BACKGROUND: Split cord malformation (SCM) is an uncommon congenital anomaly in which the spinal cord is longitudinally divided into two hemicords separated by a fibrous, cartilaginous, or osseous septum. Historically described as diastematomyelia or diplomyelia, the unified classification proposed by Pang et al. [Neurosurgery. 1992;31(3):451-80] distinguishes two main subtypes: type I, characterized by two hemicords within separate dural sacs divided by a rigid spur, and type II, in which hemicords share a single dural sac separated by a fibrous band. Composite SCM, involving multiple nonconti

BACKGROUND: Tethered cord syndrome (TCS) is most commonly associated with congenital spinal dysraphism, thickened filum terminale, lipomas, or split cord malformations (SCM ). Bony causes of tethering are typically described in association with diastematomyelia. Isolated intradural bony spurs without cord splitting or extradural communication are exceptionally rare.
CASE DESCRIPTION: We report a young adult female presenting with long-standing progressive lower limb sensory disturbances, gait tightness, and urinary dysfunction. A delayed neurological diagnosis resulted in multiple orthopedic

Prenatal diagnosis of coarctation of the aorta (CoA) remains a challenging diagnosis in fetal cardiology. In rare situations, extrinsic compression of the aorta can produce a pseudocoarctation-like appearance, characterised by elongation and kinking of the aorta. This appearance is rarely recognised prenatally and is not well documented. We report a fetus at 24+4 weeks with ventricular disproportion and arch kinking, initially raising suspicion for CoA. Echocardiography demonstrated preserved haemodynamics and no discrete narrowing, but findings were consistent with extrinsic compression of th

Spinal dysraphisms are a heterogeneous group of congenital spinal cord anomalies resulting from defective neurulation. They include both open and closed forms, with spina bifida (SB) being the most clinically recognized. These conditions frequently coexist with other anomalies, such as syringomyelia and diastematomyelia, significantly complicating clinical presentation and management. We report the case of a male neonate born at term with a lumbar spinal defect consistent with SB occulta. Initial evaluation revealed a closed, skin-colored lesion with preserved neurological function. Early spin

Primary leptomeningeal melanomatosis is a rare and aggressive variant of primary central nervous system melanoma. Diagnosis is often challenging due to nonspecific radiologic features. We hereby report the 18F-FDG PET/CT and MRI findings in a case of primary leptomeningeal melanomatosis in a 47-year-old woman with a previously unidentified diastematomyelia. Spinal MRI showed diffuse abnormal signal. 18F-FDG PET/CT showed hypermetabolism most notably in the lumbar and caudal portion of the spinal cord, which was helpful to identify an appropriate site for biopsy.

Hemimyelomeningocele (HMM) is a rare split cord malformation where only one hemicord forms a myelomeningocele-like sac, and the opposite hemicord undergoes normal neurulation. We aimed to compile all published HMM cases to concisely summarize embryology, presentation, imaging, associated anomalies, management, and outcomes of the disorder. Following PRISMA 2020, we searched PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar (1968-Feb 2025) for studies with confirmed human HMM, extracting clinical, radiological, surgical, and follow-up data. Of 688 records screened, 25 articles met incl

BACKGROUND: There is a paucity of literature to guide the surgical management of the simultaneous presentation of a diastematomyelia, two hemicords, and a unilateral bar unsegmented vertebra, a form of formation failure.
OBSERVATIONS: An 11-year-old female presented with acute-onset back pain, urinary incontinence, and lower extremity numbness. Evaluation revealed T7 diastematomyelia with a tethered cord and a right-sided T6-11 nonsegmented unilateral bar, resulting in thoracolumbar levoscoliosis (maximal Cobb angle of 49°). Initial surgical decompression and spinal cord detethering were perf

BACKGROUND: Pierre-Robin syndrome (PRS) is a disorder characterized by mandibular hypoplasia, leading to upper airway obstruction and feeding difficulties due to backward displacement of the tongue (glossoptosis). Cervical diastematomyelia is another rare congenital condition in which the spinal cord splits into two hemicords at the level of the cervical spine.
CASE DESCRIPTION: We report a unique case of a 6-month-old boy who has cervical diastematomyelia and PRS. At birth, our patient had mandibular hypoplasia, respiratory distress, a posterior parietal region swelling, and reduced tone in

BACKGROUND: The unified theory of split spinal cord malformation (SCM) comprises type I, with each one of the hemicords in one own dural tube, separated by a bone spur, and type II, with two hemicords in one dural tube, separated by a nonrigid septum. This classification is still evolving, with denominations SCM type 0.5 proposed for incomplete forms, SCM type 1.5 for lesions combining features of SCM types I and II, and SCM type III for the composite lesions occurring on different spinal levels.
OBSERVATIONS: A newborn male patient presented with a complex SCM with two placodes, one in each

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges to successful patient outcomes. SCMs are rare and can progress to cause significant neurological deficits including pain, weakness, sensory disturbance, bowel and/or bladder dysfunction. In these circumstances, surgical management of TSC with SCM is indicated. Reoperation for recurrent TCS, particularly in the context of retained or regrown bony septa and prior spinal instrumentation, is technically demanding and carries elevated risk.

