Microtia, along with trauma, represents one of the main causes of external ear malformation. Different clinical techniques were developed for the reconstruction of the auricle, but they all have some drawbacks. This work is focused on the development of an innovative 3D porous scaffold, printed by Fused Deposition Modelling (FDM) and based on laser-scanned images of the healthy contralateral ear of the patient. The scaffold was printed using polycaprolactone (PCL) and thermoplastic polyether urethane (TPU) to mimic the components of the cartilage and adipose tissue, respectively. After the opt

OBJECTIVE: This study aims to compare the postoperative thoracic deformity and scar aesthetic outcomes of auricular reconstruction between the conventional large-incision technique and small-incision technique with perichondrium preservation for costal cartilage harvesting.
METHODS: A retrospective analysis was performed on 134 patients who underwent auricular reconstruction using tissue expanders and autologous costal cartilage harvested from the right chest wall between January 2021 and September 2023. Patients were divided into two groups according to the harvesting technique: the traditio

Treatment decision-making is an integral but complex part of healthcare, particularly in the context of craniofacial surgeries. The aim of the current study was to explore caregiver narratives to inform future surgical care delivery and best practice. 'Life Story' narrative interviews were conducted with US English- and Spanish-speaking caregivers (n = 62) of children aged 3-17 years with craniofacial microsomia (CFM). Extracts relating to treatment decision-making were inductively coded using Reflexive Thematic Analysis. Four themes were identified: 1) 'Grappling with Difference' exemplifies

BACKGROUND: This study introduces a construction method for a multilayered three-dimensional costal cartilage auricular framework and retrospectively analyzed its clinical effectiveness in auricular reconstruction with an expanded postauricular skin flap.
METHODS: We designed multiple techniques for constructing a multilayered three-dimensional costal cartilage auricular framework to enhance stability and aesthetic outcomes. Postoperative follow-up included a comprehensive morphological assessment of the reconstructed ears, as well as evaluations of postoperative complications and patient sat

BACKGROUND: Serratus anterior plane block (SAPB) and parasternal block (PSB) are used in multimodal analgesia strategies to improve postoperative comfort and recovery. Their combined effectiveness for pediatric auricular reconstruction (AR) patients needs clarification.
METHODS: A study was conducted on 100 children aged 8 to 12 undergoing autologous rib cartilage transplant in AR. Subjects were randomized into two groups: the ultrasound (US)-directed SAPB plus modified PSB group (SAPB+PSB group) and the US-guided SAPB alone group (SAPB group). The primary outcome was the 24 h postoperative Q

BACKGROUND: Auricular deformities are common in newborns and require early diagnosis and timely intervention. Several factors highlight the necessity of a machine learning-based diagnostic solution: the high prevalence of these conditions, the narrow time window for effective non-surgical treatment, limited medical resources, and the importance of both physical and mental well-being. This study presents a novel artificial intelligence (AI) model to identify and classify common sub-types of auricle deformities, using photos taken with mobile devices.
METHODS: The dataset was made up of the ope

Patients suffering from microtia have limited treatment options for auricular reconstruction due to donor-site morbidity, complications, and unaesthetic outcome. Therefore, tissue engineering emerged as an alternative therapeutic option. Here, we generated and characterized human auricular cartilage using differentiated human adipose mesenchymal stem cells (hASCs) combined with human auricular chondrocytes. The differentiated hASCs were analysed for their morphology, phenotype, gene, and protein expression of chondrogenic markers, and biochemical composition at different time points in 2D and

Reconstructing auricular tissue is challenging because ear cartilage has few blood vessels and limited regenerative capacity. Traditional methods that utilize autologous costal cartilage or synthetic polymers often lead to donor site morbidity and suboptimal biocompatibility. In this study, we introduce 3D-printed scaffolds composed of decellularized human cartilage-derived bioink combined with polycaprolactone (PCL), designed to enhance both tissue regeneration and mechanical stability. The decellularization process effectively removed cellular components while preserving glycosaminoglycan an

BACKGROUND: Autologous microtia reconstruction is a highly specialized procedure with the potential to significantly improve patient quality of life. However, it has a complex patient pathway including a multistaged procedure, the environmental impact of which has yet to be explored.
METHODS: A retrospective study of 23 patients undergoing autologous microtia reconstruction at a single center was performed. Process mapping and life cycle analyses were performed for surgical and inpatient stay, including facilities, consumables, medical gases, equipment, food and linen, and travel. Estimates f

This study aims to investigate the effects of ethnic, racial, and socioeconomic differences on outcomes for patients undergoing surgical management for microtia or anotia in the United States. Using the Pediatric Health Information System (PHIS) database, 2928 patients presenting for primary microtia or anotia reconstruction between 2017 and 2023 were analyzed. The majority of patients were non-White (55.2%, 1617/2928), with a significant proportion identifying as Hispanic/Latino (48.5%, 1421/2928). Non-White patients were more likely to present at a later age for surgery (10.8 versus 9.4 y; P

Caregiver psychosocial adjustment can be impacted by their children's early treatment for craniofacial microsomia (CFM). A deeper understanding of caregiver early experiences is needed to advance family-centered clinical care. This study aimed to describe the range of caregiver experiences of CFM care for their young children to inform recommendations for providers. As part of the "Craniofacial Microsomia: Accelerating Research and Education" (CARE) program, health histories and narrative interviews were completed with a national sample of US caregivers (N=42) of children with CFM aged 3 to 17

OBJECTIVE: This systematic review aims to determine the impact of social determinants of health (SDoHs), such as race, ethnicity, socioeconomic status, and insurance status, on access to care for Hispanic children with otologic conditions. Identifying targetable disparities is vital to improve health equity in the United States.
REVIEW METHODS: A systematic review of PubMed and Scopus databases, all years through February 2023, for articles evaluating health disparities related to pediatric otology conditions or settings was conducted. Inclusion criteria included all peer-reviewed publication

Microtia is a congenital malformation characterized by underdevelopment of the external ear. While chondrocyte dysfunction has been implicated in microtia, the specific cellular abnormalities remain poorly understood. This study aimed to investigate mitochondrial dysfunction in microtia chondrocytes using single-cell RNA sequencing. Cartilage samples were obtained from patients with unilateral, non-syndromic microtia and healthy controls. Single-cell RNA sequencing was performed using the 10 × Genomics platform. Bioinformatic analyses including cell type identification, trajectory analysis, an

PURPOSE: This study evaluated the audiometric outcomes and complications of simultaneous bone conduction device (BCD) implantation and auricular reconstruction, and the comparative effectiveness of three BCDs.
METHODS: In total, 41 patients with bilateral microtia (ranging from 8 to 16 years old) who underwent combined surgery from January 2018 to January 2024 were retrospectively analyzed. Audiometric parameters (free-field thresholds and speech recognition scores) and complications were compared across the groups of patients who received the Baha Attract (n = 13), Sophono (n = 18), and Bone

BACKGROUND AND PURPOSE: Use of autogenous cartilage for ear reconstruction is the gold standard; however, its harvest is associated with morbidity and limited availability in children. This study systematically reviews literature on the safety and efficacy of ear reconstruction with cartilage allograft.
DESIGN: A systematic review per the PRISMA guidelines.
PATIENTS: Patients with a history of acquired or congenital ear defects who underwent reconstruction with cartilage allografts.
RESULTS: The search yielded in 178 articles and 10 studies were included. All were retrospective case series
