Commisuroplasty for Macrostomia in Tessier 7 Cleft Using Modified Vermillion Square Flap Technique.

BACKGROUND: The transverse facial cleft, as described by Tessier, is a rare anomaly that affects both aesthetics and function. Various techniques for commissuroplasty have been discussed in the literature, often with inconsistent results. In this case series, the authors used a modified vermilion square flap technique to achieve satisfactory oral function and aesthetic outcomes. METHODS: Five Tessier 7 cleft patients underwent reconstruction using vermilion square flap technique with W-plasty for the skin closure to help create a less noticeable scar. Evaluation was conducted at least 1 year

Macrostomia Surgery Using Small Z-plasty for Construction of the Corner of the Mouth.

Macrostomia is a type of transverse facial cleft (termed number 7 in the Tessier classification) and a disorder in which there is a cleft on 1 or both sides of the corner of the mouth. The Kaplan technique is used for the treatment of macrostomia and is characterized by forming the corners of the mouth with a single continuous flap of vermilion. In this technique, there are no suture lines at the commissure; thus, it does not cause contracture at the corners of the mouth. We performed a small Z-plasty on the outer edge of the flap at the corner of the mouth, constructed using the Kaplan techni

Lip Reconstruction Following Cancer Resection.

BACKGROUND: An analysis of all possible lip defects and a comparison of outcomes following various reconstructive techniques is lacking in the literature. METHODS: An analysis of lip reconstructions following cancer resection from 1994 to 2024 was performed. RESULTS: A total of 221 patients underwent lip reconstruction. Tobacco use (p = 0.003), diabetes mellitus (p = 0.042), and concurrent mandible or maxillary reconstruction (p = 0.019) were associated with surgical complications. Recurrent disease (p = 0.011) and oral commissure resection (p = 0.020) were associated with oral incompetence.

A Rare Case of Bilateral and Midline Cleft Palate With Bilateral Macrostomia.

A 6-day-old girl presented with bilateral macrostomia, micrognathia, and a complex cleft palate consisting of a midline submucous cleft, left lateral submucous cleft, and a right lateral cleft with partial soft-palate agenesis. Owing to Pierre Robin Sequence, surgery was deferred until airway stability. Bilateral macrostomia repair was performed at 12 months, followed by one-stage correction of all palatal clefts at 17 months using a right buccal myomucosal flap for oral lining. Rare clefts variably labeled as oblique clefts or soft-palate agenesis should be uniformly termed "lateral cleft pal

A rare case of unilateral parotid gland agenesis.

Parotid gland agenesis is a rare congenital anomaly with fewer than 30 cases reported in the literature. It can present with xerostomia, facial asymmetry, recurrent infections, or may be entirely asymptomatic. Due to its rarity and potential to mimic other pathological conditions, especially in cases of facial asymmetry, awareness is crucial for accurate diagnosis. We report the case of a 39-year-old man referred to our clinic for routine follow-up of a thyroid nodule. The patient had no history of systemic illness or symptoms related to salivary gland dysfunction. During neck ultrasonography,

Insulin resistance and cognitive decline: the metabolic mechanisms linking type 2 diabetes to Alzheimer's disease.

INTRODUCTION: Abnormal liver function tests (LFTs) in pregnancy require proper interpretation in order to avoid pitfalls in the diagnosis. The underlying disorder can have a significant effect on the outcome of both mother and foetus. The present study was done with the objective to study the clinical profile, incidence and possible causes of derangements of liver function tests. MATERIAL AND METHOD: Eighty pregnant women with abnormal liver dysfunction were studied prospectively. Women with chronic liver disease and drug-induced abnormal liver function test were excluded. All available LFTs

Risk factors and treatment strategies for adjacent segment disease following spinal fusion (Review).

Anaphylaxis is a severe multisystem reaction that occurs rapidly after the introduction of an antigen that would otherwise be a harmless substance. It is characterized by airway and respiratory problems, cardiovascular collapse, mucosal inflammation, and other complications, all severe symptoms that can cause death. IgE-dependent anaphylaxis involves mast cells (MCs) which are the main sources of biologically active mediators that contribute to the pathological and lethal phenomena that can occur in anaphylaxis. Antibody-mediated anaphylaxis can follow multiple pathways such as that mediated b

Positioning appliance for an implant-retained prosthesis for a patient with surgically and radiation-induced microstomia: A clinical report.

