[Scoliotic malformations with or without associated occult spinal dysraphia. A study in 82 children. Current radiological techniques, indications and findings].

The possible association of spinal dysraphia with scoliotic malformations was studied in 82 children. Scoliotic malformations are not always the result of dysraphic malformations (diastematomyelia, hydromyelia, intrathecal lipoma, medullary cone ectopia). Recognition of dysraphic scoliotic malformations from among other types of scoliotic malformations can be accomplished before myelography by detecting vertebral malformations on standard films and, if necessary, by computed tomography. Segmental vertebral malformations, affecting vertebral bodies only, do not appear to arise from malformation

Functional improvement after spinal surgery for the dysraphic malformations.

Spinal surgery for the various dysraphic malformations is generally indicated to arrest disease progression. The results of the present study suggest that certain clinical conditions and presentations are associated with an exceptionally good surgical prognosis. Sixty-nine procedures were done in 55 patients. The diagnoses included hydromyelia and tethered cord related to diastematomyelia, lipoma, lipomeningocele, previous operation, and thickened filum terminale. The operative results were evaluated for significant functional neurological and urological improvement, as well as for the relief

The syringo-hydromyelia and its surgical treatment.

In this communication, we present a homogeneous series of 12 cases out of 25 patients having undergone a surgical operation on the posterior fossa for troubles of the cranio-rachidian junction, between 1967 and 1973. Each of these 12 cases presented an Arnold Chiari's malformation and either a "syringo-hydromyelia" (6 times), or "bulbie" (6 times) communicating with the IVth cerebral ventricle. Two cases with hydrocephaly and HIC were drained before the operation on the posterior fossa. Two cases had a ventriculo-atrial drainage 4 and 11 years after the operation for a hydrocephaly revealed by

Paralysis in lambs caused by overdosing with parbendazole.

An experiment was undertaken to determine whether an overdose of the anthelmintic parbendazole could cause paralysis in lambs when given to ewes during the early stages of pregnancy. Out of a total of 68 lambs, born from ewes treated at various stages of gestation with parbendazole at 180 mg/kg, 5 showed the paralysis syndrome, 5 showed skeletal deformities, 2 were ataxic, while 1 foetus was aborted. The ewes giving birth to paralysed lambs had been treated with parbendazole at 30, 32, 37 and 53 days of gestation. Cerebral hypoplasia was observed in 2 of these lambs, while 2 others showed inte

Hydromyelia: clinical presentation and comparison of modalities of treatment.

The clinical and radiographic features of 60 cases of hydromyelia area discussed. A combination of motor and sensory symptoms and signs is the usual presentation, but pure motor or sensory forms of the disease are not infrequent. Pain and scoliosis are usually associated with a high degree of blockage. A normal spinal fluid protein content in the presence of an enlarged spinal cord is of diagnostic value. Important radiographic clues include widening of the spinal cord without venous stagnation and collapse of the spinal cord visualized with the patient in the upright position. Metrizamide com

Hypothesis; overdistention of the neural tube may cause anomalies of non-neural organs.

This hypothesis is offered by a neurological surgeon interested in anomalies of the central nervous system. It is based on accumulating evidence indicating that some neural tube defects result not from failure of the tube to close but from its rupture after closure. The central nervous system, serving all organs, is the first to develop and its maldevelopment may cause damage to other emerging structures. The neural tube closes during the fourth week and is immediately distended by a proteinaceous neural tube fluid (NTF) secreted by its lining cells at a pressure four to five times that of the

MALFORMATIONS OF THE CENTRAL NERVOUS SYSTEM INDUCED BY NEUROTROPIC DRUGS IN MOUSE EMBRYOS.

Out of a sample of fifteen neurotropic drugs consisting of seven antidepressants and anti-psychotics, two antianxiety drugs, one anticonvulsant, three opiates and two synthetic analgesics, twelve were found to be teratogenic for mouse embryos, causing malformations of the central nervous system. After single injections of the teratogenic dose administered at the very beginning of the ninth day of gestation, four days later, i.e. in 13-day-old embryos, the induced defects appeared to make up a recurring syndrome of malformations which consists of several abnormalities present in various frequen

Diffuse cord enlargement in radiation myelopathy.

Radiation myelopathy is a well-documented complication of the irradiation of neoplasms adjacent to the spinal cord. Myelography has been used to distinguish intraspinal metastases with the diagnosis of radiation myelopathy being made if the study is normal. Four cases of radiation myelopathy are here described which were associated with swelling of the spinal cord and, in three cases, this was demonstrated at myelography. The fusiform expansion of the cord, which is indistinguishable from an intrinsic tumour or hydromyelia, is maximal in the irradiated zone, but may extend beyond it. The demon

Terminal ventriculostomy in syringomyelia.

