Late diagnosis of lumbar lipomyelomeningocele with tethered cord syndrome in an adult: case report and literature review.

BACKGROUND: Lipomyelomeningocele (LMM) is the most common form of closed spinal dysraphism and is typically diagnosed in childhood. Although many patients are initially asymptomatic, progressive neurological deterioration may occur over time due to tethered cord syndrome (TCS). Adult presentation of untreated LMM is rare and poorly described in the literature, and optimal management strategies in this population remain controversial. CASE: We report the case of a 53-year-old man with a congenital lumbosacral mass and a history of partial superficial lipoma resection in childhood, who presente

Health Literacy Trends and Associations With Transition Readiness in Youth with Spinal Dysraphism.

OBJECTIVE: To assess whether health literacy (HL) changes over time are associated with transition readiness among young people with spinal dysraphism (SD), given that coordinating transition from pediatric to adult care for individuals with SD is vital. METHODS: We performed a retrospective longitudinal study of prospectively collected patient-reported outcomes from patients> 12 years old between May, 2019 and February, 2024 in a multidisciplinary spina bifida clinic. Our primary exposure was HL, measured with the BRIEF questionnaire. Our primary outcome was transition readiness, assessed wi

Ultrasound-assisted neuraxial anaesthesia in an adult patient with repaired spina bifida, scoliosis and difficult airway.

Patients with spina bifida (SB) undergoing neuraxial anaesthesia have increased risk of further neurologic injury due to altered spinal anatomy. Meanwhile, scoliosis is associated with concomitant restrictive lung diseases. While research has been focused on managing secondary comorbidities related to SB, limited attention has been given to perioperative anaesthetic care of its increasing adult population. This case presents a woman in her 30s with repaired SB, scoliosis and predicted difficult airway. She was diagnosed with oxygen-requiring pneumonia and bilateral popliteal ulcers for electiv

Blastemal-Predominant Primary Extrarenal Nephroblastoma Associated with Lumbosacral Lipomeningocele in an Infant: A Case Report and Focused Review.

Primary extrarenal nephroblastoma, or extrarenal Wilms tumor, is an exceptionally rare embryonal tumor outside the kidney. Spinal/lumbosacral involvement is particularly uncommon and may mimic congenital dysraphic lesions. We report a 10-month-old female infant followed since birth for presumed sacral lipoma and operated on for suspected meningomyelocele/lipomeningocele. Imaging demonstrated a solid S1 to S3 lesion adjacent to a thin lipomatous lesion near the conus medullaris. Histopathology unexpectedly showed a blastemal-predominant triphasic nephroblastoma with epithelial differentiation a

Tethered cord syndrome and pediatric scoliosis: workup, natural history, and surgical timing.

PURPOSE: Tethered cord syndrome (TCS) is an important consideration in pediatric scoliosis because it can present with curve progression, pain, and evolving neurologic or urologic dysfunction that may affect deformity evaluation and surgical planning. This state-of-the-art review summarizes the current literature on TCS in pediatric spinal deformity, with emphasis on diagnostic workup, scoliosis natural history, and surgical timing relative to deformity correction. METHODS: A literature review was performed focusing on pediatric TCS in the setting of spinal deformity, including dysraphism-ass

Interobserver Agreement of Filling Cystometry Parameters in Children With Neurogenic Lower Urinary Tract Dysfunction.

PURPOSE: Urodynamic studies (UDS) are the gold standard for evaluating pediatric lower urinary tract dysfunction (LUTD). However, previous studies have shown that the interpretation of cystometric findings may vary according to the observer's level of experience and the criteria used for evaluation. This study aimed to investigate interobserver differences in the reporting of cystometric parameters in children with spina bifida. METHODS: Patients who underwent UDS between 2020 and 2025 for neurogenic LUTD secondary to spina bifida were retrospectively reviewed. Studies that did not reach 80%

Diagnostic Test Characteristics of Renal Ultrasound Findings for High-Risk Bladder in Spina Bifida.

