Hybrid cavitary disc anomaly - A case series and proposal of a novel classification system.

BACKGROUND: Cavitary disc anomalies, including optic disc pit (ODP), optic nerve coloboma (ODC), and morning glory disc anomaly (MGDA), are congenital conditions with distinct embryological origins and systemic associations. Hybrid features combining these anomalies are rare, complicating diagnosis and management. PURPOSE: To describe the clinical spectrum, multimodal imaging characteristics, and management strategies of hybrid cavitary disc anomalies, and to propose a novel classification system based on anatomical and physiological overlap. METHODS: This retrospective case series included

CEP290 deficiency disrupts ciliary axonemal architecture in human iPSC-derived brain organoids.

Primary cilia are ubiquitous sensory organelles mediating various signaling modalities essential for development and cell homeostasis. Their dysfunction leads to ciliopathies, human disorders often affecting the central nervous system. CEP290 is a major ciliopathy-associated gene that encodes a centrosomal and ciliary transition zone protein. CEP290 has been implicated in different cellular functions, including cell cycle control, ciliogenesis and control of ciliary membrane protein content. To investigate CEP290 dysfunction in human neurons, we generated human induced pluripotent stem cell (i

Endoscopic third ventriculostomy versus ventriculoperitoneal shunting for hydrocephalus treatment following prenatal and postnatal myelomeningocele repair.

PURPOSE: It is not known how responses to endoscopic third ventriculostomy (ETV) and ventriculoperitoneal shunting (VPS) for myelomeningocele (MMC)-associated hydrocephalus vary with timing of MMC repair. Therefore, we compare the use of ETV and VPS after prenatal and postnatal MMC repair. METHODS: We retrospectively identified MMC patients who underwent prenatal or postnatal repair and VPS or ETV ± choroid plexus cauterization (CPC) between 2015 and 2024 at Washington University in St. Louis. Need for shunt or ETV revision (defined as surgical revision of shunt or ETV) and differences in pre

MAPK signaling and angiopoietin-2 contribute to endothelial permeability in capillary malformations.

Nonsyndromic capillary malformations (CM) are seen predominantly in skin. In Sturge-Weber Syndrome (SWS), CMs occur in the skin, leptomeninges of the brain, and choroid of the eye. >90% of CM are caused by a somatic mutation-GNAQ p.R183Q, the gene encoding the G-protein subunit Gαq. Longitudinal MRI of the brain in one SWS patient suggests developing vascular permeability. We modeled this in a transendothelial electrical resistance assay and found endothelial cells with GNAQ p.R183Q (EC-R183Q) exhibited increased permeability compared to EC wild-type. Increased vascular permeability was confir

Longitudinal Neuroimaging of a Pediatric Patient With Sturge-Weber Syndrome: From Birth to Adolescence.

Sturge-Weber syndrome (SWS) is a rare congenital neurocutaneous disorder characterized by facial port-wine stains, leptomeningeal angiomas, and various ocular abnormalities. Serial neuroimaging provides critical insights into disease progression. We report a longitudinal case of a female patient with SWS followed from one month to 12 years of age. Imaging demonstrated leptomeningeal angiomas, progressive underlying brain atrophy, cortical calcifications, choroid plexus enlargement, and hemi-calvarial hypertrophy correlating with the patient's clinical deterioration and seizure activity. This c

Prenatally diagnosed chromosome 1p36 deletions: a retrospective case series, literature review, and genotype-phenotype correlations.

BACKGROUND: Chromosome 1p36 deletion syndrome is the most common terminal autosomal deletion disorder. While postnatal features are well-defined, prenatal characterization remains less comprehensive. This study aims to delineate the prenatal sonographic and genetic spectrum of this syndrome and explore genotype-phenotype correlations. METHODS: A retrospective analysis was conducted on 21 consecutive prenatally diagnosed cases of 1p36 deletion (2017–2025) from a single tertiary center. Data on maternal demographics, ultrasound findings, genetic results, inheritance patterns, and pregnancy outco

The Role of genetic sonography in chromosomal anomaly detection in low-resource settings: A case report of Edwards syndrome.

