Malaria, primarily caused by Plasmodium falciparum (Pf), remains a significant public health concern in Southeast Asia and sub-Saharan Africa, disproportionately affecting pregnant women, leading to adverse maternal and fetal outcomes. Placental malaria (PM) is characterized by sequestration of P. falciparum infected erythrocytes in intervillous space, which causes activation of immune response at maternal-fetal interface and altered expression of angiogenic regulators like VEGF, sVEGFR1, Angiopoietins (ANG), sEndoglin, and placental growth factor (PLGF). Emerging evidence suggests that the la

BACKGROUND: Cerebral malaria is a complication of Plasmodium falciparum infections where the integrity of the blood-brain barrier (BBB) is compromised, frequently leading to persistent neurological sequelae or death. The current treatment for cerebral malaria is based on rapid elimination of the parasite with intravenous anti-malarial drugs, but there is no adjunctive treatment that could address the loss of BBB integrity, potentially decreasing sequelae and improving the survival rate. Prenylation, a cellular process that incorporates isoprenoid lipids to specific protein sites, is known to r

Cerebral malaria (CM) is a severe complication of Plasmodium infection, classically attributed to parasite sequestration and neuroinflammation. Here, we uncover a spleen-centered humoral autoimmune circuit that drives CM pathology. Proteomic analyses identify CD36 as a dominant host-derived antigen enriched in infected red blood cells (iRBCs), triggering anti-CD36 autoantibody production in patients with falciparum malaria. Although contributing to iRBCs clearance, these autoantibodies also target other CD36-expressing cells, thereby driving thrombocytopenia, endothelial injury, and macrophage

Plasmodium knowlesi is an emerging zoonotic malaria species and a leading cause of malaria in Malaysia. Although severe disease is increasingly recognised, cerebral malaria remains uncommon and is traditionally associated with Plasmodium falciparum. We report a fatal case of cerebral malaria caused by P. knowlesi in a previously healthy 61-year-old woman with a history of frequent orchard visits for peridomestic agricultural activities. She presented with rapid neurological deterioration and circulatory shock, complicated by an acute pontine infarction. Peripheral blood smear and polymerase ch

Opsoclonus myoclonus syndrome (OMS) is a rare inflammatory disease of the central nervous system characterized by ocular movements, ataxia, irritability, myoclonus, and sleep disturbances and is thought to result from variable etiologies. We report a case of OMS following severe malaria in a patient living with Human Immunodeficiency Virus (HIV). This is a 47-year-old woman, known to be HIV-positive, for the past 6 years, and on highly active antiretroviral therapy (HAART) with Tenofovir-Lamivudine-Atazanavir, to which she is strictly compliant. Her last viral load was undetectable. Two weeks

UNLABELLED: Primary CNS lymphoma (PCNSL) is a rare subtype of non-Hodgkin lymphoma with the worst outcomes amongst all extranodal lymphomas. There is a scarcity of data on real-world outcomes of primary CNS lymphoma (PCNSL) owing to the rarity of the disease. This study analyzed the demographic patterns, risk stratification, treatment regimens used, & outcomes of patients treated at Tata Memorial Center Mumbai, India. This is a retrospective analysis of newly diagnosed primary CNS lymphoma patients treated at our centre over seven years from January 2013 to December 2019. A total of 142 patien

PLAIN ENGLISH SUMMARY: There are new ways to engage people with science and research but many patient support groups and charitable organisations still hold traditional meetings to provide updates on their activities and to report new developments in their field of interest. These meetings often feature presentations given by medical doctors or, in the case of research-focussed organisations, by research scientists.Receiving feedback from people who are confused and sometimes upset by some types of information, and the way it is presented at meetings, made us think about better ways for resear

Malaria, caused by Plasmodium spp., remains a major public health problem. Cerebral malaria is its deadliest form, with a 15-25% mortality rate, despite artemisinin-based treatments. In addition, the World Health Organization (WHO) strictly defines cerebral malaria as the presence of coma, 1 h after a seizure or the correction of hypoglycemia, in patients with P. falciparum parasitemia. Consequently, 25% of survivors experience neurocognitive and behavioral sequelae, particularly in children. However, more recently, neurocognitive and behavioral impairments were also reported in severe non-cer

A 37 year old woman presented with a history of weakness and lethargy for many years. She had been amenorrhoeic for 5 years after a still birth at 37 weeks gestation, due to severe falciparum malaria. On examination, she appeared pale and lethargic, with a blood pressure of 100/70 mm Hg. Her systemic examination was unremarkable. Laboratory tests revealed low 9.00 am serum ACTH and cortisol levels, along with low free T4, FSH and LH levels. A short synacthen test showed an inadequate cortisol response at 30 minutes. MRI of the Pituitary gland displayed an empty sella. She was diagnosed with hy

