Fluconazole. Review and situation among antifungal drugs in the treatment of opportunistic mycoses of human immuno-deficiency virus infections.

Fluconazole is a novel triazole antifungal drug chiefly used in the treatment of opportunistic mycoses in immuno-compromised patients, particularly those with the acquired immuno-deficiency syndrome (AIDS). In comparison with other antifungal drugs, fluconazole has outstanding physical and pharmacokinetic properties, such as an excellent aqueous solubility allowing a parenteral formulation, high bioavailability by the oral route, even distribution throughout the tissues, including the central nervous system and the cerebro-spinal fluid, a long half-life (permitting once daily administration),

P&T Committee review of fluconazole: an effective alternative to antifungal therapy.

Fluconazole is a new antifungal agent available in both oral and parenteral formulations. According to the experts in this roundtable discussion, fluconazole represents a major clinical advance in the treatment of candidiasis and cryptococcosis in cancer patients, patients with AIDS, organ transplant recipients, and other patients at risk for opportunistic mycoses. The pharmacokinetic profile for fluconazole permits infrequent dosing and also makes it ideal for tissue site infections. Fluconazole's low toxicity gives it an advantage over currently available antifungal therapy and will permit p

[Antimycotic therapy].

Because deep opportunistic mycoses are relatively rare, clinicians frequently lack routine in the application of antimycotics. It is the purpose of this review to summarize, from a practical point of view, rationale, indications, applications and toxicity of antimycotic therapy for the mycoses autochthonous to Europe. Amphotericin B remains the standard therapy for most deep opportunistic mycoses, while the newer azoles are the first line agents to be used for superficial forms of candidiasis.

Itraconazole in opportunistic mycoses: cryptococcosis and aspergillosis.

Striking results were obtained with oral itraconazole therapy in two opportunistic mycoses. Of 28 patients with cryptococcal meningitis, 18 achieved complete responses, including 16 of 24 patients with acquired immunodeficiency syndrome. Other manifestations of cryptococcosis were similarly responsive. In aspergillosis 12 of 15 patients responded, including 8 of 10 immunocompromised hosts. These patients included those with invasive pulmonary disease (4/5), skeletal disease (2/2), pleural disease (1/2), and pericardial, sinus, mastoid, hepatosplenic, or nail disease (1/1). These results with i

Pulmonary defense mechanisms against opportunistic fungal pathogens.

Though of critical importance, nonimmune host defense mechanisms against aspergillosis and mucormycosis are not completely understood. Prevention of these infections presumably requires control of either spore germination and/or hyphal growth by the host. The data suggest that the host provides an important barrier to infection by control of spore or conidia germination, the critical step involving conversion of the fungus to its tissue-invasive form. The mechanisms of host defense against A. fumigatus are not strictly dependent on inhibition of conidia germination. Rather, pulmonary defense a

[Treatment of mycoses and new antifungal agents].

The multiplication of iatrogenic factors, nosocomial diseases and acquired immunodeficiency syndrome is responsible for an ever increasing number of deep opportunistic mycoses, namely candidiasis, aspergillosis and cryptococcosis. The advent of a wide variety of rare opportunistic fungi is a fairly recent and worrying phenomenon. To combat these infections, often very serious, our therapeutic armentarium is rather scanty. Moreover, the prescription of the available drugs is limited by their toxicity, their spectrum and their route of administration or by the emergence under treatment of resist

Experimental basis for the clinical epidemiology of fungal infections. A review.

Based on the concept that the agents of deep fungal infections can be divided into primary pathogens and opportunists the experimental basis for the clinical epidemiology of mycoses is outlined. Kinetics of experimental infections with opportunists and primary pathogens discriminate between the two fungal categories. Natural resistance eliminates opportunists and prevents the establishment of progressive infection in the normal host. Primary pathogens call upon mechanisms of adoptive cell mediated immunity for their control. Therefore athymic mice which are not more susceptible to opportunists

Nosocomial fungal infections. Old problems and new challenges.

