[Levator strenght measurements. Oculomyodynamometry of the levator palpebrae superioris (author's transl)].

The author describes a method of levator strength measurement with the oculomyodynamometer with which he measured a maximum strength of the levator muscle in adults of 58 G. The levators were much weaker in congenital ptosis. In three such cases the levators had strengths of only 9 G, 5 G and 5 G. In cases of acquired ptosis the reduced strength of the levators was on an average about half normal. This method allows 1. better diagnosis with levator paresis, 2. a control of its behaviour during the clinical course, 3. profiting from the measurement results for correcting operations on the levat

Repair of ptosis using frontalis muscle and fascia lata: a 20-year review.

The results of an analysis of the surgery ("Repair of Ptosis using Frontalis Muscle and Fascia Lata") carried out on 316 lids is presented. The main indication for this operation is a large congenital ptosis with minimal levator function. Conclusions from the study indicate that autogenous fascia is an ideal tissue for lid suspension, and is better than stored fascia. Stored fascia does present some problems but is useful in small children or when the patient does not want a scar on the leg. Bilateral repair for unilateral ptosis is recommended for symmetrical results. The best ages for surger

[Myotubular or centronuclear myopathy; report of a case and review of the literature].

A case of centronuclear or myotubular myopathy in a 13-year-old boy who was admitted with hypotonia and congenital ptosis is reported. Clinical, electromyographic and pathologic studies are discussed. A review of the electron microscopic features and comparison with out findings is included. Review of the literature disclosed near 50 cases of centronuclear myopathy. The diferential diagnosis is discussed.

Surgical repair of extreme bilateral ptosis by modified anterior approach.

This report describes an apparently unique anterior approach surgical repair of a congenital ptosis which had progressed with age and had functionally almost blinded the patient. Poor tear production was the reason that neither conventional maximal levator resection nor sling procedures were considered desirable. A method of levator resection with no interference with the conjunctiva is presented, although since performance of the surgery, a similar method has been published. A few caveats of ptosis surgery are offered.

Clinical and ultrastructural correlation in congenital and acquired ptosis.

The levator superioris muscle has been studied with the light microscope and the electron microscope in congenital and acquired ptosis. Ultrastructural finding correlated closely with the clinical entities. Congenital ptosis appears to be on a myogenic basis. Acquired ptosis ultrastructurally indicates a regenerative process.

Euryblepharon with associated ocular anomalies.

Two cases of euryblepharon with other associated ocular abnormalities have been described. Case one showed flattening of the bridge of the nose and an abnormally large distance between the two medial canthi. Case two showed congenital ptosis in one eye and concomitant alternating divergent squint.

[Differential diagnosis of ptosis (author's transl)].

An accurate differential diagnosis of ptosis results if one refers the functional disorders of the levator to the different levels of the motor unit: Muscle, synapsis, peripheral neuron and nucleus. Therefore ptosis can be classified into myogenic, synaptogenic and neurogenic forms. This classification is also based on electromyographic findings from the levator muscle. Myogenic forms of the ptosis are: Senile ptosis, chronic progressive ocular muscle dystrophy, acute exophthalmic myositis and affections of the levator together with general systemic myopathies. Synaptogenic forms of ptosis are

Probable Marden-Walker syndrome: evidence for autosomal recessive inheritance.

Two first cousins, the offspring of consanguineous marriages, had features suggestive of Marden-Walker syndrome. Phenotypic similarities and differences for Schwartz-Jampel syndrome have been discussed. Main features of the Marden-Walker syndrome are failure to thrive, marked motor and mental retardation, and multiple malformations in the form of peculiar facies associated wilth poor muscle mass, mild congenital joint contractures, pigeon breast, kyphoscoliosis and arachnodactyly. Peculiar facies is due to blepharophimosis, congenital ptosis, hypoplastic mandible and low-set and malformed ears

Aniridia and congenital ptosis.

Congential ptosis associated with aniridia was found in 3 patients from 2 pedigrees. Nonsurgical aphakia, a condition not previously reported in association with aniridia, was also found in one case. The association of aniridia and congenital ptosis is suggested as evidence for the common mesodermal etiology of both anomalies.

Hereditary ptosis.

Weakness of the external eye muscles was believed to be of genetic origin in 94 of 97 patients studied and was familial in 73 patients. Thirty patients had congenital ptosis alone or with weakness of other ocular muscles. The lack of progression and high incidence of other congenital anomalies suggested that congenital ptosis had in most instances a development etiology. Neurogenic congenital ptosis was documented in one instance and was suspected in another patient. The most common causes of progressive ophthalmaplegia with onset after birth was myotonic dystrophy and ocular muscle dystrophy.

Genetic heterogeneity in the Waardenburg syndrome.

Waardenbury syndrome is divided into three groups: Type I, with dystopia; Type II, and "pseudo-Waardenburg" syndrome, without dystopia; the third has unilateral congenital ptosis. Using Waardenburg's original variables a, b and c, ratios a/b and c/a, Cotterman's L function, newly described indices X and Y and some phenotypic manifestations, a confident neat separation of each type can be obtained. New cases of all types and electron micrographs of the hair pigment anomaly are described.

Unusual facies, joint hypermobility, genital anomaly and short stature: a new dysmorphic syndrome.

Three brothers with a previously unrecognized pattern of malformations are presented. The syndrome is characterized by short stature; a broad, prominent forehead, hypertelorism, congenital ptosis, a broad, short nose with anteverted nostrils, a long, broad upper lip, low-set, abnormally shaped and posteriorly rotated ears; simian palmar creases; brachyclinodactyly; short fingers; ligamentous laxity allowing for hyperextensibility of the fingers, genu recurvatum, flat feet; and an anomalous penoscrotal configuration resulting in "saddle" deformity with scrotal folds incircling the base of the p
