Context-dependent effects of adaptive immunity on IFN-α-mediated neurotoxicity in Aicardi-Goutières syndrome.

Aicardi-Goutières syndrome (AGS) is a severe neuroinflammatory disorder characterized by chronic overexpression of interferon-alpha (IFN-α) in the central nervous system (CNS), leading to progressive encephalopathy, developmental regression, and high mortality. Recent work has identified the brain microvasculature, particularly the blood-brain barrier (BBB) endothelial cells, as a central mediator of IFN-α-induced neurotoxicity. This microangiopathy is accompanied by the infiltration of B and T lymphocytes into the brain parenchyma, but their contribution to disease progression remains unclear

No Evidence of Impact of Genetic Sex on Hyperthermia-Induced Neural Tube Defects in Chicken Embryos.

BACKGROUND: In mammals, cranial neural tube defects (NTDs) are more likely to occur in females. It has been proposed that the sex bias in NTDs was due to the presence of the inactive X chromosome in the somatic cells of female embryos and that the inactive X acted as a heterochromatic sink, competing for silencing factors with the rest of the genome. Such competition increased the likelihood of abnormal gene regulation. To test this hypothesis, we used an animal model that lacks dosage compensation, the chicken Gallus gallus. If the heterochromatic sink hypothesis was correct, no sex bias will

Microstructure imaging in patients undergoing evaluation for epilepsy surgery or low-grade glioma: Clinical utility of a novel diffusion MRI method.

OBJECTIVE: To investigate a recently developed MRI technique for mapping the Mean Diffusivity of Tissue (MDT), which improves diffusion MRI imaging of the cerebral cortex by reducing partial volume effects from cerebrospinal fluid (CSF). As cortical lesions are common in patients with focal epilepsy, we explored the clinical value of MDT imaging in patients with drug resistant epilepsy and low-grade glioma. METHOD: This is a prospective study in which 18 patients undergoing evaluation for epilepsy surgery and five patients undergoing evaluation for low-grade glioma were recruited. Patients un

Seizure outcomes after VNS therapy in children with drug-resistant epilepsy due to monogenic etiologies versus malformations of cortical development.

OBJECTIVE: Vagus nerve stimulation (VNS) is an established adjunctive therapy for drug-resistant epilepsy (DRE). However, evidence regarding its efficacy in children with structural and nonstructural etiologies of epilepsy remains limited. Herein, the authors aimed to explore the effectiveness of VNS in patients with either etiology. METHODS: In this retrospective single-center study, authors evaluated children (ages 2-18 years) with DRE due to nonlesional monogenic epilepsies (MEs) or malformations of cortical development (MCDs) who had undergone VNS device implantation between 2008 and 2024

Surgical management of a lenticulostriate artery aneurysm and an associated initially less-appreciated arteriovenous malformation: two-dimensional video.

Surgical management of the basal ganglia vascular lesions needs a personalized surgical strategy due to intricate neurovascular anatomy and eloquent surrounding structures. This report describes the successful resection of a perforator aneurysm and an initially less-appreciated arteriovenous malformation (AVM) at the basal ganglia in a 15-year-old male. Preoperative digital subtraction angiography (DSA) and high-resolution magnetic resonance imaging (MRI) showed a deep-seated basal ganglia aneurysm on a lenticulostriate artery (LSA) perforator with a distal suspicious early drainage vein adjac

SLC38A3 deficiency reveals a critical role of blood-derived glutamine in brain development.

Biallelic mutations in SLC38A3 lead to postnatal progressive microcephaly, epilepsy and intellectual disability. However, the underlying pathophysiology remains unknown. Here, we identified Slc38a3 expressed at the vascular endothelium as a critical glutamine transporter that mediates blood-to-brain influx of glutamine through the blood-brain barrier. Endothelial selective deletion of Slc38a3 (Slc38a3-cKO) lowered the influx of glutamine across the blood-brain barrier and decreased brain glutamine levels in mouse pups. This was associated with lower transfer of glutamine carbons to glutamate a

Intraoperative ultrasonography in the surgical management of Chiari I malformation: A systematic review and meta-analysis of outcomes and applications.

BACKGROUND: Chiari malformation type I (CM-I) is a neurological disorder characterized by cerebellar tonsillar herniation and is often associated with syringomyelia. Posterior fossa decompression (PFD) is the primary treatment, although the optimal extent of decompression remains a topic of debate. Intraoperative ultrasonography (IOUS) offers real-time visualization to support informed surgical decision-making. This study aimed to assess the effectiveness and safety of IOUS-guided surgery in CM-I. METHODS: This systematic review followed PRISMA guidelines and searched the literature up to July

The Effect of Phosphoric Acid on the Development of Neural Tube Defects in Chick Embryos.

BACKGROUND Neural tube defects (NTDs) are among the most common congenital malformations and arise from disruption of early neurulation. Phosphoric acid is a widely used food additive; however, its potential effects on early neural tube development have not previously been evaluated in experimental neurulation models. This proof-of-concept study aimed to investigate the embryotoxic and teratogenic effects of phosphoric acid on neural tube development in a chick-embryo model of neurulation, at a single tested concentration. MATERIAL AND METHODS Fertilized pathogen-free chicken eggs (n=30) were

Epilepsy-associated FOXJ3 variants link a transcriptional program of the PTEN-mTOR pathway to neuronal specification and cortical lamination.

