Choroidal, optic disc and crystalline lens colobomas are rare ophthalmological conditions, but they can (either directly or by association) present with retinal detachments that are difficult to manage. Due to their rarity and variability of presentation, evidence of different approaches to treatment strategies is mainly found in case reports or small case series. This article reviews the basic pathogenesis of these conditions and the authors' experience in how various treatment options address the underlying mechanisms. Sub-categories of retinal detachments complicating choroidal coloboma and

PURPOSE: To reassess the anatomical basis of optic disc pit maculopathy using swept source optical coherence tomography and to characterize the broader structural abnormalities comprising the optic pit complex.
METHODS: Sixteen patients with optic disc pit maculopathy were imaged using a high-resolution swept source optical coherence tomography system (DREAM optical coherence tomography). Cross-sectional and volume-rendered scans were analyzed for lamina cribrosa defects, intraneural cavitations, and pathways for fluid entry into or beneath the retina.
RESULTS: All eyes demonstrated lamina c

We report 3 pediatric cases of persistent fetal vasculature (PFV) and myelinated retinal nerve fibers (MRNF) coexisting in the same eye. All cases presented with congenital cataracts, vitreous strands, and an unusual axial length (AL) profile-specifically, the affected eyes exhibited longer ALs than their fellow eyes, contrary to typical PFV patterns. Preoperative Color Doppler imaging consistently revealed a linear echogenic band extending from the optic disk to the posterior lens capsule. Intraoperative and postoperative fundus examinations confirmed the presence of MRNF in each case, with v

Congenital optic disc anomalies are uncommon causes of vision loss that may be recognized shortly after birth but may also be identified in later childhood and sometimes well until adulthood. They may be unilateral or bilateral and isolated or associated with other systemic findings. In some conditions the origin of the developmental abnormality is understood as in the case of optic disc colobomas resulting from failure of closure of the embryonic fissure whereas in others the cause is hypothetical. Recognition of these anomalies is usually by direct observation though ancillary testing may ai

BACKGROUND: Optical coherence tomography (OCT) can be a valuable tool for non-invasively monitoring the optic nerve status in children with craniosynostosis. However, it is currently unknown whether optic nerve parameters derived from handheld OCT are comparable to those derived from table-mounted OCT, which is more widely used. This study aims to assess the feasibility and reproducibility of handheld and table-mounted OCT in craniosynostosis.
METHODS: This was a cross-sectional study conducted at Great Ormond Street Hospital (GOSH), London. Twenty children aged 4-18 years with a clinical/gen

BACKGROUND: To report a rare case of unilateral hybrid optic disc anomaly characterized by the coexistence of optic disc pit and optic disc coloboma in the absence of maculopathy, evaluated with multimodal imaging including optical coherence tomography angiography (OCTA). CASE PRESENTATION: A 42-year-old asymptomatic woman was found to have coexisting optic disc pit and temporal optic disc coloboma in the right eye. Visual acuity was normal, and no structural or clinical evidence of maculopathy was detected. Optical coherence tomography (OCT) demonstrated preserved macular architecture. Fluore

BACKGROUND: Orbital apex syndrome (OAS) is a rare, sight-threatening condition caused by injury or compression of neurovascular structures in the orbital apex, which, depending on the underlying etiology, may even pose a life-threatening risk. Prompt recognition and intervention are essential to prevent permanent visual loss and systemic complications.
CASE PRESENTATION: We report a 10-year-old child presenting with acute left-sided proptosis, eyelid edema, ophthalmoplegia, and optic disc edema, following a history of orbital cellulitis and meningitis in early childhood. Imaging revealed a mi

A 22-year-old woman had an 8-year history of progressive bilateral vision loss and of diabetes mellitus. Her mother had diabetes and two first cousins had severe congenital deafness. On examination, her visual acuities were 6/36 bilaterally, with absent colour vision and gross optic disc pallor. Her HbA1c was 94 mmol/mol (20-42). Visual evoked potentials were bilaterally symmetrically prolonged with P100 140 ms. Optical coherence tomography showed significant thinning of the peripapillary retinal nerve fibre layer. MR scan of brain and orbits showed small optic nerves and chiasm. Genetic testi

Chiari type I malformation (CMI) is a congenital hindbrain anomaly that may disrupt cerebrospinal fluid (CSF) dynamics and lead to intracranial hypertension. Although papilledema is a well-recognized manifestation of elevated intracranial pressure, it is typically bilateral and symmetric. Asymmetrical papilledema is uncommon, particularly in pediatric patients, and may pose a diagnostic challenge. We report a 10-year-old boy with no neurological or visual complaints who was referred after asymmetrical optic disc edema was detected during a routine ophthalmic examination. Visual acuity was pres

BACKGROUND: Arachnoid cysts (ACs) are the most common brain cysts, comprising 1% of intracranial lesions, with a higher prevalence in children (2.6%) than adults (0.3%-1.7%). Intraorbital ACs are extremely rare, with only a few cases reported to date, especially in children. While most ACs do not exhibit any symptoms, some cases of symptomatic ACs require surgical intervention to prevent brain damage. This article presents a unique case of a large intraorbital AC with preserved visual acuity in a 3-year-old female with a concise literature review of all intraorbital AC cases reported to date.

INTRODUCTION: Hydrocephalus is an etiologically heterogeneous disorder that may result from acquired insults or monogenic causes. More than 100 genes have been implicated in congenital and early-onset hydrocephalus, but MAP7D3 has not previously been associated with disturbances of cerebrospinal fluid (CSF) circulation. We report an adult patient with obstructive hydrocephalus in whom a novel frameshift variant in MAP7D3 was identified by exome sequencing.
CASE PRESENTATION: A 38-year-old woman presented with a 6-month history of progressive predominantly occipital headache without nausea, vo

This retrospective case series presents the characteristic features of superior segmental optic nerve hypoplasia (SSONH) in four patients who were initially misdiagnosed with normal-tension glaucoma (NTG), aiming to raise awareness of this rare condition in Türkiye. Four patients (two females and two males) with a mean age of 38 years were included. All were initially diagnosed with NTG and treated with brimonidine drops for three years. Comprehensive ophthalmological examinations were performed, including optic disc photography, optical coherence tomography retinal nerve fiber layer (RNFL) an

We report a very rare autosomal recessive metabolic disorder in monozygotic twin sisters caused by the steroid 5a-reductase type 3 (SRD5A3) gene defect, a subtype of congenital disorder of glycosylation (CDG). SRD5A3 activity is required for N-glycosylation of proteins. This step is important for the protein to gain its function. The condition is characterized by severe neurodevelopmental delay, cerebellar atrophy or hypoplasia, ocular abnormalities, and ichthyotic skin changes. We describe 20-month-old female monozygotic twins born to non-consanguineous South Asian parents. Notably, both twin

Wolfram syndrome is a rare childhood neurodegenerative disease characterized by diabetes followed by severe and rapid optic atrophy leading to blindness before the age of 20. Patients often develop other symptoms, such as deafness and neurological dysfunction. Wolfram syndrome is caused by mutations in the WFS1 gene, which encodes wolframin protein. Despite decades of intensive research, the complex mechanisms of optic neuropathy are not fully understood, and there are currently no therapies to prevent vision loss in Wolfram patients. Here, we showed that the Wfs1 knockout mice produced by the

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm
