Microtubule binding protein Togaram1 is required for proper development of mammalian forebrain and neural primary cilia.

Proper forebrain development relies on precise spatial and temporal control of early neural stem cell (NSC) proliferation and later neurogenesis. Brain malformations can arise when these processes are defective. Joubert Syndrome (JS) is a neurodevelopmental disorder that is diagnosed by a mid-hindbrain malformation, but can also include forebrain defects such as microcephaly, which are less studied. One gene recently linked to Joubert Syndrome with microcephaly is Togaram1, which encodes a TOG domain microtubule binding protein shown to affect primary cilia. In the embryonic dorsal forebrain,

Clinical and functional characterization of a novel homozygous non-canonical splice mutation (c.1910-15_1910-11delinsTTACA) in CEP290 causing Joubert syndrome.

BACKGROUND: Joubert syndrome (JS) is a rare, predominantly autosomal recessive neurodevelopmental disorder characterized by hypotonia, motor delay, intellectual disability, oculomotor apraxia, and the hallmark "molar tooth sign" on axial view of MRI. JS is genetically heterogeneous, with pathogenic variants identified in more than 40 genes involved in primary cilia function. Among these, CEP290 is one of the most frequently mutated genes. RESULTS: In this study, we investigated two children-an 11-year-old boy (the proband) and his 5-year-old sister-both presenting with a similar phenotype con

Delayed recognition of Joubert syndrome in a child initially diagnosed with autism spectrum disorder without intellectual impairment.

Joubert syndrome (JS) is a rare ciliopathy characterised by the molar tooth sign on brain MRI, cerebellar vermis hypoplasia, hypotonia and developmental delay. We report an elementary school-aged boy initially diagnosed with autism spectrum disorder without intellectual impairment because of early language delay, social communication difficulties and stereotyped behaviours. Developmental screening at 4 years of age using the Denver Developmental Screening Test II demonstrated delays in language and social communication domains. At presentation to our clinic at 8 years of age, cognitive evaluat

Whole-Genome Landscape of Retinal Hemangioblastomas.

PURPOSE: Retinal hemangioblastomas (RHs) are rare benign vascular tumors that present mostly as manifestations of Von Hippel-Lindau (VHL) syndrome. In contrast to other VHL syndrome-associated tumors, only a handful of molecular studies have been published on RHs. It remains unclear whether VHL alterations alone drive RH tumorigenesis. METHODS: Whole-genome sequencing (WGS) was performed on germline- and RHs-derived DNA of five patients. Additionally, transcriptomes of four unaffected retinas and five choroid controls were compared with those of RH biopsies. RESULTS: Heterozygous germline mi

Neuro-ophthalmic disorders resulting from defects in the gamma tubulin ring complex: a clinically oriented review.

The gamma-tubulin ring complex (γ-TuRC) is a highly conserved and ubiquitously expressed complex necessary for proper microtubule nucleation and mitotic spindle function. While it is well established that the microtubule network plays a critical role in proper neurodevelopment, the clinical phenotypes associated with defects in the γ-TuRC have only been characterized recently. Generally, the neurologic features associated with γ-TuRC defects include microcephaly associated with chorioretinopathy (MCCRP), lissencephaly, cerebellar atrophy, motor and speech delay, and intellectual disability wit

PAX6-associated aniridia and RPGR-related X-linked retinitis pigmentosa: a rare dual Mendelian molecular diagnosis.

An exceptionally rare case of dual Mendelian molecular diagnosis is presented in a young male with both PAX6-associated congenital aniridia and RPGR-related X-linked retinitis pigmentosa. The patient presented in childhood with photophobia, iris hypoplasia, foveal hypoplasia on optical coherence tomography and reduced multifocal electroretinography amplitude, consistent with PAX6-related aniridia confirmed by familial variant testing. In early adulthood, progressive nyctalopia, visual field constriction and widespread peripheral pigmentary retinopathy emerged. Whole-exome sequencing revealed a

A HaloTag Knock-In Resource for In Vivo and In Vitro Analysis of Endogenous Polycystin-2 Localization, Turnover, and Transport.

