PURPOSE: To evaluate the spectrum of developmental anomalies observed in patients with the caudal regression syndrome and relate them to the pathogenesis of this syndrome.
METHODS: Nineteen children with caudal regression were investigated with MR.
RESULTS: The level of vertebral agenesis varied from T-11 to S-5. In 9 of the 19 children the characteristic high-ending wedge-shaped cord terminus was observed. A separation of the anterior and posterior spinal roots of the cauda equina was observed in 9 patients. Four patients had a tethered spinal cord, in 1 in combination with a wedge-shaped c

The frequency of occurrence of conjoined nerve roots (CR) in the cauda equina has been reported to range from 0.3% to 2%. We found intradural conjoined nerve roots in 13 of 123 (10.6%) patients undergoing selective dorsal rhizotomy for spasticity due to cerebral palsy. The anomaly occurred most commonly at L5-S1 and S1-S2. There was no consistent relationship to other anomalies of segmental anatomy. Our finding of a significantly higher number of conjoined nerve roots in these patients is primarily due to the surgical exposure of the entire cauda equina. This anomaly is more common than previo

Between October 1982 and August 1987, 20 patients underwent magnetic resonance imaging (MRI) and subsequent surgical release of a tethered spinal cord. The tethering was caused by a thick filum terminale in 6 patients. On MRI scans, the conus medullaris was at L4 in 2 patients, at L2 in 3 patients, and the filum terminale appeared thick in 1 patient. The spinal cord was tethered to an intradural lipoma correctly demonstrated by MRI in 6 patients. Increased epidural fat was misdiagnosed as an intradural lipoma in one patient and a lipomatous stalk was not identified in 2 other patients. Scar ti
