Immunopharmacological mechanisms underlying SARS-CoV-2 spike-induced persistent pain in mice.

SARS-CoV-2 infection is frequently associated with persistent pain, yet the underlying mechanisms remain poorly understood. Given that viral structural proteins can act as pathogen-associated molecular patterns, we investigated whether the Spike protein is sufficient to induce long-lasting nociceptive alterations and the immune pathways involved. Intraplantar administration of recombinant Spike protein in mice elicited robust and persistent mechanical and thermal hypersensitivity, accompanied by transient local inflammation, sustained systemic cytokine alterations, and prolonged spinal neuroin

Screening of anti-metastasis drugs by targeting angiopellosis and cancer cluster extravasation.

Metastasis accounts for 90% of cancer-related deaths. Extravasation is a necessary step for cancer metastasis. Currently, there are no drugs that specially target extravasation. Most cancer therapies target either proliferation or angiogenesis. We previously identified "angiopellosis" as the dominant mechanism by which vascular endothelial cells undergo conformational changes and actively "push" circulating cancer cells out of blood vessels. In this study, we developed an image-based high-throughput drug screening assay by coculturing cancer membrane-coated spheres with endothelial monolayers.

Association of ACE2 gene polymorphisms with risk of pulmonary arterial hypertension in neonates with congenital heart disease.

BACKGROUND: Studies have confirmed a correlation between angiotensin-converting enzyme 2 (ACE2) gene polymorphisms and the risk of hypertension; however, its correlation with congenital heart disease (CHD)-related-pulmonary arterial hypertension (PAH) risk in neonates has not been reported. METHODS: The study enrolled 321 Han Chinese neonates, comprising 113 healthy controls and 208 patients with left-to-right shunt CHD. Among the CHD patients, 98 cases were classified as the PAH subtype [CHD PAH (+)]. Tag SNP genotyping was performed using Sanger sequencing. Associations between three ACE2 S

Prolonged benzo[b]fluoranthene exposure increases tissue-specific mutational burden, expansion of mutant cells, and mutagenic potency.

Chronic exposure to polycyclic aromatic hydrocarbons (PAHs) is associated with increased risk of cancer through a mutagenic mode of action. We investigated how exposure duration influences mutagenicity in bone marrow (BM) and liver of MutaMouse males orally exposed to increasing doses of benzo[b]fluoranthene (BbF), a priority PAH, for 28, 90, or 180 days. We applied Duplex Sequencing (DS) across 20 endogenous loci to assess dose- and time-dependent effects on mutation frequency, clonal expansion of mutant cells, mutation distribution, and spectra. Mutation frequencies increased with BbF dose a

Cardiorespiratory adaptations to high-intensity interval training in young boxers: A randomized controlled study.

BACKGROUND: High-intensity interval training (HIIT) has been recognized for its effectiveness in enhancing cardiorespiratory fitness. However, the application of HIIT within boxing training remains underexplored. This study aimed to investigate the physiological adaptations induced by a structured 4-week HIIT-boxing regimen, with a specific focus on pulmonary function (PF), aerobic capacity, and autonomic cardiovascular regulation, assessed by heart rate recovery (HRR). METHODS: Sixteen trained young individuals were randomly assigned to either the HIIT-boxing group or the control group. Comp

Predicting first falls among older adults with chronic conditions and polypharmacy using routinely collected health records.

BACKGROUND: Falls represent a major clinical and financial challenge for healthcare systems. Accurately predicting first falls remains challenging, especially when using routinely collected data. OBJECTIVE: Develop and evaluate a predictive model to identify elderly people at risk of first fall-defined as a fall after a 90-day fall-free period-in the Basque Country using routinely collected health records. METHOD: A retrospective study included patients aged ≥65 with at least two chronic conditions among heart failure, chronic obstructive pulmonary disease (COPD), and diabetes. Data on demog

Objective Return-to-Sport (RTS) Decision Frameworks for Adolescent Shoulder and Elbow Injuries: Tests, Thresholds and Exercise Targets.

BACKGROUND: Adolescent shoulder and elbow injuries are increasing due to sports specialization and year-round training, yet return-to-sport decisions typically rely on adult-derived heuristics that fail to address the unique physiology, growth plate vulnerability, and recovery needs of developing athletes aged 10-19 years. OBJECTIVE: This review proposes a novel, objective three-pillar RTS framework integrating: (1) foundational tissue capacity, (2) dynamic movement quality and neuromuscular control, and (3) sport-specific load tolerance. We synthesize current evidence to establish provisiona

Development of Oligo-PROTACs That Target C/EBPα.