INTRODUCTION: Spina bifida (SB) can cause neurogenic bladder dysfunction and renal deterioration. Guidelines recommend renal ultrasound (RUS) for surveillance, but its utility in screening for high-risk bladder is unclear. We evaluated the performance of RUS findings for detecting high-risk bladder in youth with SB. MATERIALS AND METHODS: We conducted a retrospective cohort study of patients with myelomeningocele at a pediatric SB clinic who had a RUS within 3 months of urodynamic studies (UDSs). RUS parameters included hydronephrosis, thinned parenchyma, hypoplasia, and scarring. High-risk b

[In utero treatment of spina bifida].

Dysraphisms are characterized by an anomalie of the vertebrae and spinal cord. Only the prevalence of open dysraphisms is known, which in France is around 5 per 10,000 pregnancies. The prenatal diagnosis rate for open dysraphisms exceeds 95% in France, with a high rate of termination of pregnancy, close to 90%. Open dysraphisms (myelomeningocele and myeloschisis) represent the most severe forms and are associated with lower limb paralysis (the severity of which depends on the level of the defect), urinary sphincter incontinence (neurogenic bladder), and fecal incontinence, making a request for

Open fetal myeloschisis closure: pearls for uterine exposure and closure.

Fetal closure of an open neural tube defect offers the potential to limit progressive neurological injury and improve long-term outcomes. Myeloschisis presents unique challenges due to the technical complexity of mobilizing the skin and achieving adequate soft tissue coverage, particularly in the sacral region. In this video, the authors demonstrate an open fetal myeloschisis closure in a fetus with an S1-level defect and associated grade 3 hindbrain herniation. The preoperative counseling process, surgical technique, and perioperative course are highlighted, with a focus on pearls for the exp

A Hybrid CNN-Transformer Model with Crossover Boosted Cheetah Optimization for Prenatal Spina Bifida Identification from Ultrasound Images.

OBJECTIVES: Spina bifida is a birth defect caused by the incomplete closure of the neural tube around the spinal cord. Earlier, various deep learning-based models were developed, but they failed to capture the fine details from the ultrasound images that reduce the detection accuracy. Hence, to overcome these challenges, an automated and trustworthy deep learning framework is significant for accurate identification of spina bifida from second-trimester fetal ultrasound images. METHODS: This research proposes a novel hybrid convolutional neural network-transformer model for spina bifida identi

Perioperative shear wave elastography for the evaluation of spinal cord release in pediatric dysraphism: first experience in 21 patients.

OBJECTIVE: Shear wave elastography (SWE) is a recent method for evaluating soft tissue stiffness. Its application to the spinal cord is limited by the ultrasound bone barrier. Spinal dysraphism can cause neurological symptoms impairing bladder, bowel, and lower limb function due to a mechanical stress known as tethered cord syndrome. Detethering of the spinal cord is a part of surgical treatment. This study aimed to assess the feasibility of intraoperative elastography to guide surgery in patients with spinal dysraphism involving traction of the spinal cord. METHODS: This single-center retros

Transamniotic stem cell therapy (TRASCET).

Transamniotic stem cell therapy (TRASCET) constitutes the least invasive approach to prenatal stem cell delivery described to date. This strategy is largely based on the recently discovered active transport of donor cells from the amniotic fluid to the placenta directly through the amnio-placental interface. From a simple amniocentesis, via the placenta, donor cells of different phenotypes can reach the fetal circulation and virtually any fetal anatomical site, including, and particularly, the fetal bone marrow. As a result, TRASCET has shown broad therapeutic potential in an assortment of ani

MRI-Assisted Planning for Spinal Anesthesia in a Pregnant Woman with Radiographic Spina Bifida Occulta: A Case Report.

Background and Clinical Significance: Spina bifida occulta may be associated with occult spinal dysraphism, including a low-lying conus medullaris or tethered cord, which can increase the risk of neurological injury during neuraxial anesthesia. We report a case in which preoperative magnetic resonance imaging (MRI) and lumbar ultrasonography supported individualized planning for spinal anesthesia for cesarean delivery. Case Presentation: A 31-year-old woman at 37 weeks of gestation was scheduled for elective cesarean delivery because of marginal placenta previa. She had chronic low back pain,
