Congenital malformations of the inner ear: histologic findings in five temporal bones.

The histopathologic findings in five temporal bones from three patients with congenital malformation of the inner ear are described. The external contour of the cochlea was deformed in two temporal bones, while the internal architecture was abnormal in all five temporal bones. Intracochlear abnormalities included defects in the interscalar septum, deficiencies in the modiolus, and a paucity of neural elements. Spiral ganglion cell populations varied, but were substantially diminished in all five temporal bones. Enlargement of the vestibule and semicircular canals (lateral and posterior) was se

Mesenchyme remaining in temporal bones from patients with congenital anomalies. A quantitative histopathologic study.

Our hypothesis that more mesenchyme remains for a longer time in the middle ear clefts of infants with congenital anomalies in the ear and/or other parts of the body than in the ears of infants without such anomalies has been tested. One hundred four human temporal bones obtained from 81 individuals with congenital anomalies in the ear and/or other parts of the body, with ages ranging from 24 weeks' gestation to 35 years, were processed histologically, stained with H & E, and examined under the light microscope. The slides were projected under a microprojector, and the mesenchyme remaining in

Further application of VM-plasty.

The use of VM-plasty in the repair of web contracture and its application for other regions are presented. We refined VM-plasty so that both ends remain in the "dog ear" form, eschewing the conventional excision and instead making flaps that are set in a zigzag shape. This procedure results in an increase in the effect of extension to the lateral direction, in addition to preventing recontracture. This method is used not only to repair finger web contracture but also for other contracted scars. No recontracture has been observed, and good results have been obtained.

Neurologic findings in children with ear malformations.

Cephalic neural crest cells contribute to the formation of the external and middle ears, the supporting cells of the statoacoustic ganglion, other cranial nerve components, and the face. The anlage of otic sensory structures receive inductive stimuli from adjacent rhombencephalic tissue. The complex series of interactions that guide organogenesis of the outer, middle, and inner ear structures may explain why neurologic dysfunction is likely to be associated with malformations of the ear. We reviewed the records of 100 patients with complex ear anomalies with or without hearing loss. Mean age w

Auditory brainstem responses in young children with Down's syndrome.

Auditory brainstem evoked responses (ABRs) were studied in 37 Japanese infants and children with Down's syndrome to determine the level of the lesion causing their unresponsiveness to sound stimuli. ABR thresholds, peak latencies of waves I and the peak interval latency of wave I-V were measured. For comparison with ABR thresholds, behavioral audiometry was performed. Thirty-two percent of the patients (12 cases) with Down's syndrome showed no responses by ABR as well as behavioral audiometry. The other ABR abnormalities with respect to age-matched controls were classified into 6 types: shorte

[Diagnosis and operative sequela of rare abnormalities of the inner ear].

Two cases of congenital unilateral deafness in otherwise normal children are presented. X-ray examination showed a partition of the inner ear canal in one of them. A review of the literature showed this lesion to be very rare, but it is more frequent in combination with other malformations such as the Klippel-Feil-syndrome, Wildervanck-syndrome and Thalidomide embryopathy. The second child showed an extensive malformation of the cochlear and vestibular parts of the inner ear and of the internal auditory meatus. The vestibulocochlear nerve was absent. An arachnoidal cyst protruding into the tym

Auditory and temporal bone abnormalities in CHARGE association.

CHARGE association is a recently described cluster of congenital defects including ocular coloboma, heart disease, choanal atresia, retarded development and/or CNS abnormalities, genital hypoplasia, and ear anomalies. Although congenital hearing loss has been reported in CHARGE association, no information regarding the underlying temporal bone disease is available in the literature to date. The authors evaluated four patients with multiple anomalies consistent with CHARGE syndrome. Two surviving patients have bilateral severe hearing loss on auditory brain stem response testing. Two patients d

Value of CT scan in the diagnosis of temporal bone diseases in children.

CT scan of the temporal bone has become a frequently ordered test for diagnosis of temporal bone pathology. We retrospectively examined our experience with CT scans of the temporal bones in 124 consecutive children from January 1983 to September 1984 in an attempt to assess its usefulness in diagnosis and treatment of ear disease in children. Patients were divided into six categories according to their pre-scan diagnosis, (trauma, congenital aural atresia, dizziness, facial nerve paralysis, middle ear disease, congenital sensorineural hearing loss) and CT findings were compared to data found b

Tympanometry screening in developmentally delayed individuals.

Residents of the Huronia Regional Centre for the developmentally disabled (N:900) underwent otoscopic screening (canal "clear" vs "not clear") and tympanometric screening, including tympanometry and physical volume measures (Ss exhibiting Type B tympanograms were classified "hard positive"). Results showed that (1) by otoscopy, 53.2% of all Ss examined had canals "not clear". (2) Overall, degree of retardation was loosely related to prevalence of abnormal otoscopy. (3) The etiologies for mental retardation of "metabolism" and "chromosomal abnormalities" (including all Down's Syndrome cases) we

Cochlear neuronal populations in developmental defects of the inner ear. Implications for cochlear implantation.

