Risk prediction of mechanical complications in acute myocardial infarction patients with prior malignancy.

BACKGROUND: Acute myocardial infarction (AMI) patients with prior malignancy have been largely understudied, despite potentially facing higher risks of adverse outcomes. This case-control study aimed to identify independent risk factors for in-hospital mechanical complications among AMI patients with prior malignancies. METHODS: This study enrolled AMI patients with prior malignancy who were hospitalized for treatment. Patients were divided into complication and control groups based on the occurrence of in-hospital mechanical complications. The mechanical complications in this study were defi

Three different surgical approaches for complex transposition of the great arteries with ventricular septal defect and pulmonary stenosis.

The treatment of complex transposition of the great arteries with a ventricular septal defect and pulmonary stenosis/left outflow tract obstruction poses significant challenges. The decision between performing a Rastelli procedure, a Nikaidoh procedure or a double root rotation depends on the cardiac anatomy, especially the location of the ventricular septal defect, its spatial relationship with the great arteries, the presence of straddling of the atrioventricular valve and the appreciation of the pulmonary valve. In our centre, our goal is to provide the most physiologic repair tailored to e

Update on Surgery for Complex Transposition of the Great Arteries: Focusing on Aortic Root Translocation.

Transposition of the great arteries (TGA) is a congenital heart defect (CHD) characterized by discordance between the ventricles and great arteries. The arterial switch operation generally provides excellent early and long-term outcomes in cases in which both great arteries are well developed. Furthermore, various surgical techniques, including the Rastelli procedure, réparation à l'étage ventriculaire (REV) procedure, and pulmonary root translocation have been introduced to improve the outcomes in patients with complex TGA accompanied by significant left ventricular (LV) outflow tract obstruc

Echocardiographic Assessment of Cardiac Phenotype Predicts Complications and Guides Intensive Care Management Following Pulmonary Valve Balloon Dilation in Neonates With Pulmonary Atresia/Critical Pulmonary Stenosis With Intact Ventricular Septum.

INTRODUCTION: Pulmonary valve balloon dilation (PVBD) has largely replaced surgical decompression as the preferred treatment for neonates with pulmonary atresia with intact ventricular septum (PAIVS) or critical pulmonary stenosis (CPS). This study aims to evaluate whether echocardiographic morphological and functional assessment can predict early complications and guide intensive care management following PVBD in this population. METHODS: We retrospectively analyzed 27 neonates with PAIVS or CPS who underwent PVBD between 2017 and 2023. Patients were divided into Group A (tripartite right ve

Characterization of an induced pluripotent stem cell line NCHi025-A from a 1-year-old female with pulmonary stenosis harboring a heterozygous HAND2 variant.

HAND2 is a transcription factor that plays a vital role in the development of the heart, limbs, and pharyngeal arch. Functional defects in HAND2 have been shown to cause congenital malformations in the extremities of the body and the heart. NCHi025-A iPSC line was generated from a 1-year-old female with pulmonary stenosis and harbors a novel de novo heterozygous variant of uncertain significance within HAND2 (NM_021973.3; c.247delG; p.Val83CysfsTer16). This variant causes a frameshift and premature stop codon that is predicted to truncate the protein. NCHi025-A is a pluripotent stem cell line

Characterization of a Novel GATA4 Missense Variant p.Gly303Trp in a Family with Septal Heart Defects and Pulmonary Stenosis.

Congenital heart disease (CHD) represents a prevalent group of structural cardiac anomalies often associated with alterations in key transcription factors including NKX2-5, TBX5, and, particularly, GATA4. GATA4 is a zinc finger transcription factor essential for regulating genes involved in cardiogenesis. Here, we report the identification of a novel heterozygous missense variant in GATA4 (NM_002052.5:c.907G>T, p.Gly303Trp) in a family with a history of CHD. The proband, exhibiting ventricular septal defect (VSD) and pulmonary stenosis, was referred for genetic evaluation after recurrent spont

Using Multimodality Imaging and 3-Dimensional Printed Models to Guide Decision-Making for Complex Congenital Biventricular Repair.

BACKGROUND: Determining the optimal surgical approach for complex congenital heart disease (CHD) can be challenging using conventional 2-dimensional transthoracic echocardiography (TTE) and cross-sectional imaging, including cardiac magnetic resonance (CMR) and computed tomography. CASE SUMMARY: An 11-month-old female patient presented with heterotaxy syndrome, dextrocardia, double-outlet right ventricle, and complex pulmonary valve stenosis. Fetal echocardiography and postnatal TTE established the complexity of the cardiac anatomy and led to a multidisciplinary discussion to plan a complex s

Beyond a mirror-image procedure: a case report of leadless pacemaker implantation in dextrocardia with situs inversus.

