BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine

Acute lung injury (ALI) affects over 10% of patients hospitalised in critical care, with acute respiratory distress syndrome (ARDS) being the most severe form of ALI and having a mortality rate in the region of 40%. There has been slow but incremental progress in identification of biomarkers that contribute to the pathophysiology of ARDS, have utility in diagnosis and monitoring, and that are potential therapeutic targets (Calfee CS, Delucchi K, Parsons PE, Thompson BT, Ware LB, Matthay MA, Thompson T, Ware LB, Matthay MA, Lancet Respir Med 2014, 2:611--620). However, a major issue is that ARD

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

INTRODUCTION: Anophthalmia/microphthalmia (A/M) and anterior segment dysgenesis (ASD) are severe ocular anomalies impacting eye morphology, occurring in 30 per 100,000 live births. Genetic research has identified over 30 genes linked to A/M anomalies, with their products mainly involved in eye organogenesis.
AIMS AND OBJECTIVES: This study examined two consanguineous A/M families to identify disease-associated pathogenic mutations and predict their functional impact.
METHODOLOGY: Patients were clinically examined using A-scan and ophthalmic ultrasonography. Whole exome sequencing (WES) ident

Mutations in the MFRP (membrane-type frizzled-related protein) gene are associated with a spectrum of ocular diseases. Here, we report on a patient with MFRP-linked autosomal recessive retinitis pigmentosa (arRP) with nanophthalmos who exhibited yellow deposits circumferentially along with far temporal intraretinal pigment migration. In future studies, we plan to explore the amelioration of MFRP-associated phenotypes in patient-specific induced pluripotent stem cell (iPSC)-derived retinal pigment epithelium and in vivo using the classical Mfrprd6 mouse model of RP. To effectively screen gene e

INTRODUCTION: Cataract may cause severe visual impairment in eyes with microphthalmos (MO) and uveal coloboma (UC). Despite their similarities, distinguishing between these conditions is crucial for cataract surgery. We aimed to compare complications, nucleus hardness, surgical strategies, and outcomes of cataract surgery between MO and UC.
METHODS: This retrospective comparative study included 19 eyes with MO and 20 with UC. Preoperative, intraoperative, and postoperative data of eyes that underwent cataract surgery were analysed.
RESULTS: MO eyes had lower high-hardness nucleus rates (p =

Microphthalmia and anophthalmia (M/A) are rare congenital eye anomalies with a birth prevalence of up to 1 in 10,000 births. The etiology of M/A can involve environmental and/or genetic factors, with a genetic origin identified in approximately 50% of cases through analysis of key genes. The transcription factor SOX2 is the most commonly implicated gene, accounting for around 15% of M/A cases. Recent studies have shown that the RNA-binding protein Rbm24 post-transcriptionally regulates Sox2 expression in mice and zebrafish, with Rbm24 null models exhibiting eye phenotypes in both species. Rbm2

The homeobox-containing transcription factor PAX6 is a key regulator of eye development. Pathogenic heterozygous PAX6 variants lead to variable ocular phenotypes, most commonly haploinsufficiency-induced aniridia. Missense variants are typically associated with milder ocular conditions, although variants in the DNA-binding paired domain which alter target binding lead to severe ocular phenotypes including bilateral microphthalmia, similar to SOX2-anophthalmia syndrome. However, the variant-specific pathway disruption resulting in phenotypic heterogeneity is not well understood. To investigate

Nanophthalmos is an uncommon developmental ocular disorder characterized by a diminutive eye resulting from impaired growth, which may occur sporadically or within familial contexts. Both environmental and genetic factors are implicated in the induction of nanophthalmos. The ocular biometric characteristics associated with nanophthalmos include reduced ocular axis, shallow anterior chamber, thickened scler and microcornea, which may lead to complications such as secondary uveal effusion syndrome and angle-closure glaucoma, as well as comorbidities like congenital cataract. Typically, the diagn

Nivelon-Nivelon-Mabille syndrome (NNMS, #600092) is an extremely rare genetic disorder characterized by microcephaly, central nervous system abnormalities, skeletal anomalies, and 46,XY disorders of sex development. It is caused by biallelic variants in the HHAT gene, which encodes the Hedgehog acyltransferase (HHAT) protein. To date, only eight patients with NNMS have been reported in the literature. In this study, four new patients from two unrelated families were presented, exhibiting distinct phenotypic features including 46,XY gonadal dysgenesis, microcephaly, microphthalmia, ocular colob

