Transverse facial cleft repair: Preventing pigmentation around the postoperative scar by excising the pigmented white lip at the cleft margin-A retrospective case series of Japanese patients.

Numerous transverse facial cleft repair techniques and designs have been described; however, a complication of surgical reconstruction is scar formation with brown discoloration. This study investigated the histological basis of the darker white lip at the cleft margin, its role in postoperative brown discoloration, and the potential esthetic benefits of extending the excision. In this retrospective analysis, patients with transverse facial cleft were divided into limited and extended excision groups. Histological assessment of melanin pigment was performed using Fontana-Masson staining, and p

Surgical Strategies Before Auricular Reconstruction: A Treatment Algorithm for Craniofacial Anomalies in Goldenhar Syndrome.

Goldenhar syndrome (GS) presents complex craniofacial malformations and challenging timing for auricular reconstruction. This study characterizes the craniofacial phenotype and staged surgical management preceding ear reconstruction. Single-center retrospective cohort of 30 patients (2019-2024). Craniofacial anomalies and all procedures performed before auricular reconstruction were recorded. Categorical data were summarized as n (%), and ages as median. Auricular anomalies were universal; microtia occurred in 27/30 (90.0%) and external auditory canal atresia in 43.3%. Hemifacial microsomia wi

Clusters in craniofacial microsomia and microtia according to facial morphology and craniofacial anomalies.

UNLABELLED: Associated craniofacial malformations such as ocular anomalies and macrostomia are frequently seen in craniofacial microsomia (CFM). This study assessed associations between objectively measured facial morphology and clinically observed craniofacial malformations in patients with CFM in order to identify clinically significant patient clusters based on these characteristics for preferential screening purposes. In this multicenter cross-sectional cohort study, facial morphology was assessed from three-dimensional (3D) photographs using asymmetry index (ASI) and facial signature (FS)

Transverse facial cleft repair using a small triangular flap technique.

Transverse facial cleft is a rare congenital malformation frequently associated with anomalies of the first and second branchial arches. We report a case of transverse facial cleft repaired using a small triangular flap technique designed to improve aesthetic and functional outcomes while minimizing scar formation. A 5-day-old infant presented with a right transverse facial cleft and bilateral accessory auricles without additional anomalies. Surgical repair was performed at 5 months of age. A 2-mm triangular flap was designed at the commissure, followed by excision of redundant skin and mucosa

Long-Term Functional and Aesthetic Outcomes of Bilateral Tessier No. 7 Cleft With Associated Anomalies: A Case Report With 20-Year Follow-up.

We report the long-term functional and aesthetic outcomes of a male patient with bilateral severe Tessier no. 7 clefts associated with mandibular hypoplasia, cleft palate, right soft-palate hypoplasia, and congenital bilateral facial nerve palsy. The patient required enteral nutrition from birth because oral feeding was impossible. Through staged reconstruction, including oral floor muscle release, bilateral commissuroplasty, palatoplasty, pharyngeal flap surgery, tracheostomy, and later tracheostomy closure, he ultimately achieved full oral intake, phonation, and social adaptation in adulthoo

A Novel Advantage to Microtia Reconstruction Utilizing Ear Molding: A Case Report of Goldenhar Syndrome's Clinical Presentation and Surgical Reconstruction.

This case report describes a unique approach to reconstructing microtia in a patient with microtia and craniofacial microsomia. Microtia is a congenital outer ear deformity that can vary in severity, from minor abnormalities to complete absence of the ear. The traditional methods for microtia reconstruction are autologous reconstruction using the patient's rib cartilage or alloplastic reconstruction using porous polyethylene. Autologous reconstruction typically begins around age six to 10 years of age when the rib cage is sufficiently large enough, while alloplastic reconstruction may be perfo

Infantile epileptic spasms syndrome as a new phenotype in TOP2B deficiency caused by a de novo variant: a case report and literature review.

Background: Type II DNA topoisomerases (EC5.99.1.3) are enzymes that catalyze topological changes during DNA replication and gene transcription in an ATP-dependent manner. Vertebrates have two isoforms: topoisomerase IIα and β. Type II topoisomerase β is encoded by TOP2B. For TOP2B, a number of germline pathogenic variants have been identified as causative for human diseases, including Hoffman syndrome, ablepharon-macrostomia syndrome with immunodeficiency, B-cell immunodeficiency, distal limb anomalies, and urogenital malformations syndrome. To date, only 14 patients with the above diseases f

Unusual Bilateral Oblique Craniofacial Cleft with Atypical Cleft of Palate: A Case Report with 3 Years of Follow-Up.

