Dynamometric Analysis of Normative Auricular Stiffness and Comparison With Operated Prominent Ears.

AIM: Stiffness of the auricular cartilage is the main determining factor for the choice of operative technique of the prominent ear deformity. The aim of this study is to evaluate the stiffness of normal appearing ears objectively and quantitatively, compare the results with the operated prominent ear patients, and present prospective short-term dynamometric evaluation of the operated prominent ear patients. PATIENTS AND METHODS: A total of 190 volunteers without ear deformities were recruited and 9 age groups were formed: group (5-9), group (10-14), group (15-19), group (20-24), group (25-29

Relapsing Polychondritis Presenting with Meningoencephalitis and Dementia: Correlation with Neuroimaging and Clinical Features.

PURPOSE: Relapsing polychondritis (RP) is a rare systemic autoimmune disease affecting cartilaginous and non-cartilaginous structures. Neurological involvement is rarer but results in profound disability. Early identification and treatment of underlying RP may promote neurological recovery. CASE REPORT: We illustrated a 53-year-old man diagnosed with dementia. Neuroimaging and cerebrospinal fluid studies disclosed meningoencephalitis. "Prominent ear sign" was evident on diffusion-weight magnetic resonance imaging. After glucocortisone administration, the improvement of clinical manifestations

New otoplasty approach: a laterally based postauricular dermal flap as an addition to Mustarde and Furnas to prevent suture extrusion and recurrence.

Prominent ear is the most common deformity of the external ear. The major causes can be an underdeveloped antithetical fold, concha hypertrophy, and prominence of the ear lobule. Since Ely's first aesthetic correction of the prominent ear in 1881, more than 200 different techniques have been described, but the choice of procedure still remains the surgeon's preference. This report presents the laterally based posterior auricular dermal flap technique as an adjunct to the conventional cartilage-sparing otoplasty. An elliptical skin incision was planned according to the classic prominent ear cor

Cartilage-sparing techniques versus percutaneous adjustable closed otoplasty for prominent ear deformity.

OBJECTIVE: The goal of this study is to follow longitudinally the prominent ears treated by either cartilage-sparing techniques (CSTs) or percutaneous adjustable closed otoplasty (PACO) and compare them as to efficacy, reoccurrence, complications, and patient contentedness. METHODS: The CSTs were applied to the first group, that is, 17 patients (32 ears) with stiff auricular cartilage; whereas PACO was applied to the second group, 15 patients (28 ears) with soft auricular cartilage. Auriculocephalic distances were recorded at 4 levels preoperatively, at the completion of the surgery, and agai

Reconstructing a large helical rim defect: synergy of a postauricular perforator flap, diced cartilage, and otoplasty technique.

Large full thickness helical ear defects are a challenge to reconstruct. A 25-year-old woman presented to us with loss of a large portion of the helical rim after an assault. A successful 2-stage reconstruction was performed, incorporating the principles of perforator flaps, otoplasty techniques, and use of diced cartilage more commonly used for rhinoplasty. There was a 5 × 0.5-cm central helical defect of the right ear. In the first stage, the original defect was recreated and a 7 × 1.5-cm inferiorly based postauricular artery perforator flap was raised from the right postauricular sulcus. Di

Asymmetric Z-plasty for telephone deformity in prominent ear correction.

There have been plenty of surgical techniques for the correction of prominent ears. "Telephone deformity" or "reverse telephone ear" has been described as the undesired result of the inappropriate correction of the prominent ear, mainly the deficient correction of the lobule. We have performed an asymmetric Z-plasty to the lobule to overcome this deformity not only by excision of the excess skin but also by transposition of the soft tissue. Between 2005 and 2011, the technique currently described was performed in 19 patients. Preoperative, intraoperative, and postoperative standardized photogr

A modification of incisionless otoplasty for correcting the prominent ear deformity.

This article describes a modification of the incisionless otoplasty. We investigated the complication rates, recurrence risks, and patient satisfaction with this modified procedure. In total, 26 patients (49 ears) complaining of prominent ear were operated on. Auriculocephalic distances were measured at three different levels, pre-operatively, at the end of the surgery, and at 4 weeks and 6 months after surgery to evaluate the efficacy of the technique. Patient satisfaction was evaluated using a visual analog scale and the global aesthetic improvement scale was applied by an independent non-pa

Otoplasty: the case for skin incision by higher volume operators.

Prominent ear correction in the paediatric population is a common and popular procedure, with demonstrable benefit in alleviating psychological distress. Posterior cartilage-sparing techniques for otoplasty have been shown to be consistently safer and better than cartilage-scoring alternatives, however excision of the posterior auricular skin during otoplasty, although a common first step, has not been shown to have any benefits over skin incision alone. In this study, we examined the association between skin excision and recurrence of ear prominence, by examining the collective surgical outco

An Unexpected Otoplasty Complication: Temporal Nerve Paresis.

Otoplasty, for prominent ear correction, is the most common aesthetic surgery procedure done in children. While the outcome often has a positive impact on the psychological state of the patient, the procedure itself does have associated complications that must be considered. Common complications include hematomas, infection, necrosis, outer ear canal stenosis, extrusion of sutures, insufficient correction, overcorrection, keloids, and hypertrophic scars. This is the first case report of facial nerve palsy as a complication following otoplasty.

An unusual cause of conductive hearing loss: bilateral complete meatal obstruction following otoplasty.

