A case of cervical diastematomyelia and syringohydromyelia in a 16-year-old female myelomeningocele patient is reported. Progressive weakness of the upper extremity led to an MR examination of the brain and spine, which revealed hydrocephalus, Chiari II malformation, cervical diastematomyelia with a syringohydromyelic cavity in each hemicord and a large dural sac in the lumbar region. Operative therapy consisted of detethering and shunting of the two syringes. Soon after surgery her symptoms improved. The need for early complete MR imaging of myelomeningocele patients presenting with new sympt

The aim of our study was to assess the influence of the Arnold-Chiari malformation, cervical cord dysraphism and hydrocephalus on hand function in subjects with meningomyelocele, but without many of the other confounding factors commonly found in this group e.g low IQ and cerebral palsy, which in themselves cause poor hand function. Three groups of subjects who attended main stream schools underwent a basic neurological examination of the upper limbs and a battery of hand function tests. Group 1 consisted of subjects with meningomyelocele and shunted hydrocephalus (n = 21), Group 2 subjects ha

Cervical meningomyelocele, spina bifida, and hydrocephalus were diagnosed in a newborn miniature colt that was unable to stand and had a cystic mass in the caudal portion of the dorsal cervical region. Results of physical examination and ultrasonographic imaging of the brain supported the diagnosis of hydrocephalus. Results of radiographic evaluation of the vertebral column were consistent with spina bifida at C5-C6. Fluid aspirated from the cervical mass resembled CSF. Radiography of the cervical region after injection of iohexol into the mass revealed herniation of the spinal cord through a

The indications for hindbrain decompression in neonates and young infants with spinal dysraphism who experience brain stem dysfunction in association with Chiari II malformations have remained controversial. This largely reflects the fact that the postoperative outcome in such patients has often been poor, which has supported the belief that much of the brain stem compromise in these patients is congenital and inherently irreversible. However, in a previous retrospective review of our operative results between 1975 and 1989, we noted that a significant component of the brain stem dysfunction i

Cervical spina bifida, either occult or overt, is relatively uncommon. These lesions tend to be associated with other abnormalities of the central nervous system and spine. For the occult dysraphic lesions, a Klippel-Feil syndrome is a particular association, especially the type I variety. For the spina bifida cystica lesions, the more common associated features include a Chiari II malformation and hydrocephalus. Spina bifida cystica in the cervical region is different from the myelomeningocele of the lower spine and is characterized by two types of abnormalities: the myelocystocele herniating

Published cases of cervical dysraphic lesions are rare. Their estimated proportion is about 5% of the entire group of spinal dysraphism. Four cases are reported, representing 2 types of lesion: 2 meningoceles with a simple fibrous band fixing the posterior aspect of the cord to the skin and 2 meningomyelocystoceles with other associated anomalies (hydromyelia, hydrocephalus, Chiari II, Peter's ocular anomaly). The clinical presentation is that of a soft posterior cervical mass without marked neurological impairement. The surgical treatment is simple, consisting in the resection of the lesion,

Between 4% and 8% of cases of spina bifida cystica occur in a cervical or cervicothoracic location. Despite a large body of literature concerning spinal dysraphism, there has been little written specifically about patients afflicted with this disorder in a cervical location. Eight children who presented at birth with posterior cervical or cervicothoracic lumps, all of which represented a dysraphic state, are discussed. Two types of abnormalities were noted. Three patients had hydromyelia with an associated myelocystocele herniating posteriorly into a meningocele sac. In these three patients th

Motion of the cerebrospinal fluid (CSF) in and around the brain and spinal cord was examined in healthy subjects and in a number of patients with abnormalities of the CSF circulation. The pulsatile motion of the CSF was determined by spin echo phase (velocity) imaging, sometimes in combination with gradient echo phase contrast cine. Differences in flow patterns across CSF spaces were observed: flow reversal in the cerebellomedullary cistern and lumbar area relative to cervical CSF, and in the posterior versus the anterior subarachnoid space in the spinal canal. Flow communication was demonstra

Simple meningoceles are infrequent forms of dysraphism and are often benign. They have been associated with other spinal anomalies. The uncommon cervical meningocele may have a higher propensity to be associated with other spinal anomalies. Four patients with cervical meningocele are presented with radiographic evaluation and clinical course. Multiple abnormalities were documented radiographically and operatively, including hydrocephalus, Chiari malformation, hydromyelia, lipomeningomyelocele, tethered cord, thickened filum terminale, diastematomyelia, Klippel-Feil syndrome, and thoracic hemiv