Focal cortical dysplasia (FCD), a major cause of drug-resistant epilepsy, involves abnormal neuronal migration and cortical architecture, yet its molecular basis remains poorly defined. Here, we identify FOXJ3 pathogenic variants in patients with autosomal dominant focal epilepsy and FCD. In the developing mouse cortex, FOXJ3 declines sharply in neural progenitors after embryonic day 15.5. In utero electroporation-mediated Foxj3 knockdown in mouse brains impairs neuronal migration, disrupts cortical lamination, and alters neuronal specification, promoting upper-layer neuron production at the e

Transient Dysphagia as a Presenting Symptom of Familial Cerebral Cavernous Malformation.

Cerebral cavernous malformations (CCMs) are vascular lesions characterized by a collection of thin-walled capillaries with slow blood flow, which are often identified incidentally on MRI. CCMs are the most common cerebral vascular malformation after developmental venous anomalies. Familial CCM (FCCM) is a rare autosomal dominant disorder characterized by several lesions throughout the central nervous system. We report the case of a 47-year-old female patient who presented to the neurology clinic with a chief complaint of transient dysphagia. An MRI of the brain without contrast, including susc

Sleep magnetoencephalography enhances detection and source imaging of seizures and fast oscillations in focal cortical dysplasia.

OBJECTIVE: Focal cortical dysplasia (FCD) causes drug-resistant epilepsy requiring presurgical evaluation. Invasive electroencephalographic (EEG) studies demonstrate that sleep modulates epileptic activity, including interictal epileptiform discharges (IEDs), fast oscillations (FOs) in the beta (14-40 Hz) and gamma (40-80 Hz) frequency bands, and seizures. This study aimed to quantify sleep-associated changes in IEDs, FOs, and seizures in FCD patients using noninvasive magnetoencephalography (MEG). METHODS: Nineteen patients with FCD were prospectively recruited and underwent simultaneous MEG

Network Disconnection Syndrome in Unruptured Brain Arteriovenous Malformations: A Multimodal Connectome Study.

BACKGROUND: Cognitive impairment is a critical yet poorly understood complication of brain arteriovenous malformations (bAVM). While traditionally attributed to hemodynamic "steal" or focal tissue destruction, it remains unclear whether cognitive variability correlates with gross structural pathology or broader network disruption. METHODS: We recruited consecutive patients with unruptured bAVM at Huashan Hospital and matched healthy controls (2017-2020). We integrated lesion frequency mapping, surface-based morphometry, and Network Diffusion Modeling (NDM) to dissect the structure-function re

Rare Ectopic Presentation of an Intrasphenoidal Rathke's Cleft Cyst - Case Report and Literature Review.

Rathke's cleft cysts (RCCs) typically present as intrasellar lesions, with exclusive sphenoid sinus involvement being exceptionally rare. We report the seventh documented case of an intrasphenoidal RCC in a 39-year-old male presenting with blurred vision and headaches. Complete resection was achieved via an endoscopic endonasal approach, and the patient remained symptom-free on follow-up. To our knowledge, this is the first report to provide a series of endoscopic images illustrating the appearance and resection technique. We review the literature on this rare entity and summarize current inve

Outcomes in children receiving single-fraction Gamma Knife radiosurgery for arteriovenous malformation.

INTRODUCTION: Stereotactic radiosurgery (SRS) is used for arteriovenous malformations (AVM), but the risk for secondary malignancy (SM), radiation necrosis (RN), obliteration rates (ObR), and late neurologic toxicities in pediatric patients is not well defined. To address this, we reviewed our two-decade-long institutional experience treating pediatric AVMs with SRS. METHODS: Retrospective review of patients receiving single-fraction SRS for AVM between 1/2006 and 12/2023. ObR and RN were determined on neuroimaging, and toxicities by clinical evaluation. Overall follow-up was 56 months (range

Longitudinal analysis of lipid changes in the sciatic nerve caused by overexpression of PMP22 in murine models of CMT1A.

Charcot-Marie-Tooth type 1A (CMT1A), a prevalent progressive demyelinating peripheral neuropathy is caused by a duplication of the peripheral myelin protein (PMP22) gene. PMP22 is crucial for formation of compact myelin, but the mechanism by which PMP22 overexpression results in CMT1A pathogenesis remains elusive. Emerging evidence points to the role of PMP22 in lipid metabolism as a key modulator of disease progression. Here we show that C3 and C22 mouse models, carrying 5 and 10 additional copies of the human PMP22 gene, have PMP22 dose-dependent lipidomic and transcriptomic alterations. Bot

Bidirectional associations between functional gastrointestinal disorders and epilepsy and the mediating role of mental health: A population-based study.

BACKGROUND: This study aimed to investigate the epidemiological and causal associations between functional gastrointestinal disorders (FGIDs) and epilepsy, with a focus on the mediating role of mental health. METHODS: Based on the UK Biobank, Cox proportional hazard models were used to estimate the risk of incident epilepsy among participants with FGIDs and the risk of FGIDs among those with epilepsy. Mediation analysis quantified the contribution of mental health to these associations. The bidirectional causal relationships between these disorders were evaluated through Mendelian randomizati

Hidden Triplets: Coexisting Spinal Epidural and Dural Arteriovenous Fistulae - A Rare Case Report and Literature Review.

A Spinal epidural and dural Arteriovenous Fistula is a rare vascular malformation that rarely presents as multiple synchronous lesions. We report a rare case of three synchronous lumbar spinal arteriovenous shunts in a 22-year-old male presenting with subarachnoid hemorrhage (SAH) symptoms. Angiographic evaluation revealed a complex vascular pathology consisting of one anterior epidural arteriovenous fistula (AVF) fed by the L3 radiculomedullary branch and two smaller dural AVFs at L4-L5 levels. Initial cranial imaging was unremarkable, but spinal MRI demonstrated intrathecal hemorrhage with p
