Exposure to ambient air pollutants during early gestation and phenotypic variation in non-syndromic microtia.

Prenatal exposure to ambient air pollution has been associated with congenital anomalies; however, its association with phenotypic variation among patients with microtia is poorly defined. In this single-center retrospective case-only study, we examined the associations of estimated ambient air-pollutant exposure during gestational weeks 5-9 and bilateral involvement and anatomical severity among patients with nonsyndromic microtia. Exposures to PM2.5, PM10, O3, NO2, SO2, and CO were assessed using a spatiotemporal model linked to residential addresses recorded at initial clinical registration

Progress in Auricular Reconstruction Techniques: A Review.

BACKGROUNDS AND PURPOSE: Microtia is a congenital malformation characterized by abnormal development of the external ear. Auricular reconstruction is the primary treatment for microtia. Because of the unique 3-dimensional anatomical structure of the auricle, the fabrication of patient-specific scaffolds remains one of the most technically demanding aspects of the reconstruction process. This review summarizes the literature and provides a comprehensive guide of the latest advances and breakthroughs in traditional autologous cartilage procedures and tissue-engineered ear scaffold technologies.

Cholesteatoma in concha-type microtia.

BACKGROUND: Concha-type microtia with congenital aural stenosis carries a risk of external auditory canal cholesteatoma, which may cause infection and lead to the loss of costal cartilage grafts during auricular reconstruction. However, awareness of this risk among plastic surgeons remains limited. We aimed to review cases of cholesteatoma in concha-type microtia and discuss optimal management strategies. METHODS: We conducted a single-center retrospective review of patients with concha-type or small concha-type microtia and congenital aural stenosis who underwent auricular reconstruction wit

Safety and efficacy of connecting the residual ear to the cartilage scaffold in the first-stage versus second-stage surgery: a protocol of a randomised controlled trial in non-expanded auricular reconstruction for concha-type microtia.

INTRODUCTION: Concha-type microtia, classified as a milder form of microtia under Nagata's classification, is characterised by the presence of more normal ear subunits compared with other types. The non-expanded auricular reconstruction using autologous rib cartilage is a standard procedure treating conchal microtia, and this operation typically comprises two stages. To maximise the utilisation of native auricular cartilage, the lower half of a patient's ear is preserved as residual ear, which should be connected to the rib cartilage scaffold in this operation. However, it remains unclear whet

A Meier-Gorlin syndrome mutation impairs the loading of the MCM2-7 complex during DNA replication initiation.

Chromatin loading of the hexameric replicative helicase MCM2-7 complex requires coordinated interactions with the origin recognition complex (ORC), CDC6, and CDT1. MCM2-7 not bound to DNA forms a single hexamer (SH) with an open DNA entry gate between MCM2 and MCM5. Two MCM2-7 SHs can be loaded sequentially to form the double hexamer (DH) that encircles the DNA duplex. Activated MCM2-7 then unwinds DNA and initiates DNA replication. Our cryoelectron microscopy analyses show that a fraction of human MCM2-7 without DNA exists as DH. Unexpectedly, we find that the MCM3 winged helix domain (WHD) d

Surgical and Postoperative Radiotherapy for Keloids on Congenital Microtia Reconstructed Auricles: Site-specific Incision Design.

BACKGROUND: Patients who undergo auricular reconstruction have a high risk of developing keloids. The traditional incision designs for keloid excision may appear inadequate to accommodate the biomechanical characteristics of different subunits of reconstructed ears. METHODS: A retrospective cohort of 86 patients (90 ears) with auricular keloids post-reconstruction from January 2020 to January 2024 underwent the keloid core removal surgery. We designed different incisions based on specific sites: earlobe, helix, crus of helix, and others. 24-48 h after surgery, all participants underwent super

Accuracy of fiducial-based augmented reality in auricular reconstruction.

OBJECTIVES: Reconstruction with autologous costal cartilage remains the gold standard surgical treatment for total or partial ear reconstruction. The different auricular subunits are sculpted from costal cartilage and assembled. The assembly requires a particular skillset and training, including a thorough three-dimensional (3D) understanding of auricular anatomy. The correct positioning of the ear framework can be particularly difficult in patients with hemifacial microsomia because of facial asymmetry. We developed a digital workflow that can help the surgeon in the correct positioning of th

Hair Removal with 755 nm Alexandrite Laser via Subcutaneous Route in Stage-III Ear Reconstruction by Expansion.

BACKGROUND: Congenital microtia reconstruction often introduces unwanted hair onto the new auricle. Laser hair removal can address this issue, but conventional external treatments require multiple sessions, increasing the overall treatment time, discomfort, and cost. Preclinical studies suggest that delivering laser energy from the flap's underside may improve efficiency by directly targeting hair follicles. We evaluated an intraoperative subcutaneous laser depilation approach applied during stage-III surgery to improve hair removal in this setting. METHODS: A prospective cohort study include
