Infertility and Risk of Congenital Anomalies: A Population-Based Cohort Study.

BACKGROUND: Fertility treatments have been associated with an increased risk of congenital anomalies. Most studies have compared births conceived through fertility treatment with unassisted conceptions; consequently, these estimates reflect the combined effects of fertility treatment and the underlying infertility, potentially resulting in confounding by indication. OBJECTIVES: To estimate the association between infertility, independent of fertility treatment, and the risk of congenital anomalies. METHODS: This population-based cohort study included all live births and stillbirths at ≥ 20 w

Pre-operative High-Resolution CT and MRI Evaluation in Pediatric Cochlear Implant Candidates: Correlation With Surgical Findings and Outcomes.

Purpose Congenital sensorineural hearing loss (SNHL) is a major childhood disability, and early cochlear implantation offers optimal auditory and language outcomes. High-resolution computed tomography (HRCT) and magnetic resonance imaging (MRI) are essential in evaluating candidacy, identifying inner ear malformations (IEMs), assessing cochlear nerve integrity, and predicting surgical challenges. This study evaluates the role of HRCT and MRI in pediatric cochlear implant candidates and correlates imaging findings with intraoperative events and postoperative outcomes. Methods This retrospective

Cerebral vascular malformation screening in hereditary hemorrhagic telangiectasia: Balancing low diagnostic yield against high-risk hemorrhage.

BACKGROUND: Hereditary Hemorrhagic Telangiectasia (HHT) is an autosomal dominant disorder predisposed to cerebral vascular malformations (CVMs). With conflicting international guidelines on systematic CVM screening in adults, we evaluated the prevalence, clinical relevance, and risks of screening in a national Danish HHT cohort. METHODS: This combined prospective and retrospective study analyzed data from the Danish HHT Registry. The prospective arm involved systematic non-contrast brain MRI in 202 HHT patients (aged 18-65, no MRI in previous 5 years). The retrospective arm included 119 patie

Prenatal Tympanic Ring Anomaly Without Microtia: A Subtle Clue Toward Severe Early-Onset Monogenic Disorders.

OBJECTIVE: To investigate the genetic etiologies and clinical significance of fetal tympanic ring abnormalities detected during second-trimester ultrasound in the absence of microtia. METHOD: Between November 2019 and June 2024, we examined the fetal tympanic rings of 10,277 unselected pregnant women during the 20-22 weeks of morphology ultrasound. Fetal cases with abnormal tympanic rings, without microtia, underwent amniocentesis for genetic investigation, including chromosome microarray analysis and trio genome sequencing (30×). RESULTS: Five fetuses (0.05%, 5/10,277) with abnormal tympani

Silent obstruction: The hidden role of aberrant internal carotid artery in middle ear pathology.

PURPOSE: Aberrant Internal carotid artery (ICA) within the middle ear is a rare congenital vascular anomaly, occurring in approximately 1% of the population [1]. It often remains clinically silent or presents with nonspecific otological symptoms, making preoperative diagnosis challenging. This study aims to highlight the importance of early identification and safe management of aberrant ICA through a clinical case and literature review. METHODS: We report a case of 36 year old female presenting with bilateral chronic ear discharge and tympanic membrane retraction. Detailed otoscopic examinatio

HLA genotype, clinical and immunofluorescence findings of 30 patients with type VII collagen antibody positive subepidermal bullous disease.

Epidermolysis bullosa acquisita (EBA) and mucous membrane pemphigoid are mucocutaneous blistering disorders that are associated with antibodies that target type VII collagen (anti-col7). There are relatively few studies that closely investigate the clinical phenotype and HLA genotype of patients who are anti-col7 positive. We therefore characterized all patients with anti-col7-positive subepidermal bullous disease that were treated at our centre. In total, 30 patients were identified, 29 had mechanobullous skin lesions and all had oral ulceration. The most common sites for oral lesions were th

Rare Cause of Congenital Unilateral Facial Palsy: High-Resolution Computed Tomography (HRCT) Findings of Isolated Facial Canal Hypoplasia and Aplasia.

Congenital facial nerve palsy (CFNP) is an uncommon medical condition conventionally grouped into acquired (traumatic) and developmental (isolated or syndromic) categories depending on the cause. Distinction between the two etiological mechanisms is mandatory for determining further prognosis and selecting the treatment method. High-resolution computed tomography (HRCT) of the temporal bone currently remains the gold-standard diagnostic technique used to examine the complex osseous architecture of the Fallopian canal and reveal congenital abnormalities. In this report, we describe the case of

Auditory evoked-potential abnormalities in a mouse model of 22q11.2 Deletion Syndrome and their interactions with hearing impairment.

The 22q11.2 deletion is a risk factor for multiple psychiatric disorders including schizophrenia and also increases vulnerability to middle-ear problems that can cause hearing impairment. Up to 60% of deletion carriers experience hearing impairment and ~30% develop schizophrenia in adulthood. It is not known if these risks interact. Here we used the Df1/+ mouse model of the 22q11.2 deletion to investigate how hearing impairment might interact with increased genetic vulnerability to psychiatric disease to affect brain function. We measured brain function using cortical auditory evoked potential

Improvement of perceived cochlear implant sound quality through individualized psychoacoustic-based frequency fitting.

OBJECTIVE: In recent years, there has been a trend toward more individualization in the fitting of cochlear implants (CI). Here, a new individualized approach to frequency band allocation was used. This approach is based on binaural perceptual pitch matching. PATIENT: The patient had congenital bilateral progressive sensorineural hearing loss due to Usher Syndrome. He had used hearing aids in both ears since the age of 4 years. In his mid-40s, he received a CI on his right ear and, ten months later, a second CI on the left ear. INTERVENTION: Adjustments to the frequency band allocations were

[Congenital cholesteatoma of the middle ear with total atresia of the external auditory canal: two cases report].

Congenital stenosis and atresia of the external auditory canal are the most common developmental anomalies of the external ear. Cases of combination of these malformations with cholesteatoma of the temporal bone have been repeatedly described in the world literature. This combined pathology is more typical of congenital stenosis of the external auditory canal. Total atresia of the external auditory canal is rarely associated with cholesteatoma. Cholesteatoma found behind the atretic plate is classified as congenital, excluding other possible mechanisms of it's formation. The article presents t

Facial Nerve Abnormalities in Congenital Middle Ear Malformations With Comments on Preoperative Detectability: A Report of Two Cases.

Facial nerve abnormalities and congenital middle ear malformations originate in similar developmental stages and are often concomitant. Preoperative recognition of such a condition is desirable to prevent any damage, but it is often difficult. Here, we report two cases of middle ear malformations associated with facial nerve abnormalities, describe the computed tomography (CT) appearance, and discuss its preoperative detectability. Case 1 was a 40-year-old female with a facial nerve abnormality associated with stapes ankylosis. She underwent stapes surgery. The facial nerve was hanging over th

The Presentation Patterns of Tessier 7 Clefts.

Given its rarity, there are few large-number studies regarding Tessier 7 clefts. A review and classification of 81 Tessier 7 patients who presented to our craniofacial center was performed. The most common Tessier 7 clefts were middle positioned, followed by agenetic, superiorly rotated, and inferiorly rotated clefts, respectively. A total of 77/81 patients had simple macrostomia and 4/81 patients had macrostomia with muscle diastasis. The most common associated anomalies were craniofacial microsomia, skin tags, and ear anomalies occurring in 80%, 71%, and 37% of patients, respectively. In thi