A natural AQP0 mutation, CatTohm, resulted in smaller eyes, and lenses with bilateral dominant cataracts in mice. Our objective was to characterize this mutation and explore the possible reasons for CatTohm causing dominant cataracts. We studied lens morphology, transparency, functional alterations and cytotoxicity. Lens morphology and nuclear fiber cell organization were severely affected. Water permeability (Pw) of oocytes expressing CatTohm-AQP0 cRNA (12 ± 2 μm/s) reduced markedly (P < 0001) compared with WT-cRNA-expressing oocytes (25 ± 2 μm/s); co-expression of both cRNAs decreased the Pw

Microphthalmia and coloboma are structural malformations of the eyes that arise from defective morphogenesis and are among the most severe defects associated with paediatric blindness. Frizzled class receptor 5 (FZD5) is a Wnt receptor expressed in the developing eye, and individuals with variants in FZD5 exhibit microphthalmia/coloboma, supporting a role for this receptor in human eye formation. Here, we show that zebrafish fzd5 mutants homozygous for complete loss-of-function or predicted dominant-negative alleles display no obvious eye defects during embryogenesis. Rather, they develop eye

Severe visual impairment in the newborn period can be attributed to prenatal and perinatal etiologies. Prenatal causes include various congenital anomalies such as anophthalmos or microphthalmos, congenital cataracts, congenital glaucoma, retinal dystrophies, and retinoblastoma. Acquired perinatal causes include ophthalmia neonatorum, cerebral visual impairment related to neurologic insult, and retinopathy of prematurity. Severe visual impairment can negatively affect a child's growth, development, social skills, and educational abilities, but prompt recognition and treatment of visual deficit

We report a rare case of posterior microphthalmos with prominent papillomacular folds, pseudo disc edema and high hyperopia in a 12 year old child with no systemic anomalies. The patient was diagnosed as high hyperopia two years ago but the posterior findings were missed. The anterior segment of the eye was normal on slit lamp examination but the appropriate diagnosis was missed. Hence detailed evaluation of every hyperopic child should be done for the timely management. This case report highlights the importance of detailed fundus and biometric evaluation to avoid missing the diagnosis of pos

Colobomatous microphthalmia is a potentially blinding congenital ocular malformation that can present either in isolation or together with other syndromic features. Despite a strong genetic component to disease, many cases lack a molecular diagnosis. We describe an autosomal dominant oculo-vertebral-renal (OVR) syndrome in six independent families characterized by colobomatous microphthalmia, missing vertebrae and congenital kidney abnormalities. Genome sequencing identified six rare variants in the orphan nuclear receptor gene NR6A1 in these families. We performed in silico, cellular, and zeb

We report the case of a 74-year-old woman with high hyperopia and nanophthalmos who presented with a progressive central scotoma in her left eye. Best-corrected visual acuity was limited to hand motion, and examination revealed a full-thickness macular hole associated with retinal detachment. Pars plana vitrectomy with the inverted internal limiting membrane (ILM) flap technique and 20% sulfur hexafluoride (SF6) tamponade was performed, but hole closure was not achieved. A second surgery including phacovitrectomy, retinal massage and ILM flap repositioning resulted in successful anatomical clo

Anophthalmia/microphthalmia (A/M) are rare congenital ocular malformations involving the absence or underdevelopment of the eyes, and they display considerable clinical and genetic heterogeneity. Establishing a genetic diagnosis for A/M is critical because it facilitates early intervention, informed genetic counseling, and the prevention of disease transmission in high-risk families. This study explored the genotypic and phenotypic landscape of A/M in 10 Pakistani families meeting specific criteria: confirmed A/M phenotype, residence in Khyber Pakhtunkhwa, no prior genetic testing, and informe

PURPOSE: FOXE3 encodes a highly conserved, lens-enriched transcription factor essential for eye development. Biallelic mutations in FOXE3 are associated with a spectrum of ocular anomalies, ranging from congenital cataracts to complex microphthalmia (CM), with severity and penetrance correlating with genotype. This study aimed to investigate the regulatory landscape of FOXE3 and its contribution to CM.
METHODS: In a patient with CM, a truncating FOXE3 variation (p.Cys240*) was identified alongside a second, trans-acting regulatory variant (rv: rs745674596G>A) located 3 kb upstream of FOXE3. T