A branchial cleft cyst (BCC) is a neck mass, present from birth, caused by incomplete development of the branchial arches during fetal development. It commonly presents as a solitary, painless mass in the neck of a child or young adult. We present a case of a large intramedullary BCC in a patient with Klippel-Feil syndrome (KFS). A male in his 60s presented with long-lasting neck pain and fast-progressing paraparesis. MRI showed a large intramedullary cystic tumour in the cervicothoracic spine with accompanying cervical diastematomyelia and extensive cervical vertebrae fusion. The patient was

INTRODUCTION: Adults with spina bifida have impaired mobility and often rely on assistive devices like crutches for walking, which exert high torque demands on the upper extremities. Blood flow restriction training (BFRT) may improve strength and gait measures by increasing intensity of training when applied during walking for individuals with limited walking tolerance. However, no studies have investigated the application of BFRT to the upper extremities for individuals who rely on crutches for walking.

BACKGROUND: Tethered cord syndrome (TCS) can present with neurologic, urologic, and/or orthopaedic symptoms, but little research has focused on the orthopaedic conditions that result in tethered cord release (TCR). This study aims to categorize orthopaedic findings associated with TCS and identify which conditions require further surgical intervention post TCR.
METHODS: This retrospective cohort study involved 247 patients from our tertiary referral center, all enrolled in the National Spina Bifida Patient Registry (NSBPR) and who underwent TCR between 2007 and 2017. Patients were grouped by

Diastematomyelia, a rare congenital anomaly of the spine, is characterised by the division of the spinal cord and is often associated with other spinal and cutaneous malformations. Diagnosis is primarily conducted via prenatal ultrasound and fetal Magnetic Resonance Imaging (MRI). The postnatal evaluation is carried out using Computed Tomography (CT) and MRI, which are essential for comprehensive assessment and therapeutic planning. Prognosis depends on the presence of neurological symptoms, whose severity may correlate with specific radiological findings. Treatment ranges from surgery in symp

BACKGROUND: The human tail is just the distal remnant coccygeal appendage of the embryonic tail. It may be formed by muscle or fatty tissue and by cartilage or bone covered with skin. Many cases may be associated with spinal dysraphism. The case presented here is of a child with a human pseudotail in the region of the lumbar spine associated with diastematomyelia.
CASE DESCRIPTION: A 4-year-old girl presented with pain and bulging in the lumbar spine. On neurologic examination, there were no motor or sensory deficits, the urinary and anal sphincters were normal. Imaging examinations (computed

While horseshoe kidneys are the most common renal fusion anomaly, various complex variant anatomies exist. One rare form is the retroaortic retrocaval reverse horseshoe kidney, where the upper poles fuse posterior to both the aorta and inferior vena cava. These intricate renal anomalies increase complication risk and complicate surgical interventions. We report a 3-year-old boy diagnosed with a retroaortic retrocaval reverse horseshoe kidney during a spinal deformity evaluation. This case is one of few instances of this specific variant documented in the literature and is the first to be assoc

Congenital neural tube defects rarely manifest in adulthood, and malignant transformation of meningocele is exceptionally rare. This report describes a rare case of a 56-year-old male with a congenital lumbosacral meningocele that underwent malignant transformation to epithelioid angiosarcoma, coexisting with multiple spinal anomalies, including tethered cord and diastematomyelia. Following admission and diagnostic workup, the patient underwent surgical intervention. Tumor recurrence occurred 3 months after surgery, and received combined radiotherapy and targeted therapy. The aim of this paper is to present the currently used classification of split cord malformation. Split cord malformation (SCM) is a developmental defect arising during neurulation, resulting in abnormal neural tube development, with the formation of a division within the spinal cord and dural sac. The terms diastematomyelia and diplomyelia are used in the literature to describe this defect. In 1992, Pang proposed the term SCM to describe all dysraphic spinal cord defects and classified them into type I and type II, depending on the nature of the sagittal septum within the spinal canal and the