CCAAT/enhancer-binding protein alpha (C/EBPα) is a critical lineage-defining transcription factor and tumor suppressor. Despite its clinical significance in conditions like acute myeloid leukemia and hepatocellular carcinoma (HCC), C/EBPα has remained undruggable due to its intrinsically disordered regions and lack of small-molecule binding pockets. In this study, we describe the development of a novel Oligonucleotide-based PROteolysis-TArgeting Chimera (Oligo-PROTAC or O'PROTAC) strategy designed to achieve targeted degradation of endogenous C/EBPα. We designed and synthesized a 10-nucleotide

Nanomedicine in Lung Transplantation: Emerging Solutions for Clinical Challenges.

Lung transplantation (LT) remains the definitive treatment for select patients with end-stage respiratory failure. Despite refinements in donor selection, organ preservation, and perioperative management, LT is still associated with the shortest median survival among all solid organ transplants. Major clinical bottlenecks include airway complications (AC), ischemia-reperfusion injury (IRI), organ rejection and chronic lung allograft dysfunction (CLAD), and suboptimal donor organ quality. Current management is constrained by delayed diagnosis, anatomical delivery barriers, and the off-target to

Living donor outcomes in an emerging living donor liver transplantation program: A single-center experience from Malaysia.

BACKGROUND: Living donor liver transplantation (LDLT) has become an essential strategy to address the shortage of deceased donor organs in Malaysia. Universiti Malaya Medical Centre (UMMC), the second public institution in the country to establish a liver transplantation program, initiated its adult LDLT program in 2017 and became the first center in the country to successfully perform a living-related adult right lobe LDLT. However, data on donor outcomes from emerging LDLT programs remain limited. AIM: To evaluate donor outcomes and perioperative results of an emerging LDLT program in Malay

Why Organised Cancer Screening Works: Lessons from Slovenia and the Next Phase of Evidence-Based Implementation.

Slovenia's experience shows that cancer screening becomes effective only when a test is embedded in an organised, population-based programme that reaches the target population equitably, assures quality across the whole pathway and uses individual- and programme-level data for improvement. Since the national roll-out of Programme ZORA for cervical cancer screening in 2003, followed by DORA for breast cancer screening in 2008 and Programme Svit for colorectal cancer screening in 2009, Slovenia has built a coherent screening infrastructure with visible population effects. Cervical and colorectal

The pan-tumor landscape, allelic status, and genomic complexity of SMARCA4 alterations.

PURPOSE: The clinical implications of distinct SMARCA4 alteration classes remain incompletely defined. We performed a pan-cancer analysis to characterize SMARCA4 mutation classes and their associations with allelic status, genomic context, and therapeutic outcomes. PATIENTS AND METHODS: We analyzed 68,920 tumor-normal paired samples sequenced with the MSK-IMPACT next-generation sequencing assay between 2015 and 2023. Oncogenic or likely oncogenic SMARCA4 alterations were curated using OncoKB and classified based on prior functional and structural literature. Allele-specific copy number, ploid

Loss of Sox10 prevents tumor initiation in vivo and induces luminal-to-basal reprogramming in Neu+ tumor cells.

The SRY-HMG-Box transcription factor SOX10 plays a critical role in neural crest development, but its function in epithelial tumorigenesis remains unclear. Here, we identify SOX10 as a key regulator of tumor-initiating activity in Neu-driven mammary cancers. Genetic ablation of Sox10 in the luminal compartment of MMTV-Neu (NIC) mice resulted in delayed but normal mammary gland development. Sox10 deletion resulted in a reduction in mammary progenitors and a complete loss of tumor initiation in Sox10-deficient luminal cells. CRISPR/Cas9-mediated Sox10 inactivation in Neu-transformed tumor cells

Factors associated with unplanned readmission after elective gastroenterological surgery: An analysis of 1,824 participants with clean-contaminated wounds enrolled in two randomized controlled trials for the prevention of surgical site infection.

