[Choanal atresia: personal surgical technique].

Choanal atresia is a rare otorhinolaryngological pathology. It occurs in 1 out of 5,000 children born alive and can be associated with other major or minor congenital abnormalities. The most important associations go under the acronym of C.H.A.R.G.E. (Coloboma, Heart disease, Atresia choanae, Retarded growth and/or development, Genital hypoplasia and Ear anomalies and/or deafness). The authors briefly describe the most salient clinical features and possible, though rare, complications of this anomaly (i.e., prolonged apnea, ab ingestis pneumonia, sinusitis, all the way to death by asphyxia). [Vertical vergence in convergence].

PURPOSE: To report on a 7-year-old boy with a small left-over-right deviation (-VD) which increased when the head was tilted to the left shoulder and during convergence. METHODS: The squint angles were measured by the unilateral and alternate prism cover test at distance and near fixation when the head was in ortho-position and when it was tilted. RESULTS: At distance fixation (D) there was a latent deviation of-VD 3 degrees. With near fixation (N) at 0.3 m the vertical phoria increased to-VD 18 degrees. The angle of deviation was not influenced by (N) convex lenses in front of the fixating

Retinal structure and function in an animal model that replicates the biochemical hallmarks of desmosterolosis.

Desmosterolosis is a rare, autosomal recessive, human disease characterized by multiple congenital anomalies in conjunction with grossly elevated levels of desmosterol and markedly reduced levels of cholesterol in all bodily tissues. Herein, we evaluated retinal sterol composition, histology, and electrophysiological function in an animal model that exhibited the biochemical features of desmosterolosis, produced by treating pregnant rats and their progeny with U18666A, an inhibitor of desmosterol reductase. Treated rats had cataracts, were substantially smaller, and had markedly high levels of

Exclusion of chromosome 15q21.1 in autosomal-recessive Weill-Marchesani syndrome in an inbred Lebanese family.

We report an inbred family where 3 siblings had short stature, brachydactyly, limitation of joint movements, microspherophakia, luxated lenses, glaucoma, and heart malformations. Parents of the affected siblings were relatively short, but did not have any of the other features present in their siblings. Those clinical features are consistent with the Weill Marchesani syndrome (MIM 277600). Both autosomal-recessive and autosomal-dominant pedigrees have been reported, with a possible linkage to chromosome 15q21.1 in the latter. Linkage analysis at 15q21.1 in this Lebanese family allowed us to ex

Autosomal dominant cataracts and Peters anomaly in a large Australian family.

Peters anomaly is a congenital corneal opacity with underlying defects in the posterior stroma, Descemets membrane and corneal endothelium. It is a disorder resulting from abnormal migration or function of neural crest cells and may include abnormalities of other anterior segment structures, such as the lens and iris. We report a family in which anterior segment abnormalities, including Peters anomaly and cataracts, were inherited in an autosomal dominant fashion. Although the PAX6 gene on chromosome 11 has been shown to be involved in some cases of anterior segment developmental defects, we f

Cataract mutations and lens development.

The lens plays an essential role for proper eye development. Mouse mutants affecting lens development are excellent models for corresponding human disorders. Moreover, using mutations in particular genes the process of eye and lens development can be dissected into distinct steps. Therefore, three mouse mutants will be described in detail and discussed affecting three essential stages: formation of the lens vesicle, initiation of secondary lens fiber cell formation, and terminal differentiation of the secondary fiber cells. The mutant aphakia (ak) has been characterized by bilaterally apakic e

[Visual prognosis factors in congenital cataract].

PURPOSE: Study of different factors which can change the visual prognosis in congenital cataract. METHODS: One hundred seventy eyes of 100 children with congenital cataract are operated by extracapsular cataract extraction with limbic incision. Posterior rhexis and anterior vitrectomy were associated in children under 2 years. Implantation was used in 42%, contact lens in 8%. Other cases were corrected by glasses. The prognostic factors studied were: uni- or bilaterality, partial or complete aspect of cataract, age of apparition and management, and association with other malformations. RESUL

Ciliary body is not hyperplastic in Weill-Marchesani syndrome.

PURPOSE: To demonstrate the in-vivo morphology of the ciliary body in Weill-Marchesani syndrome which might be of pathophysiological importance for the spherical shape of the crystalline lens. METHODS: Three juvenile patients with Marchesani syndrome were examined by ultrasound biomicroscopy of the anterior segment. RESULTS: The in-vivo configuration of the ciliary body in these Weill-Marchesani patients with normal axial length of the eye showed no signs of hyperplasia, and was even relatively small. CONCLUSION: Spherophakia and hyperplasia of the ciliary body are not necessarily associate

Ultrasonographic diagnosis of persistent hyperplastic tunica vasculosa lentis/persistent hyperplastic primary vitreous in two dogs.