Edwards syndrome (trisomy 18) is a severe chromosomal disorder associated with multiple congenital anomalies and high perinatal mortality. In low-resource settings where advanced prenatal screening and genetic testing are limited, genetic sonography plays a pivotal role in early detection. We report the case of a 37-year-old primigravida who presented at 20 weeks' gestation with multiple fetal anomalies detected on second-trimester ultrasound, including bilateral choroid plexus cysts, a strawberry-shaped skull, a ventricular septal defect, and persistently clenched fists. These hallmark findin

Effective bilateral neuroendoscopic approach for managing intermittent triventricular hydrocephalus with septated choroid plexus cysts of the third ventricle: a case report and comprehensive literature review.

INTRODUCTION: Choroid plexus cysts (CPCs) are rare, benign neuroepithelial cysts that can cause obstructive hydrocephalus, although most are asymptomatic. Symptomatic CPCs are uncommon, and hydrocephalus from these cysts, especially intermittent in nature, is rarely reported. Diagnosing CPC-induced intermittent hydrocephalus can be challenging due to the cysts' CSF-like content and thin walls, often requiring advanced MRI sequences for detection. CASE REPORT: We present the case of a previously healthy 6-month-old male who developed intermittent triventricular hydrocephalus, presenting with p

Microsurgical Anterior Transcallosal Resection of Colloid Cysts of the Third Ventricle.

Colloid cysts of the third ventricle are benign and rare lesions of the central nervous system. Although gross total resection is the standard treatment of these lesions, there are multiple surgical routes proposed and discussed in the literature. Similarly, the origin of these colloid cysts remains a topic of debate. Long-term outcomes of anterior transcallosal approach have been analyzed and compared with the recent meta-analyses published in the literature. Additionally, the origin of these cysts is explored using immunohistochemical analysis.Eighty operations on 76 patients by a single sur

Colloid cysts of the third ventricle show consistent expression of PAX8 but lack other features of thyroid, müllerian or intestinal differentiation.

Colloid cyst of the 3rd ventricle (CC) is an unusual cystic lesion of uncertain histogenesis, occurring exclusively in the 3rd ventricle close to the foramen Monro. In the past, its pathogenic relationship to neurenteric cyst (NEC) of central nervous system or Rathke cleft cyst (RCC) has been suggested. Thus, we evaluated expression of selected tissue-specific transcription factors and other proteins to assess its putative origin. Tissue samples of 15 CCs (8 females and 7 males, mean age 46.7 years), 7 RCCs (7 females, mean age 45.6 years), 8 neurenteric cysts (5 females and 2 males, mean age

Anteromedial striatum and globus pallidus infarcts: Uncommon complications of cerebral herniation in the setting of acute subdural hemorrhage.

Herniation of the temporal lobe through the tentorial notch often leads to compression of the ipsilateral posterior cerebral artery and stretching of the ipsilateral oculomotor nerve. Arterial compression classically results in infarcts of the inferior temporal and occipital lobes. Herniation of the cingulate gyrus beneath the falx cerebri often leads to compression of branches of the ipsilateral anterior cerebral artery, including the pericallosal artery, which classically results in infarcts of the cingulate gyrus and medial corpus callosum. A less common consequence of transtentorial hernia

Profound Near Fatal Respiratory Dysfunction in a Neonate With Meningomyelocele: A Narrative With Neurosurgical Lessons.

The association between spina bifida, specifically myelomeningocele, and autonomic dysfunction is known although rare. This case highlights the severe respiratory compromise that can occur in paediatric patients secondary to myelomeningocele. We describe a case of a neonate who experienced profound respiratory dysfunction following a successful lumbosacral myelomeningocele repair on Day 1 of life, on the background of a prenatal diagnosis of Arnold-Chiari Type II malformation and congenital hydrocephalus. In addition, the patient was found to have an incidental choroid plexus papilloma which w

Thalamic Infarctions: A Case Study of Inferolateral and Posterior Choroidal Branch Involvement in a Patient Without Classic Vascular Risk Factors.

INTRODUCTION: Thalamic infarctions represent a rare subset of ischemic strokes, characterized by a diverse array of clinical manifestations and significant diagnostic challenges. These infarctions can lead to a wide range of neurological deficits, depending on the specific vascular territory involved. Infarctions within the posterior circulation territory of P2, particularly those affecting the inferolateral and posterior choroidal arteries, are associated with distinct neurological symptoms, including contralateral sensory loss, hemiparesis, hemiataxia, thalamic hand, and various sensory and
