National trends in orthopedic surgical procedures among adults with spina bifida: A MarketScan database study.

BACKGROUND: Prenatal screening and folic acid supplementation have decreased spina bifida (SB) incidence, while advances in multidisciplinary care have increased life expectancy for those born with the condition. Historically, orthopedic outcomes research has focused largely on childhood, as earlier assumptions regarding limited survival into adulthood contributed to the exclusion of adults with SB from longitudinal studies. As a result, knowledge of orthopedic procedures performed on adults with SB may help to guide treatment in childhood and transition to adult care. OBJECTIVE: To describe

Bulbar "Red Flag" Symptoms and a 21-Year Diagnostic Delay in Adult Arnold-Chiari Type II Malformation With Tracheoesophageal Fistula and Syringomyelia: A Case Report.

The simultaneous occurrence of Arnold-Chiari Malformation Type II (ACM II), syringomyelia, and an acquired tracheoesophageal fistula (TEF) in a young adult is exceptionally rare and represents a major diagnostic challenge. Arnold-Chiari Malformation II is characterized by the herniation of cerebellar structures through the foramen magnum. It usually presents in infancy in association with myelomeningocele; co-existing TEF and syringomyelia in an adult, as in our patient, is seldom documented. Notably, the present case represents a Chiari II phenotype without spinal dysraphism, lying outside th

Recurrent Bacterial Meningitis in an Infant Caused by a Sacral Dermal Sinus Tract.

Congenital dermal sinus tract is a rare form of closed neural tube defect, connecting the epidermis to a deeper layer. It presents in only one in 2,500 live births. This connection between the outside world and the spinal cord is a nidus for bacterial meningitis and other infections, and dedicated imaging is essential in identifying such a tract. We present a case of a 9-month-old previously healthy male who repeatedly presented with intermittent fevers despite recurrent antibiotics. Cerebrospinal fluid (CSF) analysis consistently demonstrated neutrophilic pleocytosis, elevated protein, and hy

Intradural contractions: spinal lipoma surgery with equivocal electromyography activity. Illustrative case.

BACKGROUND: Intradural spinal lipomas are a form of closed dysraphism and a frequent cause of tethered cord. Although usually adipocyte predominant, rare lesions may contain additional histological elements that can yield unexpected intraoperative behavior. OBSERVATIONS: A 13-year-old boy presented with progressive right foot deformity, distal weakness, gait limitation, and a low conus at S1-2 with a large terminal lipoma on spine MRI. He underwent detethering with debulking and L5 osteoplastic laminoplasty under intraoperative neuromonitoring (IONM). During microsurgical dissection, a pink b

Case Report: Novel Insights Saccular Limited Dorsal Myeloschisis - A New Indication for Prenatal Surgery? Case Series and Scoping Review.

INTRODUCTION: Limited dorsal myeloschisis (LDM) is a rare type of closed spinal dysraphism characterized by a fibroneural stalk connecting a midline skin defect to the underlying spinal cord. When saccular, LDM can mimic open spina bifida (OSB) lesions such as myelomeningocele (MMC) in prenatal imaging, potentially leading to misdiagnosis and inappropriate management. CASE PRESENTATION: We retrospectively analyzed eight cases of LDM diagnosed at the Dr. Franco Ravera Zunino Hospital in 2024. Clinical, imaging, surgical, and histopathological data were collected. Three cases were misdiagnosed

Bilateral Lumbar Artery Perforator (LAP) Flaps for Extensive Lumbosacral Defects: A Report of Two Cases.

Extensive lumbosacral defects remain a major reconstructive challenge because of limited regional tissue mobility, contamination risk, proximity to neural structures, and frequent association with prior surgery or spinal instrumentation. Although lumbar artery perforator (LAP) flaps are recognized as muscle-sparing options for posterior trunk reconstruction, their capacity to safely reconstruct very large midline lumbosacral defects and their applicability in rare adult presentations of lumbosacral myelomeningocele remain insufficiently defined. We report two adult female patients who underwen

Perioperative Predictors of Length of Stay and Readmission After Simple Cystectomy and Urinary Diversion in Nonambulatory Adults With Neurogenic Bladder.

OBJECTIVE: To identify factors associated with readmission and length of stay (LOS) after simple cystectomy in neurogenic patients to inform Enhanced Recovery After Surgery (ERAS) pathway development. ERAS protocols improve outcomes after oncologic cystectomy, but guidance for neurogenic populations is limited. METHODS: This retrospective study identified patients with Multiple Sclerosis (MS), Spinal Cord Injury (SCI), Spina Bifida (SB), or related conditions who underwent cystectomy (2013 and 2023). Exclusion criteria were age ≤18, cystectomy without diversion, and ambulatory patients. Prima

Diagnostic overshadowing in primary progressive multiple sclerosis with coexisting tethered cord syndrome and long-standing urological dysfunction: a case report and literature review.

