Split cord malformations in adults.

PURPOSE: Split cord malformations represent a small group among tethered cord syndromes. This paper presents the largest series of adult patients with this disorder reported to date. Neuroradiological features, clinical symptoms, surgical management and data on short- and long-term outcomes are analyzed. METHODS: 94 adults (mean age 54.7 ± 15 years) presented with split cord malformations between 1991 and 2024 and were evaluated (follow-up of 64 ± 84 months). Radiological features, intraoperative findings, and neurological examinations before and after surgery were analyzed. Long-term outcome

Upper extremity blood-flow restriction training applied during walking in an adult with a rare form of spina bifida: a case report.

INTRODUCTION: Adults with spina bifida have impaired mobility and often rely on assistive devices like crutches for walking, which exert high torque demands on the upper extremities. Blood flow restriction training (BFRT) may improve strength and gait measures by increasing intensity of training when applied during walking for individuals with limited walking tolerance. However, no studies have investigated the application of BFRT to the upper extremities for individuals who rely on crutches for walking. CASE DESCRIPTION: This case describes a 29-year-old female with diastematomyelia, a rare

Characterization of Orthopaedic Indications Among Patients Undergoing Tethered Cord Release.

BACKGROUND: Tethered cord syndrome (TCS) can present with neurologic, urologic, and/or orthopaedic symptoms, but little research has focused on the orthopaedic conditions that result in tethered cord release (TCR). This study aims to categorize orthopaedic findings associated with TCS and identify which conditions require further surgical intervention post TCR. METHODS: This retrospective cohort study involved 247 patients from our tertiary referral center, all enrolled in the National Spina Bifida Patient Registry (NSBPR) and who underwent TCR between 2007 and 2017. Patients were grouped by

Diastematomyelia on imaging: Clinical and radiological considerations of this spinal abnormality.

Diastematomyelia, a rare congenital anomaly of the spine, is characterised by the division of the spinal cord and is often associated with other spinal and cutaneous malformations. Diagnosis is primarily conducted via prenatal ultrasound and fetal Magnetic Resonance Imaging (MRI). The postnatal evaluation is carried out using Computed Tomography (CT) and MRI, which are essential for comprehensive assessment and therapeutic planning. Prognosis depends on the presence of neurological symptoms, whose severity may correlate with specific radiological findings. Treatment ranges from surgery in symp

Human pseudotail associated with diastematomyelia: A case report and literature review.

BACKGROUND: The human tail is just the distal remnant coccygeal appendage of the embryonic tail. It may be formed by muscle or fatty tissue and by cartilage or bone covered with skin. Many cases may be associated with spinal dysraphism. The case presented here is of a child with a human pseudotail in the region of the lumbar spine associated with diastematomyelia. CASE DESCRIPTION: A 4-year-old girl presented with pain and bulging in the lumbar spine. On neurologic examination, there were no motor or sensory deficits, the urinary and anal sphincters were normal. Imaging examinations (computed

Retroaortic retrocaval reverse horseshoe kidney associated with vertebral and spinal cord anomalies.

While horseshoe kidneys are the most common renal fusion anomaly, various complex variant anatomies exist. One rare form is the retroaortic retrocaval reverse horseshoe kidney, where the upper poles fuse posterior to both the aorta and inferior vena cava. These intricate renal anomalies increase complication risk and complicate surgical interventions. We report a 3-year-old boy diagnosed with a retroaortic retrocaval reverse horseshoe kidney during a spinal deformity evaluation. This case is one of few instances of this specific variant documented in the literature and is the first to be assoc

Congenital lumbosacral meningocele with malignant transformation to epithelioid angiosarcoma: A case report of complex spinal anomalies.

Congenital neural tube defects rarely manifest in adulthood, and malignant transformation of meningocele is exceptionally rare. This report describes a rare case of a 56-year-old male with a congenital lumbosacral meningocele that underwent malignant transformation to epithelioid angiosarcoma, coexisting with multiple spinal anomalies, including tethered cord and diastematomyelia. Following admission and diagnostic workup, the patient underwent surgical intervention. Tumor recurrence occurred 3 months after surgery, and received combined radiotherapy and targeted therapy. Follow-up magnetic re

Partially calcified giant hemorrhagic syringomyelia and hematomyelia.