There are many causes of conductive hearing loss. Otoplasty is a commonly performed cosmetic surgery to correction for prominent ear. We discussed an unusual cause of conductive hearing loss, bilateral complete meatal obstruction following otoplasty, in this article. This complication was probably due to unsuitable placement of the Furnas sutures. In the literature, as a cause of conductive hearing loss, unilateral complete meatal obstruction has been described rarely, but bilateral complete obstruction has not been defined. Hearing loss recovered completely after revision surgery. Correct dia

Factors affecting professional delay in diagnosis and treatment of oral cancer in Iran.

BACKGROUND: Total colonic aganglionosis (TCA) is present in 4% - 5% of the patients with Hirschsprung's disease and has a high surgical mortality rate between 13% - 23%. Diagnosis and treatment of TCA is still a major challenge for pediatric surgeons. Many techniques with several advantages and disadvantages were established for its treatment. We have performed State's pull-through as total colectomy and ileo-proctostomy with long posterior rectal myotomy in TCA and severe dysmotility disorders. METHODS: In this retrospective study, 13 patients with TCA were evaluated from 1992 through 2012 i

Splitted posterior auricular muscle flap combined with traditional otoplasty.

OBJECTIVE: This article presents a modified technique to split and reposition the posterior auricular muscle flap as an adjunct to traditional otoplasty. BACKGROUND: Although numerous techniques have been described for the correction of prominent ear deformity, there is still a continuing debate on which operative method achieves the best results. Recurrence of the deformity or the need for secondary revisions is still among the common problems. We aimed to focus on a technical modification of the use of the postauricular muscle flap to strengthen the effect of otoplasty and to obtain more pr

Dysmorphic features in subtelomeric 20p13 deletion excluding JAG1: a recognizable microdeletion phenotype?

We report a 19 year-old patient carrying a terminal 20p microdeletion. She displayed clinical features resembling those of two other previously described patients. We suggest that a specific phenotype can be associated with this chromosomal anomaly. Mental retardation, epilepsy, and dysmorphic signs including low-set ears and overfolded helices seem highly characteristic of this syndrome and may define major diagnostic criteria of a recognizable phenotype. Delayed closure of fontanella, delayed permanent teeth eruption, visual disturbances, prominent ear lobes, prominent nasal root and ridge,

Antihelical shaping of prominent ears using conchal cartilage-grafting adhesion.

OBJECTIVES/HYPOTHESIS: No specific otoplasty method has been considered as the absolute standard in patients with prominent ears showing thick and strong auricular cartilage. STUDY DESIGN: Retrospective case series study. METHODS: The authors reviewed the chart and preoperative and postoperative photos of patients (N = 66) who underwent otoplasty consecutively with a new antihelical shaping technique using conchal cartilage-grafting adhesion between 2003 and 2010. Three mattress sutures between the posterior scapha and concha wall were performed to create an antihelical tubing. A piece of co

Identification of congenital auricular deformities during newborn hearing screening allows for non-surgical correction: a Mayo Clinic pilot study.

OBJECTIVE: To introduce a Mayo Clinic pilot study in which newborn hearing screeners are trained to identify congenital auricular deformities, allowing for non-surgical correction with a simple splint initiated in the immediate neonatal period. METHODS: Newborn hearing screeners received education on evaluation of congenital auricular deformities. Ten infants with 19 affected ears amenable to treatment with a simple splint were enrolled between June 15 and December 10, 2009. Splinting was initiated prior to the infant's discharge from the hospital. RESULTS: Congenital auricular deformities a

Correction of the bilateral question mark ear deformity using double-opposing z-plasty and a chondrocutaneous flap.

OBJECTIVE: To describe an alternative technique for correcting a rare and complex ear deformity. INTRODUCTION: The question mark ear deformity has been described as a congenital cleft between the helix and the lobule in addition to a prominent ear. Here we describe a novel technique using double-opposing Z-plasty and a chondrocutaneous flap to correct the deformity without significant reduction in auricle height or creation of a donor-site defect. METHODS: An anterior/posterior double-opposing Z-plasty and a superiorly based chondrocutaneous flap were designed at the level of the cleft. Flap

Creating a neoconchal complex using the adjustable conchal sliding technique in prominent ear correction.

An oversized aural concha plays a significant role in prominent ear deformities and should be taken into consideration during preoperative examination. In the current study, we present a procedure known as the conchal sliding technique as an alternative to more disruptive methods. Twenty-four patients (9 females and 15 males; 47 ears in total) underwent a conchal sliding procedure between 2006 and 2010. During the surgery, a wide subperichondrial dissection is performed after a posterior elliptical incision. After the concha is exposed as a hemisphere, it is split along its long axis to reveal

Neonatal molding in deformational auricolar anomalies.

INTRODUCTION: Congenital auricular anomalies can be categorized either as malformational or deformational. The first are characterized by a partial absence of the skin or cartilage resulting in a constricted or underdeveloped pinna and require surgical correction. Deformations are characterized by a misshaped but fully developed pinna and are best treated by auricular molding. AIM: Authors want to present their case load in treatment of infants affected by deformational auricolar anomalies and describe their techniques using early splinting for congenital auricular deformities, like prominent

Understanding the unfavorable result after otoplasty: an integrated approach to correction.

The correction of an unfavorable outcome after otoplasty requires a thorough understanding of the anatomy of prominent ear and recognition of the spectrum of secondary deformities and their origin. The goal of this article is to describe the causes of postotoplasty deformity, including both undercorrection and overcorrection. The latter presents the more complicated reconstructive problem, as both skin shortage and permanent cartilage disruption need to be addressed. The authors propose an algorithm for revision otoplasty based on clinical findings and patient concerns. Finally, a case with ov
