Emerging antimalarial drug targets to combat drug-resistant malaria parasites.

The emergence and rapid spread of multidrug resistance in Plasmodium parasites, particularly in the Greater Mekong region and increasingly in Sub-Saharan Africa. Resistance has led to an urgent need to develop novel anti-malaria therapies with an effective mechanism. Several non-classical targets, including Na++ pump PfATP4 (Plasmodium falciparum ATPase 4), PI4K (Phosphatidylinositol 4-kinase) and PfNCR1(Plasmodium falciparum Niemann-Pick Type C1-Related protein), are highly active against the asexual stage of Plasmodium. We summarise the progress achieved on key emerging targets over past dec

Triple Vector-Borne Co-infection During Pregnancy in the United Arab Emirates: A Case Report at 21 Weeks' Gestation.

Pregnancy is a unique physiological state characterised by altered immune responses that may predispose women to severe manifestations of infection. Vector-borne diseases such as malaria and dengue pose significant risks to pregnant women and their foetuses, including miscarriage, preterm labour, placental abruption, and intrauterine growth restriction (IUGR). Co-infection with multiple vector-borne pathogens is extremely rare but can exacerbate maternal morbidity and complicate foetal outcomes. This case report aims to describe the clinical presentation, diagnosis, management, and maternal an

Type III Interferons (lambda) in Parasitic Infections: Balancing Mucosal Defense and Immune Modulation.

Parasitic infections are a significant global health issue, with pathogens often entering the hosts through mucosal surfaces. Type III interferons (IFN-λ) are key mediators of immune responses in these infections. Produced mainly at epithelial barriers, IFN-λ induces a specific set of IFN-stimulated genes (ISGs) that boost antiviral and antimicrobial defense without causing widespread systemic inflammation. This review examines functions of IFN-λ diverse in parasitic infections, focusing on roles in antimicrobial responses and immune modulation. Understanding these effects may lead to new ther

Machine learning and network pharmacology-assisted discovery of natural product inhibitors targeting Plasmodium falciparum lysyl-tRNA synthetase.

The emergence of drug-resistant Plasmodium falciparum necessitates novel antimalarial strategies. Lysyl-tRNA synthetase from Plasmodium falciparum (PfLysRS), an essential enzyme involved in protein biosynthesis, represents a promising molecular target for structure-based drug discovery and development of effective antimalarial agents. In this study, an integrated computational workflow identified three natural product-derived lead compounds, MolPort-001-742-110, MolPort-000-778-336, and MolPort-001-740-194, through structure-based virtual screening with favourable binding affinity and interact

Translational Pharmacokinetic and Pharmacodynamic Evaluation of Mefloquine Against Plasmodium falciparum Using a Hollow Fiber Infection Model.

This study established a plastic-based hollow fiber infection model (HFIM) to reproduce human-like mefloquine exposure and quantify antimalarial pharmacodynamics (PD) against blood-stage Plasmodium falciparum. Asynchronized ring-stage 3D7 parasites were maintained in the extracapillary space under continuous-flow culture conditions that supported sustained growth across two consecutive 48 h intraerythrocytic life cycles. The observed parasitemia dynamics were comparable in static and dynamic drug-free controls. Target total plasma concentration time profiles for intravenous mefloquine were gen

Detection of Plasmodium vivax and Plasmodium falciparum using colorimetric loop-mediated isothermal amplification from dried blood spots collected in active and passive malaria surveillance in the Peruvian Amazon.

BACKGROUND: As malaria transmission declines in regions approaching elimination, highly sensitive diagnostic tools are needed to detect low-density infections, particularly in active and passive surveillance settings. Loop-mediated isothermal amplification (LAMP) is a promising molecular alternative to conventional diagnostics. This study evaluated the performance of a colorimetric LAMP (cLAMP) assay for detecting Plasmodium vivax and Plasmodium falciparum from dried blood spots (DBS) in the Peruvian Amazon. METHODOLOGY/PRINCIPAL FINDINGS: The cLAMP assay was applied to DNA extracted from DBS

Different Factors Drive Initiation of Gametocytogenesis Versus Gametocyte Carriage in Plasmodium falciparum Infections.

BACKGROUND: Gametocyte carriage varies across Plasmodium falciparum infections. The extent to which this variation reflects differences in the initiation of gametocytogenesis and/or gametocyte maturation remains unclear. We investigated determinants of initiation of gametocytogenesis and gametocyte carriage in P. falciparum infections from Malawi. METHODS: A total of 1108 P. falciparum infections from a cohort study conducted in Malawi between 2019 and 2020 underwent ccp4 and pfmget qRT-PCR to measure gametocyte carriage, ap2-g and sbp1 RT-PCR to quantify gametocytogenesis initiation in ring-

De novo minibinders targeting RH5 achieve nanomolar inhibition of blood-stage malaria replication.

