Unveiling ocular developmental disorders through short-read whole-genome sequencing.

Congenital eye malformations represent a clinically and genetically heterogeneous group of disorders. Despite advances in genetic testing, fewer than half of affected patients receive a definitive molecular diagnosis. However, obtaining a molecular diagnosis is crucial for these patients and their families. Whole genome sequencing (WGS) is now standard practice in the genetic investigation of patients, but its contribution has not been extensively evaluated in patients with ocular malformations. In this work, we performed short-read WGS in a cohort of 100 families presenting with eye developme

Retinal detachment complicating ocular coloboma.

Choroidal, optic disc and crystalline lens colobomas are rare ophthalmological conditions, but they can (either directly or by association) present with retinal detachments that are difficult to manage. Due to their rarity and variability of presentation, evidence of different approaches to treatment strategies is mainly found in case reports or small case series. This article reviews the basic pathogenesis of these conditions and the authors' experience in how various treatment options address the underlying mechanisms. Sub-categories of retinal detachments complicating choroidal coloboma and

[Infantile hemangiomas: diagnosis and modern therapeutic approaches].

Infantile hemangiomas are benign vascular tumors that occur exclusively in infants and display a characteristic growth pattern. They usually become noticeable shortly after birth, proliferate over a period of 4-6 months, remain stable in size for a variable duration, and then gradually involute, although residual changes may persist. Diagnosis is typically made clinically, with careful attention to potential complications, functional impairments, and rare associated malformations such as those seen in PHACES and LUMBAR syndromes. A thorough understanding of the different types of hemangiomas,

Point-of-Care Ultrasound for Quantitative Assessment of Pupillary Dysfunction in Acute Internal Carotid Artery Dissection.

BACKGROUND AND PURPOSE: Diagnosing internal carotid artery dissection (ICAD) can be challenging even for experienced clinicians, as one of its most specific signs-Horner syndrome (HS)-is often missed during routine neurological examination (NE). This study aimed to evaluate pupillary reactivity in ICAD patients using ultrasound pupillometry (UP) and compare quantitative UP findings with standard NE. METHODS: We enrolled 33 consecutive patients with acute ICAD and an age- and sex-matched healthy control group without ocular pathology. HS was assessed at admission using standard NE. UP was perf

Case Report: Ischemic central retinal vein occlusion in a patient with relapsed mantle cell lymphoma as a manifestation of suspected cancer-associated hypercoagulability.

BACKGROUND: Ischemic central retinal vein occlusion (ischemic CRVO) is a severe, vision-threatening vascular event. While most cases stem from conventional cardiovascular or local anatomical etiologies, its presentation in association with relapsed mantle cell lymphoma (MCL) and suspected cancer-associated hypercoagulability is rare. This report highlights that retinal vascular occlusion in a patient with relapsed MCL represents a rare ocular manifestation of severe systemic prothrombotic activity. CASE PRESENTATION: An 89-year-old male with established stage IV MCL (CD20+, cyclin D1+, SOX-11

The eyes and Parkinsonian syndromes.

Parkinsonian degenerative syndromes, including Parkinson disease (PD), progressive supranuclear palsy (PSP), multiple system atrophy (MSA), corticobasal degeneration (CBD), and dementia with Lewy bodies (DLB), are frequently associated with ophthalmic manifestations that contribute substantially to impaired quality of life, reduced functional independence, and increased falls risk. These visual disturbances remain under-recognised in routine neurological practice despite their diagnostic and therapeutic relevance. This review summarises the spectrum of ophthalmic manifestations across Parkinso

Bilateral Anophthalmia Associated With Ventricular Septal Defect, Atrial Septal Defect, and Patent Ductus Arteriosus: A Rare Neonatal Case Report.

BACKGROUND: Anophthalmia is a rare congenital ocular malformation characterized by the complete absence of the eye and its associated ocular structures. Bilateral anophthalmia is particularly uncommon and is frequently associated with multisystem congenital anomalies. Congenital heart disease represents one of the most commonly reported systemic associations; therefore, all infants diagnosed with bilateral anophthalmia should undergo comprehensive systemic evaluation, including detailed cardiovascular assessment. However, the coexistence of bilateral anophthalmia with multiple congenital cardi

Diabetic Retinal Disease Revisited: Integrating Neurovascular Unit Dysfunction and Hemodynamics in Pathophysiology and Treatment.

Studies have identified the involvement of microvascular injury, neurovascular unit (NVU) dysfunction, and retinal hemodynamic abnormalities in diabetic retinal diseases, including diabetic retinopathy and diabetic macular edema (DME). Although chronic hyperglycemia is the central driver of retinal damage, mounting evidence suggests that neuronal dysfunction, glial activation, impaired neurovascular coupling, and altered retinal circulation may occur before the development of clinically visible vascular lesions. These changes may reflect an early mismatch between neuronal metabolic demand and

Beyond the Eye of the Tiger: A Case Report of Adult-Onset Pantothenate Kinase-Associated Neurodegeneration Mimicking Progressive Supranuclear Palsy-Parkinsonism.

Pantothenate kinase-associated neurodegeneration (PKAN) is an autosomal recessive neurodegeneration with brain iron accumulation disorder that usually begins in childhood or adolescence; very late adult presentation can resemble more common atypical parkinsonian syndromes. We report a 54-year-old woman whose neurologic symptoms began insidiously at approximately 47 years of age with gait and balance impairment, retropulsive falls, micrographia, and dysarthria. Dysphagia, choking on liquids, urinary incontinence, and loss of independence evolved later. She had been labeled as having progressive

Comparative associations of immune-inflammatory, immune-lipid, and metabolic composite indices with hyperuricemia among Chinese adults with glycemic abnormalities: a health examination-based cross-sectional study.

BACKGROUND: Hyperuricemia (HUA) commonly coexists with glycemic abnormalities and may reflect complex metabolic disturbances involving inflammation, lipid dysregulation, insulin resistance, and impaired renal urate handling. However, evidence directly comparing immune-inflammatory, immune-lipid, and metabolic composite indices in relation to HUA among individuals with glycemic abnormalities remains limited. This study aimed to compare the associations of these composite indices with HUA among Chinese adults with glycemic abnormalities. METHODS: This large-sample health examination-based cross
