Aberrant Molecular Myelin Architecture in Charcot-Marie-Tooth Disease Type 1A and Hereditary Neuropathy With Liability to Pressure Palsies.

Charcot-Marie-Tooth Disease Type 1A (CMT1A) and Hereditary Neuropathy with Liability to Pressure Palsies (HNPP) are the most common inherited peripheral neuropathies and arise from copy number variation of the Peripheral Myelin Protein 22 (PMP22) gene. While secondary axon degeneration has been proposed as the primary driver of disability, our prior work demonstrated pronounced neuromuscular impairment in CMT1A model mice in the absence of overt axonal loss, prompting investigation into primary myelin dysfunction. Here, we reveal that altered PMP22 dosage profoundly disrupts molecular architec

Chiari Decompression in Patients with Spontaneous Intracranial Hypotension: Presenting Symptoms and Treatment Response.

Spontaneous intracranial hypotension (SIH) is a debilitating and often misdiagnosed condition. One important differential diagnosis is Chiari malformation type I, which differs markedly in its pathophysiology, clinical manifestation, and treatment. We describe 7 patients with SIH, initially misdiagnosed as having Chiari malformation type I, who underwent posterior fossa decompression. Five patients developed temporary symptom relief after posterior fossa decompression, and 6 patients developed rebound intracranial hypertension. Our findings emphasize the importance of considering SIH when eval

Aicardi-Goutières Syndrome caused by SAMHD1 mutation: Pathogenesis and Beyond.

Aicardi-Goutières Syndrome (AGS) is a rare monogenic autoinflammatory disorder primarily affecting the central nervous system. It is characterized by elevated levels of type I interferon (IFN-I) in the cerebrospinal fluid. Mutations in SAMHD1 gene cause AGS type 5. The primary function of SAMHD1 is to maintain genome stability by regulating the dNTP pool through its enzymatic activity. This review comprehensively describes the role of loss-of-function mutations in SAMHD1 in the pathogenesis of AGS. It covers the molecular structure and function of SAMHD1, as well as its relationship with type

Pseudo-healthy image synthesis via location-guided diffusion models for focal cortical dysplasia lesion localization.

Focal cortical dysplasia (FCD) is a developmental disorder frequently linked to drug-resistant focal epilepsy, where surgical intervention is often the most promising treatment. Unfortunately, conventional neuroradiological methods often struggle to detect subtle FCD cases, which may result in missed surgical opportunities for patients. To address this challenge, we have developed an innovative approach to FCD lesions detection using conditional diffusion models guided by complementary location-based modal factors. Our method involves employing a classifier to identify epileptic sites while di

Spectral entropy variability of intraoperative electrocorticography predicts outcome after epilepsy surgery in people with focal cortical dysplasia.

OBJECTIVE: Epilepsy surgery in people with focal cortical dysplasia (FCD) requires accurate removal of all epileptogenic tissue, and outcome is difficult to predict. We explored whether spectral entropy, a fast computable electroencephalographic (EEG) feature, could estimate epileptic activity in intraoperative electrocorticography (ioECoG) and forecast postsurgical outcomes. METHODS: We included people with FCD pathology who underwent ioECoG-assisted resective surgery. We analyzed ioECoG recordings acquired before and after resection. We computed spectral entropy across eight frequency bands

Diverse Genetic Etiologies of Unilateral Polymicrogyria.

OBJECTIVE: Polymicrogyria (PMG) is one of the most common human malformations of cortical development and is often classified by its radiographic pattern of distribution. Unilateral polymicrogyria (uPMG) is a subtype of PMG affecting a portion or all of one cerebral hemisphere. As most PMGs occur bilaterally, there has been no specific investigation as to whether the genetic underpinnings of uPMG comprise a subset of or a distinct entity from bilateral PMG. In this study, our goal was to assess both the genetic etiology of uPMG and the value of diagnostic genetic testing in this setting. METH

Sleep enhances gamma oscillations in the seizure onset zone and broadband activity in the irritative zone of focal cortical dysplasia.