Malaria kills ∼600,000 people annually despite recently deployed vaccines. The parasite's blood-stage invasion of erythrocytes, driven by the essential, non-redundant interaction between Plasmodium falciparum RH5 and erythrocyte basigin, remains a leading therapeutic target. Here, we designed de novo mini-proteins ("minibinders") to occlude this interface. From ∼1,500 computationally generated designs, we synthesized 18 candidates ranging from 9.3 to 16.9 kDa which were easily expressed in E. coli as soluble, well-folded proteins. Of these, 7 minibinders blocked blood-stage replication of P. f

PfPHAST: Plasmodium falciparum Public Health Amplicon Sequencing Tool, a Streamlined Panel for Malaria Genomic Surveillance.

Genomic tools can support malaria control policy through surveillance of Plasmodium falciparum populations, tracking antimalarial drug resistance, pfhrp2/3 deletions that compromise rapid diagnostic tests, and selection at the circumsporozoite protein (PfCSP) vaccine target, as well as through molecular correction of therapeutic efficacy studies (TES). Multiplex Amplicons for Drug, Diagnostic, Diversity, and Differentiation Haplotypes using Targeted Resequencing (MAD 4 HatTeR), a comprehensive amplicon sequencing panel covering up to 276 targets, supports these applications but is tailored to

Additional blood meals increase mosquito infectivity by accelerating the transcriptional development of Plasmodium sporozoites.

Sporozoites, the infectious stage of Plasmodium malaria parasites, must invade two different tissues in two very different hosts to ensure transmission: the mosquito salivary glands and the human liver. Whether infectivity to hepatocytes is affected by physiological components, such as the availability of nutrients within the Anopheles female, is not well understood. Here we show that Plasmodium falciparum sporozoite infectivity to hepatocytes is increased if mosquitoes take an additional blood meal while oocysts are developing. Infection rates of HC04.J7 hepatocytes at 24 h were 1.8-fold high

Protective near-germline antibodies to malaria elicited by RTS,S/AS01 E vaccination in African infants.

Malaria is a leading cause of childhood mortality, yet infant immune responses to vaccination remain poorly understood. Here, we characterize 88 monoclonal antibodies (mAbs) directed against the Plasmodium falciparum circumsporozoite protein cloned from African infants immunized with the RTS,S/AS01 E malaria vaccine, identifying potent antiparasitic mAbs exhibiting molecular features resembling those observed in adults and minimal somatic hypermutation. Importantly, germline-reverted variants of four mAbs inducing sterile protection in vivo maintained high binding affinity and Fab-dependent pa

Dynamic hinge-motion of PfRIPR revealed by malaria invasion inhibitory antibodies.

The PfPCRCR complex is essential for invasion of human erythrocytes by the deadliest malaria parasite, Plasmodium falciparum. Antibodies against each subunit of PfPCRCR prevent erythrocyte invasion and the PfRH5 component is currently the most advanced blood-stage malaria vaccine. Central within PfPCRCR is PfRIPR. This complex molecule contains a core and a flexible tail and allows PfPCRCR to bridge the parasite and erythrocyte during invasion. In this study, we generated a small panel of human monoclonal antibodies against PfRIPR. We structurally characterised four PfRIPR tail-binding antibod

An Extensive Review of Recent Progress In Kinase-targeting Antimalarial Drug Discovery.

Malaria remains a major global health challenge, with the rise of drug-resistant Plasmodium falciparum strains necessitating the development of novel therapeutic strategies. Kinases, which regulate essential processes throughout the parasite life cycle, including invasion, egress, development, proliferation, gametogenesis, and transmission, have emerged as promising drug targets, leading to significant advances in kinase inhibitor development over the past five years. This review examines recent advances in kinase-targeted antimalarial drug discovery, with particular emphasis on inhibitor scaf

A multiscale framework for heme crystallization inhibition: From single-molecule step-capping to parasite suppression.

Heme crystallization is central to malaria parasites' survival during the erythrocytic stage of the disease, yet how clinically relevant antimalarial drugs modulate this process at the molecular level remains unclear. Here we employ stage-resolved parasite assays and multiscale in vitro approaches to explore the interactions between lumefantrine (LF), one of the popular components of current combination therapies, and hematin crystallization. Pulse LF suppresses Plasmodium falciparum during both ring and trophozoite stages. Under biomimetic conditions, LF markedly reduces β-hematin crystal gro

Spatial and genetic dynamics of Plasmodium falciparum histidine-rich protein2 in sub-Saharan Africa.

BACKGROUND: Malaria rapid diagnosis tests (mRDTs) detecting Plasmodium falciparum histidine-rich protein 2 (Pfhrp2) are the most widely adopted diagnostic tool in malaria-endemic areas. However, extensive polymorphisms in Pfhrp2 could modify Pfhrp2 epitopes, rendering it undetectable by Pfhrp2-based mRDTs. This study investigates the spatial distribution and population dynamics of Pfhrp2 in sub-Saharan Africa (sSA). METHOD: Pfhrp2 sequences (215) were retrieved from public repositories and analysed with MAFFT v7, DnaSP, and various packages in R. We computed genetic diversity metrics, neutral
