Congenital atresia of the ear.

Congenital atresia of the ear requires prompt diagnosis and an early assessment of hearing. In bilateral atresia, aural rehabilitation should be started early in life to avoid late sequelae of speech problems. In unilateral atresia, the need is not urgent if the child hears normally in the other ear. Polytomography and audiometric testing are the two most important parts of the patient's evaluation. Precise audiometric testing may be difficult, or even impossible, in these children. The indications for surgery are presented. The risk of injuring an abnormally placed facial nerve remains a dete

The differentiation of the columella in avian embryos treated with hadacidin with some observations on other skeletal abnormalities.

Single doses of N-formyl hydroxyaminoacetic acid (hadacidin) were injected into chick embryos HH stages 5-30. Embryos surviving for 8+ days total incubation time were recovered and examined for gross and microscopically observable defects. Particular attention was paid to the development of the columella. Survival was only 20%, but a high incidence of cranio-facial and hind limb defects was observed in most age groups. While three categories of columella defect were observed, reflecting some degree of age dependency, this chondrogenic tissue is less susceptible to teratogenic action than other

Ear disease in relation to age in the cleft palate child and adolescent.

Ninety-eight cleft palate subjects were studied to ascertain the eventual adolescent and adult hearing. There were 72 complete clefts of lip and palate and 26 clefts of the palate alone. The incidence or side of ear disease did not appear to be influenced by this type of cleft. Other systemic anomalies were present in 11 cases with congenital middle ear abnormalities in 3 and sensory neural hearing loss in 8 subjects. Progressive improvement in incidence of hearing loss from 94% at 4 years to 42% at 20 years was noted with the strict definition of hearing loss as 10 dB conductive loss at any o

[Syndrome of symphalangism and stapes fixation: an autosomal dominant hereditary disease (author's transl)].

The autosomal dominant hereditary syndrome of symphalangism and stapes fixation consists of multiple synostoses, most frequently symphalangism, in addition to synostoses of carpal and tarsal bones and radio-ulnar synostoses, various other malformations of the limbs (restricted joint movement or syndactylism) and variable degrees of conduction disorders due to stapes fixation, starting in childhood. A family tree of four successive generations with various forms of the syndrome is described.

Inner ear morphology in Down's syndrome.

A comparative study was made on four pairs of temporal bones from patients with Down's syndrome (trisomy 21) and 15 pairs of temporal bones from other infants of the same age range. Spiral reconstructions showed chochlear length to be slightly shorter in temporal bones from patients with Down's syndrome than that in the controls. Based upon these dimensional measurements, a developmental anomaly of the vestibular apparatus was found.

The use of computerized tomography in otolaryngology.

Computed axial tomography represents a major advance in otolaryngology in the area of neuro-otology. Now that third and fourth generation scanners with their coned-down imagery are available with much better resolution, the scanner has become extremely useful in the entire field of otolaryngology. Major advantages of this technique are its noninvasive character and the absence of morbidity. This encourages its application to large numbers of patients with disorders at an early stage in the disease processes than was previously deemed prudent with other invasive diagnostic techniques. Intensive

[Correcture of congenital defects of the wing and the tip of the nose (author's transl)].

Congenital hypoplasia of the nasal wing can appear as a defect (congenital notch) or as a flattening and deficiency of the cartilagineous support. The combination with other deformities is seen frequently, 2 own cases: 1. Congenital notch of the left ala nasi. Operation: forming a window in the wing by incision, implantation of a composite graft from the ear, undisturbed and complete healing. 2. Hypoplasia of both cartilages of the ala nasi, flatness and dropping of the nasal tip, combined with deformities in both middle ears. Operation: Lifting the nasal tip, median suture between the cartila

[The neurovascular island pedicle: basic considerations and its application in the face (author's transl)].

