Impact of fetal myelomeningocele repair on the clivus-supraocciput angle and third ventricle anatomy: evaluation of the outcome of endoscopic third ventriculostomy and choroid plexus cauterization.

OBJECTIVE: Fetal myelomeningocele repair (FMMR) has shown significant promise in decreasing the incidence of myelomeningocele (MMC)-associated hydrocephalus. In this study, the authors report on the reversal of hindbrain herniation via FMMR. They also describe biometric changes in third ventricle anatomy, which influences the outcome of endoscopic third ventriculostomy and choroid plexus cauterization (ETV/CPC). METHODS: At the Arnold Palmer Hospital for Children and Winnie Palmer Hospital for Women & Babies, the maternal and fetal inclusion and exclusion criteria developed by the Management

Adult presentation of a vein of Galen malformation as cognitive decline in a 47-year-old multiparous female: illustrative case.

BACKGROUND: Vein of Galen malformations (VOGMs) are congenital arteriovenous shunts that typically present in neonates and infants. Symptomatic neonates have a poor prognosis and face high mortality without dedicated neonatal intensive care and pediatric neurointervention in specialized centers. OBSERVATIONS: The authors present the case of a 47-year-old Puerto Rican female with a history of chronically compensated heart failure, progressive cognitive decline, and new-onset seizures. On presentation, the patient had a Glasgow Coma Scale score of 6, and CT imaging revealed intraventricular hem

Hydranencephaly in Neonate With Prenatal Exposure to Alcohol During Pregnancy.

Hydranencephaly (HE) is a rare congenital condition characterized by near-total absence of the cerebral hemispheres, replaced by cerebrospinal fluid, most commonly resulting from in utero bilateral internal carotid artery occlusion; although maternal alcohol consumption is associated with fetal neurodevelopmental abnormalities, its direct role in HE remains unclear. We report a case of a 42-year-old multiparous woman with chronic alcohol use throughout pregnancy who delivered a female neonate at 40 + 5 weeks via spontaneous vaginal delivery. The neonate presented with facial asymmetry and a bo

Neuroendoscopic treatment of a rapidly enlarging choroidal fissure cyst: a case report and literature review.

Choroidal fissure cysts (CFCs) are rare cystic lesions arising in the region of the choroidal fissure. Most are discovered incidentally and remain clinically silent. Rapid interval enlargement with associated mass effect is distinctly uncommon. We report an infant with a rapidly enlarging choroidal fissure cyst. A cystic lesion in the choroidal fissure was first detected on neuroimaging at 6 months of age. Follow-up imaging at 10 months demonstrated marked cyst enlargement with compression of adjacent brain tissue. The patient subsequently underwent neuroendoscopic cyst fenestration with fistu

Bilateral fornix bodies and posterolateral thalamus ischemia, septum pellucidum cyst.

Purpose: Bilateral damage of fornix bodies has been reported in a few cases of brain tumors and in exceptional cases of ischemia. Methods: a 73-year-old man presented with acute right ataxic hemiparesis and right hypoesthesia. Brain MRI showed an acute ischemia of both fornix bodies and of the left posterior lateral thalamus, along with a septum pellucidum (SP) cyst. MR angiography revealed a 7-mm long stenosis of the initial P2 of the left posterior cerebral artery. Neuropsychological evaluation documented an amnestic syndrome. Fornix bodies and posterolateral thalamus are supplied by the med

Reduced choroid plexus volume aggravates the glymphatic system burden in patients with patent foramen ovale-related migraine with aura.

OBJECTIVE: The choroid plexus (ChP) produces cerebrospinal fluid (CSF) and serves as a crucial component of the glymphatic system. This study investigated the characteristics of choroid plexus volume (CPV) changes in patients with patent foramen ovale-related migraine with aura (PFO-M), analyzed its correlation with migraine severity (including both visual analogue scale [VAS] and monthly migraine frequency [MMF]), and elucidated the mechanism of chronic long-term injury on ChP as well as the therapeutic significance of PFO closure. METHODS: A prospective study was conducted to enroll 76 pati

Neuron-specific deletion of ADAR1 induces brain malformation and early postnatal lethality.