Infection with Plasmodium falciparum carries a significant risk of cerebral malaria (CM). Children are particularly susceptible to human CM (HCM) which manifests as an acute neurovascular encephalopathy leading to high levels of mortality. Occurring in parallel with CM, malarial retinopathy (MR) is readily detected on ophthalmoscopy as one or more of: white-centered retinal hemorrhage, retinal whitening, and vessel discoloration. It leads to several distinct types of blood retinal barrier (BRB) breakdown. The precise molecular mechanisms underpinning CM and MR remain ill-defined, but parasitem

Cerebral malaria (CM), a life-threatening complication of Plasmodium falciparum infection, is characterized by the sequestration of infected erythrocytes in the brain microvasculature. Our study investigated the potential of repurposing tyrosine kinase inhibitors targeting BCR-ABL1 (BCR-ABL drugs), which are also known to be effective against P falciparum blood-stage parasites, for mitigating inflammation and blood-brain barrier breakdown in CM. Our analysis demonstrated differential protective effects of BCR-ABL drugs on primary human brain microvascular endothelial cells exposed to thrombin

The application of neuroimaging techniques to patients with Plasmodium falciparum infection has uncovered a wide range of brain changes not only in cerebral malaria but also in noncomatose patients. We propose several hypotheses to unify findings across the spectrum of clinical malaria in adults and highlight the urgent need to evaluate potential long-term effects of cerebral alterations on neurocognition in this understudied age group.

Acute kidney injury (AKI) is increasingly recognized in pediatric severe malaria, but an understanding of contributory mechanisms is lacking. We evaluated potential hemodynamic contributors to AKI in children with cerebral malaria. Structural AKI was defined using urine neutrophil gelatinase-associated lipocalin (uNGAL) values >125 ng/mL. We included 62 patients (median age 53.5 [31, 71] months), and 38 (61%) had AKI at hospital admission. Blantyre Coma Score <2 (aOR 9.1, 95%CI 1.8-43, P = .003), a low-flow phenotype on transcranial doppler ultrasound (TCD) (aOR 2.3, 95%CI 1.1-7.7, P = .05), a

Severe malaria is associated with kidney and brain injury, yet potential mechanisms linking both complications remain understudied. We investigated the associations between kidney and brain injuries in a cohort of Indian adults and children with severe Plasmodium falciparum malaria. We found that acute kidney injury was prevalent in both adults (64.4%) and children (71.4%). We also found that plasma levels of the structural kidney injury biomarker neutrophil gelatinase-associated lipocalin (NGAL) were strongly associated with acute kidney injury severity (P < .0001) and negatively correlated w

Cerebral malaria (CM) is a deadly complication of P. falciparum infection. Although adults with CM have a higher mortality rate, CM affects mostly children under the age of 5 years. Neurological symptoms and signs include impaired consciousness, coma, seizures, and increased intracranial hypertension. Upon survival of a CM episode, persistent neurologic deficits occur in a subset of surviving children. These sequelae include recurrent seizures, behavioral deficits, loss of developmental milestones, learning disabilities and attention deficit hyperactivity disorder, which can remain with the su

Jaundice is a common presentation of Plasmodium falciparum malaria, which arises from the accumulation of circulating bilirubin. It is not understood whether it represents an adaptive or maladaptive response to Plasmodium spp. infection. We found that asymptomatic P. falciparum infection in humans was associated with a higher ratio of unconjugated over conjugated bilirubin and parasite burden compared with symptomatic malaria. Genetic suppression of bilirubin synthesis by biliverdin reductase A (BVRA) increased parasite virulence and malaria mortality in mice. Accumulation of unconjugated bili

Malaria, a global health challenge, remains a leading cause of morbidity and mortality, particularly in sub-Saharan Africa, Southeast Asia, and South America. While traditionally associated with fever and systemic complications, the neurological impact of malaria, including stroke, has become a significant concern. This review aims to examine the incidence, clinical presentation, and outcomes of stroke in individuals with malaria, highlighting the role of malaria in both ischaemic and haemorrhagic strokes. A literature search identified nine studies published between 1999 and 2024, comprising

Sickle-cell disease (SCD) is a group of inherited blood disorders in which a mutation in the β-globin (HBB) gene causes red blood cells to produce abnormal hemoglobin, known as Hb S. SCD is characterized by an autosomal-recessive pattern of inheritance, implying that for a child to manifest the condition, they must inherit an Hb S allele from both parents (HbSS) or one Hb S allele and another β-globin variant, such as Hb C or β-thalassemia (HbSC, HbS/β-thal). It has been observed that (heterozygote) carriers of one copy of the sickle-cell trait (HbAS) are typically healthy and can even gain pa

BACKGROUND: Cerebral malaria (CM), a fatal neurological complication of Plasmodium falciparum infection, is partially driven by neuronal injury. Emerging evidence highlights exosomes as vital mediators of mast cell-neuron interactions in neurological disease progression. While mast cells and their exosomes were previously shown to exacerbate experimental cerebral malaria (ECM) severity, the specific role of mast cell-derived exosomes in CM-associated neuronal injury remains unclear.
METHODS: Exosomes were isolated from resting and lipopolysaccharide (LPS)-activated P815 mast cells (denoted as
