Palatoplasty for bilateral oblique cleft of the soft palate accompanied by Tessier number 5·7 cleft.

Craniofacial cleft is a rare congenital deformity in which there are fissures of a facial structure classified by Tessier. A palatoplasty to reconstruct the muscle sling and to close the cleft of the velum was performed. Acceptable postoperative appearance of the velum was seen after 6 months. Although further follow-up is still needed for velopharyngeal function, this case report provides details of the anatomical deformities and our surgical approach to this atypical bilateral cleft palate.

Barber-Say syndrome in a father and daughter.

We report on a father to daughter transmission of Barber-Say syndrome (BSS), a rare, congenital disorder characterized by severe generalized hypertrichosis, macrostomia, ocular telecanthus, bulbous nose and atrophic skin. These two cases further support the autosomal dominant inheritance. Both presented with the typical BSS symptoms but the phenotypic expression in the father was milder. Treatment is challenging for both patients and doctors, requiring a multidisciplinary approach.

A rare bilateral Tessier no. 6 and 7 clefts.

The authors describe a patient with bilateral Tessier no. 6 and no. 7 clefts. The case consists of macrostomia combined with maxillary, zygomatico-orbito-temporal deformities, dental anomalies and mandibular retrusion. In addition, the cleft is located bilaterally in the maxillary arch with a double deciduous posterior dentition. This is rare. The clinical and radiological features are reported and the treatment plans are discussed.

Macrostomia: a study of 15 patients seen in Lagos, Nigeria and proposal for a classification of severity.

We made a descriptive epidemiological study of patients who presented with isolated macrostomia during a mass screening programme and repair of orofacial cleft deformities in Lagos, Nigeria. Detailed histories of environmental, gestational, and hereditary factors that may contribute to the malformation were taken, and physical examinations and specialist consultations to detect associated congenital malformations were made. Three patients had unilateral macrostomia and in 12 it was bilateral and symmetrical. The mean (SD) age was 8 (2) years, range 3 months to 32 years. There were 6 men and 9

Fiberoptic Intubation Using LMA as A Conduit and Cook Airway Catheter as An Exchanger in A Case of Tessier 7 Facial Cleft Syndrome.

Any anaesthesiologist handling a paediatric airway must have a detailed understanding of the differences in airway anatomy, signs and symptoms of airway compromise and common paediatric airway abnormalities. In addition to various equipments needed to manage a difficult airway, there should be a clear plan for evaluation, preparation and management of life threatening complications. We share our experience of successfully managing a difficult airway of a 5 year old child with Tessier 7 facial cleft syndrome. We emphasize the importance of preoperative evaluation, preparation and use of various

Refined simple line closure for macrostomia repair: designing a mucosal triangular flap on the commissure region.

Congenital macrostomia is a relatively rare deformity. A number of different methods for its correction have been reported in the past. Here, we report our refined method of correcting macrostomia. Our method is characterized by creation of a small triangular mucosal flap on the cleft region of the lower red lip where the tissue appears identical to the normal commissure. Adequate advancement of this flap into the mouth along with proper reconstruction of muscle continuity and simple line closure of the skin enable correction of macrostomia with a natural-looking commissure in a symmetric posi

The surgical repair of macrostomia.

BACKGROUND: Many surgical techniques have been described for the repair of macrostomia. However, most were designed for the repair of unilateral macrostomia. Most reports of repairs involved small numbers of patients. This has not allowed for evaluation of complications and comparison of various techniques. OBJECTIVE: This study describes a modification of an older technique designed for unilateral macrostomia. Details of the operative technique used for the repair are described, along with the principles underlying the various steps. MATERIALS AND METHODS: This study involves 13 patients op

Case report supporting that the Barber-Say and ablepharon macrostomia syndromes could represent one disorder.

We report on a 7-year-old girl with unequivocal features of Barber-Say syndrome (BSS): generalized hypertrichosis especially at the back, dry lax skin, macrostomia, thin lips, cup-shaped ears, bulbous nose, hypoplastic nipples, and abnormal external genitalia. She also demonstrated conductive hearing impairment and microblepharon. BSS has been reported with ectropion (not present in our patient), but ablepharon and microblepharon (i.e., absent or hypoplastic eyelids) have always been considered as hallmarks of ablepharon macrostomia syndrome (AMS). This is the first report of microblepharon in

Insulin--a growth hormone.

Insulin is a peptide hormone with a high degree of homology with the insulin-like growth factor I (IGF-I). Its biological actions are interlaced with those of GH and IGF-I. The objective of this study is to review the growth promoting actions of insulin. The experimental evidence consists of the use of organ cultures of neonatal mice condilar cartilage insulin which stimulates the cartilage cell differentiation and maturation. Injection of insulin to hypohysectomized rats stimulated tibial growth. Clinical evidence is manifold. Babies with diabetes and hypoinsulinemia are short, whereas babies

Tessier no. 7 cleft: a new subclassification and management protocol.

