Persistent third branchial apparatus.

Neck sinuses, cysts or fistulae arising from the third branchial apparatus, have seldom been reported. Between 1979 and 1989, 16 cases were diagnosed as persistent third branchial apparatus based on a fistula open or in proximity to the pyriform sinus. There were 8 boys and 8 girls whose ages ranged from newborn to 13 years. Esophagogram was performed in 6; 4 showed a fistula tract leading to the pyriform sinus. Others were demonstrated at surgery. The main presentations were suppurative thyroiditis (5), lateral neck fistula (5), cyst (3), mass (1), abscess (1), and esophageal stricture (1). T

Branchial cleft anomalies: a five-year retrospective review.

Branchial cleft cysts, sinuses and fistulas are among the most commonly encountered congenital anomalies in pediatric otolaryngic practice. They can present difficulties in differential diagnosis and surgical management. In order to study the clinical presentation and surgical management of branchial cleft anomalies, the operative records of the Children's Hospital of Philadelphia (CHOP) were reviewed for a five year period, January 1, 1982 through December 31, 1986. All patients with a pathologically-confirmed post-operative diagnosis of a branchial cleft anomaly were included in a retrospect

Catheterization of branchial sinus tracts. A new method.

The complete surgical extirpation of branchial cleft abnormalities is essential in avoiding recurrent disease. The most technically difficult maneuver is often the localization and excision of associated sinus tracts and fistulae. The authors communicate a simple yet effective technique for dealing with this difficulty utilizing the Fogarty embolectomy catheter.

Fourth branchial pouch anomaly.

We present an extremely rare case of fourth branchial fistula in a 23-year-old male. The characteristic clinical feature was a recurrent left lower neck abscess which did not respond to appropriate medical and surgical therapy. Radiography and a computed tomographic scan with contrast material revealed a fistula running from the apex of the left pyriform sinus (internal opening) to the left lower neck abscess. The fistula tract was excised surgically. Histological examination of the excised fistula revealed a squamous epithelial lining and subepithelial lymphoid tissue. This pyriform sinus fis

[Congenital fistula of the 4th pharyngeal pouch and cleft].

This is the first description of a fistula arising from both the fourth branchial cleft and the fourth pharyngeal pouch in a 62-year-old woman. This developmental anomaly ran from the piriform sinus to the skin of the left side of the neck. A small quantity of liquid was seen to flow from the external opening of the fistula during drinking or swallowing. Surgery revealed a tract inferior to the superior laryngeal nerve and lateral to the recurrent laryngeal nerve. This fistula penetrated the larynx in the region of the cricothyroid joint and ended in the piriform sinus. This patient had neithe

[An unusual lateral cervico-facial fistula].

Lateral cervical sinuses of fistula are usually derivatives of the branchial clefts and pouches and have a more or less typical course in the neck. We present a patient with an extremely long sinus extending from the left supraclavicular area, via the cheek up to the left infraorbital rim. We could not find a similar report in literature. The diagnostic evaluation as well as the successful surgical removal of the malformation through a skin incision in the neck and a second incision in the sulcus gingivobucalis are described. On the basis of a brief review in the embryology of the congenital a

[Duplication of the auditory canal. A vestige of the 1st branchial cleft].

Anomalies of the first branchial cleft appear as duplicated auditory canals; they are rare clinical entities. Patients will present with a history of recurrent fistulas of the neck or parotid gland close to the angle of the mandible. There are two distinct malformations associated with the first cleft: A simple sinus which is lined with squamous epithelium and runs parallel to the auditory canal (Type I); the Type II anomaly, on the other hand, has a close, though variable relationship to the facial nerve and contains cartilage, a mesodermal derivate, within the wall. Embryologically it appear

Fistulae of the fourth endobranchial pouch.

Eight cases of laterocervical cystic tumor occurring during childhood are described. Three cases presented in the neonatal period as an obstructive tumor. Five cases presented in bigger children as recurrent abcess of the neck. Two of those were initially considered as acute suppurative thyroiditis. Laryngoscopy revealed, in all cases, a fistula originating from the apex of the pyriform sinus. Considering the pharyngeal connections, the anatomical route and the pathological reports, we believe that these cysts are derived from the 4th endobranchial pouch. A review of the literature of the past

[Fistulas of the 3rd and 4th branchial clefts].

The third and fourth branchial fistulas are exceptionally rare; both originate from the pyriform sinus, extend through the inferior constrictor of the pharynx and cross either the upper or the lower aspect of the superior laryngeal nerve, downwards and frontwards. Like the more common first and 2nd branchial fistulas, a cervical abscess is the usual consequence of these fistulas which should be treated by radical excision.

Congenital cervical salivary fistula.