A histological study was made to determine the cochlear neuronal populations of 20 human ears having hearing loss caused by developmental defects. The neuronal populations ranged from 7677 in an ear with Mondini dysplasia to 30 753 in an ear with DiGeorge's syndrome, the norm for young human subjects being 35 000 neurons. The length of the cochlear (spiral) ganglion varied from 7.3 mm to 14.8 mm, the norm for human subjects being 12 mm. The sensorineural hearing losses in all cases were attributable to malformation or degeneration of the sensorineural structures. The hearing loss was moderate

Consequences of unremitting middle-ear disease in early life. Otologic, audiologic, and developmental findings in children with cleft palate.

To learn whether chronic otitis media with effusion during early life has lasting otologic, audiologic, or developmental consequences, we evaluated 24 closely matched pairs of children with repaired palatal clefts whose treatment had been equivalent except with regard to persistent otitis media during early life. One group had undergone early (mean age, 3.0 months) myringotomy with placement of tympanostomy tubes, followed by assiduous monitoring and an aggressive treatment program to maintain ventilation in the middle ear. The other group had undergone initial myringotomy later (mean age, 30.

[Do hemoglobinopathies correlate with genetically-induced hearing disorders?].

In hereditary deafness a battery of audiological and medical investigations are needed to find out the aetiological factors. We tested the blood of children impaired hearing for hemoglobinopathies. Such anomalies might provide a clue to the aetiology. We found abnormal structures of hemoglobin in 5 children suffering from Fanconi's anemia. Four of these children had prenatal conductive hearing loss. The other children showed no abnormal hemoglobin.

Aural abnormalities in Klippel-Feil syndrome.

Klippel-Feil syndrome, the triad of short neck, low occipital hairline, and limitation of movement of the head, is often associated with other serious congenital anomalies of the nervous, urogenital, and cardiovascular, and respiratory systems. Hearing loss is the second most common associated anomaly following neurologic defects. Histologic studies of the temporal bones of an infant with Klippel-Feil syndrome whom we studied showed severe anomalies in one ear consisting of microtia, aural atresia, maldeveloped ossicles, fixed stapes, hypoplastic facial nerve, short cochlea, and large diameter

The fragile X syndrome I: familial variation in the proportion of lymphocytes with the fragile site in males.

A review of 61 males with the fragile X positive form of the Martin Bell syndrome from 30 families seen in the past 4 years suggests that the number of lymphocytes with the fragile site on the X chromosome (fra(X) ) in retarded males tends to be characteristic for the individual and similar to that found in other retarded males in the same family. The number of lymphocytes with fra(X) was not correlated with height, weight, occipitofrontal circumference, ear length or mean testicular volume in adults nor with age over the whole series.

Spontaneous encephalocele of the middle ear.

Mature glial tissue was seen in the middle ear of a patient with no previous pathologic precedents, and a clinical diagnosis of serous otitis media was made. That anomaly was detected, and its corresponding clinical, roentgenographic, and pathologic findings are described. The results led us to diagnose it as a congenital (spontaneous) encephalocele of the middle ear. To our knowledge, other case of pure congenital encephalocele of the middle ear has been reported.

Temporal bone pathology in DiGeorge's syndrome.

DiGeorge's syndrome is characterized by partial or complete absence of the parathyroid and thymus glands and is often associated with other developmental anomalies, particularly of the structures arising from the third and fourth pharyngeal pouches. The temporal bone findings in three cases of DiGeorge's syndrome are presented. Patients with this condition have a high incidence of Mondini dysplasia in both ears, sometimes with other anomalies of the external or middle ears. Hearing may range from normal to profound deafness and may manifest sensorineural, conductive, or mixed losses of varying

Inner ear and renal diseases.

Certain conditions (Alport's syndrome, kidney transplantation, dialysis) are followed by a progressive inner ear hearing loss. On the other hand, pharmacological substances that act on the transport mechanisms of the tubular epithelium may also produce inner ear disturbance. Experimental data on the possible pathomechanisms are discussed in reference to results reported in the literature.

Radiographic features of the ear-related developmental anomalies in patients with mandibulofacial dysostosis.

Among the conditions that arise from disturbances in development of the first two branchial arches, the symmetric syndrome of mandibulofacial dysostosis and the asymmetric anomalies of the hemicraniofacial microsomia represent a characteristic pattern of craniofacial malformation distinct from other first and second arch syndromes. Since the usual embryonic aberration in both of these anomalies involves the first and second branchial arch derivatives, malformations of the external and middle ear are common. The external auditory canal is absent in the vast majority of the cases. Incudomallear

Conductive hearing loss, middle ear ossicular anomalies, malformed thickened lop auricles, and micrognathia. A rare autosomal dominant congenital syndrome.

A rare congenital autosomal dominant syndrome of thickened bilateral lop auricles, conductive hearing loss, ossicular anomalies, and micrognathia is reported. The anomaly is presumably a defect in the first and second branchial arch development during the sixth and seventh weeks of gestation with an auricle abnormality (first and second arch), abnormal incus and malleus (first and second arch), abnormal stapes (second arch), and micrognathia (first arch). Recognition of low-set or malformed auricles with a unilateral or bilateral conductive hearing loss should alert the otolaryngologist to pos