BACKGROUND: Unplanned readmission is a fundamental metric of quality of care associated with significant clinical and financial burden. Surgical site infection (SSI) is one of the most common clinical indications for unplanned readmission following surgery. This study assessed factors associated with unplanned readmission within 30 days post-discharge after elective gastroenterological surgery under the implementation of SSI prevention bundles. METHODS: A total of 1,973 patients enrolled in two previous randomized controlled trials to evaluate the efficacy of intraoperative interventions for

Deep Learning-Driven Discovery and Engineering of an Efficient PETase for Depolymerization and Detoxification of PET Microplastics Under Physiological Conditions.

Microplastics (MPs) accumulation in ecosystem and human organs poses urgent environmental and health risks, yet few enzymes efficiently degrade polyethylene terephthalate (PET) under physiological conditions. We leveraged deep learning to mine unexplored sequence space across 246 million proteins, discovering AhPETase, an evolutionarily distinct hydrolase with low homology (<50% sequence identity) to known PET-degrading enzymes. This noncanonical biocatalyst efficiently depolymerizes PET at 37°C, outperforming all typical PETases and achieving a 7.76-fold enhancement over IsPETase, one of the

Age-Opposed Rewiring of Upstream Translation-Initiation Pattern Across Different Types of Human Cancers.

The 5' mRNA leader regulates translation through its length and upstream open reading frames (uORFs), but its systematic remodeling in cancer remains unclear. We analyzed isoform-weighted 5' leader features across The Cancer Genome Atlas, the Genotype-Tissue Expression transcriptomes and the Clinical Proteomic Tumor Analysis Consortium proteogenomic cohorts. Tumor-associated shifts frequently opposed normal tissue aging trajectories, with the clearest signals in lung adenocarcinoma and lung squamous cell carcinoma. Raw closed-uORF burden defined the sharpest paired-supported lung cores, wherea

Extracellular vesicles in pharmacology: Innovations in drug delivery and therapeutic applications in cancer.

Extracellular vesicles (EVs) are a diverse population of membrane nanoparticles secreted by nearly all cell types, playing a key role in intercellular communication by transferring bioactive macromolecular cargo. In cancer, EVs shape both the local tumour microenvironment and distant premetastatic niches. They are essential regulators of tumour initiation, progression, immune modulation, angiogenesis and metastatic dissemination. Due to their abundance in biofluids, EVs have attracted attention as diagnostic and prognostic biomarkers for early detection and assessment of therapeutic response.

Prospective multi-centre evaluation of guideline-based artificial intelligence to streamline multidisciplinary tumour board.

INTRODUCTION: Multidisciplinary tumour board (MTB) or tumour board (TB) is the "gold standard" for providing oncological patients' diagnosis and treatment. Often, MTBs are time-intensive, capacity-constrained, and absent or inconsistent in many routine hospitals. Despite that, in MTB, a few experienced specialists in the oncological field, sometimes the final decision is mistaken (4-47%), the necessity of treatment plan changing (12-64%), etc. Using artificial intelligence (AI) for cancer patients' treatment is a modern approach, but the main idea of our investigation should be to possibly rep

Metyrapone in the Management of Severe Hypercortisolism Secondary to Ectopic Cushing's Syndrome.

Ectopic Cushing's syndrome (ECS) is a rare but important cause of ACTH-dependent Cushing's syndrome. Metyrapone, a steroidogenesis inhibitor targeting 11-β hydroxylase, has shown effectiveness in controlling hypercortisolism. However, little is known about the practical aspects of its initiation, use, and effectiveness in ECS. We present three cases of severe hypercortisolism secondary to ECS managed with metyrapone and report our experience. Case 1: a 56-year-old male with high-grade neuroendocrine carcinoma. Case 2: a 67-year-old man with a history of stage IIB non-small cell lung cancer (NS

Bilateral pulmonary embolism in anti-NMDA receptor encephalitis presenting as acute psychosis.

BACKGROUND: Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is an autoimmune disorder that frequently presents with prominent psychiatric manifestations, often leading to misdiagnosis as a primary psychiatric illness. Early recognition is critical, as timely immunotherapy significantly improves outcomes. CASE PRESENTATION: A 26-year-old previously healthy man presented with acute psychosis following significant emotional stress and first-time marijuana use 3 days earlier. Initially diagnosed with brief psychotic disorder, he developed catatonia and fever. Cerebral spinal fluid (CSF) an
