Considering the high morbidity and mortality of deep-seated opportunistic mycoses in severely immunosuppressed patients, strategies for prophylaxis appear to be indicated. Exposure to Aspergillus spp. can be prevented by air filtration which has been shown to reduce the rate of infection. However, Candida infections are predominantly caused by colonizing fungi; therefore drug prophylaxis is more promising. Prospective randomized studies proved the effectiveness of fluconazole (FLU) to prevent infections in patients after bone marrow transplantation.

Invasive opportunistic mycoses are common complications in patients suffering from hematological disorders. Brain abscesses in the immunocompromised host are known to be most frequently caused by fungi of the Aspergillus species and are often associated with concomitant pulmonary disease. As the penetration of the currently available antifungal agents into the brain tissue is limited, only very few patients have been described to survive this life-threatening condition. We report the case of a 62 year old female patient who presented with multiple aspergillus brain abscesses during prolonged n

Pradimicins are a new class of antifungal compounds currently undergoing preclinical and early phase I clinical trials. The pradimicin structure is characterized by an aglycone of dihydrobenzo (alpha) naphthacenequinone with substitutions by a D-amino acid and hexose sugar. Pradimicins possess a novel mechanism of action consisting of a specific binding recognition to terminal D-mannosides of the cell wall of Candida albicans, resulting in the formation of a ternary complex consisting of D-mannoside, pradimicin, and calcium that leads to disruption of the integrity of the fungal cell membrane.

Report of three cases. Newly appeared immunosuppression related conditions have led to an increase in morbidity of systemic opportunistic mycoses. Rhinosinusal infection has also increased in frequency. Three invasive nasosinusal mycoses are presented: two cases of mucormycosis and a simultaneous infection by mucormycosis and candida albicans. In all cases, and despite treatment, progression of the disease could not be stopped and the patients died. The main characteristics of invasive nasosinusal mycoses are reviewed, to obtain early diagnosis. Early therapy improves the prognosis, and thus c

The addition of itraconazole and fluconazole to the antifungal armamentarium has resulted in less toxic, easily administered therapy for both endemic and opportunistic mycoses. Itraconazole has become the treatment of choice for most patients with mild-to-moderate histoplasmosis, blastomycosis, and sporotrichosis. Coccidioidomycosis is effectively treated with either itraconazole or fluconazole; fluconazole has become the drug of choice for the treatment of coccidioidal meningitis. In several large cooperative trials, fluconazole has been shown to have a useful role in the treatment of patient

Invasive fungal infections, including disseminated candidiasis and invasive pulmonary aspergillosis, are important causes of morbidity and mortality in neutropenic patients. The recent development of fluconazole, itraconazole, lipid formulations of amphotericin B, and recombinant cytokines have expanded our therapeutic armamentarium. Clinical trials have elucidated new strategies for utilizing these compounds in the prevention and treatment of opportunistic mycoses. The population of more severely immunocompromised patients, however, continues to expand and the spectrum of drug-resistant fungi

Fungi of the genera Aspergillus, Candida, and Cryptococcus and the class Zygomycetes are the most common causes of thoracic opportunistic mycoses in immunocompromised patients. Candidiasis and zygomycosis usually manifest as severe, often life-threatening, pneumonias. Aspergillus species are commonly implicated as the causative organisms in a broad spectrum of pulmonary disorders, ranging from hypersensitivity lung disease in atopic patients to invasive pneumonia in immunocompromised patients. Cryptococcus neoformans infects both immunologically normal and abnormal patients, with variable clin

The frequency of systemic fungal infections have increased significantly in the past decade. Opportunistic systemic fungal infections are increasingly posing difficult management challenges to the clinician. To the aid of the clinician, new systemic antifungal agents have also emerged in the last two decades. To effectively utilise the old and new antifungal agents, a clear understanding of their clinical pharmacology and therapeutic applications is needed. Amphotericin-B remains effective therapy in many severe disseminated mycoses including candidaemia and cryptococcosis but problems with to

The opportunistic character of deep-seated mycoses depends on granulocyte-based defense in candidosis and aspergillosis. Therefore, haematological, patients represent the group of highest risk. Mucocutaneous candidosis is controlled by macrophages. Cryptococcus neoformans forces its way into the human host via causing an imbalance in the CD8-T-cell suppressor system. An aggravating synergism exists between Cryptococcus invasion and HIV-infection which explains the severe course of cryptococcosis in AIDS patients. The following pathways of transmission are observed in opportunistic mycoses: In

The typical clinical picture and mycological and histological basis of chromomycosis is compared with the clinical course, which is encountered more often nowadays. It refers also to the affection of the vascular, respiratory, lymphatic system, the brain and the dissemination in compromised patients. For this reason chromomycosis should rank among opportunistic mycoses. Chromohyphomycosis characterized by skin nodules occurs very rarely. Subcutaneous abscesses also called phaeomycotic cysts, are more frequent especially after corticoid and immunosuppressive therapy. Among three Czechoslovak ca

Phagocytic host defense mechanisms play a pivotal role in the prevention and containment of invasive, disseminated candidiasis, aspergillosis, and other opportunistic mycoses. The specific processes involved depend upon both the species, growth phase, and surface properties of the fungal target and the type and differentiation of the effector phagocyte. The nature and outcome of phagocyte-fungal interactions are also influenced by definable surface features of the organisms, binding of opsonic serum components, triggering of particular phagocyte plasma membrane receptors, and modulating effect

In compromised patients, invasive mycoses are being observed at an increasing frequency and with a broadening spectrum of causative fungi. The rapid and definitive diagnosis of such opportunistic mycoses requires the synoptical consideration of predisposing clinical conditions, of culture isolates, and of the results of nonculture techniques. At present, the latter comprise the monitoring of specific antibodies and of fungal antigens, and the microscopical examination of suspected biopsies. The validity of the individual techniques (e.g., the monitoring of antibodies or antigens) varies with i

The most frequent pathogens of opportunistic mycotic infections are Candidae. The clinical manifestation of infection is very varied. With regard to the extent of the affection the following are differentiated: 1. mucocutaneous candidosis, 2. organ affections and 3. disseminated candidosis. The incidence of mycoses caused by Aspergillae, cryptococci, and mucors is of a lower order, these infections are, however, associated with a relatively higher mortality. The authors review briefly different fungi and describe the most important clinical manifestations of infection.

The field of opportunistic mycoses in the patient with cancer is rapidly changing. Not only are fungal infections increasing in frequency in this patient population, but these infections are occurring earlier during the course of cytotoxic chemotherapy, and newer fungi are increasingly recognized as potentially lethal pathogens. Candidiasis remains the most commonly encountered infection. The spectrum of disease includes candidemia and acute and chronic disseminated candidiasis. Pulmonary aspergillosis and disseminated aspergillosis are common and remain relatively resistant to therapy. Dissem

Patients with neoplastic diseases are predisposed to develop invasive fungal infections as the result of impairments of host defense mechanisms due principally to pharmacologic immunosuppression resulting from intensive cytotoxic chemotherapy, ablative radiation therapy, and corticosteroids. Candida species, Aspergillus species, and emerging opportunistic fungal pathogens comprise the principal etiologic agents of opportunistic mycoses in cancer patients. This paper reviews the recent progress, particularly during the year of 1991, in management of invasive fungal infections and the current pr