Syringomyelia is a rare condition characterized by the formation of a fluid-filled cyst within the spinal cord, leading to myelopathy. In addition, the pathological enlargement of the central canal is referred to as hydromyelia or cleft-like syrinx. We present a case of idiopathic syringomyelia and hematomyelia in a 50-year-old female patient with a 5-year follow-up on her disease progression. Magnetic resonance imaging (MRI) images revealed low-signal intensity on T1 and high-signal intensity on T2, with elevated hemorrhagic signal intensity on T1 and low peripheral signal intensity on T2. Split cord malformation - a simple, current classification based on CT and MRI neuroimaging studies.

The aim of this paper is to present the currently used classification of split cord malformation. Split cord malformation (SCM) is a developmental defect arising during neurulation, resulting in abnormal neural tube development, with the formation of a division within the spinal cord and dural sac. The terms diastematomyelia and diplomyelia are used in the literature to describe this defect. In 1992, Pang proposed the term SCM to describe all dysraphic spinal cord defects and classified them into type I and type II, depending on the nature of the sagittal septum within the spinal canal and the

Adult Type II diastematomyelia with tethered cord and associated spinal anomalies: A case report.

Spinal dysraphism consists a group of congenital anomalies due to defective neural tube closure, among which diastematomyelia or split cord malformation is rare. Split cord malformation is classified into Type I and Type II, with Type II being less common and often asymptomatic. These anomalies may coexist with various spinal anomalies, as tethered cord, neural lipoma, hemimyelocele, and dorsal dermal sinus, forming a complex spectrum. Adult presentations are particularly uncommon and usually incidental. We herein report a 23-year-old male with chronic low back pain and a congenital midline lu

Comparison of Intraspinal Abnormalities Prevalence in Congenital Scoliosis: Is Multiple Hemivertebra Associated with Higher Risk than Single Hemivertebra?

BACKGROUND: The increasing utilization of magnetic resonance imaging has facilitated the detection of intraspinal abnormalities in congenital scoliosis (CS) caused by hemivertebra. However, the risk of intraspinal abnormalities across different hemivertebra patterns remains unclear. The aim of this study was to compare the prevalence of intraspinal abnormalities between single hemivertebra and multiple hemivertebra and identify key associated risk factors. METHODS: A total of 1,048 patients with CS caused by hemivertebra who received surgical correction were included. The radiographic and cli

Dorsal 3D-printed plate stabilisation of atlantoaxial instability associated with atlanto-occipital overlap in three dogs.

Atlantoaxial instability is a common condition in toy breed dogs, sometimes associated with occipital dysplasia. Surgical stabilisation is challenging due to the small bone size and proximity to the spinal cord. This case report describes three dogs treated with a custom 3D-printed dorsal plate for atlantoaxial instability, considering the presence of atlanto-occipital overlap. Each patient underwent pre- and postoperative computed tomography, and follow-up was obtained via telephone for the surviving dogs. The use of a dorsal 3D-printed plate appears to offer a feasible and clinically accepta

Challenging Case of Sitting Imbalance as an Indication of Surgery for Grotesque Congenital Deformity of Spine - A Case Report.

INTRODUCTION: Among the various musculoskeletal manifestations of post-acute Coronavirus disease 2019 (COVID-19) syndrome, osteonecrosis and pyogenic infections are of serious concern due to their potential for joint destruction and the need for major surgery, even among young patients. Spontaneous septic arthritis is rare among healthy adults. Very few cases of osteonecrosis of the femoral heads (ONFH) with concomitant septic arthritis of the hip following a COVID-19 infection are reported in the literature. Imaging of Craniovertebral Junction Instability, Fixation, and Stenosis in Children.

Craniovertebral junction (CVJ) instability, fixation, and stenosis in children are closely related conditions that are often challenging to diagnose and are associated with significant morbidity. Groups at higher risk for CVJ abnormalities include children with trisomy 21, juvenile idiopathic arthritis, upper respiratory infection or other inflammatory conditions of the head and neck, and certain skeletal dysplasias. Radiography, CT, and MRI play complementary roles in the evaluation of pathologic conditions of the CVJ. CVJ morphometry is helpful in characterizing osseous relationships and sug
