Clinical aspects of the branchio-oto-renal syndrome.

The branchio-oto-renal syndrome, first defined in 1976, is an autosomal dominant disorder characterized by anomalies of the external, middle, and inner ear in association with preauricular sinuses, branchial cleft anomalies, and varying degrees of renal dysplasia, including aplasia. Less frequently expressed phenotypic abnormalities include lacrimal duct aplasia and the stigmata of renal dysgenesis known as Potter facies. Although the precise incidence of the disorder is unknown, it may be more common than is generally appreciated and appears to be distinct from other autosomal dominant otobra

Abnormal facial development in the mouse mutant first arch.

The first arch mutation in mice, far, causes specific severe craniofacial defects, different from those caused by other craniofacial mutations. The purpose of the present study was to define the facial defects in far homozygotes, to identify heterozygote expression, if any, and to investigate the embryogenesis of the defect. A genetic study and a developmental study were done. The genetic study showed that only far homozygotes, observed on day 18 of gestation, have cleft palate, pointed snout, and deficiency and disorganized pattern of maxillary vibrissae. Approximately 75% have open eye(s). I

Persistent pharyngeal pouch derivatives in the neonate.

Internal branchial sinuses are rare compared to other branchial anomalies. A case is described in which such a sinus presented as an air-containing inflammatory lateral neck mass in a neonate and evidence is presented to support the theory that this originated from the fourth pharyngeal pouch. Review of six similar cases from the literature shows this to be a well-defined clinical entity.

Developmental deficiencies of the upper facial skeleton due to partial elimination of mesencephalic neural crest cells in the chick embryo.

With the aim to test the hypothesis that cells derived from the mesencephalic portion of the neural crest, are involved in the process of differentiation of various upper facial bones, in 41 chick embryos of the 6-somite stage (approx. 26 hours of incubation) the anterior and middle thirds of this part of the neural crest were partially eliminated by micro-laser irradiation, either unilaterally or bilaterally. Of the 14 embryos sacrificed at the age of 12 days, a number of 6 proved to have developed harelip and/or cleft palate conditions. In these embryos, in addition a reduction or absence of

Brief clinical report: a new syndrome of hemangiomatous branchial clefts, lip pseudoclefts, and unusual facial appearance.

Two unrelated children (one male, the other female) had unusual craniofacial anomalies consisting of hemangiomatous branchial clefts, lip pseudoclefts, and identical unusual facial appearance. One also had unilateral microphthalmia and both had congenital nasolacrimal duct obstruction. Two similar, sporadic cases from the literature were also identified. These four cases form the basis of a new, distinctly recognizable pattern of malformation.

Mandibular morphogenesis and craniofacial malformations.

Four questions have been addressed in this review. What is morphogenesis? I conclude that morphogenesis is simply development or change of shape and distinguish morphogenesis both from differentiation and from growth, although predominant growth along one axis can alter shape. Morphogenesis can be considered at all levels from the molecule to the population. I concentrated on cells, tissues, and organs. How does the mandible develop? A brief overview of the origin, migration, and differentiation of the neural crest and other cells which constitute the mandible has been provided. How is mandibu

Bilateral branchial cleft sinuses associated with intrauterine and postnatal growth retardation, premature aging, and unusual facial appearance: a new syndrome with dominant transmission.

A mother and son with bilateral branchial sinuses, intrauterine and postnatal growth retardation, unusual facial appearance, and premature aging in the mother are reported. No other members of the family are similarly affected. No hormonal or systemic cause of growth retardation was identified. Chromosomal studies with G-banding were normal. It is suggested that this syndrome is a dominant trait, the mother being the initial mutant.

[Diagnosis and treatment of congenital cysts and fistulae of the neck].

The diagnosis and operative treatment of congenital cervical fistulae and cysts are in many cases difficult, especially abnormalities of the first branchial cleft and of the sinus cervicalis. In our hospital 90 congenital cervical fistulae and cysts were treated surgically between April 1972 and August 1979, excluding cysts of the thyroid gland, lymphangiomas etc. Four of these children with a median cyst and two children with fistulae of the first branchial cleft had recurrences due to partial obliteration and fibrosis of the ducts following inflammation. Other causes for recurrence were atyp

Embryonic development of the head and neck: part 2, the branchial region.