BACKGROUND: Primary progressive multiple sclerosis (PPMS) may be diagnostically challenging when coexisting structural spinal abnormalities produce overlapping neurological and urinary manifestations. Tethered cord syndrome (TCS) and PPMS can both present with progressive myelopathy and lower urinary tract symptoms, increasing the risk of diagnostic overshadowing. CASE PRESENTATION: A 42-year-old woman with a history of sacrococcygeal meningocele and prior surgical resection presented with a 20-year history of urinary dysfunction and a 2-year history of progressive ascending sensory symptoms

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The intersection of infant substance exposure, foster care placement & child welfare policy.

OBJECT: The author studied the specific issue of ventral tethering in cases of type II split cord malformation (SCM). He also discussed the issue of ventral tethering in patients with type I SCM in whom a bony septum and dural sleeve have been resected. METHODS: Eleven (21%) of 52 patients with type II SCM were found to have a ventral fibrous septum tethering the hemicords to the ventral dura. Four categories of ventral septa were noted: 1) pure ventral intradural septa (three patients); 2) complete dorsoventral intradural septa (three patients); 3) dorsoventral septa continuous with a dermal

Spinal cord tethering rarely occurs in the cervical region. In adults, it usually results from previous operations. However, congenital origin is always diagnosed and treated early in the infant period. We report a 12-year-old boy with cervical spinal dysraphism which was erroneously diagnosed as focal muscular atrophy, a benign form of motor neuron disease. The patient was brought to our hospital because of rapid deterioration of symptoms. Careful evaluation disclosed a hairy dimple at the nuchal area, which led to the correct diagnosis. X-ray of the cervical spine showed spina bifida from C(

Updated classification with spinal dysraphism and treatment outcomes of arteriovenous shunts below conus: a retrospective cohort study.

BACKGROUND: Arteriovenous shunts below conus medullaris (AVS-BC) are understudied, particularly those associated with spinal dysraphism. This study aimed to refine the classification and management of AVS-BC. METHODS: A retrospective analysis of patients with AVS-BC from two centers over two decades was performed, focusing on clinical presentations, angioarchitecture, and treatment outcomes. AVS-BC was classified into eight subtypes based on angioarchitecture, dural relation, and spinal dysraphism presence. Treatment efficacy was evaluated using changes in the modified Aminoff and Logue's Sca

The Intrauterine Treatment of Open Spinal Dysraphism.

BACKGROUND: Open spinal dysraphism is a congenital malformation that causes major morbidity. Its consequences include sensory and motor impairment as well as bladder- and bowel dysfunction. It is often also associated with prenatal ventriculomegaly, which, in turn, necessitates postnatal treatment with a ventriculoperitoneal shunt in approximately 80% of cases. Prenatal therapy with coverage of neural tube defect can reduce the shunt rate and preserve motor function. In this review, we describe the different surgical procedures and their outcomes. METHODS: This review is based on publications

Having more resilience factors only gets you so far in the context of neighborhood disadvantage: understanding differential predictors of internalizing symptoms and body mass index in youth with spina bifida.

OBJECTIVE: The primary aim of this study was to determine if neighborhood disadvantage predicted internalizing symptoms and body mass index (BMI) in youth with spina bifida (SB), while accounting for several sociodemographic factors. We also explored whether resilience factors helped explain associations between neighborhood disadvantage and internalizing symptoms or neighborhood disadvantage and BMI. METHODS: Participants (n = 69, Mage=14.13, Range: 8-20, 51% biological female, 25% Hispanic/Latinx White, 28% Other/Multiracial, 48% non-Hispanic/Latinx White) were drawn from a retrospective ch

Long-term efficacy of Mirabegron-anticholinergic combination in paediatric neurogenic bladder.

INTRODUCTION: The Mirabegron-anticholinergic (MAC) combination has proven effective as a step-up strategy in managing paediatric neurogenic bladder following anticholinergic medication and botulinum toxin (BTX) therapy. This study assesses the long-term efficacy of MAC in children with neurogenic bladder. PATIENTS AND METHODS: A retrospective chart review was conducted from 2015 to 2023, including consecutive paediatric patients receiving Mirabegron (25/50 mg) with an anticholinergic agent (solifenacin 16, tolterodine 7, oxybutynin 7, trospium 1). MAC was started where previous therapy had fa
