Angiogenic switching in cerebral cavernous malformations driven by MAP3K3-PIK3CA synergy.

Cerebral cavernous malformations are common vascular anomalies in the CNS that predispose individuals to seizures and haemorrhagic stroke. Familial forms are linked to germline loss-of-function mutations in CCM1-3, and sporadic lesions frequently harbour somatic gain-of-function mutations in MAP3K3 and PIK3CA. However, the mechanisms by which these somatic mutations drive lesion development remain incompletely understood, and no medical therapies are currently available. Here, we investigated the cooperative effects of MAP3K3I441M and PIK3CAH1047R mutations using transgenic neonatal and adult

Resolution of generalized tonic seizures following focal ablative or resective surgery.

OBJECTIVE: Focal brain lesions may underlie generalized tonic seizures, as seen in Lennox-Gastaut syndrome, by engaging bilateral neural networks. However, this seizure type is often not considered surgically remediable. Here, we describe the resolution of apparent electroclinically classic generalized tonic seizures in children originating from a unifocal brain lesion following resective or ablative surgery. This study aims to contribute to emerging evidence that prompt removal of a lesion may resolve generalized seizures by ameliorating aberrant network activity. METHODS: Boston Children's

Outcomes of Predrug Resistant Versus Postdrug Resistant Surgery in Children With Focal Cortical Dysplasia-Related Epilepsy: Drug Resistance Should Not Be the Threshold for Surgical Candidacy.

BACKGROUND: Surgical intervention has become an established treatment option for epilepsy, but its traditional indications are limited to drug resistant cases. This study compares outcomes of early surgery (predrug resistance) with traditional surgery (postdrug resistance) in focal cortical dysplasia (FCD)-related epilepsy, providing clinical evidence for early surgical intervention. METHODS: Medical records of FCD-related epilepsy children who underwent 1.5 T or 3T brain magnetic resonance imaging at our center (Jan 2008-Dec 2022) were reviewed. Children were divided into early surgery and t

Charcot-Marie-Tooth disease and related neuropathies.

Charcot-Marie-Tooth disease (CMT) subsumes many different inherited neuropathies. CMT and related neuropathies are among the most common inherited neurological disorders, affecting ~1 in 2,500 people globally and including both sexes. Mutations in genes that cause demyelinating forms of CMT often affect the proteins of the myelin sheath, the unfolded protein response, endosomal signalling and recycling, or key transcription factors. Mutations in genes that cause axonal forms often affect mitochondrial biology, aminoacyl-tRNA synthetases, molecular chaperones or the axonal cytoskeleton. All for

Folic acid supplementation and neural tube defects: a cross-sectional study of knowledge and practice among pregnant mothers in Lira, Northern Uganda.

BACKGROUND AND AIM: Folic acid requirements increase during pregnancy to meet the metabolic needs, DNA synthesis, and rapid cell division during fetal development. Low levels of folate before and during pregnancy augment the chances of poor pregnancy outcomes, like neural tube defects. Awareness of the importance of folic acid in the prevention of neural tube defects is low among women of childbearing age. This study intended to explore the level of knowledge and practices of pregnant mothers regarding folic acid supplementation in preventing neural tube defects in Lira, northern Uganda. STUDY

Prognostic factors for spinal cord cavernous hemangioma resection and optimal surgical timing for symptomatic patients.

OBJECTIVE: Spinal cord cavernous hemangioma (CH) is a rare intramedullary vascular lesion with a risk of hemorrhage and progressive neurological deterioration. While resection is the standard treatment for symptomatic CH, the optimal timing for surgery remains uncertain. This study aimed to examine surgical outcomes, find prognostic factors of functional independence, and suggest the optimal surgical timing in functionally dependent patients. METHODS: A retrospective analysis was conducted on the medical records of 40 patients who underwent resection for CH at a tertiary medical institution b

Transvenous approach: a promising strategy for endovascular treatment of tentorial dural arteriovenous fistulas.

OBJECTIVE: Tentorial dural arteriovenous fistulas (TDAVFs) are uncommon, high-risk intracranial vascular malformations characterized by severe clinical courses. Transarterial embolization (TAE) has been widely recognized as the primary treatment modality. Nevertheless, limited data exist regarding the safety and efficacy of transvenous embolization (TVE). The current study reports a comparative analysis of TAE and TVE in a consecutive TDAVF cohort, providing detailed insights into both clinical and angiographic outcomes. METHODS: A retrospective cohort analysis was conducted on 53 patients wi

Comprehensive segmentation of focal cortical dysplasia by combining surface-based and whole-brain MRI deep learning algorithms: a proof-of-concept study.

