Congenital Limb Duplication and Spinal Dysraphism: A Rare Case Report Highlighting Single-Stage Multidisciplinary Surgery and Life-Changing Outcome.

BACKGROUND Congenital polymelia, defined as the presence of supernumerary limbs, is an exceedingly rare anomaly with an estimated incidence of less than 1 per million live births. Its coexistence with spinal dysraphism creates a complex clinical picture requiring intricate diagnostic workup and surgical planning. The purpose of this case report is to contribute to the existing literature on limb duplication. CASE REPORT A 19-month-old girl presented with an additional limb and the inability to walk independently. Physical examination revealed a "tail-like" appendage with features resembling a

Spina bifida in Africa: A systematic review and Meta-Analysis of Presentation, Management, and outcomes.

BACKGROUND: Neural tube defects (NTDs), particularly spina bifida, contribute significantly to pediatric mortality and disability in Africa. Limited healthcare resources and variation in management strategies across regions contribute to disparate outcomes. This systematic review and meta-analysis evaluate the epidemiological profile, clinical presentation, management, and outcomes of spina bifida across African regions. METHODS: A PRISMA-guided systematic review was conducted using PubMed, Google Scholar, Cochrane, and Scopus databases. Retrospective studies and case reports reporting spina

"Inappropriate" spinal ultrasounds for suspected occult spinal dysraphism.

OBJECTIVE: Spinal ultrasound is used to investigate occult dysraphism in neonates with sacrococcygeal dimples. Inappropriate imaging studies may be performed despite the Choosing Wisely Canada guidelines. The objective of this study was to assess the appropriateness of spinal ultrasounds performed in 2 Canadian provinces and to compare the diagnostic accuracy of spinal ultrasound and MRI in predicting the need for neurosurgical intervention. METHODS: This multicenter retrospective cohort study identified spinal ultrasounds in neonates younger than 6 months of age from January 1, 2017, to Dece

Cumulative Antibiotic Exposure in Children With Spina Bifida.

PURPOSE: Spina bifida (SB) is a common congenital condition associated with frequent antibiotic use, but cumulative antibiotic exposure in the pediatric SB population remains underexplored. To test our hypothesis that the pediatric SB population has higher cumulative antibiotic exposure because of genitourinary (GU) infections, we will (1) quantify total antibiotic burden by age, (2) assess diagnoses associated with antibiotic prescriptions, and (3) identify differences in prescribing patterns, particularly for GU infections. MATERIALS AND METHODS: This retrospective cohort study used MarketS

Fetal Instability and Resuscitative Management During Fetal Intervention: A Retrospective, Descriptive Analysis of 245 Fetal Cardiac and Spina Bifida Procedures.

OBJECTIVE: The heterogeneity of fetal surgery has led to variability in the nature and frequency of fetal instability. Understanding these differences is critical for optimizing management. METHODS: We reviewed all fetal atrial septal interventions (FASI) and fetoscopic myelomeningocele (fMMC) repairs with fetal instability performed at our institution between March 2012 and April 2025. We described and compared the frequency, timing, etiology, and management of fetal instability. RESULTS: During the study period, 28 unique patients underwent 35 FASI procedures, and 210 patients underwent fM

Associations of arsenic exposure and folate in maternal leukocyte DNA methylation: a case-control study of mothers with spina-bifida affected children.

BACKGROUND: In Bangladesh, more than a quarter of drinking water tubewells are contaminated with arsenic above the national standard (50 µg/l), while nearly half exceed the World Health Organization guideline (10 µg/l). Among other negative health consequences, arsenic is a suspected environmental risk factor for neural tube defects (NTDs), including spina bifida. Maternal folate status protects against NTDs, though recent evidence suggests arsenic attenuates folate’s protective effects. Arsenic is methylated prior to excretion with methyl groups produced in one-carbon metabolism, for which fo

Cutaneous stigmata indicative of occult spinal dysraphism in two ancient Roman statues.

Symptoms of tethered spinal cord syndrome often present later in life and go undiagnosed due to a lack of appreciation of the associated cutaneous stigmata. This is especially true with occult spinal dysraphism. Here, we report cutaneous signs of occult spinal dysraphism identified in two ancient Roman sculptures. Although the significance of such findings may not have been realized, the association between cutaneous stigmata of the back and underlying malformations involving the spinal cord is now recognized, as such findings are typically imaged to evaluate for spinal cord tethering.