BACKGROUND: Tessier described rare craniofacial clefts anatomically. The no. 7 cleft is a lateral facial cleft consisting of macrostomia, lateral facial muscular diastasis, and bony abnormalities of the maxilla and zygoma. Early computed tomographic imaging provided preliminary insight into the bony abnormality. This article reviews this patient group, defining the clinical and radiological features, to advise optimal protocol management. METHODS: A retrospective case-note review of 15 patients with Tessier no. 7 clefts managed by the Australian Craniofacial Unit over the past 25 years was pe

Pallister-Killian Syndrome (PKS) as a Cause of Mental Retardation.

Pallister-Killian syndrome (PKS; OMIM: # 601803) is a rare sporadic syndrome of multiple congenital anomalies attributable to the presence of a de novo mosaic supernumerary isochromosome 12p [i(12p)]. The syndrome presents with a recognizable pattern of findings including: pigmentary skin changes, characteristic facial features (sparse anterior scalp hair, flattened midface, macrostomia, and coarsening of the facial features), and developmental delay. The developmental phenotype of PKS is quite variable, but most are considered to fall into the profound range of developmental retardation. We r

An isolated bilateral pure macrostomia in a 2-year-old girl.

Congenital macrostomia is a lateral orofacial cleft between the maxillary and mandibular components of the first branchial arch. Bilateral macrostomia is a rare entity. To date, 17 cases have been reported in the literature, with more than 50% isolated. We present a case of bilateral transverse facial cleft (macrostomia) in a 2-year-old girl. It was not associated with any other anomaly. The cleft involved only the soft tissues of the face. Hence, it was a pure macrostomia. She was the third female child of the family, with 2 elder sisters normal. Nutritional deficiency during intrauterine lif

A case of Goldenhar-Gorlin syndrome with unusual association of hypoplastic thumb.

A case of Goldenhar-Gorlin syndrome in a seven-month-old male infant presented with the features of epibulbar dermoid, microtia and hemifacial microsomia associated with thumb defect. The dermoid was bilateral and microtia was unilateral. Preauricular appendages and pits were double and single respectively on both the sides. Hemifacial microsomia was unilateral and was associated with cleft lip, macrostomia, dental misalignment, large tongue and high arched palate. The association of hypoplastic thumb with Goldenhar-Gorlin syndrome has not been documented in the past.

Myoplasty for congenital macrostomia.

OBJECTIVE: To describe the different myoplasty techniques that could be used for limited commissural reconstruction. METHODS: Twelve cases of congenital macrostomia are reported, with different cleft lengths and termination sites. For each case, an orbicular myoplasty was performed, and in the case of extension to the area of the tragus or tonsillar pillars, a masseteric myoplasty or pharyngoplasty was performed. Functional and aesthetic results were analyzed. RESULTS: Functional results were excellent, with normal phonation, facial expression, and deglutition in the case of posterior extens

The Tessier number 5 facial cleft: surgical strategies and outcomes in six patients.

The Tessier no. 5 facial cleft is an extremely rare congenital malformation. Only 26 cases have been described in the English-language literature. The cleft begins in the upper lip just medial to the oral commissure, extending across the cheek as a groove ending at the junction of the middle and lateral thirds of the lower eyelid. The bone involvement usually includes an alveolar cleft in the premolar region, extends across the maxilla lateral to the infraorbital nerve, up to the infraorbital rim and orbital floor. The goals of the surgical procedure include reconstructing the lower eyelid, re

Fraser and Ablepharon macrostomia phenotypes: concurrence in one family and association with mutated FRAS1.

To date, Fraser syndrome (FS) and Ablepharon macrostomia syndrome (AMS) have been considered distinct disorders, but they share strikingly similar patterns of congenital abnormalities, specifically craniofacial anomalies. While recent research has led to the identification of the genes FRAS1 and FREM2 as the cause of FS, the genetic basis of AMS continues to be enigmatic. We report on the concurrence of AMS-like and Fraser phenotypes in a Brazilian family. Both affected sibs were homozygous for a novel splice site mutation in the FRAS1 gene. Extensive studies on mRNA expression indicated that

An apparent duplication of the mouth in a patient with midline dysraphism and a teratoid polyp: A 15-year postsurgical follow-up.

True median cleft of the upper and lower lip occurs very rarely. Complete or partial facial duplication is an extremely rare malformation. Approximately 26 cases have been reported since 1900, spanning a wide spectrum of clinical severity. Most cases appear to share a number of features, including cleft palate, duplication of the tongue, orbital hypertelorism and macrostomia. An unusual patient with midline clefts of the upper and lower lips with an apparent duplication of the mouth and a teratoid polyp, with fusion of the maxilla and mandible, is reported.