Adenosine deaminase acting on RNA 1 (ADAR1), which mediates adenosine-to-inosine RNA editing, is expressed as two isoforms, p110 and p150. Deletion of Adar1 p150 in mice results in embryonic lethality caused by aberrant activation of melanoma differentiation-associated protein 5 (MDA5)-mediated sensing of unedited endogenous transcripts, whereas Adar1 p110-specific deficient mice die postnatally through RNA editing-independent mechanisms. ADAR1 mutations cause Aicardi-Goutières syndrome (AGS), a congenital autoinflammatory disease accompanied by encephalopathy with a type I interferon (IFN) si

First-trimester fetal neurosonography.

First-trimester ultrasound has evolved beyond basic screening to permit structured neurosonographic assessment of the developing fetal brain. Historically, detailed evaluation of intracranial anatomy was deferred to the mid-trimester, based on assumptions regarding developmental maturity and technical limitations. However, advances in imaging technology, improved understanding of early neuroembryology, and refinement of standardized scanning planes have shifted this paradigm. Major structural anomalies that disrupt early cranial morphology, including acrania-exencephaly sequence, holoprosencep

Anatomical and clinical factors associated with multiple-stage embolization of brain arteriovenous malformation as a stand alone treatment.

INTRODUCTION: Embolization is one of the modalities used to treat brain arteriovenous malformations (bAVMs), which often require a multiple-stage approach. Factors influencing the need for a multiple-stage embolization are poorly defined and not very well understood. The aim of this study was to establish those factors. PATIENTS AND METHODS: We analysed 117 consecutive patients undergoing embolization due to bAVM with intention to treat. Patients undergoing embolization prior to surgery and with history of previous interventions or stereotactic radiosurgery were excluded. We used univariate a

Multimodal imaging and intravitreal faricimab for polypoidal choroidal vasculopathy associated with a choroidal nevus in genetically confirmed Usher syndrome type 2: a case report.

BACKGROUND: Usher syndrome is an autosomal recessive disorder characterized by retinitis pigmentosa (RP), sensorineural hearing loss, and vestibular dysfunction. Polypoidal choroidal vasculopathy (PCV) is characterized by branching neovascular networks and polypoidal lesions. While typically classified within the pachychoroid spectrum, PCV can also manifest in eyes without choroidal thickening. Conversely, RP is usually associated with choroidal thinning, which may lead to underdiagnosis of PCV. To the best of our knowledge, PCV has not previously been reported in patients with Usher syndrome.

Translational animal model for genetic predisposition to anophthalmia/microphthalmia.

Anophthalmia and microphthalmia (A/M) are severe congenital malformations that are responsible for ∼11% of childhood blindness. Despite clinical advances, approximately 70-80% of human A/M cases lack a molecular diagnosis. This study aims to characterize a spontaneous, non-syndromic model of the microphthalmia, anophthalmia, and coloboma (MAC) spectrum in the laboratory opossum (Monodelphis domestica), providing a novel experimental opportunity for embryonic developmental and ocular pathology research, as well as translational research into preventions and treatments of these conditions. Unravelling Cholera's Footprints in India: A Data-Driven Investigation of its Burden, Risk, and Strain Variability.

INTRODUCTION: Binge Eating Disorder (BED) has high lifetime prevalence rates, low treatment success rates, and high rates of treatment dissatisfaction, early discontinuation of care, and recurrence. Complementary and integrative health (CIH) interventions (non-mainstream practices used with conventional approaches for whole-person treatment) hold potential to overcome many treatment barriers and improve BED treatment outcomes. Some CIH interventions have empirical support for use in eating disorders. However, little is known about the current state of CIH use in BED. METHODS: This mixed-metho

Congenital HSV-2 infection with atypical skin lesions, chorioretinitis, and neurodevelopmental sequelae: a case report.

BACKGROUND: Neonatal herpes simplex virus (HSV) infection, primarily caused by HSV-2, carries a significant risk of severe morbidity, including neurological impairment and developmental delays, without timely diagnosis and intervention. The early identification of atypical cutaneous manifestations at birth and the prompt empirical initiation of high-dose intravenous acyclovir in suspected cases are crucial to prevent systemic dissemination and improve outcomes. Nevertheless, due to the unique neonatal immune response and limitations of current therapies, significant long-term sequelae such as
