Analysis of Craniofacial Microsomia and Facial Asymmetry: Approaches in Facial Feminization Surgery.

BACKGROUND: Comprehensive facial analysis among transgender and gender-diverse (TGD) individuals undergoing facial feminization surgery (FFS) has created new opportunities to investigate facial asymmetry in adults. Although craniofacial microsomia (CFM) is typically identified in infancy, milder forms may go unrecognized, leaving gaps in diagnosis and treatment. OBJECTIVES: The authors of this study examine facial asymmetry, including CFM, among TGD patients undergoing FFS and discuss appropriate surgical strategies. METHODS: Adults registered male at birth undergoing de novo FFS from 2020 t

Lateral Orbital Rim Advancement and Balanced Orbital Decompression for Exorbitism Regression in Crouzon Syndrome.

Crouzon syndrome is a syndromic craniosynostosis characterized by brachycephaly, midfacial hypoplasia, exorbitism, and malocclusion. Although current treatments can effectively address these deformities, they generally involve extensive staged procedures associated with prolonged treatment duration and substantial financial cost. For postpubescent and adult patients with relatively mild manifestations whose primary concern is noticeable exorbitism, less invasive options are limited. We describe a minimally invasive technique termed lateral orbital rim advancement combined with balanced orbital

Granulomatosis with polyangiitis presenting as anophthalmic socket pain.

Ocular and orbital symptoms are among the most common presenting signs in granulomatosis with polyangiitis (GPA). This report describes a patient with bilateral anophthalmos presenting with several weeks of left socket pain that was unresponsive to analgesics and low-dose oral steroid, without periorbital inflammation or orbital lesions on imaging. She was initially diagnosed with orbital neuralgia secondary to extensive prior orbital fractures and hardware and was scheduled for hardware removal. While awaiting surgery, she was admitted 1 month later for pneumonia and was diagnosed with GPA fo

Variability, asymmetry and sexual dimorphism in craniofacial anomalies in Loeys-Dietz syndrome 2: geometric morphometric analysis in mice.

Loeys-Dietz syndrome is a rare connective tissue disorder characterized by life-threatening aortic aneurysm and distinctive craniofacial anomalies. It is caused by mutations along the transforming growth factor beta (TGF-β) signaling pathway (LDS1-6). We previously showed that craniofacial anomalies varied among LDS subtypes and that LDS2, caused by mutations in the TGFBR2 gene, exhibited the most severe and variable phenotype. In this study, we performed a thorough qualitative and quantitative analysis of craniofacial anomalies in a mouse model for LDS2, through micro computed tomography and

Assuming the right approach in cerebrofacial arteriovenous metameric syndrome: a case report.

Cerebrofacial arteriovenous metameric syndrome (CAMS) is a rare nonhereditary disease, amounting to only 0.5% of all cerebral vascular malformations. It is characterized by vascular malformations in a metameric distribution involving the cerebrofacial region. We present a case of a 14-year-old girl in whom the first vascular lesion appeared on the tip of her nose at the age of 18 months. This lesion reoccurred on several occasions during her childhood, with exacerbations coinciding with common rhinitis and several nose bleeds. She was unsuccessfully treated with laser therapy and topical selec

The Use of Virtual Surgical Planning and 3D Printed Patient Specific Guides to Optimize Esthetic Outcomes of Hydroxyapatite Cement Onlay Cranioplasty.

Craniofacial procedures such as fronto-orbital remodelling can result in forehead contour irregularities and bitemporal hollowing many years following the initial surgery. Hydroxyapatite cement (HAC) cranioplasty is a secondary surgical procedure that can be used to recontour the forehead and superior orbital rim. This approach relies on the surgeon's subjective assessment of form and aesthetics both preoperatively and intraoperatively. The highly artistic nature of this procedure means that outcomes, although often good, can be inconsistent and may result in undercorrection or overcorrection.

Posterior Cranial Decompression in ERF-Mutated Multisuture Craniosynostosis.

Multisutural craniosynostosis, involving premature fusion of cranial sutures, leads to abnormal skull shape and elevated intracranial pressure (ICP), threatening neurodevelopment. This case report describes a 6-day-old infant with multisutural craniosynostosis associated with an ERF gene mutation. Imaging confirmed elevated ICP. At 2 months, posterior cranial and foramen magnum decompression effectively reduced ICP, deferring fronto-orbital advancement. By age 2, the child had achieved normal developmental milestones with stable cranial morphology and no Chiari I malformation. Early posterior

One ECG with 2 Rare Findings: Wellens Syndrome With Prolonged QT Interval in Acute Myocardial Infarction Due to LAD Occlusion.