The authors report their experiences with two patients suffering from syringomyelia on whom terminal ventriculostomies were carried out. The good results which can be obtained if the patient is in a relatively good neurological condition are pointed out. The principle of terminal ventriculostomy is based on the assumption that syringomyelia is hydromyelia that has become symptomatic; with this in mind, it seems reasonable to relieve the patient's symptoms by draining the distended central canal at the lowest possible level. The father of the operation is Dr. Gardner, who has thoroughly studied

Campomelic syndrome--possible role of intrauterine viral infection.

Two infants whose malformations fulfilled the clinical, radiographic and pathologic criteria of campomelic syndrome, were additionally found to have features suggestive of intrauterine viral infection. The presence of hydrocephalus and hydromyelia in both neonates and focal proliferative villitis in the placenta of one suggests that intrauterine viral infection may play a role in the pathogenesis of campomelic syndrome.

Klippel-Feil syndrome, iniencephalus, anencephalus, hindbrain hernia and mirror movements: overdistention of the neural tube.

Bony anomalies encountered in the 'no neck' form of Klippel-Feil syndrome (KFS) are a wide, short, fused, bifid, retroflexed spinal canal; craniolacunia, cranium bifidum, and acrania. The only symptom may be mirror movement (MM). The CNS anomalies are hindbrain hernia, hydrocephalus, hydromyelia, syringomyelia, meningocele, myelocele, encephalocele, and anencephalus. In severe KFS, i.e. iniencephalus (IN) and in anencephalus (AN), the inion is in contact with the back. In both there is hindbrain hernia and the left thorax may contain the stomach tethered to an anterior spina bifida. KFS result

Scoliosis and hydrocephalus in myelocele patients. The effects of ventricular shunting.

Developmental scoliosis is a common cause of increasing disability and deformity in long-term myelocele survivors, and is believed to result from a paralytic collapsing spine. The possible etiological role of compensated hydrocephalus and hydromyelia was assessed by determining the effect of ventricular shunting on 11 myelocele patients with developmental scoliosis. After successful shunting, one patient with a 47 degrees curve continued to deteriorate. Three cases with curves greater than 60 degrees were stabilized for short periods, but eventually required spinal fusion. Seven cases with cur

Uncommon syndromes of cerebellar vermis aplasia. II: Tecto-cerebellar dysraphia with occipital encephalocele.

Two new cases are reported of a rare syndrome of malformation consisting of occiptal encephalocele, aplasia of the vermis and deformity of the tectum. Less consistent components are aplasia of mammillary bodies, fusion of thalami, anomalies of cerebral gyral patterns, bifid atlas or bifid occipital squame, elevation of torcular, and cervical hydromyelia. One of the present cases had survived to the age of eight years with a ventricular shunt. This syndrome, of which five cases are known, is thought to be a tandem malformation in which either an occipital dysraphia or an encephalocele induces a

A myelographic technique for cysts in the spinal canal and spinal cord.

A collapsing cord in six patients with hydromyelia and a collapsing intraspinal arachnoid cyst in one patient are demonstrated. The authors describe the technique for two-position gas myelography and demonstrate that both intra- and extra-medullary intraspinal processes may collapse during myelography.

Inverse cerebellum and occipital encephalocele. A dorsal fusion defect uniting the Arnold-Chiari and Dandy-Walker spectrum.

The malformation of inverse cerebellum and occipital encephalocele is situated morphologically between the Arnold-Chiari and Dandy-Walker malformations. In the three reported cases, hydrocephalus was not present, concomitant malformations of the lamina terminalis were present in two, and polymicrogyria was found in all three. The primary defect in the malformation is a complex occipital encephalocele composed of miniature hemispheres connected to the brain stem by an extension of the midbrain tectum. The cerebellar folia extend ventrally and cover the basilar artery. We propose that the enceph

A study of experimental syringomyelia by scanning electron microscopy.

Experimental hydromyelia and syringomyelia, induced by kaolin in dogs, has been investigated by scanning electron microscopy. This study demonstrated that communication between the 4th ventricle and the spinal central canal is normally occluded by a plug of acellular material lying within the central canal. This plug is broken down following the onset of hydrocephalus and communication is rapidly established between the ventricles and the central canal. The canal distends and ruptures dorsally into the spinal gray matter with the development of extensive intramedullary cavities, lined by glial