OBJECTIVE: Focal cortical dysplasia (FCD) is a leading cause of drug-resistant epilepsy and is associated with sleep-related seizures, yet the underlying electrophysiological mechanisms during different brain states remain poorly understood. We investigated whether fast oscillations (FOs) within the seizure onset zone (SOZ) and irritative zone (IZ) change significantly during sleep compared with wake in FCD patients. We analyzed multiple frequency bands-beta (14-20 Hz), gamma (40-80 Hz), ripple (80-250 Hz), and broadband BGR (14-250 Hz) to provide comprehensive information about sleep-related

Biallelic Rare COL18A1 Variants in Patients With Neurological Phenotypes Without Severe Ophthalmologic Abnormalities.

COL18A1 encodes the α1 chain of collagen type XVIII, a nonfibrillar collagen expressed in vascular and epithelial basement membranes. Biallelic pathogenic variants in COL18A1 have been associated with Knobloch syndrome, a condition defined by ophthalmologic abnormalities, though patients often have some or all of brain malformations, epilepsy, and intellectual disability. We reviewed our research and clinical databases for patients with seizures and biallelic COL18A1 variants suspected to be pathogenic. Three patients were identified, all of whom had epilepsy and global development impairment,

A novel patient-Centered approach to clinical trial readiness in rare diseases: Application in Aicardi-Goutières Syndrome (AGS).

INTRODUCTION: Aicardi-Goutières Syndrome (AGS) is a genetic type 1 interferonopathy that causes white matter abnormalities and intracranial calcifications, resulting in varying degrees of neurologic impairment and systemic manifestations. Novel disease-modifying therapies for AGS are forthcoming. The 2022 Food and Drug Administration guidance, "Patient-Focused Drug Development" (PFDD), emphasizes the importance of including patients' voices early in the design of clinical trials. This represents an urgent unmet need in rare disease research. In this study, we propose and pilot a new methodolog

Circulating lncRNAs Remark Expression Profile of Cerebrovascular Malformation Endothelial Cells.

Among cerebrovascular malformations, brain arteriovenous malformation (bAVM) and cerebral cavernous malformation (CCM) primarily affect microvessels. Brain AVM originates from arterioles due to impaired expression of endothelial differentiation markers, whereas CCM arises due to aberrant sprouting angiogenesis. Recent advances in the field of lncRNAs have linked perturbations of the competitive endogenous RNA (ceRNA) lncRNA-miRNA-mRNA axis to the development of several pathological conditions. However, this mechanism remains poorly investigated in cerebrovascular phenotypes. Based on RNA-seq d

Stress-Driven Selective Neuronal Vulnerability in Charcot-Marie-Tooth Disease: From Prodromal Pathology to Therapeutic Implications.

Charcot-Marie-Tooth (CMT) disease represents the most prevalent inherited peripheral neuropathy with a broad range of clinical manifestations, inheritance patterns, and causative genes. The primary pathological hallmark is progressive degeneration, predominantly affecting sensory and motor neurons, leading to prominent sensory deficits and progressive motor impairments. While neuropathy-causing mutations in the ubiquitously expressed small heat shock protein HSPB1 account for a subset of axonal CMT cases, the mechanisms underlying the selective vulnerability of peripheral neurons remain poorly

Incidence, prevalence, burden, and disability rate of neural tube defects in sub-Saharan Africa, 1990-2021: understanding epidemiology.

OBJECTIVE: Neural tube defects (NTDs) represent a significant global concern, impacting an estimated 300,000 individuals each year. Sub-Saharan Africa (SSA) is considerably affected due to various biopsychosocial factors, dietary issues such as folic acid deficiency, and fumonisin-contaminated maize consumption, and sex-based disparities. This study sought to understand epidemiology and trends in NTD in SSA from 1990 to 2021. METHODS: The Global Burden of Disease database was utilized to obtain epidemiological data on the variables of interest, which included disability-adjusted life years (D

Microsurgical resection of brain arteriovenous malformations with or without preoperative embolization: a 10 year observational cohort study.

