Primary congenital glaucoma (PCG) is a rare developmental disorder caused by dysgenesis of the trabecular meshwork and anterior chamber angle. We report a five-year longitudinal case of severe bilateral PCG in a male presenting at two months of age with photophobia, tearing, corneal edema, and markedly elevated intraocular pressure (IOP). The patient required four surgical interventions, including deep sclerectomy with mitomycin C and trabeculotomy ab externo, to achieve long-term IOP control. Serial axial length measurements documented rapid buphthalmos, and ultrasound biomicroscopy revealed

A 10-year-old Saudi girl with a homozygous GLIS3 gene deletion presented with the classic triad of neonatal diabetes mellitus, congenital hypothyroidism, and congenital glaucoma. During routine surveillance, an incidental complex left renal mass was identified. Comprehensive imaging and histopathological analysis following a core needle biopsy diagnosed a benign multicystic nephroma with features of a mixed epithelial and stromal tumor (MEST). This represents a novel renal manifestation, expanding the phenotypic spectrum of GLIS3-related disorders beyond simple cystic dysplasia. Management fav

Zika virus (ZIKV) remains a significant global public health threat due to its association with severe neurological and ocular abnormalities, including microcephaly and congenital glaucoma in infants. Viruses often exploit host metabolic programs, such as energy and lipid metabolism, to support their replication. However, how ZIKV-driven metabolic reprogramming affects the anterior segment of the eye, especially trabecular meshwork (TM) cells, remains poorly defined. In this study, we investigated the roles of AMP-activated protein kinase (AMPK) signaling, fatty acid (FA) metabolism, and lipid

Degeneration of retinal ganglion cells (RGCs) and their axons, as observed in ischemic retinopathy and glaucoma, results in irreversible vision loss. Elevated intraocular pressure (IOP) is a key factor driving these conditions, with existing treatments focused on IOP reduction often proving inadequate to halt disease progression. Here, we report that CaMKII-based gene therapy robustly protects RGCs and preserves visual function in mouse models of ischemic injury and congenital glaucoma, positioning CaMKII as a broad therapeutic target for retinal diseases driven by acute and chronic IOP elevat

BACKGROUND: To evaluate and compare the long-term clinical outcomes of combined viscocanalostomy and repeat trabeculotomy (VCO+retbo) versus repeat trabeculotomy alone (retbo) in eyes with primary congenital glaucoma (PCG) that failed initial angle surgery.
METHODS: This prospective comparative study evaluated 164 eyes with PCG that exhibited an insufficient response to primary trabeculotomy, divided into VCO+retbo (n = 82) and retbo (n = 82) groups. Treatment success was defined as an intraocular pressure (IOP) between 6 and 21 mmHg with a ≥ 20% reduction from baseline, categorized as comple

This case report explores a unique instance of primary congenital glaucoma (PCG), where the expected progression was unexpectedly halted without intervention. A 17-year-old female patient presented with diminished vision and left eye exotropia since early childhood. Examination revealed normal right eye vision, but the left eye had significantly reduced visual acuity, manifest exotropia, and megalocornea with Haab striae. Intraocular pressure was within normal range. Anterior segment examination showed prominent iris processes and anterior iris insertion. Fundus examination indicated a healthy

Primary congenital glaucoma (PCG) is an early-onset eye disorder, particularly prevalent in populations with high consanguinity rates. In Pakistan, approximately 63% of marriages are reported to be consanguineous, contributing to an increased burden of autosomal-recessive disorders. The genetic basis of PCG in the highly consanguineous Pakistani population is largely unexplored. This study was conducted to broaden the genetic landscape of PCG in families of Pakistani origin. PCG families identified through clinical characterization were recruited in the study. All probands were initially inves

UNLABELLED: Despite advances in early diagnosis and surgical treatment of primary congenital glaucoma, the condition of the cornea, including opacity, endothelial dysfunction, and persistent edema, often remains underestimated, although it largely determines residual vision and the effectiveness of visual rehabilitation.
OBJECTIVE: The study aimed to determine clinical and diagnostic criteria for glaucomatous keratopathy in children and to develop a classification to improve the effectiveness of diagnosis and treatment.
MATERIAL AND METHODS: We examined 360 children with congenital glaucoma

