Hearing loss and congenital rubella in Atlantic Canada.

There were 41 children born between 1973-1976 in Atlantic Canada who were victims of a severe rubella epidemic. This paper investigates prevalent audiometric patterns, incidence of middle ear disease, evidence regarding progressivity of hearing loss and any relationship between presence of middle ear disease and progressivity. Both surgical and audiological records were utilized in a combined retrospective-prospective study. Results indicate a typically flat audiogram for our group, with possible errors in the analysis of high frequency data accounting for differences between our center and th

Temporal bone histopathology of atresia auris congenita with chromosome aberration.

Temporal bone findings in two infants demonstrating congenital aural atresia with microtia were presented focusing mainly on the external and middle ears. These two cases were proved to have chromosome aberration that is, 13-15 trisomy and extra chromosome in group G individually. Temporal bones showed the multiple anomalies through the external, middle and internal ears. The external canals were filled with connective tissue in one case and with bony plate in the other to form aural atresia. There were no development of the tubotympanic recess in a case of 13-15 trisomy. The oval window area

Nose-, sinus- and ear-symptoms in 27 patients with primary ciliary dyskinesia.

Nose-, sinus- and ear-symptoms, signs and tests were studied in 27 patients with primary ciliary dyskinesia. All subjects had had daily nasal discharge since birth or early childhood. X-ray examination showed sinusitis, and most patients had frequent episodes of sinusitis symptoms. The frequency of common colds was apparently not increased. As a general rule, the patients had chronic secretory otitis media, but few episodes of acute otitis media. Repeated suctions, punctures, and insertion of tubes had been disappointing, but more radical surgery of the nose and sinuses had helped some of the

Mesodermal induction defect as a possible cause of ear malformations.

Deafness due to inner ear anomalies is rarely associated with malformations of the auricles. We describe two brothers with profound congenital sensorineural deafness, abnormal vestibular function, normal ossicles, and delayed motor development. Since the external and inner ear originate from distinctly separate structures, the embryogenesis of this malformation association is less clear than in the more common association of external and middle anomalies, where the latter two structures are derived from the first and second branchial arches. The combination of auricular and inner ear anomalies

Results of the Widmaier-Perko palatoplasty in clefts of secondary palate.

The Widmaier-Perko palatoplasty was evaluated in post alveolar clefts. The speech results of 42 Chinese patients revealed 78.5% as having acceptable speech. In post alveolar clefts of the soft palate only 80% had acceptable speech compared to 76% acceptable speech for clefts of the soft and hard palate. This compared favourably with other similar reported studies. No dental occlusal or growth disturbances were noted. There was significant middle ear disease in the patients examined. This type of palatoplasty does not disturb the palatine bone periosteum and may be helpful in preventing growth

Malleus Teflon piston prosthesis.

The malleus Teflon piston prosthesis is very useful to reconstruct the sound conducting mechanism of the middle ear in absent or inadequate incus, congenital malformation of the middle ear, and epitympanic fixation of the incus and/or malleus. It has a thin-walled loop at its upper end to lessen the tendency for ulceration of the overlying drum with an inside diameter of 1 mm to fit around the mid portion of the handle of the malleus at an angle of 30 degrees to the shaft of the prosthesis. There is a cut in the distal part of the loop through which it is snapped onto the handle of the malleus

Polytomography and congenital anomalies of the ear.

Conventional radiography and even high resolution computerized axial tomography leave much to be desired in demonstrating the minutiae of the anatomy of the temporal bone. Multidirectional tomography remains the examination of choice radiographically in cases of congenital anomalies of the temporal bone. Details of the inner, middle and even external ear in the presence of atresia are more clearly delineated by this method than any other to data. Such information is of inestimable value to the surgeon in the diagnosis and surgical correction of anomalies of the temporal bone. Important feature

[Sibling case of osteosclerosis with cranial nerve symptoms].

We experienced two cases of "Osteosclerosis" who were 12 and 15 year old sisters. Previous reported cases of this disease are 50 cases and among them only one patient was reported in Japan. Osteosclerosis seems to be inherited as an autosomal recessive trait. Parental consanguinity is also observed. The peculiar facies are evident in infancy, characterized by broad, flat nasal bridge, ocular hypertelorism and prognathic, broadened mandible. Commonly, they have cutaneous syndactylies in bilateral hands and feet, especially between the second and third finger and toe. Roentgenographically, hyper

[The Noonan syndrome].

Two cases of Noonan syndrome are observed. The Noonan syndrome is a Turner-like syndrome with normal chromosomal constitution. Essential criteria in diagnosing the syndrome are the facial expression (hyper-telorisme, low-set and malformed ears, micrognathie, fish-like mouth). The short webbed neck and the cubitus valgus. These children are rather small and mental retardation can be present, especially in comparison with their siblings. Associated cardiac malformations occur in approximately 50% of the cases. We insist on audiologic findings. One case has a hearing impairment due to a malformat

[Surgical reconstruction of the sound conduction mechanism in congenital atresia of the ear (author's transl)].