The embryology of the branchial (pharyngeal) region is reviewed in terms of the embryonic pharynx, branchial apparatus and derivatives, tongue, larynx, and trachea. Derivatives of the branchial pouches, grooves (clefts), and arches are analyzed in relation to each other and to the first arch syndrome. Other common head and neck defects attributable to abnormal development of the branchial region are also discussed, particularly those defects involving the incomplete degeneration or the abnormal migration of branchial tissues.

Symposium: Congenital anomalies of the middle ear. III. Congenital anomalies of the middle ear.

Congenital anomalies of the middle ear have been considered rare. The tremendous volume of middle ear surgery performed during the last 15 years has brought attention to numerous anomalies of the facial nerve, ossicles, and middle ear cleft, suggesting that middle ear anomalies are not as rare as once believed. This paper discusses anomalies of the descending and horizontal facial nerve which have been previously reported. The literature of anomalies of the ossicles is reviewed. A very rare anomaly of the carotid artery presenting in the middle ear is described and its surgical treatment outli

[Syndrome of the 1st and 2nd branchial arches. Report of 3 cases].

Three newborn babies were studied in our hospital with the following associated unilateral deformities: macrostomia, hemignathia and anomalies of the external ear. The main symptom was the inability for suction causing some weight loss. These cases could be logically named "syndrome of the first and second branchial arches" because of their common origin. We discuss the pathogenesis and the differences with other diseases including those of hereditary origin. The initial management is discussed.

[Corneal dermoid cysts with other abnormalities (author's transl)].

Out of 22 patients with corneal dermoids, 9 showed other abnormalities consistent with the 1st branchial arch syndrome. In 4 cases there was hare lip, divided jaw and /or cleft palate along-side other abnormalities which made impossible a sure classification into one of the recognised syndromes. It is assumed that the anomalies arise in the 5th and 6th intrauterine week. The corneal dermoids were always unilateral and in only one case confined in position to the cornea.

Symposium on sensorineural hearing loss in children: early detection and intervention. Genetic factors in deafness of early life.

One half of all cases of childhood deafness are genetically caused, and this proportion will become increasingly larger as other causes come under control. The mode of transmission of hereditary hearing loss may be dominant, recessive, or X linked. Many varieties of childhood deafness can be distinguished by their accompanying anomalies. Other types of hereditary deafness occur, without associated abnormalities. Certain of these types may be delineated by the mode of transmission, age of onset, stability or progression, and audiometric findings. Further research on delineation of nonsyndromic

[A case of Goldenhar syndrome: acute vitamin A intoxication in the mother during pregnancy].

The authors describe the case of a boy aged 2 1/2, who showed at birth a bilateral epibulbar dermoid, preauricular appendices on the left side and other malformations suggesting Goldenhar's syndrome. The history disclosed that in the second month of pregnancy the mother had accidentally swallowed 10 ml of an oily solution of Vitamin A during a laboratory manipulation. In their discussion, the autonrs analyse the teratogenic action of Vitamin A during embryonic development and review the aetiologies found in cases of Goldenhar's syndrome.

Median facial malformations and their implications for brain malformations.

For purposes of identifying craniofacial syndromes which predict brain malformations the face can be regarded as developing from 2 sources. The median and paramedian structures derive from the embryonic segment of the face, termed the frontonasal prominence. The lateral structures of the face derive from the branchial arches. Certain patterns of median plane facial anomalies predict a severe brain malformation, while other patterns, although producing equally grotesque facial malformations, bear little association with a malformed brain. Orbital hypotelorism may occur in a variety of syndromes

Variability versus "incidental findings" in the first and second branchial arch syndrome: unilateral variants with anophthalmia.

Four cases of hemifacial microsomia with unilateral anophthalmia are presented and various nosologic problems in the branchial arch dysplasias are discussed. Hemifacial microsomia with unilateral anophthalmia may or may not represent a formal genesis syndrome and nosologic entity different from other types of hemifacial microsomia. The disorder is not in reality a branchial arch dysplasia, but rather a complex dysmorphogenetic syndrome.