The island pedicle flap has in comparison with other skin flaps a number of important advantages which include elasticity, minimal shrinkage, absence of dog ears, success in irradiated areas and innecessity of a second stage procedure. On the head the most common donor areas for an island flap are postauricular, nasolabial, submental, and less cosmetically satisfying frontal and temporal regions. For large reconstrictions, however, only those pedicles which contain a branch of the superficial temporal or frontal arteries suffice. The major indications for an island pedicle flap include defects

Pinna anomalies associated with congenital conductive hearing loss.

This paper compares the normal pinna with the abnormal congenitally malformed pinna. Then the abnormal pinna is related to case histories where a hearing loss was found due to a congenital malformation of the middle ear ossicles. Careful examination of the pinna, therefore, may be a simple diagnostic sign of an underlying potentially surgically correctable hearing loss. An abnormal pinna is more often associated with a middle ear anomaly than with any other congenital malformation including a urinary tract anomaly.

Management of middle ear effusions in infants with cleft palate.

Middle ear effusion is now generally recognized as a virtually universal complication in infants with cleft palate. Such infants may therefore be assumed to experience conductive hearing loss of some degree or other throughout infancy. Hoping to prevent not only deficits in intellectual and language development, but also permanent otic and auditory handicaps, we have routinely treated such infants as early as practicable with myringotomy and tympanostomy tube insertion. Subsequently, we have repeated the operation whenever blockage or extrusion of the tubes resulted in recurrence of persistent

Hearing loss in prosthetically treated adults with cleft palate.

The effect of obturator treatment of cleft palate on middle ear disease and hearing loss has not been established. This study serves as an otologic and audiologic review of a previously reported adult population with cleft palate. A substantial improvement in hearing levels subsequent to obturation was demonstrated; however, it was believed to be based on stabilization of pathologic features of the ear with age. A statistically significant correlation of hearing level with length of use of prosthesis or with type of lesion could not be demonstrated. Results are compared with those reported on

Hemangiomas of the internal auditory canal.

Three patients who presented with symptoms and signs of acoustic tumors are described: one of them had an arteriovenous malformation and the other two had cavernous hemangiomas. The common feature in all these patients was the presence of marked facial weakness with or without fasciculations and a complete loss of hearing in the presence of a small tumor. The cerebrospinal fluid protein was normal in these patients. Polytomography and Pantopaque cisternograms were used to make the diagnosis. Since these lesions are circumscribed, they are suitable for removal using microneurosurgical technique

Recurrent meningitis secondary to idiopathic oval window CSF leak.

Bacterial meningitis remains a life-threatening infection even in the present antibiotic era; thus, any abnormality which predisposes a patient to a recurrence of this serious disease, must be identified and corrected. This report describes the histroy of a 12-year old boy with a profound neurosensory hearing loss, a related absence of vestibular function and a Mondini-type of temporal bone dysplasia who developed recurrent episodes of meningitis which were due to an idiopathic cerebrospinal fluid otorrhea. Even though the meningitis was labyrinthogenic in origin, the patient did not experienc

Symposium: Congenital anomalies of the middle ear. III. Congenital anomalies of the middle ear.

Congenital anomalies of the middle ear have been considered rare. The tremendous volume of middle ear surgery performed during the last 15 years has brought attention to numerous anomalies of the facial nerve, ossicles, and middle ear cleft, suggesting that middle ear anomalies are not as rare as once believed. This paper discusses anomalies of the descending and horizontal facial nerve which have been previously reported. The literature of anomalies of the ossicles is reviewed. A very rare anomaly of the carotid artery presenting in the middle ear is described and its surgical treatment outli

Temporal bone histopathological findings in trisomy 13 syndrome.

This study reports the histopathological findings of 14 temporal bones from infants with trisomy 13 syndrome. The most primitive anomalies in the structures of the inner and middle ears in the present series are those of the semicircular canals, particularly of the horizontal canals: flattened horizontal canal cristae, absence or opening of the utricular endolymphatic valve, small facial nerve, and obtuse angle of the geniculate area of the facial nerve. Each ear demonstrated more than one of those anomalies. The anomalies present features similar to those found in the structures of the normal
