In addition to a distinct malformation (pachymicrogyria, heterotaxic lamination of the cerebellar cortex, olivary dysplasia), unusual degenerative changes were found in the nervous system of 2 unrelated babies with the Zellweger syndrome. Cerebral clefts were present in 1 case. In both infants there was neuron loss and accumulation of glial nodules and globoid cells in the gray matter as well as degeneration of the white matter. There was fatty change in astrocytes and diffuse gliosis. Neurons in the column of Clarke and the lateral cuneate nucleus showed peculiar fibrillary changes. Cytoplasm

A case of craniometaphysial dysplasia with extensive degeneration of the cerebral white matter is presented. The cortex of the swollen part of the long bones was quite thin and there was striking trabecular atrophy. Both the vault and base of the skull showed marked thickening and sclerosis, leaving no interlaminal zone. It is probable that the diffuse degenerative change of the cerebral white matter with gliosis bears some resemblance to that produced by a circulatory disturbance of the great vein of Galen. Due to the narrowed foramen magnum, deformed atlase and axis, and the surrounding post

By its frequent association with malformative scoliosis and neurological symptoms diastematomyelia has an important place in the diagnostic and treatment of cyphoscoliotic neurological syndromes. In all these cases and even in cases without neurological deficits the diagnosis of disatematomyelia should be done by routine myelography. The indication for operation is often curative in cases with neurological signs and prophylactic in those without.

Kaolin-induced hydrosyringomyelia in dogs has been investigated by radioisotope ventriculography using both cerebrospinal fluid radioassay and scintigraphy. The hydromyelic central canal can be differentiated from the spinal subarachnoid space by scintigraphy, Serial studies show that hydromyelia arises rapidly to decompress the associated hydrocephalus in surviving animals. Syringomyelia, after a delayed onset, originates from the enlarged central canal. Radioisotope ventriculography may be a useful clinical aid in the diagnosis of hydrosyringomyelia.

Radioisotope ventriculography was applied clinically in myelodysplastic hydromyelia in three groups of patients: two patients with normal ventricles, two with obstructive hydrocephalus, and 16 with myelodysplasia. In the myelodysplastic group, radioassay in one patient demonstrated flow of radioistope down the hydromyelic cavity. Twenty scintigraphic studies on 16 myelodysplastic patients showed hydromyelia was present in all patients with spontaneously compensated hydrocephalus but in none of those with functional ventricular shunts. This relation between the hydromyelia and disordered ventri

Encroachment by man's overlarge forebrain is responsible for the anatomic substrate of syringomyelia, i.e. a hindbrain hernia developing in fetal life with persisting hydromyelia. Communication between the syrinx and 4th ventricle is readily disclosed at operation, but because of postmortem shrinkage, almost never at autopsy. Syringomyelia developing in 16 of 864 post-traumatic paraplegics has been classified non-communicating. However, the syrinx, its fluid, and the ultimate clinical picture are the same as in the non-traumatic, and surgical exposure has disclosed the communication. The sympt

Microsurgical exploration of 15 adults with Arnold-Chiari malformation with and without hydromyelia using 3 to 20 X magnification has led to the following conclusions. Hydromyelia, associated with Arnold-Chiari malformation, is a progressive mechanical disorder that causes spinal cord deficits by pressure distention of the cord. Arnold-Chiari malformation causes slowly or suddenly progressive bulbar dysfunction by impaction of the malformation in the foramen magnum. Decompression of both can be achieved by a suboccipital carniectomy, upper cervical laminectomy, establishing an outlet from the

A series of cats were made hydrocephalic by intracisternal injection of kaolin, and four weeks later ventriculography confirmed the presence of hydromyelia. Ten days later, laminectomy in the lower thoracic region was performed. Several days later ventriculography revealed a markedly dilated central canal beneath the entire laminectomy, and pathological examination confirmed the presence of a focal bulge confined to the same region. On sectioning, it was evident that the central canal was massively enlarged to fill the entire spinal cord beneath the bulging region. The conclusion from these fi

Although the number of bone dysplasias and their variable forms of expression seem to be increasing almost daily, in almost all instances orthopedic disease is detected at rather specific locations--the cervical spine, the thoracic and lumbar spine, and the knee and hips--and the orthopedic deformity is surprisingly constant.

The risk of neurologic complications following corrective surgery in congenital scoliosis is ten times greater than in idiopathic scoliosis, the main reasons being transfixation of the spinal cord by diastematomyelia or by other adhesions of the spinal cord. Therefore preoperative myelography is advisable in congenital scoliosis. The typical findings from 3 cases of diastematomyelia, as shown by myelography and by operation, are presented. The clinical and x-ray features are shown as well as neurological signs, which are easily overlooked.

Six cases of possible diastematomyelia studied by air myelography with tomography were thought to involve fibrous or bony septa, but in each case no septum was found. Surgical findings included prominent midline posterior fissures in the cord associated with fibrous bands, an unusual tentlike extension of the arachnoid over the spinal cord, and hemicords with no intervening septum. Recognition of an osseous septum on the plain film does not necessarily mean that the cord is completely divided by bone, as other structures derived from the mesoderm, such as fat or cartilage, may contribute to th

Three cases of spina bifida (SB) associated with diastematomyelia (DM) are described and possible differences in pathogenesis are discussed. The frequent simultaneous occurrence of both malformations and the possibility that the cord can be split above, at the level of or below the SB indicate a common pathogenesis for SB and DM. Splitting of the cord symmetrically, asymmetrically, bilateraly or even antero-posteriorly can be explained only by the existence of a destructive phenomenon which is responsible for the bifurcation of the neural tube or its anlage. The existence of such a phenomenon

The diagnosis of diastematomyelia was confirmed in 2 patients by computed tomography (CT). Abnormalities of the vertebral bodies, pedicles, lamina, and spinous processes were defined and the diastematomyelic spurs were visualized.

Five cases of myelodysplasia with progressive paraparesis are presented. Three of the five patients developed spasticity, but dissociated sensory loss and loss of sphincter control was not a prominent feature. All were found to have compensated hydrocephalus and extensive communicating hydromyelia. The use of myelography and ventriculography in the diagnosis of hydromyelia is discussed. Ventricular drainage led to clinical improvement in two cases and radiological improvement in one. The relationship of compensated hydrocephalus, meningomyelocele, and progressive hydromyelia postnatally may su

Mouse hepatitis virus (MHV3) can persist for months in strains of mice with genetically controlled "semisusceptibility" to this virus. The pathology of the chronic neurological disease induced in these animals has been investigated by conventional histology and immunofluorescence. A2G mice develop a chronic choroidoependymitis and meningitis leading to severe hydrocephalus and hydromyelia. In C3H mice a widespread vasculitis was observed, with both viral antigens and bound immunoglobulins in vessal walls. No significant glomerulonephritis was found. Systemic amyloidosis was present in the sple
