Defining genetic and physiopathological bases of orbital hypertelorism: about a systematic literature review.

Orbital hypertelorism is observed in a wide spectrum of congenital disorders, such as in case of frontonasoethmoidal tumors or encephaloceles, craniofrontonasal dysplasia, craniofacial clefts, syndromic craniosynostosis, Rasopathies and various malformative and multisystemic congenital syndromes. Yet its underlying pathophysiological mechanisms remain incompletely understood. Our systematic literature review aimed to delineate and classify the different conditions associated with true orbital hypertelorism based on both genetic bases and physiopathological mechanisms of orbital hypertelorism.

Congenital Corneal Staphyloma in an Asian Infant With Kabuki Syndrome Confirmed by a KMT2D Mutation.

This case report presents the first case of congenital corneal staphyloma in an Asian pediatric patient with Kabuki syndrome confirmed by a KMT2D gene mutation. A female infant, born at 38 weeks and 3 days of gestation via cesarean section, was referred immediately after birth for anterior segment dysgenesis of the right eye. The right eye showed microphthalmia accompanied by corneal opacity and limbal deficiency with neovascularization. Intraocular tumors were ruled out by ocular ultrasound and MRI. The left eye appeared normal. Subsequently, the cornea continued to protrude, leading to a cli

Comparative assessment of the infraorbital canal course in patient with cleft lip and palate with control group: a CBCT study.

AIM: This study aimed to assess the course of the infraorbital canal (IOC) in cleft lip and palate (CLP) patients compared to a matched control group using CBCT imaging, evaluating the potential anatomical variations that may influence surgical risks in the maxillofacial region. MATERIALS AND METHODS: In this cross-sectional study, CBCT scans of 67 CLP patients (50 unilateral, 17bilateral) were evaluated alongside 67 age- and sex-matched controls. IOC courses were classified into three types using the Ference method: Type I (within sinus roof), Type II (juxtaposed), and Type III (protruded).

Orientation-robust latent motion trajectory learning for annotation-free cardiac phase detection in fetal echocardiography.

Fetal echocardiography is essential for detecting congenital heart disease (CHD), facilitating pregnancy management, optimized delivery planning, and timely postnatal interventions. Among standard imaging planes, the four-chamber view (4CV) provides important information for CHD diagnosis, where clinicians carefully inspect the end-diastolic (ED) and end-systolic (ES) phases to evaluate cardiac structure and motion. Automated detection of these cardiac phases is thus a critical component towards fully automated CHD analysis. However, existing approaches typically rely on manual annotation of E

Significant clinical, functional and radiological response with sirolimus in refractory complex orbital venolymphatic malformation.

Orbital venolymphatic malformations (OVLM) are rare congenital vascular anomalies that may cause progressive proptosis, visual loss, facial disfigurement and marked psychosocial burden. Management is challenging especially when the disease is multifocal, diffuse and with limited response to conventional therapies. We describe a school-aged child with a right eye refractory extensive orbital venolymphatic malformation leading to marked visual impairment and disfigurement despite multiple prior sclerotherapy sessions. After multidisciplinary evaluation, oral sirolimus was initiated with written

Bilateral coronal craniosynostosis with novel TWIST1 mutation.

INTRODUCTION: Syndromic craniosynostosis is characterized by premature fusion of one or more cranial sutures, often in association with multisystem anomalies affecting the airway, cardiovascular, musculoskeletal, and neurodevelopmental systems. Variants in genes such as TWIST1 contribute to phenotypic heterogeneity and may influence surgical timing, risk stratification, and long-term craniofacial planning. We present a severe syndromic craniosynostosis phenotype associated with a previously undescribed TWIST1 variant and discuss perioperative considerations of staged cranial vault reconstructi

Modern concepts of CSF disorders in monosutural craniosynostosis.

INTRODUCTION: Although hydrocephalus is randomly encountered in monosutural craniosynostosis (CS), CSF disorders may present more subtly and accompany these pictures. Indeed, dilation of subarachnoid spaces is frequently reported. The pathogenesis of these disorders is still under debate, as well as their prognostic significance. METHODS: A thorough review of the literature has been performed. Accordingly, pathogenic theories, eventual impact of surgical strategy, and prognostic significance are discussed. RESULTS: Most data come from scaphocephaly and trigonocephaly. Focal dilation of subar

Endoscopic management of unilateral coronal synostosis: impact on the fronto-orbital growth.

Unilateral coronal synostosis (UCS) is a disorder in which one of the six major cranial sutures are fused and can affect either the left or right side of the skull. Its clinical effects are quite significant, based on the degree of asymmetry it causes both in the cranium as well as the subcranial skull base and facial features, as well as significant ocular concerns. Open cranial vault reconstruction, often referred to as frontal orbital advancement, has long been considered the standard of care for the treatment of this condition. However, most experienced clinicians understand that it is imp

Congenital Isolated Bilateral Anophthalmia: A Case Report.

