at the Children's Hospital of Goi

Management of a large pseudomeningocele and cerebrospinal fluid fistula after microsurgical resection of recurrent lipomyelomeningocele in children.

OBJECTIVE: Symptomatic retethering in pediatric patients following complex spinal dysraphism surgery for lipomyelomeningocele (LMMC) repair occurs in approximately 20% of cases. Common complications after repeat surgery for retethering include infection, pseudomeningocele (PMC), and cerebrospinal fluid (CSF) fistula. The authors of this report aim to describe the treatment options for large PMCs or CSF fistulas in a series of patients who underwent repeat surgery for recurrent LMMC at their institution. Additionally, they review management strategies from the literature. METHODS: This retrosp

Exploring the Extent and Nature of Disordered Eating Among Canadian Adolescents and Young Adults With Spina Bifida and Hydrocephalus.

BACKGROUND: Adolescents and young adults (AYA) with spina bifida and/or hydrocephalus (SBH) are at a higher risk for disordered eating, poor body image and body dissatisfaction. Regrettably, there is limited research on the eating patterns and behaviours of AYA with SBH, as well as their body image perceptions. OBJECTIVES: The purpose of this study was to explore the nature of disordered eating behaviours among AYA with SBH and their perceptions surrounding their body image. This study represents the first investigation of its kind conducted within a Canadian population. It fills a literature

Mirabegron as part of combination therapy for treatment refractory neurogenic bladder in pediatric patients with spina bifida.

INTRODUCTION: Medical therapy for patients with neurogenic bladder secondary to spina bifida is a key element in their management. Mirabegron has been shown to improve abnormal bladder wall dynamics in the neurogenic bladder (NGB) population, but there is limited information on its efficacy as additive therapy in the treatment of refractory NGB in pediatric literature. OBJECTIVE: This study assessed the utility of mirabegron as combination therapy in pediatric patients with spina bifida who have insufficient improvement of bladder dynamics on antimuscarinic monotherapy. We hypothesized that u

Late Prenatal Development of Hindbrain Herniation in Open Spina Bifida.

INTRODUCTION: We describe 4 cases of open spina bifida initially presumed to be "closed" defects in which serial prenatal ultrasound examinations documented the occurrence of sac rupture and development of Chiari II malformation later in the pregnancy. CASE PRESENTATION: In each case, the ultrasound examinations starting from 19 to 24 weeks' gestation, showed spinal cystic lesions containing neural elements but no evidence of Chiari II malformation. The lack of hindbrain herniation rendered them ineligible for prenatal surgical repair, but follow-up imaging at 24-32 weeks' gestation demonstra

Using technology to enhance prevention of pressure injuries in the spina bifida population.

Patients with spina bifida (SB) are highly susceptible to pressure injuries (PIs) due to limited mobility and sensory deficits, and they may occur during multiple surgical procedures. The article highlights that PIs, which significantly increase hospital costs and extend patient stays, can be prevented mainly through evidence-based interventions. One of the critical technologies used at this stage is pressure mapping to monitor and optimize pressure distribution on surfaces, whether lying or seated, particularly during prolonged medical procedures. Additionally, the article emphasizes the impo

The potential impact of intraoperative neurophysiological monitoring on neurological function outcomes after postnatal spina bifida repair.

INTRODUCTION: The management of open neural tube defects (ONTD) has significantly improved with fetal surgery, but many children remain ineligible for fetal therapy. This study assesses the impact of intraoperative neurophysiological monitoring (IONM) during postnatal myelomeningocele (MMC) repair and its potential to optimize functional outcomes. PATIENTS AND METHODS: Seven newborns with thoracolumbar or lumbar MMC underwent postnatal surgical repair using IONM. Neuromonitoring included motor-evoked potentials (MEP), sensory-evoked potentials (SEP), electromyography (EMG), and bulbocavernosu

Congenital Cutaneous Hamartomas With Skeletal Muscle Differentiation Associated With LUMBAR Syndrome.

BACKGROUND/OBJECTIVES: Congenital skin anomalies have been observed in LUMBAR syndrome, but their clinicopathologic significance remains unclear. This study aimed to investigate the congenital, nonvascular skin anomalies in LUMBAR syndrome (lower body infantile hemangiomas, urogenital anomalies and hemangioma ulceration, spinal cord malformations, bony deformities, anorectal/arterial anomalies, and renal anomalies). We hypothesized that an association exists between complex skin hamartomas and LUMBAR syndrome, similar to recent findings in PHACE syndrome (posterior fossa anomalies, hemangioma,

Novel Techniques in Antenatal Imaging of Spinal Dysraphisms.

PURPOSE OF REVIEW: This review examines the imaging techniques for diagnosing spinal dysraphisms (SD), focusing on advancements in prenatal detection. RECENT FINDINGS: Prenatal ultrasound (US) is the first-line tool for detecting spinal dysraphisms, including myelomeningocele. While US is effective for early detection, it has limitations in fully characterizing defects, particularly due to factors like fetal positioning. To address these, advanced techniques such as 3D ultrasound and AI-driven algorithms have improved diagnostic accuracy. Magnetic resonance imaging (MRI) remains critical for

Financial Toxicity in Spina Bifida: Development of a Patient and Caregiver Questionnaire.

OBJECTIVE: To develop and validate the first patient- and caregiver-reported measure of spina bifida-specific "financial toxicity" (SBFT), that is, the negative side effects of healthcare-related expenditures. Assessing financial toxicity is a growing area of research in both oncology and benign urology. METHODS: We developed a 20-item questionnaire and quantitatively validated it in a population of SB patients and caregivers. We distributed these through email and online platforms. Validation was accomplished via descriptive and other statistical analyses; confirmatory factor analysis was us

Prenatal predictors of ventriculoperitoneal shunt requirement and adverse perinatal outcomes in newborns with open spina bifida.

OBJECTIVE: Spina bifida is a group of anomalies that, while non-lethal, require careful prenatal evaluation and prognosis prediction due to their association with common motor and cognitive problems. In this study, we investigated the need for postnatal ventriculoperitoneal shunting (VPS) in fetuses with open spina bifida and explored the relationship between adverse perinatal outcomes and prenatal ultrasonographic findings. METHODS: In this retrospective study, fetuses with open spina bifida that were diagnosed prenatally and resulted in a live birth were examined. The fetuses were divided i

Advances in spina bifida care: The global journey toward dignity, inclusion, and innovation.

For nearly a decade, the special issue for spina bifida (SB) in the Journal of Pediatric Rehabilitation Medicine (JPRM) has become an epicenter for qualitative, state of the art, and innovative SB research. It has been noted that the concept of "blue marble health," a policy framework to illustrate trends in the geographic distribution of neglected diseases affecting at-risk populations, may also have a place in illustrating the SB care paradigm in high-income countries such as those in North America and Europe. Concurrently, JPRM has dawned to provide insight into SB care worldwide, both in t