Introduction.Focal cortical dysplasia type II (FCD II) is a significant cause of drug-resistant epilepsy, and the full surgical resection of the lesion is linked with excellent disease-free outcomes. Its imaging hallmark is the white matter hyperintense funnel-shaped transmantle sign on T2-FLAIR magnetic resonance imaging (MRI). Manual delineation of this abnormality is challenging and inconsistent. Most current artificial intelligence (AI) segmentation tools focus on cortical features and do not fully evaluate the white matter component. We tested whether integrating an algorithm trained on w

Microglia replacement by ER-Hoxb8 conditionally immortalized macrophages provides insight into Aicardi-Goutières syndrome neuropathology.

Microglia, the brain's resident macrophages, can be reconstituted by surrogate cells - a process termed 'microglia replacement'. To expand the microglia replacement toolkit, we here introduce estrogen-regulated (ER) homeobox B8 (Hoxb8) conditionally immortalized macrophages, a cell model for generation of immune cells from murine bone marrow, as a versatile model for microglia replacement. We find that ER-Hoxb8 macrophages are highly comparable to primary bone marrow-derived macrophages in vitro, and, when transplanted into a microglia-free brain, engraft the parenchyma and differentiate into

Magnetic resonance imaging-negative type II focal cortical dysplasia as a subtle form of bottom-of-sulcus dysplasia: Electrophysiological and histopathological evidence and treatment implications.

OBJECTIVE: Magnetic resonance imaging (MRI)-negative type II focal cortical dysplasia (FCD) is challenging to localize and treat. Emerging evidence suggests that the sulcal bottom is the critical site of epileptogenicity. This study investigated whether MRI-negative type II FCD represents a subtle form of bottom-of-sulcus dysplasia (BOSD) and evaluated the safety and efficacy of sulcus bottom-targeted minimally invasive therapy. METHODS: Fifty-eight patients (mean age = 18.0 ± 10.0 years, range = 4-41) with drug-resistant epilepsy and MRI-negative scans were clinically diagnosed with FCD type

Hereditary Polyneuropathies in the Era of Precision Medicine: Genetic Complexity and Emerging Strategies.

Hereditary polyneuropathies represent a genetically and clinically heterogeneous group of disorders affecting the peripheral nervous system, characterized by progressive motor, sensory, and autonomic impairment. Advances in molecular genetics have identified key causative genes, including PMP22, MPZ, MFN2, TTR, EGR2, and CX32 (GJB1), which are implicated in Charcot-Marie-Tooth disease, Dejerine-Sottas syndrome, and related neuropathies. These conditions display substantial allelic and locus heterogeneity. Pathogenetically, mechanisms involve impaired myelin maintenance, disrupted axonal transp

Toxic Effects of Polystyrene Microplastics and Sulfamethoxazole on Early Neurodevelopment in Embryo-Larval Zebrafish (Danio rerio).

Microplastics (MPs) and antibiotics have emerged as contaminants of global concern, posing potential threats to ecosystem security and organismal health. To investigate the individual and combined toxicity of microplastics (PS-MPs) and sulfamethoxazole (SMX), we conducted a 120 h acute exposure experiment using embryo-larval zebrafish as a toxicological model. Our findings demonstrate that both PS-MPs and SMX can induce neurodevelopmental toxicity in embryo-larval zebrafish during embryonic development. Notably, PS-MPs and SMX exerted a significant synergistic effect. PS-MPs 1 µm in diameter w

Natural history of hemorrhagic events in spinal cord cavernous malformation: an updated systematic review and Meta-Analysis.

Spinal cord cavernous malformations (SCCMs) are rare lesions believed to have moderate rates of debilitating hemorrhages, often requiring complex treatment. Previous studies have reported vastly different rates of hemorrhagic outcomes over time depending on treatments. In this meta-analysis, we aim to assess the long-term natural history of hemorrhagic events and risk in patients with a SCCM. Preferred reporting items for systematic reviews and meta-analysis guidelines (PRISMA) were utilized in this work. We extracted demographic data, follow-up lengths, lesion characteristics and locations, s