Oblique facial clefts are a rare entity and can be associated with palatal anomalies like synechiae caused by amniotic bands. Their treatment is individualized following sound reconstructive surgical principles. This is a case report of a 15-month-old child with cleft palate in which the right palatal shelf was represented by a tongue-like projection, associated with macrostomia and bilateral medial and lateral oro-ocular clefts. The palatal cleft was closed in two stages and the oblique clefts in the first stage. At 4 years of age, the velopharyngeal incompetence was addressed by a superiorly

The Presentation Patterns of Tessier 7 Clefts.

Given its rarity, there are few large-number studies regarding Tessier 7 clefts. A review and classification of 81 Tessier 7 patients who presented to our craniofacial center was performed. The most common Tessier 7 clefts were middle positioned, followed by agenetic, superiorly rotated, and inferiorly rotated clefts, respectively. A total of 77/81 patients had simple macrostomia and 4/81 patients had macrostomia with muscle diastasis. The most common associated anomalies were craniofacial microsomia, skin tags, and ear anomalies occurring in 80%, 71%, and 37% of patients, respectively. In thi

Upper and lower eyelid contour and positional changes after deep skin grafts in ablepharon macrostomia syndrome.

Ablepharon macrostomia syndrome is a rare congenital disorder caused by autosomal-dominant TWIST2 mutations. This condition is characterized by redundant skin, low-set ears, macrostomia, ambiguous genitalia, and underdevelopment of the both upper and lower eyelids. The shortening of the anterior lamella, septum and levator aponeurosis lead to a severe corneal exposure within the first hours of life. Since McCarthy and West's first report in 1977, 21 AMS cases have been documented. We report a new AMS case with a quantitative analysis of palpebral fissure changes following skin grafts over the

Commisuroplasty for Macrostomia in Tessier 7 Cleft Using Modified Vermillion Square Flap Technique.

BACKGROUND: The transverse facial cleft, as described by Tessier, is a rare anomaly that affects both aesthetics and function. Various techniques for commissuroplasty have been discussed in the literature, often with inconsistent results. In this case series, the authors used a modified vermilion square flap technique to achieve satisfactory oral function and aesthetic outcomes. METHODS: Five Tessier 7 cleft patients underwent reconstruction using vermilion square flap technique with W-plasty for the skin closure to help create a less noticeable scar. Evaluation was conducted at least 1 year

Macrostomia Surgery Using Small Z-plasty for Construction of the Corner of the Mouth.

Macrostomia is a type of transverse facial cleft (termed number 7 in the Tessier classification) and a disorder in which there is a cleft on 1 or both sides of the corner of the mouth. The Kaplan technique is used for the treatment of macrostomia and is characterized by forming the corners of the mouth with a single continuous flap of vermilion. In this technique, there are no suture lines at the commissure; thus, it does not cause contracture at the corners of the mouth. We performed a small Z-plasty on the outer edge of the flap at the corner of the mouth, constructed using the Kaplan techni

A Rare Case of Bilateral and Midline Cleft Palate With Bilateral Macrostomia.

A 6-day-old girl presented with bilateral macrostomia, micrognathia, and a complex cleft palate consisting of a midline submucous cleft, left lateral submucous cleft, and a right lateral cleft with partial soft-palate agenesis. Owing to Pierre Robin Sequence, surgery was deferred until airway stability. Bilateral macrostomia repair was performed at 12 months, followed by one-stage correction of all palatal clefts at 17 months using a right buccal myomucosal flap for oral lining. Rare clefts variably labeled as oblique clefts or soft-palate agenesis should be uniformly termed "lateral cleft pal

Risk factors and treatment strategies for adjacent segment disease following spinal fusion (Review).

Anaphylaxis is a severe multisystem reaction that occurs rapidly after the introduction of an antigen that would otherwise be a harmless substance. It is characterized by airway and respiratory problems, cardiovascular collapse, mucosal inflammation, and other complications, all severe symptoms that can cause death. IgE-dependent anaphylaxis involves mast cells (MCs) which are the main sources of biologically active mediators that contribute to the pathological and lethal phenomena that can occur in anaphylaxis. Antibody-mediated anaphylaxis can follow multiple pathways such as that mediated b
