The importance of conchal resection in correcting the prominent ear.

The authors believe that conchal hypertrophy plays a more significant role in ear prominence than has been indicated in the literature. Instead of focusing on the antihelical fold, this otoplasty technique emphasizes chondrocutaneous resection. With even limited resection and resuturing of the cut concha, the antihelix yields to posterior suture placement with a soft, smooth, rounded shape unmarred by any sharp, irregular surfaces.

Otoplasty.

BACKGROUND: Auricular deformities, specifically, prominent ears, are relatively frequent. Although the physiologic consequences are negligible, the aesthetic and psychological effects on the patient can be substantial. METHODS: Otoplasty techniques are used to correct many auricular deformities, including the prominent ear, the constricted ear, Stahl's deformity, and cryptotia. Various treatments and techniques have been developed for the correction of these deformities, including methods that excise, bend, suture, scratch, or reposition the auricular cartilage. RESULTS: The multitude of dif

Anthropometric growth study of auricle of healthy preterm and term newborns.

OBJECTIVE: Auricular abnormalities are important for early diagnosis of the birth defects in the prematures and newborns. Auricular antropometric studies in healthy premature and mature population depend on their gestational age are limited and insufficient. The aims of this study were to reveal antropometric growth and dynamics of the auricle in the healthy newborns from the 28th to the 42nd gestation weeks. MATERIALS AND METHODS: A total of 600 newborns were evaluated in 40 groups. Each group comprising 20 preterm or term newborns according to their sexes and gestational weeks. Six surface

Otoplasty.

Multiple techniques have been advocated over the years for the appropriate and natural-appearing correction of the prominent ear. There is no single technique that can re-create the complex three-dimensional nature of the otherwise normal human ear. We have to evaluate each individual ear and patient and correct the abnormalities with a technique that each surgeon feels comfortable with and that can repeatedly provide gratifying results. Ultimately, the simplest technique that can obtain the maximum effect should be employed. A concentration on the male patient and specific concerns with these

Deletion involving the TWIST locus and the HOXA cluster: a contiguous gene syndrome on 7p?

Deletion of TWIST on 7p21 leads to Saethre-Chotzen syndrome, whereas deletion of the HOXA cluster on 7p15.2 leads to hand-foot-genital syndrome. We report here a patient with 46,XY,del(7)(p15.2p21) who had craniosynostosis, maxillary hypoplasia, prominent ear crus, rectoperineal fistula, and hypoplastic fifth fingers. Using fluorescence in situ hybridization, we demonstrated the deletion to encompass the TWIST locus and the HOXA cluster. We suggest that many, if not all, of the features of this patient could be accounted for by combined haploinsufficiency of the TWIST and HOXA cluster. Hence,

Cartilage-sparing complete otoplasty technique: a 10-year experience in 100 patients.

Otoplasty is one of the most frequently performed esthetic surgical procedures in children and adolescents. Several techniques can give satisfactory results, but few address all the components of the prominent ear deformity. The author reports on the evolution and application of a cartilage-sparing otoplasty technique that addresses all the components of the prominent ear deformity. One hundred patients (200 ears) were treated over a 10-year period. An 8% revision rate and minimal complications were encountered.

Otoplasty: evaluation, technique, and review.

Prominent ear deformity is the most common abnormality of the external ear. Over two hundred different techniques have been described to correct this deformity. Many of these techniques have proven successful in their ability to achieve high patient satisfaction despite the significant variations in these techniques. From this perspective otoplasty is a privileged procedure allowing the surgeon great latitude in his approach and ability to achieve patient satisfaction. Despite high patient satisfaction, each technique has inherent strengths and weaknesses. The art of otoplasty is in the abilit

Surgical correction of prominent ear using modified tube technique and posterior approach.

In patients with a prominent ear, the most common deformities are a poorly developed antihelical fold and the formation of excessive conchal cartilage, particularly in the posterior conchal wall. As such, the main goal for surgical correction of a prominent ear is to narrow the conchoscaphal angle by folding the antihelix and reducing the concha. In the current study, cartilage-sparing tubing otoplasty is refined by the addition of a minimal dissection of the edge of the conchal and scaphal cartilages, and horizontal mattress sutures between the two edges. Nine patients with 13 cases were oper

Anthropometric growth study of normal human auricle.

OBJECTIVE: The aim of this study was to reveal the anthropometric growth of auricula from birth to the age of 18 years and to bring out the dynamics of ear growth. MATERIAL AND METHODS: A total of 1552 children in 50 groups were evaluated. Six surface measurements were performed directly on the right auricle of the subjects: the length from the superaurale to subaurale, the width from the tragus to helix, the width from the tragus to antihelix, the conchal depth, the height from the helix to mastoid at superauraler level, and the height from the helix to mastoid at tragal level. Combined otoplasty technique: chondrocutaneous conchal resection as the cornerstone to correction of the prominent ear.

The failure to correctly analyze the complex deformity that comprises the prominent ear is the most common cause of the undesired surgical result. Recognizing conchal hypertrophy as one of the leading causes of the prominent ear deformity has led to a versatile and reproducible otoplasty technique that is easily modified to the common side-to-side variation in deformity. From January of 1993 to August of 2000, chondrocutaneous conchal resection has been used as the cornerstone to a combined sequence of maneuvers in otoplasty. The operation was performed on 47 patients (40 bilateral cases and 7

New modification in otoplasty: anterior approach.

After harvesting the conchal cartilage in a large series of secondary rhinoplasties using the anterior approach, there was a marked improvement in the shape of the auricle, with unnoticeable scarring hidden under the convolution of the antihelix. In this series of 250 cases, there were no hypertrophic scars or keloid formations. Based on these favorable results, a new modification of otoplasty was developed using only the anterior approach. Between 1992 and 2000, 108 otoplasties were performed on 55 patients to correct the prominent ear using only the anterior approach. All maneuvers used in m

Contact dermatitis complicating pinnaplasty.

Proflavine allergy is uncommon, occurring in approximately 6% of patients attending contact dermatitis clinics. Proflavine wool is used by many surgeons in the UK as a dressing that can be moulded to conform to the contours of a corrected prominent ear. It may have bacteriostatic properties. We present a case where contact dermatitis in response to proflavine developed after pinnaplasty. This caused diagnostic confusion, a lengthened hospital stay and an unsightly hypertrophic scar.

The crumpled-ear deformity.

The common characteristics of the crumpled-ear deformity, the steps for surgical correction, and the authors' experience are described in this article. Commonly, the ear has a folded-over appearance; a normal length and width when unfurled; and wrinkling of the skin and cartilage of the helical rim, scapha, antihelix, and concha. Principles of correction of this deformity include (1) superficial scoring of the concave segments of the cartilage to open the wrinkled segments, (2) creating the antihelical fold by a mattress-suture technique, (3) repositioning of the helical rim, and (4) repositio

Closed anterior scoring for prominent-ear correction revisited.

The closed-anterior-scoring technique has been used over the past 3 years to correct 56 prominent ears in 32 patients at the West Midlands Regional Plastic Surgery Unit at Wordsley Hospital. A review was carried out to assess the result of this surgical procedure. We briefly discuss the historical development of other surgical techniques for prominent-ear correction, and describe in detail the operative technique for this procedure, which includes closed scoring and suturing of the cartilage. We used this technique to treat 24 patients with bilateral prominent ears and eight patients with unil

Aesthetic otoplasty with remodeling of the antihelix for the correction of the prominent ear: criteria and personal technique.

A brief account of some important aspects of the embryological formation of the external ear is presented. Family and patient motivations to correct the aesthetic aspect of the prominent ear caused by the lack of development of the antihelix are discussed. The criteria for the selection of the patients and the ideal age for the surgical procedure are analyzed. The objectives of otoplasty for the correction of this deformity and the personal technique of the author are described. Emphasis is placed on the author's original contributions to this procedure and the results. The possible postoperat

The fissura antitragohelicina: an anatomic aid to the correction of prominent ears.

One of the important features of correction of prominent ears involves the creation of an antihelical fold in the ear cartilage. The precise and symmetrical location of this fold is crucial for the aesthetic result. This study investigated the use of the fissura antitragohelicina, a constant anatomic landmark, as a guide to the correct line for the new antihelix. In the first part of the study, 16 cadaveric ears were dissected. The fissura antitragohelicina was present in each specimen, and measurements of the distance between the fissura antitragohelicina and the helix and the antihelix were