Ultrasonography was performed on a Basset hound and a Doberman clinically suspected of persistent hyperplastic tunica vasculosa lentis/persistent hyperplastic primary vitreous. In both dogs, hyperechoic lenses with a triangular-shaped echodense structure retrolentally were visible, and a very thin hyperechoic strand was seen penetrating the anechoic vitreous from this retrolental tissue to the area of the optic nerve. Using color Doppler imaging, blood flow was evident in parts of the retina in both dogs. With power Doppler imaging there was blood flow in the lens and hyperechoic strand of the

Teratogen update: congenital rubella.

It is apparent that there are many unanswered questions about the pathogenesis of CRS. For instance, the chance of embryonic infection decreases in the second semester only to increase again in the third trimester. This is presumably due to unspecified changes in the placenta. When the embryo is infected early in the first trimester it does not appear to have any conventional immunological response to prevent spread of the virus. Yet it has been suggested that only 1 in 10(3) to 10(5) of its cells become infected. If this is true, what controls the spread of the virus in the early embryo? Why

Prenatal detection of congenital inherited cataracts.

In a high-risk pregnancy for ocular defects, routine second-trimester ultrasonography revealed a highly echogenic area within the fetal left eye with no other associated abnormalities. The diagnosis of congenital cataracts was confirmed after birth. Examination of the fetal face is an important part of prenatal ultrasonography, and visualization of the fetal eyes and lenses should be included in the routine assessment of facial anatomy. This is especially important in pregnancies at risk for congenital cataracts or for other anomalies affecting the fetal eye.

Isolation of a new homeobox gene belonging to the Pitx/Rieg family: expression during lens development and mapping to the aphakia region on mouse chromosome 19.

We recently reported the positional cloning of a homeobox gene involved in the pathogenesis of Rieger syndrome, RIEG1 , and its mouse homolog, Rieg1 . Rieg1 (also independently described as Pitx2) is highly homologous to the Ptx1/Potx gene product, suggesting that there may be additional members of this novel Pitx family. The Pitx genes play an important role in eye, tooth, pituitary and umbilical region development as evidenced by Rieger syndrome and iris hypoplasia phenotypes, resulting from mutations in the RIEG1 gene and by expression studies. In order to characterize further the Pitx gene

Persistent hyperplastic primary vitreous in male twins.

Persistent hyperplastic primary vitreous (PHPV) occurs sporadically and few familial occurrences have been reported. The authors report PHPV unassociated with other congenital anomalies in male twins. One underwent a lensectomy for the management of angle-closure glaucoma and the other was treated for amblyopia. These cases provide further evidence to suggest autosomal recessive inheritance, though the possibility of developmental error cannot be excluded.

Congenital aphakia: a clinicopathologic report of three cases.

BACKGROUND: Congenital aphakia is a rare condition that has been classified as primary when no lens induction of the surface ectoderm occurs and secondary when lens development takes place but later is resorbed or expelled in utero. METHODS: The authors report the clinical and pathologic findings in three infants with congenital aphakia whose eyes were enucleated either at surgery at 11 months or at autopsy after 1 and 3 days of life. RESULTS: Two cases classified as primary congenital aphakia had severe microphthalmos, anterior segment aplasia, or anomalous development and posterior choroid

Teratogen-induced eye defects mediated by p53-dependent apoptosis.

BACKGROUND: Many birth defects are believed to involve gene-environment interactions, although the mechanisms involved are poorly understood. Apoptosis is a common effect of many kinds of environmental stresses on the developing embryo; therefore, mechanisms of teratogenesis may be approached within the context of the cell death program. The p53 tumor suppressor gene encodes a transcription factor which functions as a critical regulator of apoptosis in response to environmental stress. RESULTS: To investigate the relationship between p53-dependent apoptosis and teratogenesis, we subjected day

Septo-optic dysplasia associated with bilateral complex microphthalmos.

An 8-month-old girl was examined because of corneal clouding and microphthalmos. The fundi of both eyes could not be visualized because of corneal clouding. Orbital and cranial computerized tomographic scanning and magnetic resonance imaging demonstrated bilateral microphthalmos and presumed retinal dysplasia, hypoplasia of the optic nerves and chiasm, agenesis of the septum pellucidum, thinning of corpus callosum, and a normal pituitary infundibulum. Cerebral cortex and white matter were unremarkable. Other ocular malformations were anterior segment dysgenesis in the right eye and congenital