KEY POINTS: A HaloTag knock-in resource allows direct visualization of endogenous polycystin-2 (PC2), enabling quantitative analysis of its localization, turnover, and transport dynamics. PC2-HaloTag labeling establishes PC1-dependent and Tulp3-dependent control of PC2 ciliary targeting in vivo and in cells. BACKGROUND: Polycystin-2, encoded by PKD2 , is a cation channel essential for kidney physiology. Dysfunction of Pkd2 causes autosomal dominant polycystic kidney disease. Currently, our understanding of cystogenesis in the kidney is limited by the difficulty of visualizing the localization

Residual risk of clinically significant copy number variations in fetuses with ultrasonographic soft markers following exclusion of non-invasive prenatal screening-detectable findings.

OBJECTIVE: To evaluate the residual risk of clinically significant copy number variations (CNVs) in fetuses with ultrasonographic soft markers by its various types after excluding theoretically non-invasive prenatal screening (NIPS)-detectable abnormalities. METHODS: This study included 2005 fetuses with soft markers undergoing chromosomal microarray analysis (CMA) between 2015 and 2024. Theoretically NIPS-detectable findings were categorized into three subgroups: 3-chromosome NIPS, 5-chromosome NIPS, and genome-wide NIPS. The residual risk was calculated and compared with a low-risk control

Chromatin remodelling subunit SMARCB1 is implicated in dendrite development and complex brain functions.

SMARCB1 encodes a core component of the BAF chromatin remodelling complex and pathogenic variants in this gene are associated with neurodevelopmental disorders such as Coffin-Siris syndrome and intellectual disability with choroid plexus hyperplasia. The relationship between reduced or dysfunctional SMARCB1 protein products and severe functional brain changes associated with Coffin-Siris syndrome, including intellectual disability and severely delayed language and motor development, remains largely unknown. We performed cellular, molecular, and behavioural analyses of a Coffin-Siris syndrome m

Ophthalmic features of WAGR syndrome: strategies for timely diagnosis, surveillance and improved quality of life.

A toddler presented with photophobia and difficulty in focusing on objects since the age of 2 months. Further examination revealed complete aniridia, horizontal jerk nystagmus and a history of genitourinary anomalies. The suspicion of WAGR syndrome, a rare genetic disorder, was raised. Genetic testing confirmed a pathogenic deletion associated with WAGR syndrome, a rare contiguous gene deletion disorder characterised by aniridia, an increased risk of Wilms tumour, genitourinary abnormalities and variable neurodevelopmental involvement. At the most recent follow-up, neurodevelopmental evaluatio

Clinical presentation of hydrocephalus following pre- and postnatal myelomeningocele repair: a Hydrocephalus Clinical Research Network study.

OBJECTIVE: Despite improvements in rates of hydrocephalus with prenatal repair of myelomeningocele (MMC), a significant number of children still require CSF diversion. While previous studies have focused on uncovering predictors of future hydrocephalus, differences in timing and presentation of MMC-associated hydrocephalus following pre- and postnatal surgery are not well-characterized. This study aimed to determine how age, head size, and ventricle size differ at hydrocephalus presentation in patients treated with pre- and postnatal surgery for MMC. METHODS: The Hydrocephalus Clinical Resear

Choroid plexus cyst of the spinal cord presenting with signs of spinal cord compression in a 55-year-old male: a case report.

BACKGROUND: Choroid plexus cysts are benign, typically asymptomatic cystic lesions found almost exclusively in the brain's lateral ventricles, often noted during second-trimester fetal ultrasounds and usually resolving spontaneously before the 28th week of gestation. Spinal intramedullary choroid plexus cysts remain extremely rare, with only few cases documented in  the literature to date. This case describes a 55-year-old man with thoracic spinal cord compression due to a choroid plexus cyst, highlighting a rare differential diagnosis for adult spinal cord lesions. CASE PRESENTATION: A 55-ye

Natural history and prognostic significance of fetal choroid plexus cysts: a systematic review.

OBJECTIVES: Fetal choroid plexus cysts (CPCs) are common incidental findings during mid-trimester ultrasound. Although typically benign, their detection often causes parental concern because of potential associations with chromosomal abnormalities, particularly trisomy 18. This systematic review summarizes current evidence on the natural history, associated anomalies, and prognostic implications of CPCs to guide prenatal counseling. METHODS: We systematically searched PubMed, Scopus, Web of Science, Embase, and the Cochrane Library for original studies published between 2010 and 2025. Eligibl