To determine the value of ultrasound scanning of the craniocervical junction in neonates via a posterior approach, we examined 50 infants with normal posterior fossae, 10 with congenital abnormalities, and eight with intracranial hemorrhage. Good evaluation of the cisterna magna, medulla, tonsils, vermis, cervical cord, and central canal was possible in most cases. In nine patients with spinal dysraphism, all displayed a Chiari II malformation; of these, a kink at the medullary cord junction was seen in six, and a cerebellar peg was noted in four. In one case, the Chiari malformation was confi

The possibility to diagnose slight masked malformations increased in the last years both by improving technical ultrasonic equipment and progressive personal experience of the examiners. With regard to time and technique it is impossible to treat fully all possibilities of ultrasonics, because the time-patient-ratio would be to enormous, the malformation rate would be to low and not any malformation is followed by an obstetric consequence. Additionally most of the malformations can be diagnosed or excluded only in determined gestational weeks. Therefore we present as a compromise a checklist w

From 1976 to 1981 111 transurethral operations were performed in 93 children; 39 bladder neck incisions or resections, 43 urethrotomies, 6 resections of urethral valves, 13 transversal incisions of ureteroceles, 7 transurethral biopsies in suspected bladder tumors and 3 resections of the neck of a bladder diverticulum were done. Complications were seen especially in the myelodysplasia-group. Multi-resistant bacteriuria was found in many children even before therapy. Hospital-induced fungal infections were seen twice. Malfunctioning of the hydrocephalus drainage-system developed once after tran

Bony anomalies encountered in the 'no neck' form of Klippel-Feil syndrome (KFS) are a wide, short, fused, bifid, retroflexed spinal canal; craniolacunia, cranium bifidum, and acrania. The only symptom may be mirror movement (MM). The CNS anomalies are hindbrain hernia, hydrocephalus, hydromyelia, syringomyelia, meningocele, myelocele, encephalocele, and anencephalus. In severe KFS, i.e. iniencephalus (IN) and in anencephalus (AN), the inion is in contact with the back. In both there is hindbrain hernia and the left thorax may contain the stomach tethered to an anterior spina bifida. KFS result

The orthopaedic surgeon is often the first consultant to whom a patient with syringomyelia is referred. The disease is not as rare as he may suppose, but its early presenting features are very variable; if he relies solely on such familiar features as pes cavus and scoliosis, he may well miss the diagnosis. The commonest presenting symptom is pain in the head, neck, trunk or limbs; headache or neckache made worse by straining is particularly significant. A history of birth injury also may suggest the possibility of syringomyelia, especially if any spasticity subsequently worsens. Neurological

The authors report a study of 92 human embryos and four fetuses with myeloschisis. The characteristics of embryonic myeloschisis compared with spina bifida cystica in infants are: 1) the lesion is often more diffuse, involving the whole spinal cord (12 embryos); 2) the cervical cord is frequently affected (23 of the remaining 80 embryos); 3) holoprosencephaly is frequently associated (18 embryos); 4) meningocele is not found; and 5) hydrocephalus and Arnold-Chiari malformation are not yet developed. Hydrocephalus and Arnold-Chiari malformation are found in myeloschistic fetuses. Almost all emb

The classification of the developmental malformations of the posterior fossa associated with hydrocephalus proposed by Chiari is systematic and comprehensive. It is based upon the relative position of cerebellum and brainstem in relation to the foramen magnum and the upper cervical canal. Types I to II are degrees of a similar abnormality in which a conical deformity of the posterior midline cerebellum and the elongated brainstem lie at or below the foramen magnum. In the Type IV deformity, in contrast, the cerebellum and brainstem lie fully within the posterior fossa and their position in rel

The temperature control of 11 infants with severe abnormalities of the central nervous system has been investigated. 4 were normothermic and 7 poikilothermic. Necropsy of the latter showed that either the hypothalamus was absent or disorganized, or the long tracts through the brainstem were unidentifiable, or in one case interrupted by a cervical myelocele. The lateral horn cells, sympathetic nervous system, and brown fat appeared normal. Where the normal reflex responses to warm and cool stimuli were absent, direct stimulation of the effector organs was attempted. Noradrenaline both stimulate
