Congenital spinal and cord lesions in children and computed tomographic metrizamide myelography.

Computed tomographic metrizamide myelography (CTMM) has developed into a highly sophisticated and accurate neuroradiologic technique in the diagnosis of childhood spinal dysrhaptic states. Of the 84 patients studied by this technique, 31 with the dysrhaphic spine also had lesions such as a tethered and low conus, diastematomyelia, or neuroenteric cysts. We believe that the exquisite pathologic anatomy obtained by CTMM obviates all neuroradiologic techniques other than a preliminary anteroposterior and lateral spinal roentgenograph.

Diastematomyelia--rare and unusual features.

Diastematomyelia is an uncommon congenital anomaly of the spine. Four unusual variations of diastematomyelia are presented. These include (1) double diastematomyelia, (2) adult presentation, (3) diastermatomyelia without a septum and unusual termination of two halves of the spinal cord, and (4) recurrence of a fibrous band following removal of an osseous septum.

[Diastematomyelia. About a series of 6 cases (author's transl)].

The authors report 6 cases of diastematomyelia in children. They stress the rarety of this spinal cord diformity whose pathogenesis is obviously of dysraphic origin. From the clinical point of view, "orthopedic syndrom" is the most common complaint (cypho-scoliosis and - or - malformation of the feet) in a child with dorsal or lumbar cutaneous dysplasia. Neurological disorders or sphincterian disturbance are less frequent. Radiologic examination shows the high frequency of spina occulta. The diagnosis is assessed when bony spur is demonstrated on films. Vertebral body diformities are not const

[Diastematomyelia--a harmless finding or cause of late neurological disturbance? (author's transl)].

Diastematomyelia is said to produce neurological lesions. Several new cases of diastematomyelia are reported without any neurological defect. They are discussed and compared with similar cases from the literature. Apparently the fixation of the spinal cord by diastematomyelia seems not to be the primary cause of the "tethering cord syndrome". Other conditions which are sometimes associated with diastematomyelia (myelodysplasia, dysraphia of the spinal cord etc.) may be the primary cause of progressive neurological lesions.

Uncommon syndromes of cerebellar vermis aplasia. II: Tecto-cerebellar dysraphia with occipital encephalocele.

Two new cases are reported of a rare syndrome of malformation consisting of occiptal encephalocele, aplasia of the vermis and deformity of the tectum. Less consistent components are aplasia of mammillary bodies, fusion of thalami, anomalies of cerebral gyral patterns, bifid atlas or bifid occipital squame, elevation of torcular, and cervical hydromyelia. One of the present cases had survived to the age of eight years with a ventricular shunt. This syndrome, of which five cases are known, is thought to be a tandem malformation in which either an occipital dysraphia or an encephalocele induces a

A myelographic technique for cysts in the spinal canal and spinal cord.

A collapsing cord in six patients with hydromyelia and a collapsing intraspinal arachnoid cyst in one patient are demonstrated. The authors describe the technique for two-position gas myelography and demonstrate that both intra- and extra-medullary intraspinal processes may collapse during myelography.

[Caudal regression syndrome and diastematomyelia. What is contained in Hohl's first publication of sacral dysgenesis? (author's transl)].

Hypoplasias and defective development of the caudal end of the spine are called caudal regression-syndrome. Occasionally such patients also show diastematomyelia. This combination, clinically important and of developmental interest is described on the basis of the first description and an observation of the author. The means of diagnosis are discussed.

Myelography with metrizamide in intraspinal and spinal abnormalities.

Myelography with gas or iophendylate is difficult in patients with spinal anomalies. Metrizamide (Amipaque) has been used to evaluate 20 patients with Chiari malformations, achondroplasia, and conditions related to scoliosis. Lateral cervical puncture was uncomplicated in 10 patients with Chiari malformations. Definition of spinal subarachnoid space was adequate to demonstrate or exclude associated intraspinal tumors or diastematomyelia.

The diagnosis of diastematomyelia by computed tomography.

CT scanning has proven efficacy for the demonstration of normal and altered anatomy of the soft tissues of the head, providing information not available with any other modality. The introduction of total body CT has enabled us to study the entire spine and its contents. This new modality is extremely useful in diagnosing a variety of abnormalities of the spine, including diastematomyelia. A case is presented and the application of CT scanning to the spine is discussed.

Inverse cerebellum and occipital encephalocele. A dorsal fusion defect uniting the Arnold-Chiari and Dandy-Walker spectrum.

The malformation of inverse cerebellum and occipital encephalocele is situated morphologically between the Arnold-Chiari and Dandy-Walker malformations. In the three reported cases, hydrocephalus was not present, concomitant malformations of the lamina terminalis were present in two, and polymicrogyria was found in all three. The primary defect in the malformation is a complex occipital encephalocele composed of miniature hemispheres connected to the brain stem by an extension of the midbrain tectum. The cerebellar folia extend ventrally and cover the basilar artery. We propose that the enceph

A study of experimental syringomyelia by scanning electron microscopy.

Experimental hydromyelia and syringomyelia, induced by kaolin in dogs, has been investigated by scanning electron microscopy. This study demonstrated that communication between the 4th ventricle and the spinal central canal is normally occluded by a plug of acellular material lying within the central canal. This plug is broken down following the onset of hydrocephalus and communication is rapidly established between the ventricles and the central canal. The canal distends and ruptures dorsally into the spinal gray matter with the development of extensive intramedullary cavities, lined by glial

Expanding lesions of the cervical spinal canal.

Attention has recently been directed to decrease in size of the spinal canal, notably in relation to production of posterior osteophytes in spondylosis. Tumors of the cord, roots, or meninges, syringolmyelia and hydromyelia may enlarge sufficiently in size or be critically located so that expansile change of the cervical spinal canal may result.

Familial osteosclerosis with abnormalities of the nervous system and meninges.

A mother and daughter with osteosclerotic dysplasia are described. The daughter had generalized osteosclerosis, flattening of the angles of the mandibles, high-arched palate, mandibular and facial bone hypoplasia, a large sella turcica, and spacious foramen magnum, platybasia, basilar impression, widened spinal cord with enlarged intervertebral foramina, and scalloping of the posterior surfaces of the vertebral bodies. Radiographic contrast studies and operative intervention revealed multiple thoracic and lumbar meningoceles and an "empty" sella, as well as evidence of maldevelopment of the sp

The diagnosis of congenital spinal disorders in computed tomography (CT).

9 patients (8 girls, 1 boy) with dysraphic disorders of the spinal column were examined by CT. In 6 cases no diastematomyelia was established. Among the 8 girls, an intraspinal bony septum was identified in 7 instances and a splitting of the cord without identifiable septum was seen in 1 girl. In 6 patients with symptoms dysplastic changes of the cord independent of the segmental level could be identified, such as variable thickness and a heterogenous absorption pattern of the cord. Three girls with symptomless diastematomyelia showed no striking absorption pattern of the spinal cord. Asymptom

Striated muscle cells in the leptomeninges in cerebral dysplasia.

Non-neoplastic foci of skeletal muscle cells are rarely noted within human central nervous system. This report describes two children with developmental anomalies and striated muscle in the leptomeninges. Embryological displacement of notochordal or prechordal mesoderm in certain types of cranio-vertebral-cerebral dysplasia may account for the origin, the restricted sites and the infrequent observation of leptomeningeal striated muscle cells.