GZF1-related phenotype (GZF1RP) has been referred to by different names, including autosomal recessive Larsen syndrome (LRS) and "joint laxity, short stature, and myopia". Only ten patients from five families have been reported, all of whom carry biallelic variants of GZF1. They share short stature and joint dislocation with LRS; however, they present with severe ocular manifestations, suggesting that GZF1RP may be specific. In this study, we described three new patients with severe ophthalmologic phenotypes, including congenital glaucoma and abnormal iris morphology. We identified the GZF1: c

PKHD1, the gene primarily mutated in human autosomal recessive polycystic kidney disease, is one of the top 20 genes associated with primary open angle glaucoma (POAG) and associated endophenotypes in Genome-Wide Association Studies. Here, we show that Pkhd1 del3-67/del3-67 mutant mice develop congenital glaucoma due to anterior segment dysgenesis. Using a combination of genetic, epigenetic, bioinformatics and mouse developmental biology approaches, we show that Pkhd1 del3-67/del3-67 mice lack Tfap2b and AP-2β expression in a subset of periocular mesenchymal cells at E13.5 and its derivatives.

IMPORTANCE: While FOXC1 single-nucleotide variants and deletions are well-established causes of Axenfeld-Rieger syndrome, few FOXC1 duplications have been reported. This study investigated families with duplications encompassing the FOXC1 gene to refine the associated phenotypic spectrum and contribution to glaucoma.
OBJECTIVE: To investigate the prevalence and phenotype of FOXC1 duplications in 2 large glaucoma registries.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective observational genetic cohort study included participants recruited from the Australian & New Zealand Registry of Adv

Glaucomas are heterogeneous optic neuropathies associated with extracellular matrix dysregulation, abnormal ocular morphogenesis, and inflammatory signaling. Targeted deep sequencing of 586 primary congenital glaucoma (PCG) cases and 1,757 controls identified rare pathogenic variants in multiple genes, including WT1 and MMP9. Notably, WT1 variants clustered within the nuclear export sequence. Further, functional analyses showed that combined wt1-pax6 suppression in zebrafish disrupted ocular morphogenesis, highlighting developmental interdependence. In human trabecular meshwork cells, WT1 acte

Children's visual health serves as a strategic cornerstone for national health and human capital, exerting a lifelong influence that profoundly shapes individual development and social well-being. At present, visual impairment among Chinese children is characterized by a high overall prevalence and a complex, multifactorial etiology. Major conditions include refractive errors, amblyopia, strabismus, congenital cataract, congenital glaucoma, retinopathy of prematurity, and cerebral visual impairment. A substantial proportion of these impairments are preventable or curable; however, the current

Genome-wide association studies report single nucleotide polymorphisms (SNPs) in the PKHD1-TFAP2B genomic interval are associated with primary open-angle glaucoma (POAG) but do not distinguish the causal gene. While neural crest cell (NCC)-specific Tfap2b inactivation causes anterior segment dysgenesis (ASD) and congenital glaucoma (CG), PKHD1 mutations cause autosomal recessive polycystic kidney disease. We now show that Pkhd1del3-67/del3-67 mice also exhibit CG and ASD. Integrating genetic, epigenetic, bioinformatic and developmental analyses, we show that Pkhd1del3-67/del3-67 mice lack Tfap

Retinol-binding protein 3 (RBP3) is a photoreceptor-derived glyco-lipoprotein that plays a central role in retinoid trafficking between photoreceptors and the retinal pigment epithelium. Beyond its canonical role in the visual cycle, emerging evidence suggests that RBP3 may participate in retinal metabolic regulation, neurovascular protection, inflammatory modulation, and disease-related immune responses. This review critically summarizes current evidence regarding RBP3 in diabetic retinopathy, retinal degeneration, pathological myopia, primary congenital glaucoma, and uveitis. Among these con