Congenital anophthalmia is the complete absence of the eye resulting from a failure of optic vesicle formation during early embryogenesis. It may present as a unilateral or bilateral condition and can occur as an isolated anomaly or as part of a syndromic disorder. We report the case of a full-term newborn admitted on the second day of life for the management of bilateral congenital anophthalmia. Family history was notable for anophthalmia in three cousins belonging to the shared parental lineage, suggesting a possible genetic predisposition. The diagnosis is primarily clinical and is confirme

Orbital Apex Syndrome Secondary to Middle Cranial Fossa Meningocele: A Rare Pediatric Case.

BACKGROUND: Orbital apex syndrome (OAS) is a rare, sight-threatening condition caused by injury or compression of neurovascular structures in the orbital apex, which, depending on the underlying etiology, may even pose a life-threatening risk. Prompt recognition and intervention are essential to prevent permanent visual loss and systemic complications. CASE PRESENTATION: We report a 10-year-old child presenting with acute left-sided proptosis, eyelid edema, ophthalmoplegia, and optic disc edema, following a history of orbital cellulitis and meningitis in early childhood. Imaging revealed a mi

Traumatic Carotid-Cavernous Fistula in a Child with Nasal Penetrating Injury.

BACKGROUND: Traumatic penetrating carotid-cavernous fistula (CCF) is an uncommon complication of craniofacial trauma, particularly in pediatric patients. Prompt recognition is essential to prevent irreversible visual and neurological deficits. CASE PRESENTATION: We describe a 2.5-year-old boy who sustained a penetrating nasal injury caused by a screwdriver. He initially presented with epistaxis and subsequently developed left-sided proptosis, chemosis, and restricted ocular motility. Early imaging studies were inconclusive, and orbital cellulitis was initially suspected. DIAGNOSIS AND MANAGE

Ophthalmic manifestations of Chiari malformation: a systematic review.

OBJECTIVE: Chiari malformation (CM) comprises congenital or acquired hindbrain herniations (>5-mm cerebellar tonsillar descent below the foramen magnum) that disrupt cerebrospinal fluid flow and compress neural structures. While neurological symptoms are well recognized, ophthalmic manifestations, such as papilledema, diplopia, and nystagmus, which are uncommon in the overall CM population but may serve as important diagnostic clues when present, remain undersynthesized. This systematic review summarizes the ophthalmic manifestations of CM to identify symptom patterns, mechanisms, and implicat

Tessier-3 Clefts: Clinical Features and Staged Reconstruction.

BACKGROUND: Tessier-3 clefts are rare craniofacial malformations involving the oro-naso-orbital axis and are frequently associated with orbitolacrimal and inferomedial orbital abnormalities. Published series remain limited, and the relationship between anatomic involvement, reconstructive sequencing, and childhood operative burden remains poorly characterized. PATIENTS AND METHODS: A retrospective case series included 9 patients with Tessier-3 clefts treated at a tertiary craniofacial center between 2015 and 2024. Clinical records, operative reports, computed tomography imaging, and clinical

Geometric Morphometric Analysis of Forehead Morphology in Unicoronal Synostosis: Identifying the Pivot Line.

Unicoronal synostosis (UCS) causes asymmetric forehead morphology and is commonly managed with fronto-orbital advancement (FOA). However, optimal reconstruction remains challenging as the location of the forehead's pivot line, the hinge axis for advancement, often remains uncertain. We conducted a geometric morphometric analysis using 235 cranial landmarks in 16 patients with UCS (aged <2 y) and 11 age-matched controls. CT-derived cranial models were assessed with generalized Procrustes analysis and principal component analysis (PCA) to evaluate morphologic asymmetry and identify potential piv

Diabetes and optic atrophy in a young adult: consider Wolfram syndrome.

A 22-year-old woman had an 8-year history of progressive bilateral vision loss and of diabetes mellitus. Her mother had diabetes and two first cousins had severe congenital deafness. On examination, her visual acuities were 6/36 bilaterally, with absent colour vision and gross optic disc pallor. Her HbA1c was 94 mmol/mol (20-42). Visual evoked potentials were bilaterally symmetrically prolonged with P100 140 ms. Optical coherence tomography showed significant thinning of the peripapillary retinal nerve fibre layer. MR scan of brain and orbits showed small optic nerves and chiasm. Genetic testi

Unfolding tale of post-traumatic globe luxation into the ethmoid sinus: an unusual encounter in ophthalmology.

Traumatic displacement of the eyeball into the paranasal sinuses is an exceptionally uncommon sequelae following head injury, particularly when the orbit is the site of major impact. We present a case of a ruptured globe dislocation into the ethmoid sinus with extensive bilateral orbital fractures in a male in his early 20s, following a high-velocity motor vehicle (two-wheeler) road traffic accident 3 weeks ago. He also sustained a right optic canal fracture with encephalocele, which led to traumatic optic neuropathy. The case was managed through a multidisciplinary staged surgical approach. T