Care trajectories of children with spinal dysraphism treated in Reunion Island between 2006 and 2024.

BACKGROUND: Spinal dysraphism (SD) requires lifelong multidisciplinary follow-up to prevent complications and optimize functional outcomes. In geographically fragmented healthcare systems, continuity of care may be challenging, particularly when patients cross regional or national borders to access specialized services. This study aimed to describe care trajectories and identify factors associated with optimal follow-up among children with SD managed in the southwestern Indian Ocean region. METHODS: We conducted a retrospective cohort study including children with SD who received active postna

Anesthetic Management of a Parturient With Lipomyelomeningocele and Tethered Cord for Cesarean Section.

Lipomyelomeningocele (LMMC) presents significant anesthetic challenges due to spinal cord tethering and distorted anatomy, which make the gold-standard neuraxial anesthesia technically difficult and neurologically risky. We report the case of a 29-year-old primigravida with LMMC and prior neurosurgeries scheduled for an elective cesarean section. Given the risk of direct neural injury and unpredictable block spread, a planned general anesthesia (GA) was performed, and a multimodal analgesic strategy was employed. The procedure was uneventful, resulting in a healthy neonate and a mother with ex

A Needle Lasso Method for Port Site Closure and Amniotic Membrane Plication in Fetoscopic Surgery.

INTRODUCTION: Amniotic membrane plication is associated with lower rates of preterm birth following fetoscopy. METHODS: We describe a novel needle lasso technique for amniotic membrane plication during port site closure in laparotomy- and mini-laparotomy-assisted fetoscopic spina bifida repair. RESULT: This approach enables precise placement of full-thickness sutures around the port tract, even when direct suturing is technically challenging. We achieved amniotic membrane plication in all the cases, and delivery beyond 34 weeks of gestation without membrane-related complications. CONCLUSION

Co-existing limited dorsal myeloschisis, spinal dermal sinus tract, and dermoid cyst in a 3-month-old female infant: Illustrative case.

Limited dorsal myeloschisis (LDM) and spinal dermal sinus tracts (DST) are rare forms of closed spinal dysraphism that are usually considered distinct entities, and their coexistence with a dermoid cyst is exceptionally uncommon. We describe a 3-month-old female infant with a congenital lumbosacral swelling; MRI revealed a subcutaneous cyst with imaging features suggestive of an epidermoid-type lesion, a fibro-neural stalk, and associated tethered cord, syrinx, and dorsal tenting. The patient underwent complete excision of the cyst and removal of the fibro-neural stalk along with its dural att

Surgical Treatment of Tethered Cord Syndrome by Release of Filum Terminalis: A Review.

Tethered cord syndrome (TCS) is a disease caused by pathological fixation of the spinal cord, most commonly due to a thickened filum terminale, postoperative adhesions, or congenital dysraphism. Progressive neurological, urological, and orthopedic manifestations result from chronic cord traction and impaired vascular supply. Surgical detethering remains the standard treatment, with the classic intradural sectioning of the filum terminale being the most widely used technique. Recent developments, however, include minimally invasive tubular and endoscopic approaches, spinal column shortening pro

Pre- and postoperative urodynamic risk stratification in infants and young children undergoing tethered cord release.

INTRODUCTION AND OBJECTIVES: Primary tethered cord syndrome (TCS) can lead to neurological and urologic dysfunction. While postoperative urodynamic studies (UDS) are standard for evaluating bladder function after spinal cord detethering (SCD), the role of preoperative UDS remains unclear, particularly in non-toilet trained (NTT) infants and children. This study assesses the utility of preoperative UDS by 1) comparing pre- and postoperative findings and risk stratification, and 2) examining correlations with long-term urologic outcomes. STUDY DESIGN: We retrospectively reviewed records of chil

Evaluating occult spinal dysraphism in children with midline cutaneous lesions: when to image?

BACKGROUND: Occult spinal dysraphism (OSD) represents a group of congenital spinal anomalies in which neural tissue remains covered by intact skin. Because the skin and nervous system share a common ectodermal origin, midline lumbosacral cutaneous lesions are frequently associated with underlying spinal abnormalities. Identifying which cutaneous markers warrant spinal imaging remains clinically important, particularly in children presenting with bowel or bladder dysfunction. OBJECTIVE: To evaluate and emphasize the importance of the possible association between various midline lumbosacral cut