A study of 35 lesions diagnosed as branchial cleft sinuses or fistulas identified a homogeneous subgroup characterized by ectopic salivary tissue, draining serous or mucoid fluid through a short sinus tract to the base of the neck anteriorly. These lesions should more properly be called congenital cervical salivary fistulas, since they may not arise from the branchial apparatus but rather from the embryonic placodal duct.

Recurrent suppurative thyroiditis due to fourth branchial pouch sinus.

Acute suppurative thyroiditis is a rare disease in childhood. The etiology of this condition is not always obvious. The description of a pharyngeal pouch fistula extending from the pyriform sinus to the thyroid gland or perithyroid area, high-lighted the etiopathogenesis of some of the thyroiditis cases. Presented here are the clinical, radiological and pathological aspects of an internal pharyngeal fistula manifesting with recurrent suppurative thyroiditis, until it was completely removed. The clinical features of caudal branchial pouch remnants and their role in the development of acute supp

Third branchial cleft sinus: an unusual cause of neck abscess.

Sinuses and fistulae of the third branchial cleft or pouch are rarely encountered. We have cared for a child with a history of multiple neck masses and abscesses, in whom the causative factor seems to have been an unrecognized branchial cleft sinus. This case suggests the need for contrast radiography of the hypopharynx in children with recurrent neck abscess, and dramatizes the need to exercise all reasonable diagnostic options before proceeding with surgery of newly presenting neck masses in children.

Subcutaneous bronchogenic anomalies.

Two cases of congenital anomalies, one cyst and one sinus, presenting in the suprasternal notch are reported. No communication with deeper structures was found at surgery, and histology was compatible with previous descriptions of subcutaneous bronchogenic cysts. The literature is reviewed, and the similarity between these lesions and many cases of branchial fistula is stressed.

First branchial cleft anomalies.

Our experience with anomalies of the first branchial cleft is reviewed. This includes 38 cases, the largest series collected to date. A new classification is proposed based on the anatomic findings--whether cyst, sinus, or fistula is present. The embryology and pathology of first cleft anomalies are discussed and an approach to the diagnosis and management of these lesions is given. In addition, a patient with a branchial cleft defect involving the middle ear space is reported.

[Diagnosis and treatment of congenital cysts and fistulae of the neck].

The diagnosis and operative treatment of congenital cervical fistulae and cysts are in many cases difficult, especially abnormalities of the first branchial cleft and of the sinus cervicalis. In our hospital 90 congenital cervical fistulae and cysts were treated surgically between April 1972 and August 1979, excluding cysts of the thyroid gland, lymphangiomas etc. Four of these children with a median cyst and two children with fistulae of the first branchial cleft had recurrences due to partial obliteration and fibrosis of the ducts following inflammation. Other causes for recurrence were atyp

The fourth branchial cleft anomaly.

The fourth branchial cleft anomaly has long been postulated. A case in which a lower neck sinus tract extended under the clavicle near the subclavian vessels is presented. The distinguishing features of bronchogenic and branchogenic anomalies are discussed. It appears that unless a complication (e.g., abscess, inflammatory process) of the fourth branchial cleft sinus tract occurs within the mediastinum, mandating full exploration, the complete form of the anomaly may continue to remain undocumented.

[Results of the surgical treatment of children with residual developmental anomalies of the 1st and 2d branchial clefts and thyroglossal duct].

The purpose of this study was assessment of the results of surgical treatment of children with developmental anomalies derived from the first and second branchial clefts and thyroglossal duct. Normal and abnormal development of branchial arches and clefts, pharyngeal pouches and thyroglossal duct, as well as the anatomy nd topography of the resulting developmental anomalies are discussed. The observed developmental anomalies are classified into: cysts, cysts with sinuses developing as a result of inflammatory processes, puncture, incision, operation, dermal sinuses draining externally, dermal

Genetic hearing loss with preauricular sinus and branchiogenic fistula.

A mother and her two children had hearing loss associated with bilateral preauricular sinus and branchiogenic fistula. All six cochleas studied showed two turns rather than 2 1/2 turns. Complete studies including audiometry, tympanotomy findings, and temporal bone polytomography of these anomalies are reported. Similar cases reported in the English literature are reviewed. It seems that all previous cases may have had an abnormal cochlea as was seen in these cases.

Branchial cleft anomalies: a review of 87 cases treated at the Toronto General Hospital.

The embryology, anatomy and pathology of branchial cleft anomalies are discussed and 87 cases reviewed. The most frequent anomaly was branchial cleft cyst, of which there were 77 cases. Treatment in all cases consisted of complete excision. There were five cases of external branchial sinus and five cases of complete branchial fistula. Sinograms were helpful in demonstrating these lesions. Excision presented little difficulty. No proved case of branchiogenic carcinoma has been found in the Toronto General Hospital. Five cases are described in which the original diagnosis was branchiogenic carcinom