BACKGROUND Wellens syndrome is considered as equivalent to ST elevation myocardial infarction and is highly specific for critical stenosis of the proximal left anterior descending (LAD) coronary artery. Prolonged QT interval is a risk factor for life threating arrhythmia characterized by a prolongation of the QT interval (QTc) longer than 440 ms corrected for heart rhythm on the ECG. It might be seen in patients suffering from acute myocardial infarction and constitutes a negative prognostic factor. These 2 ECG pathologies are rarely seen together. Computed Tomography Analysis of Craniofacial Features in Japanese Patients With Cleidocranial Dysplasia.

Cleidocranial dysplasia is a congenital malformation syndrome characterized by skeletal and dental abnormalities as well as distinctive craniofacial features. Most previous reports have relied on two-dimensional radiographs, and no comprehensive three-dimensional investigations have been conducted. This study aimed to provide a detailed overview of craniofacial features in patients with cleidocranial dysplasia using computed tomography. Craniofacial characteristics of 11 Japanese patients were assessed using three-dimensional computed tomography reconstructions. Qualitative evaluations were pe

A Standardized Surgical Technique for Tessier Medial Transnasal Canthopexy.

Medial canthopexy is a surgical technique that allows correction of the position of the medial canthus, corresponding to the medial angle of the palpebral fissure. Abnormal position of the medial canthus is observed in the case of telecanthus (excessive intercanthal distance) or medial canthal dystopia (displaced medial canthus), encountered in congenital craniofacial malformations or after craniofacial trauma or tumoral resection that affects the orbits. Correcting the position of the medial canthus is a challenge for the training surgeon, due to the difficulty of the techniques. Medial trans

Use of Chitosan Granules for Refractory Intraoperative Bleeding in Orbital Vascular Malformations.

Severe intraoperative bleeding unresponsive to conventional hemostatic methods occurred in 4 patients with distensible orbital venous malformations (n = 3) or orbitotemporal arteriovenous malformations (n = 1) (ages: 15-27 years; 3 women, 1 man) during surgical excision of the vascular masses. The bleeding originated from the intraorbital soft-tissue or intraosseous components of the lesions. In all cases, hemostasis was achieved following the application of chitosan granules with compression. Estimated blood loss ranged from 150 to 700 ml, and 1 patient required a blood transfusion. Low-cost beetle-wing fronto-orbital advancement using nonabsorbable suture fixation for non-syndromic trigonocephaly: A single center experience.

BACKGROUND: Trigonocephaly results from premature fusion of the metopic suture, leading to restricted frontal bone growth and a triangular forehead. Its incidence has increased, and fronto-orbital advancement (FOA) with frontal remodeling is now the standard surgical treatment, However, the use of absorbable fixation system may increase costs and limit accessibility. This study evaluates the aesthetic and functional outcomes of a modified Dhellemmes (Lille) beetle-wing FOA using nonabsorbable sutures as a low-cost alternative for nonsyndromic trigonocephaly. METHODS: Twenty-four patients aged

Replicating the remnant iris using an optometry trial frame: A clinical technique.

Anophthalmia, resulting from trauma, carcinoma, or congenital causes, necessitates orbital rehabilitation to address both psychological and cosmetic needs. Traditional methods for positioning ocular prostheses have frequently been inaccurate because of errors caused by parallax and differences in facial symmetry. A novel technique is described for replicating the remnant iris using an optometry trial frame to improve the precision and efficiency of custom orbital prosthesis fabrication. The described technique uses a trial framework for precise iris alignment, facilitating measurement transfer

We report a 3-year-old boy with macrocephaly, papilledema, and headaches associated with bilateral hypoplastic jugular bulbs leading to intracranial hypertension. Initial imaging was misinterpreted as idiopathic intracranial hypertension, and medical therapy with acetazolamide failed. Repeat MRI/MRV demonstrated thrombosis of the right transverse sinus with orbital collateral drainage. Angiography confirmed occlusion of the right jugular bulb and severe stenosis of the left. Due to progressive symptoms and developmental risk, rescue endovascular stenting of the right internal jugular vein was
