BACKGROUND: Arachnoid cysts (ACs) are the most common brain cysts, comprising 1% of intracranial lesions, with a higher prevalence in children (2.6%) than adults (0.3%-1.7%). Intraorbital ACs are extremely rare, with only a few cases reported to date, especially in children. While most ACs do not exhibit any symptoms, some cases of symptomatic ACs require surgical intervention to prevent brain damage. This article presents a unique case of a large intraorbital AC with preserved visual acuity in a 3-year-old female with a concise literature review of all intraorbital AC cases reported to date.

Myopathy with extrapyramidal signs (OMIM #615673) is a rare autosomal recessive mitochondrial disorder caused by biallelic loss-of-function variants in Mitochondrial calcium uptake protein 1 ( MICU1 ), which encodes the gatekeeper of the mitochondrial calcium uniporter complex. We report a 7-year-old Indian girl with global developmental delay, congenital nonfatiguable right ptosis, proximal-predominant myopathy without calf hypertrophy, multi-system dysmorphism (elongated facies, baggy cheeks, large prominent ears, partial webbed neck, bilateral clinodactyly, fetal finger pads, pes planus, an

Juvenile myasthenia gravis (JMG) is a rare autoimmune disease acquired in childhood, comprising 8%-15% of all myasthenia gravis cases depending on geographic and ethnic populations. Ocular myasthenia gravis presents as ptosis with extraocular movement restriction and is frequently misdiagnosed as third nerve palsy or congenital ptosis when bilateral. Early diagnosis prevents disease progression and unnecessary interventions. We report a case of JMG initially misdiagnosed as Guillain-Barré syndrome (GBS) to highlight diagnostic pitfalls. A 4-year-old girl presented with left eye ptosis, followe

Congenital levator synkinesis represents a complex spectrum of neurodevelopmental disorders characterized by involuntary coupling of eyelid movement with activation of extraocular or masticatory muscles. This review proposes a clinical classification of levator synkinesis into excitatory, inhibitory, and combined phenomena, based on whether synkinetic input results in eyelid elevation, depression, or a combination of both. The underlying pathophysiology is most consistent with a developmental dysinnervation process, likely involving aberrant neural connectivity "miswiring" or, supranuclear rel

We report orbital asymmetries associated with congenital ptosis. This was a prospective, observational study enrolling all cases of idiopathic, nonsyndromic congenital ptosis presenting to a tertiary university center between 2023 and 2025. Exophthalmometry was performed using both Hertel and Naugle devices by 2 independent observers. Average intereye differences were calculated. Axial length was measured using optical biometry. Any other nonsyndromic eyelid or periocular findings were noted. N=26 patients were enrolled. Average age was 8.8±8.5 years (range 7 mo-26 y). Exophthalmometry consist

OBJECTIVES: To investigate nocturnal lagophthalmos after levator surgery for simple congenital ptosis, focusing on its correlation with office-based (clinical) lagophthalmos and factors contributing to clinical-nocturnal discrepancy.
METHODS: A retrospective review was conducted on 109 children (median age, 7 years) who underwent unilateral levator surgery between 2021 and 2024. Lagophthalmos was evaluated clinically in the office and nocturnally using photographs obtained during sleep. Bell's phenomenon was assessed during daytime examination by observing upward and outward globe rotation on

Frontalis muscle flap suspension (FMFS) is a well-established surgical option for severe ptosis in patients with poor levator muscle function. Although its efficacy has been widely reported in congenital ptosis, the application of FMFS in elderly patients remains insufficiently addressed. Age-related anatomic and physiological changes, including chronic frontalis muscle hyperactivity, reduced tissue elasticity, and decreased ocular surface tolerance, may increase the risk of postoperative complications in this population. This retrospective case series presents 3 elderly male patients (aged 83

Purpose: Monocular elevation deficiency (MED), formerly termed double elevator palsy, is a rare ocular motility disorder characterized by restriction of elevation in all gaze positions. Ptosis frequently accompanies MED but may be mistaken for simple congenital ptosis, potentially resulting in suboptimal surgical planning. While frontalis sling and levator resection are the most frequently employed techniques for MED-associated ptosis, these procedures carry a considerable risk of lagophthalmos and suboptimal cosmetic outcomes. To date, the use of Müller muscle - conjunctival resection (MMCR)

In recent decades, there has been a call for change among all stakeholders involved in scoliosis management. Parents of children with scoliosis have complained about the so-called "wait and see" approach that far too many doctors use when evaluating children's scoliosis curves between 10° and 25°. Observation, Physiotherapy Scoliosis Specific Exercises (PSSE) and bracing for idiopathic scoliosis during growth are all therapeutic interventions accepted by the 2011 International Society on Scoliosis Orthopaedic and Rehabilitation Treatment (SOSORT). The standard features of these interventions a

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Subcutaneous indwelling of nonabsorbable threads for tissue fixation and tension reduction is often used in plastic surgery and is applied in breast reduction surgery, umbilicoplasty, and in frontalis muscle lifting for congenital ptosis. However, in a few cases, exposure of the indwelling thread necessitates its removal. Exposure of the indwelling threads mostly occurs due to exposure of the knots. Therefore, we require a novel method to ensure that the knot of the indwelling thread remains unexposed. Hence, we created a stab incision to place the indwelling nonabsorbable threads ligated with

BACKGROUND: Nance-Horan syndrome (NHS) is a rare X-linked dominant disorder caused by pathogenic variants in the NHS gene on chromosome Xp22.2-Xp22.13. Clinical manifestations consist of congenital cataracts, along with dysmorphic facial features and dental anomalies and, in certain instances, intellectual disability. This study aimed to identify the genetic cause responsible for NHS in a Chinese family with four individuals primarily presenting with congenital cataracts.
METHODS: Genomic DNA was collected from six family members, including four affected individuals (three females and one mal

PURPOSE: Prostaglandin-associated periorbitopathy (PAP), which causes skin hardening, ptosis, and deepening of the upper eyelid sulcus (DUES), has been suggested to affect corneal shape due to increased eyelid pressure. In this study, we performed a corneal shape analysis using anterior segment optical coherence tomography (AS-OCT) in patients with PAP accompanied by blepharoptosis.
STUDY DESIGN: Retrospective study.
METHODS: Fourier harmonic corneal shape analysis using anterior segment OCT was performed in patients with a margin reflex distance-1 (MRD-1) of 2 mm or less (176 eyes of 101 ca