The frequency of nosocomial mycoses has substantially increased in recent years, primarily because of the use of intensive therapeutic regimens in patients with cancer and organ transplantation. Furthermore, newer fungi previously considered as contaminants or harmless colonizers have now emerged as significant human pathogens, particularly in the immunocompromised host. Only through the collaboration between clinicians, pathologists, microbiologists, and infection control officers can these fungal infections be recognized and treated early. It is hoped that the proper implementation of infect

[General review of maxillofacial mycoses].

It is interesting and useful to review the different mycoses which can affect oral and maxillofacial tissues. In this review, all mycoses which can involve these tissues are described, based on a simple classification into superficial and deep mycoses. The superficial mycoses described are the dermatophytoses, the Tineas, the piedras, but also the candidoses, which are superficial opportunistic infections. The deep mycoses are subdivided into subcutaneous, systemic, and opportunistic mycoses. Subcutaneous fungal infections are sporotrichosis, lobomycosis, rhinosporidiosis, entomophtoromycoses,

An overview of pulmonary fungal infections.

An overview of important pulmonary mycoses is presented. Four endemic mycoses occur in restricted geographic areas: histoplasmosis, blastomycosis, coccidioidomycosis, and paracoccidioidomycosis. Three opportunistic mycoses have causative agents that are ubiquitous in nature but cause infection only in compromised hosts: aspergillosis, mucormycosis, and candidiasis. Cryptococcosis is acquired by inhalation but usually presents as a chronic meningitis. Pulmonary sporotrichosis is rare; the usual subcutaneous form of the disease is acquired by inoculation. The clinical features of these infection

Host-parasite relationship in opportunistic mycoses.

Aspergillosis and mucormycosis are opportunistic fungal infections that share several unique features. The etiologic agents of aspergillosis and mucormycosis are ubiquitous in the environment, but are opportunistic organisms and usually infect only patients predisposed by some underlying disease or treatment. These infections are typically characterized by hyphal tissue invasion and a predilection of the organism for blood vessel invasion with hemorrhage, necrosis, and infarction. Also, these organisms are not dimorphic, like the true pathogenic dimorphic fungi, as they grow both in the enviro

The pathogenesis of opportunistic mycoses in man.

A variety of fungi capable of growth at around 37 degrees C are potential pathogens of suitably compromised animals, including man. While awareness of diseases by these opportunistic fungi is increasing, details of their pathogenesis are largely unknown, although ability of the host to inhibit spore germination appears to be central to resistance to infection.

Systemic mycoses in the United States, 1980-1982.

To evaluate temporal trends in the incidence of systemic mycoses resulting in hospitalization, we reviewed hospital discharge data from the Professional Activity Study of the Commission on Professional and Hospital Activities for 1980-1982. In comparison with results from similar studies in 1970 and 1976, the incidences of the mycoses that are more likely to be opportunistic and/or hospital-acquired infections (candidiasis, aspergillosis, cryptococcosis) increased in 1980-1982 as did the incidences of sporotrichosis and actinomycosis. At the same time the incidences of most community-acquired

[Antifungal prophylaxis: up-date and prospectives].

Author reviews the now available strategies for the prophylaxis of primary and opportunistic mycoses. Chemoprophylaxis of opportunistic mycoses that are the most frequently observed fungal infections in Italy, gives satisfactory protection in endogenous fungal infections. On the contrary, fungal exogenous infections are not easily treated. In fact, systematically acting drugs which are also easy to use, are still lacking. Strict reverse isolation seems to achieve good results in these infections.

[Opportunistic mycoses in pediatrics].

The two most important pathologic conditions leading to mycotic opportunistic infections in children are impairment of mechanisms of defense due to immunosuppressive drugs and congenital defects of immunity. Other circumstances belong to pediatrics such as prematurity or cystic fibrosis. A few examples are chosen to illustrate these situations: congenital candidiasis, chronic mucocutaneous candidiasis, chronic dermatophytic disease, neonatal candidemia, mycotic infections in chronic granulomatous disease and aspergillosis in cystic fibrosis.