When treating a patient with microstomia, prosthesis design can become challenging and often requires modifications. The patient's ability to insert and remove the appliance may also be a problem. This clinical report describes the fabrication of a positioning appliance for a patient with microstomia. The appliance used the maxillary teeth as a guide to orient the prosthesis appropriately and engage the mandibular LOCATOR abutments.

Lower Lip Reconstruction after Skin Cancer Excision: A Tailored Algorithm for Elderly Patients.

BACKGROUND: Lower lip reconstruction is crucial to restore oral integrity post-cancer excision. A perfect balance between form and function should be achieved. With an aging demographic, adapting surgical methods to meet the unique needs of the elderly becomes imperative. Our study aims to introduce a specialized algorithm for lower lip reconstruction; it was tailored to geriatric patients and emphasized the use of "simpler flaps". Additionally, "Pearls and Pitfalls" were provided for surgeons approaching lower lip reconstruction. METHODS: Between January 2018 and June 2021, a retrospective s

Myomucosal composite graft: A simple technique for full-thickness vermilion defects of the lower lip.

Reconstruction of vermillion defects of the lower lip requires careful consideration of functional and aesthetic aspects. Traditionally, various local flap methods involving tissue advancement from the corner of the mouth, lateral chin, and medial cheek have been commonly employed to fill lower lip defects. However, these approaches have inherent limitations, which include technical complexity, disruption of the orbicularis oris muscle, lip tightening, microstomia, and visible scarring. To overcome these limitations, we employed a free myomucosal composite graft from the lower lip to reconstru

Prevention and treatment of microstomia.

The mouth, located in the lower third of the face, is a uniquely visible structure. It serves as a vital organ both aesthetically and functionally, playing a key role in speech, expression, and fundamental oral functions. Consequently, any alterations or defects in its shape, due to various causes, can lead to aesthetic and functional deficiencies. These issues may also result in challenges with social interactions and a decrease in confidence. In cases of microstomia, various surgical approaches are proposed based on the location, extent, shape, and cause of the defect, leading to numerous ca

Managing Long-term Orofacial Contractures and Microstomia Through Intraoral Stretching.

Microstomia and orofacial contractures continue to garner interest regarding the effectiveness of treatment methodologies utilized to impact functional change. Oral splints are an accepted tool in the management of microstomia. However, the concepts of which oral splints to use, timing of initiation, and duration of treatment have not gained a consensus. This article reviews approaches to oral splinting and an alternative intraoral approach using splints designed to provide a graded, low load, multidirectional, and prolonged stretch specifically in facial burn survivors including those with ma

Recessive loss-of-function variants in DPH1 identified as the molecular cause in a sibling pair previously diagnosed with Fine-Lubinsky syndrome.

Fine-Lubinsky syndrome is a rare clinically defined syndrome sometimes referred to as brachycephaly, deafness, cataract, microstomia, and impaired intellectual development syndrome. Here we provide a clinical and molecular update for a sibling pair diagnosed with Fine-Lubinsky syndrome. An extensive genetic work-up, including chromosomal microarray analysis and quad exome sequencing, was nondiagnostic. However, a research reanalysis of their exome sequencing data revealed that both were homozygous for an intronic c.749+39G>A [NM_001383.6] variant in DPH1. RNAseq analysis performed on RNA from

Dental health concerns for patients suffering from facial, peri-oral burns, and inhalation injury: A persistent yet underappreciated challenge.

Demographic data reveal a correlative relationship between facial burns and profoundly impaired dental health, while inhalation injury correlates with a high Periodontal Screening and Recording Index, both of which significantly reduce quality of life for the burn injury patient. Despite these facts, few if any burn centers surveyed in the U.S. or Germany enlist specialized services and well-defined follow-up protocols for patients who might well be at risk. Facial burns represent a severe injury, the consequence of which may tragically lead to significant and long lasting functional, esthetic