125 congenital atresia of the ear have been operated from 1974 to 1980 in our O.L. department. Congenital atresia of the ear have a conductive type of 70 db which is stable. 45 had a lateral canal fenestration. 22 had an hearing improvement greater than 20 db. 45 had a tympanoplasty using a temporalis fascia graft and the ossicles when the malleo incus mass was mobile and had an adequate contact with the stapes. 25 out of these 45 cases had an hearing improvement greater than 20db. The other one had a tympanoplasty using tympano ossicular homograft with an hearing improvement greater than 20 d

Hearing loss of Down syndrome adults.

Audiological hearing-test data were obtained for 51 Down syndrome adults. Depending upon hearing-loss criteria, from 51 to 74 percent of the subjects had some degree of hearing impairment. Correlation coefficients between these hearing-acuity measurements and receptive hearing vocabulary scores suggest a moderate relationship. These data indicate that Down syndrome adults are prone to a wide variety of hearing difficulties that may reflect a variety of ear pathologies. These data and other studies have shown that both Down syndrome adults and children need frequent hearing screening and, when

Fragile bones and fragile ears.

Conductive, sensorineural and mixed hearing loss occur in osteogenesis imperfecta in autosomal dominant inheritance pattern. Hearing loss is generally due to the middle and inner ear pathology of osteogenesis imperfecta and only occasionally to the coincidental association of otosclerosis and osteogenesis imperfecta. Two lesions cause the conductive hearing loss of osteogenesis imperfecta: (1) functional ossicular discontinuity due either to stapes superstructure fracture or fibrous replacement, or (2) thick, crumbly, lightly fixed stapes footplate. Middle and inner ear changes in congenital rubella.

Temporal bone changes in a 14-month-old female infant with congenital rubella are described. The inner ears showed cochleo-saccular degeneration, with more severe involvement of the right side. Perivascular round cell infiltration was demonstrated in the base of the cochlea, stria vascularis, and superior ampullary cribriform region, consistent with viral infection. Whether the latter was related to the rubella or the the patient's terminal viral infection is undetermined. Predominant involvement of the cochlea saccule may have been related o their blood supply from the vestibulo-cochlear arte

Unilateral sudden loss of hearing: an unusual complication of cardiac operation.

Between 1969 and 1978 7,000 patients underwent cardiopulmonary bypass at Bellevue and University Hospitals. In seven of these patients, sudden loss of hearing in one ear developed immediately after the operation. Four of the seven patients showed improvement in hearing after the initial loss, although in no case did the hearing return completely to normal. None of the patients had vertigo but two were listless postoperatively. Two of the operations were for congenital heart disease; the remainder were coronary artery bypass procedures. All of the affected patients were male. There was no predi

Embryonic development of the head and neck: part 4, organs of special sense.

A synopsis of the embryonic development of the eye, the ear, and the olfactory organ and taste buds is presented. The eye is discussed in terms of the sensory and nonsensory protions of the eyeball and of such associated structures as the extrinsic eye muscles, eyelids, conjuctiva, and lacrimal apparatus. The embryology of the internal, middle, and external ear is reviewed, as well as positional changes of both the eye and ear relative to other sturctures of the head and neck. Various congenital defects affecting the eye and ear are also discussed.

New patterns in genetic and congenital otonephropathies.

In a series of chronic renal and congenitally deaf patients 24 were identified as having inborn renal and otologic disease. Sixteen patients, representing 14 families, had genetic disorders. Only two had the features of Alport's syndrome. The patients were classified as follows: 1) Probable Alport's--2 patients (1 family); 2) Atypical hereditary nephritis and sensorineural hearing loss--7 patients; 3) Renal and inner ear anomalies--1 patient; 4) Renal, inner ear and multiple anomalies--4 patients. The temporal bone pathology in one case showed primary neural atrophy and a mild Mondini malforma

Congenital middle ear cholesteatoma: two unusual cases and a review of the literature.

Forty-three patients with congenital middle ear cholesteatoma have been described since the first case in 1953. In these patients ten cholesteatomas were confined to the anterior mesotympanum in young children who had no ossicular damage. Involvement of the posterior mesotympanum (8 ears), meso and epitympanum (21 ears), and antrum and middle ear (7 ears) was associated with an 81% incidence of ossicular erosion in addition to other abnormalities. Anterior mesotympanic cholesteatomas were readily identified by the appearance of a white mass behind a translucent drum. The other congenital chole

A family with X-chromosomal recessive congenital cataract, microphthalmia, a peculiar form of the ear and dental anomalies.

An investigation has been made in a family with X-chromosomal recessive congenital cataract, microphthalmia, a peculiar form of the ear, and dental anomalies. The carrier females show only slight symptoms. They all have lens opacities, and most of them show more symptoms. For genetic counseling it is of utmost importance to examine all family members and to re-examine the females from time to time for early lens opacificaiton, which is the most constant finding. In literature families have been described with similar clinical symptoms only a few times. We have been trying to demonstrate a link