OBJECTIVES: The role of preoperative embolization as an adjunct to microsurgical resection of brain arteriovenous malformations (AVMs) remains uncertain. We evaluated clinical and angiographic outcomes of microsurgical resection performed with or without preoperative embolization and examined factors associated with selection for embolization. METHODS: We conducted a retrospective observational cohort study of consecutive patients undergoing microsurgical resection of AVMs at a tertiary centre between July 2015 and June 2025. Patients treated with microsurgery alone or with preoperative embol

Brain V12 Predicts Radiation-Induced Adverse Events and Late Complications After Stereotactic Radiosurgery for Brain Arteriovenous Malformations.

PURPOSE: Stereotactic radiosurgery (SRS) is an effective treatment for brain arteriovenous malformations (AVMs), though radiation-induced adverse events (RAEs)-including peri-nidal T2 signal changes and late radiation-induced complications (LRICs)-remain concerns. We evaluated whether brain V12 (volume of surrounding brain receiving ≥12 Gy) predicts RAEs, including chronic encapsulated hematoma and cyst formation. METHODS AND MATERIALS: We retrospectively reviewed 317 patients who underwent SRS at a single institution between 1998 and 2020. Large AVMs with a nidus volume ≥10 mL were excluded.

Optimal timing of microsurgical resection after preoperative embolization of intracranial arteriovenous malformations: A systematic review and meta-analysis of 1,469 patients.

Preoperative embolization is frequently employed to facilitate microsurgical resection of intracranial arteriovenous malformations (AVMs), but optimal surgical timing remains debated. This study evaluated whether hybrid versus staged approaches influence surgical morbidity, resection completeness, and postembolization complications to inform standardized management. A systematic search of PubMed, Embase, and OVID/Medline through February 2025 identified studies reporting outcomes after preoperative embolization followed by microsurgical AVM resection. Studies were stratified by timing: hybrid,

Preserving paleoteratological history: a study of congenital anomalies from Vilnius University's medical collection.

Vilnius University's Faculty of Medicine, with a history dating back to the 18th century, has gathered numerous anatomy and pathology specimens over time. Regrettably, insufficient preservation has led to considerable deterioration of the collection. This study aims to evaluate the quantity and variety of congenital malformation specimens from the 20th century that remain under the care of our institution. Using archival records and visual inspection, we conducted a thorough assessment of these specimens. Congenital malformations represent an important aspect of human intrauterine development,

Plasma neurofilament light chain is associated with clinical instability in chronic autoimmune neuropathies.

Neurofilament light chain (NfL) is a sensitive biomarker of axonal damage, but its clinical relevance in chronic autoimmune neuropathies remains incompletely defined. This study evaluated plasma NfL levels in patients with chronic inflammatory demyelinating polyneuropathy (CIDP) and multifocal motor neuropathy (MMN), compared with hereditary neuropathy (Charcot–Marie–Tooth disease type 1 A, CMT1A) and healthy controls, focusing on disease activity rather than diagnostic discrimination. Plasma NfL concentrations were measured using single molecule array (Simoa) technology in 41 patients (CIDP n

[Selection of Devices and Embolic Substances in Endovascular Surgery for Brain Arteriovenous Malformations].

Cerebral vascular surgery for cerebral arteriovenous malformations (cAVMs) is often challenging due to the diversity of lesions. I describe my treatment policy, embolization techniques, selection of embolic substances, and device choice. Proper use of embolization techniques is critical for safety in cAVM procedures. I use coils, NBCA, and Onyx. NBCA is employed when short-distance, reliable feeder occlusion is required. I prefer microcatheters with good contrast properties that are compatible with coils; for very thin feeders, I select microcatheter with a finer diameter. In cAVM embolization

[Devices for Transvenous Embolization of Dural Arteriovenous Fistulas].

When approaching an occluded sinus during transvenous embolization of dural arteriovenous fistulas, it is important to employ a combination of appropriate guiding and distal access catheters to ensure adequate support and effective navigation. When navigating a long and tortuous venous route, careful attention must be given to the backup support and lengths of the coaxial catheters and connectors. Effective embolization can be achieved by selecting coils according to the stage of the embolization procedure and using liquid embolic agents when indicated.
