Mortality Through 2021 Among Persons Born With Spina Bifida in Metropolitan Atlanta, 1981-2018.

INTRODUCTION: Information on prevalence, predictors, and causes of mortality is sparse among persons born with spina bifida (SB), especially adults over age 25 years. METHODS: Individuals with SB born in 1981-2018 were identified in surveillance data from the Metropolitan Atlanta Congenital Defects Program, an active population-based birth defects surveillance system, and linked with Georgia death certificates (1981-2021). Survival probability was assessed using Kaplan-Meier curves. Selected factors were examined using Cox proportional hazards regression, estimating crude (cHR) and adjusted h

Translation, cultural adaptation, and validation of the Japanese version of the quality of life assessment in spina bifida for adults.

BACKGROUND: The QUAlity of Life Assessment in Spina bifida (QUALAS) is a self-administered questionnaire that measures health-related quality of life. The Japanese versions of QUALAS for children and teenagers with spina bifida (SB) have been validated. This study aimed to develop and validate the reliability and validity of the Japanese version of QUALAS-A (QUALAS-A-J), the adult version of the instrument. METHODOLOGY: The participants were adults with SB aged ≥ 18 years. The results of cognitive interviews and a preliminary survey conducted on 16 participants were analyzed to confirm the fa

Daily activity avoidance in the context of urinary and fecal incontinence among young adults with Spina Bifida: Preliminary results from an ecological momentary assessment study.

Although studies suggest that young adults with spina bifida (YASB) often avoid important daily activities (ADL) due to urinary (UI) and fecal (FI) incontinence, no research has prospectively examined how often ADL avoidance occurs, or the context in which it occurs. In this paper, we used ecological momentary assessment (EMA) over 30 days to describe: 1) the frequency of avoidance; 2) the activities YASB most commonly avoid; and 3) preliminary association of daily avoidance with affect, incontinence anxiety and health-related quality of life (HRQoL). As part of a larger 30-day prospective stu

Learning with Patient Campaigners About a German Drug Scandal*.

The West German drug Duogynon was internationally marketed as a "hormone pregnancy test" (HPT) between the 1950s and 1980s. In the late 1960s it came under suspicion for inducing miscarriage, spina bifida, and a spectrum of birth defects similar to those caused by the sedative thalidomide. In contrast to thalidomide, medical consensus did not form around the teratogenicity of Duogynon and many people who identify as Duogynon-affected continue to campaign for recognition. The informal use of Duogynon as an abortion pill adds a further layer of shame, secrecy, and silence. In this article, we re

Torsion of a congenital human tail in a Japanese Infant: A case report.

The optimal timing of surgical intervention for a human tail remains unclear. We report a 2-month-old male infant with a tail-like structure in the sacral region, lacking continuity with the spinal cord. Although elective surgery was scheduled at 3 months of age, vascular compromise due to torsion developed on postnatal day 60, requiring emergency excision under local anesthesia. The postoperative course was uneventful, with preserved limb function and no complications at follow-up. This rare case highlights torsion as a potential complication and supports early surgical intervention, even dur

Psychometric Evaluation of Sexual Knowledge and Self-Efficacy Components of the SPARKS Survey for Adults with Spina Bifida.

BackgroundChallenges in accessing comprehensive sexual and reproductive health (SRH) information and care leave many individuals with spina bifida (SB) vulnerable to unmet SRH needs, risking unfulfilling sexual experiences and increasing susceptibility to coercion and abuse. Accurate tools are essential to measure SRH knowledge and self-efficacy in adults with SB.ObjectiveThis study aimed to evaluate the internal consistency and construct validity of domains of a newly developed survey that assesses SRH knowledge and self-efficacy in partner and provider discussions among adults with SB.Method

Automated Screening Network for Fetal Closed Spina Bifida With Semantic Enhancement and Projected Attention.

Closed spina bifida is a high-incidence developmental disorder among rare fetal diseases. Its signs in ultrasound imaging are subtle, making it prone to misdiagnosis and heavily reliant on sonographers' experience. Therefore, we propose a novel semantic enhancement framework incorporating projected attention for the automated screening of closed spina bifida through precise landmark detection. In this method, we utilize a multi-granularity deep supervision and voting mechanism to generate point-specific features and reconstruct saliency maps for each landmark, effectively reducing interference

Large terminal syringomyelia associated with terminal lipoma: mid- to long-term outcomes after tethered cord release.

PURPOSE: The management of large terminal syringomyelia (TS) at initial surgery varies in clinical practice, and outcome data stratified by spinal dysraphism subtype remain limited. We evaluated the mid- to long-term clinical course after tethered cord release (TCR) for large TS associated with terminal lipoma. METHODS: We retrospectively analyzed 12 patients with terminal lipoma (Morota Types 3-4) and large syrinx (syrinx index ≥ 0.5) who underwent TCR without routine additional syrinx procedures between 2012 and 2023. Patients with other spinal dysraphisms were excluded to evaluate a relati

Congenital Limb Duplication and Spinal Dysraphism: A Rare Case Report Highlighting Single-Stage Multidisciplinary Surgery and Life-Changing Outcome.

BACKGROUND Congenital polymelia, defined as the presence of supernumerary limbs, is an exceedingly rare anomaly with an estimated incidence of less than 1 per million live births. Its coexistence with spinal dysraphism creates a complex clinical picture requiring intricate diagnostic workup and surgical planning. The purpose of this case report is to contribute to the existing literature on limb duplication. CASE REPORT A 19-month-old girl presented with an additional limb and the inability to walk independently. Physical examination revealed a "tail-like" appendage with features resembling a