BACKGROUND: Dextrocardia with situs inversus (DC + SI) is a rare congenital anomaly characterized by mirror-image positioning of the heart and thoracoabdominal organs. Leadless pacemaker (LP) implantation in patients with DC + SI presents unique anatomical and technical challenges, and clinical experience remains limited. CASE SUMMARY: A 76-year-old woman with DC + SI presented with tachycardia-bradycardia syndrome. Given her symptomatic bradycardia, we undertook LP implantation (Micra, Medtronic, Minneapolis, MN, USA). Pre-procedural computed tomography demonstrated a mirror-image arrangemen

Effect of Infundibular Stenosis on Growth of the Pulmonary Valve Annulus in Patients With Tetralogy of Fallot.

Objective: This study aimed to determine the effect of the degree of infundibular stenosis on growth of the pulmonary valve (PV) annulus in patients with tetralogy of Fallot (ToF). Methods: We reviewed clinical data of all patients who underwent ToF repair between 2002 and 2022. The diameter of the infundibular area was measured before ToF repair using a lateral view of angiography at end-systole and divided by the body surface area (infundibular index [mm/m2]). The cohort was divided into two groups, a systemic-to-pulmonary artery shunt (SPS) and a primary intracardiac repair (ICR), and compa

Case Report: A rare case of asymptomatic unroofed coronary sinus in a dog: diagnostic imaging and genetic findings.

A 2-year-old, 3.8 kg spayed female Pomeranian was presented for a routine health examination with no clinical signs. Physical examination revealed no cardiac murmur. Blood tests were unremarkable and electrocardiography revealed normal sinus rhythm. Thoracic radiography showed a normal cardiac silhouette. Transthoracic echocardiography identified a left atrial size at the upper limit of the normal range (left atrium-to-aortic root ratio: 1.7) and a markedly visualized tubular structure along the posterior wall of the left atrium, presumed to be the coronary sinus. Color Doppler imaging reveale

The Predictive Role of Prenatal Echocardiography in the Diagnosis of Postnatal Pulmonary Stenosis.

To evaluate prenatal echocardiography predictors of postnatal pulmonary stenosis (PS) requiring infant surgery. Consecutive fetuses with suspected cardiac anomalies (n = 303) were screened. After excluding cases of induced abortion (n = 18), the remaining cohort of 285 fetuses was analyzed. Postnatal follow-up confirmed PS in 30 neonates. Diagnostic consistency (kappa statistics) and risk factors (logistic regression/ROC analysis) for surgical intervention were assessed. This study enrolled fetuses with structurally abnormal hearts prenatally diagnosed with PS, defined by accelerated pulmonary

Antimicrobial prophylaxis in dogs undergoing catheter-based cardiac interventions and pacemaker implantation - A survey.

Guidelines for prophylactic antimicrobial use for catheter-based cardiac interventions and pacemaker implementation in dogs do not exist. Optimizing antimicrobial use by rationalizing indications and the duration of administration is essential against the ever-growing threat of antimicrobial resistance. The aim of our study was to reveal the current habits of prophylactic antimicrobial use among veterinarians who perform transvascular cardiac interventions in dogs. A digital survey was offered via the mailing list of an international veterinary cardiology-interest group. The 141 respondents re

Higher Incidence of Moderate and Severe Congenital Heart Disease in Patients With Neonatal Abstinence Syndrome.

Studies suggest that in utero opioid exposure may be associated with congenital heart disease (CHD). We sought to assess the incidence of CHD in infants with neonatal abstinence syndrome (NAS). A review of a national, administrative database from January 2019 to December 2022 was conducted for neonates with an ICD-10 code for NAS and moderate or severe CHD. The incidence of CHD in NAS (the main outcome variable) was compared to those without NAS using χ2 analysis and odds ratios for CHD with significant differences. There were 4,994,919 neonatal admissions during the study period: 26,284 with

Right Ventricular Outflow Tract and Pulmonary Artery Strains During the Cardiac Cycle Prior to Pulmonary Valve Replacement.

Right ventricular outflow tract and pulmonary arteries (RVOT/PAs) in surgically repaired congenital heart disease are highly dynamic structures, dilating in systole to accommodate the ejected blood volume while also elongating due to the ventricular contraction. In this study, we aim to quantify RVOT/PA circumferential and axial strains in a population of patients assessed for transcatheter pulmonary valve replacement (TPVR). 3D RVOT/PA geometries at end diastole and systole were reconstructed from 4D computed tomography images of 20 patients (35% female; 34 ± 15 years; 50% pulmonary stenosis,
